    UNSUCCESSFUL PRIMARY PCI FOR POSTPARTAL DISSECTION OF ALL THREE MAJOR CORONARY ARTERIES

    Coronary artery dissection is rare but very dangerous condition which can result in myocardial infarction. It mostly occurs in young postpartum women. The left anterior descending artery is affected in 75% of cases and in some extremely rare cases dissection may include all three coronary arteries. Treatment guides are still not consistent. While some authors recommend medical treatment other recommend surgical one. Case report. We present a 36-year-old female patient who was admitted to our hospital with intermittent chest pain and dyspnea that occurred two hours before admission. After premedication with Aspirin 300 mg per os, Clopidogrel 300 mg per os and Enoxaparin 30 mg iv, the pain disappeared. Chest pain was repeated after five days. Coronary angiography revealed spiral dissection in medial segment of LAD. After PCI attempt, dissection progressed and ended fatal. Pathological finding was intimal dissection of the coronary arteries with loss of some parts of internal elastic lamina. It is interesting that in the literature there is no consistent opinion about therapeutic approach to SDCA and we hope that this case report will contribute to elucidating the problem

    U radu su prikazani rezultati merenja i proračuna koji su urađeni na agregatu A3 u HE 'Zvornik', a u cilju određivanja propelernih i kombinatorskih karakteristika Kaplanove turbine. Specifična pažnja je posvećena određivanju koeficijenta protoka protokomera Vinter-Kenedi. Ova merenja daju rezultate koji omogućuju rad turbine u tačkama optimalnih stepena korisnosti. .The measurement and calculation results which were performed at unit A3 in HPS 'Zvornik' are presented in this paper with the aim of Kaplan turbine propeller and combinatory characteristics determination. Special attention is given to the Winter-Kennedy discharge coefficient determination. These measurement procedures gave the opportunity for unit operation at optimal efficiencies.

    In the last three decades the authors have published papers on the concept of the supra-molecular organization and the phase state of compressed ethylene gas and their effects on the mechanism and kinetics of free-radical ethylene polymerization. The effects on the macromolecular structure of low density polyethylene (LDPE) were also explained. The importance of the phase state of compressed ethylene on the industrial process of LDPE production are presented in this paper: The start-up of polymerization, the peak in the polymerization rate curve, the stability of the reaction, the structure and properties of LDPE, the separation of unreacted ethylene and polyethylene and ethylene compression are discussed

    Significance of magnetic resonance imaging in differential diagnosis of nontraumatic brachial plexopathy

    Background/Aim. Nontraumatic brachial plexopathies may be caused by primary or secondary tumors, radiation or inflammation. The aim of this study was to present the significance of MRI in revealing the cause of nontraumatic brachial plexopathy. Methods. A two-year retrospective study included 22 patients with nontraumatic brachial plexopathy. In all the patients typical clinical findings were confirmed by upper limb neurophysiological studies. In all of them MRI of brachial plexus was performed by 1.5 T scanner in T1 and T1 FS sequence with and without contrast, as well as in T2 and T2 FS sequences. Results. Seven (32%) patients had brachial plexopathy with signs of inflammatory process, 5 (23%) patients had secondary tumors, in 4 (18%) patients multifocal motor neuropathy was established and in the same number (18%) of the patients postradiation fibrosis was found. Two patients (9%) had primary neurogenic tumors. Conclusion. According to the results of this study MRI is a method which may determine localization and cause of brachial plexopathy. MRI can detect focal nerve lesions when other methods fail to find them. Thus, MRI has a direct impact on further diagnostic and therapeutical procedures

    Integration of dry-column flash chromatography with NMR and FTIR metabolomics to reveal cytotoxic metabolites from Amphoricarpos autariatus

    Metabolomics generate a profile of small molecules from plant extracts, which could be directly responsible for bioactivity effects. Using dry-column flash chromatography enabled a rapid and inexpensive method for the very efficient separation of plant extract with a high resolution. This separation method coupled to NMR and FTIR-based metabolomics is applied to identify bioactive natural products. OPLS multivariate analysis method, was used for correlation the chemical composition of the plant extracts, Amphoricarpos autariatus, with the results of cytotoxic activity against Human cervical adenocarcinoma cell line (HeLa) and epithelial lung cancer cell line (A549). In this way, the highest contribution to the cytotoxic activity was recorded for the guaianolide sesquiterpene lactones named amphoricarpolides.     Pomegranate seeds are an excellent source of compounds with biological activity. In the present study, the pomegranate seed oil (PSO) was tested for antioxidant and cytotoxic potential. PSO was extracted by super-critical fluid extraction with previous treatment of seeds by microwaves and analysed for its fatty acid profile, carotenoid and tocopherol contents. PSO demonstrated to be a good source of polyunsaturated fatty acids (PUFAs) (83.5%), followed by monounsaturated fatty acid (MUFA) and saturated fatty acid (SFA) (9.5% and 7.0%, respectively). Concerning fatty acid profile, punicic (C18:3), oleic (C18:1) and linoleic (C18:2) acids were the predominant ones. Carotenoids and tocopherols were identified and quantified by HPLC. Results showed that the PSO was a good source of gamma-tocopherol as dominant among investigated compounds (128.6 mg/100 g). The extract was screened for its antioxidant activity using alpha-TEAC assay. Cytotoxic effect was determined by MTT test against malignant HeLa, LS174 and A549 cells (IC50 = 49.51-91.54 mu g/mL), and against normal MRC-5 and EA.hy926 cells (IC50 gt 200 mu g/mL). The best effect was exhibited on cervix adenocarcinoma cell line while no cytotoxicity was found for normal cells pointing out PSO safety

    Background/Aim. Advanced magnetic resonance techniques can provide insight in physiological changes within pathological canges and contribute to better distinquishing between different tumor types and their discrimination from non-neoplastic lesions. The aim of this study was to evaluate the role of proton magnetic resonance spectroscopy (1H-MRS) and apparent diffusion coefficients (ADC) in distinguishing intracranial glial tumors from tumor like nonneoplastic lesions, as well as for differentiating high- from low-grade gliomas. Methods. This retrospective study included 47 patients with solid brain lesions (25 nonneoplastic, 14 low-grade and 8 anaplastic glial tumors). In all patients 1H-MRS (at a TE of 135 ms and 30 ms) and diffusion- weighted imaging (DWI) were performed. The choline to creatine (Cho/Cr), choline to N-acetyl aspartate (Cho/NAA), N-acetyl aspartate to creatine (NAA/Cr) and myoinositol to creatine (mIn/Cr) ratios and the apparent diffusion coefficient (ADC) were determined. Results. The Cho/Cr ratio was significantly higher in glial tumors grade II than in non-neoplastic lesions (p = 0.008) and in glial tumors grade III than in non-neoplastic lesions (p = 0.001). The Cho/NAA ratio was significantly higher in glial tumors grade II than in non-neoplastic lesions (p = 0.037). ΔADC/ADC between glial tumors grade II and glial tumors grade III showed a statistical significance (p = 0.023). Conclusion. Our study showed that 1H-MRS and apparent diffusion coefficients can help in evaluation and differentiation of solid brain lesions
