    Systematic review of transition models for young people with long-term conditions: A report for NHS Diabetes.

    Aims For many young people with Type 1 diabetes, transition from paediatric to adult care can result in a marked deterioration in glycaemic control. A systematic review assessed the effectiveness of transition models, or components of models, for managing the transition process in young people with long-term conditions, including Type 1 diabetes. This involved identifying (i) the main barriers and facilitators in implementing a successful transition programme, and (ii) the key issues for young people with long-term conditions and professionals involved in the transition process. Methods The following databases were searched from inception to August 2012: MEDLINE, EMBASE, PsychINFO, CINAHL, ASSIA, Social Services Abstracts, Academic Search Complete, Social Science Citation Index, Cochrane and Campbell Libraries. Selected studies included young people aged 11 to 25 diagnosed with long-term conditions who were in transition from paediatric to adult secondary health care services. Results 16 systematic reviews and 13 primary studies were included from 9992 records retrieved. No single transition model was uniquely effective. The most successful transitions centred around: young person-focused; age and developmentally appropriate content and delivery; self-management education; family participation; paediatric and adult collaboration; designated transition clinics; transition co-ordinator; young person’s portfolio; specific professionals training; multidisciplinary approach; structured process embedded in service delivery. There were no distinctive characteristics of condition-specific Type 1 diabetes services. Conclusion This important and timely review summarises the key factors that need to be considered for the development of transition programmes for young people with long-term conditions, including those with Type 1 diabetes

    Measurement of the SOC State Specific Heat in ^4He

    When a heat flux Q is applied downward through a sample of liquid 4He near the lambda transition, the helium self organizes such that the gradient in temperature matches the gravity induced gradient in Tlambda. All the helium in the sample is then at the same reduced temperature tSOC = ((T[sub SOC] - T[sub lambda])/T[sub lambda]) and the helium is said to be in the Self-Organized Critical (SOC) state. We have made preliminary measurements of the 4He SOC state specific heat, C[del]T(T(Q)). Despite having a cell height of 2.54 cm, our results show no difference between C[del]T and the zero-gravity 4He specific heat results of the Lambda Point Experiment (LPE) [J.A. Lipa et al., Phys. Rev. B, 68, 174518 (2003)] over the range 250 to 450 nK below the transition. There is no gravity rounding because the entire sample is at the same reduced temperature tSOC(Q). Closer to Tlambda the SOC specific heat falls slightly below LPE, reaching a maximum at approximately 50 nK below Tlambda, in agreement with theoretical predictions [R. Haussmann, Phys. Rev. B, 60, 12349 (1999)]

    Benchmark ultra-cool dwarfs in widely separated binary systems

    Ultra-cool dwarfs as wide companions to subgiants, giants, white dwarfs and main sequence stars can be very good benchmark objects, for which we can infer physical properties with minimal reference to theoretical models, through association with the primary stars. We have searched for benchmark ultra-cool dwarfs in widely separated binary systems using SDSS, UKIDSS, and 2MASS. We then estimate spectral types using SDSS spectroscopy and multi-band colors, place constraints on distance, and perform proper motions calculations for all candidates which have sufficient epoch baseline coverage. Analysis of the proper motion and distance constraints show that eight of our ultra-cool dwarfs are members of widely separated binary systems. Another L3.5 dwarf, SDSS 0832, is shown to be a companion to the bright K3 giant Eta Cancri. Such primaries can provide age and metallicity constraints for any companion objects, yielding excellent benchmark objects.     An account is given of the 6- to 12-month survival, and causes of failure in 110 consecutive patients who underwent primary liver transplantation under treatment from the outset with FK 506 and steroids. The patient survival is 92.7%, and the first graft survival is 87.3%. At a very high frequency, the patients achieved good graft function, and they had a relatively low morbidity that was partially ascribable to minimal use and early discontinuance (in 60% of cases) of steroids. Renal dysfunction and other adverse findings were largely confined to patients with poor initial graft function and consequent apparent alteration of the kinetics of FK 506 elimination, causing functional overdosage. Results compare very favorably with our past record using conventional immunosuppression, and support the belief that FK 506 is a superior immunosuppressive agent which is suitable for chronic administration
