Qualitative analysis of psychosocial impact of diagnosis of Chlamydia trachomatis: implications for screening
OBJECTIVES: To investigate the psychosocial impact for women of a diagnosis of Chlamydia trachomatis and discuss the implications for the proposed UK chlamydia screening programme. DESIGN: Qualitative study with semistructured interviews. Interview transcripts analysed to identify recurrent themes. PARTICIPANTS: Seventeen women with a current or recent diagnosis of chlamydia. SETTING: A family planning clinic and a genitourinary medicine clinic in Glasgow. RESULTS: Three themes were identified: perceptions of stigma associated with sexually transmitted infection, uncertainty about reproductive health after diagnosis, and anxieties regarding partner's reaction to diagnosis. Most women had not previously perceived sexually transmitted infections as personally relevant; this was a function of stereotypical beliefs about who was "at risk" of sexually transmitted infection. These beliefs were pervasive and negatively affected reactions to diagnosis and produced anxiety about disclosure of the condition to others (particularly sexual partners) and future reproductive morbidity. This anxiety, given the uncertain natural history of chlamydia, may prove difficult to dispel. CONCLUSIONS: There are three primary areas of concern for women after a diagnosis of chlamydia which need to be examined in the proposed screening programme. Information provided should normalise and destigmatise chlamydial infection and positively promote genitourinary medicine services. Support services should be available because notification of partner can cause anxiety. Uncertainty about future reproductive morbidity may be inevitable; staff providing screening will require guidance in providing advice under such conditions

Extending electron orbital precession to the molecular case: Can orbital alignment be used to observe wavepacket dynamics?
The complexity of ultrafast molecular photoionization presents an obstacle to
the modelling of pump-probe experiments. Here, a simple optimized model of
atomic rubidium is combined with a molecular dynamics model to predict
quantitatively the results of a pump-probe experiment in which long range
rubidium dimers are first excited, then ionized after a variable delay. The
method is illustrated by the outline of two proposed feasible experiments and
the calculation of their outcomes. Both of these proposals use Feshbach 87Rb2
molecules. We show that long-range molecular pump-probe experiments should
observe spin-orbit precession given a suitable pump-pulse, and that the
associated high-frequency beat signal in the ionization probability decays
after a few tens of picoseconds. If the molecule was to be excited to only a
single fine structure state state, then a low-frequency oscillation in the
internuclear separation would be detectable through the timedependent
ionization cross section, giving a mechanism that would enable observation of

Using shared goal setting to improve access and equity: a mixed methods study of the Good Goals intervention
Background: Access and equity in childrenās therapy services may be improved by directing cliniciansā use of resources toward specific goals that are important to patients. A practice-change intervention (titled āGood Goalsā) was designed to achieve this. This study investigated uptake, adoption, and possible effects of that intervention in childrenās occupational therapy services.
Methods: Mixed methods case studies (n = 3 services, including 46 therapists and 558 children) were conducted. The intervention was delivered over 25 weeks through face-to-face training, team workbooks, and ātools for changeā. Data were collected before, during, and after the intervention on a range of factors using interviews, a focus group, case note analysis, routine data, document analysis, and researchersā observations.
Results: Factors related to uptake and adoptions were: mode of intervention delivery, competing demands on therapistsā time, and leadership by service manager. Service managers and therapists reported that the intervention: helped therapists establish a shared rationale for clinical decisions; increased clarity in service provision; and improved interactions with families and schools. During the study period, therapistsā behaviours changed: identifying goals, odds ratio 2.4 (95% CI 1.5 to 3.8); agreeing goals, 3.5 (2.4 to 5.1); evaluating progress, 2.0 (1.1 to 3.5). Childrenās LoT decreased by two months [95% CI ā8 to +4 months] across the services. Cost per therapist trained ranged from Ā£1,003 to Ā£1,277, depending upon service size and therapistsā salary bands.
Conclusions: Good Goals is a promising quality improvement intervention that can be delivered and adopted in practice and may have benefits. Further research is required to evaluate its: (i) impact on patient outcomes, effectiveness, cost-effectiveness, and (ii) transferability to other clinical contexts
