Determinants of gain modulation enabled by short-term depression at an inhibitory cerebellar synapse

Heat transfer in rotating serpentine passages with trips skewed to the flow
Experiments were conducted to determine the effects of buoyancy and Coriolis forces on heat transfer in turbine blade internal coolant passages. The experiments were conducted with a large scale, multi-pass, heat transfer model with both radially inward and outward flow. Trip strips, skewed at 45 deg to the flow direction, were machined on the leading and trailing surfaces of the radial coolant passages. An analysis of the governing flow equations showed that four parameters influence the heat transfer in rotating passages: coolant-to-wall temperature, rotation number, Reynolds number, and radius-to-passage hydraulic diameter ratio. The first three of these four parameters were varied over ranges which are typical of advanced gas turbine engine operating conditions. Results were correlated and compared to previous results from similar stationary and rotating models with smooth walls and with trip strips normal to the flow direction. The heat transfer coefficients on surfaces, where the heat transfer decreased with rotation and buoyancy, decreased to as low as 40 percent of the value without rotation. However, the maximum values of the heat transfer coefficients with high rotation were only slightly above the highest levels previously obtained with the smooth wall models. It was concluded that (1) both Coriolis and buoyancy effects must be considered in turbine blade cooling designs with trip strips, (2) the effects of rotation are markedly different depending upon the flow direction, and (3) the heat transfer with skewed trip strips is less sensitive to buoyancy than the heat transfer in models with either smooth or normal trips. Therefore, skewed trip strips rather than normal trip strips are recommended and geometry-specific tests are required for accurate design information
Heat transfer in rotating serpentine passages with selected model orientation for smooth or skewed trip walls
Experiments were conducted to determine the effects of model orientation as well as buoyancy and Coriolis forces on heat transfer in turbine blade internal coolant passages. Turbine blades have internal coolant passage surfaces at the leading and trailing edges of the airfoil with surfaces at angles which are as large as +/- 50 to 60 degrees to the axis of rotation. Most of the previously-presented, multiple-passage, rotating heat transfer experiments have focused on radial passages aligned with the axis of rotation. Results from serpentine passages with orientations 0 and 45 degrees to the axis of rotation which simulate the coolant passages for the mid chord and trailing edge regions of the rotating airfoil are compared. The experiments were conducted with rotation in both directions to simulate serpentine coolant passages with the rearward flow of coolant or with the forward flow of coolant. The experiments were conducted for passages with smooth surfaces and with 45 degree trips adjacent to airfoil surfaces for the radial portion of the serpentine passages. At a typical flow condition, the heat transfer on the leading surfaces for flow outward in the first passage with smooth walls was twice as much for the model at 45 degrees compared to the model at 0 degrees. However, the differences for the other passages and with trips were less. In addition, the effects of buoyancy and Coriolis forces on heat transfer in the rotating passage were decreased with the model at 45 degrees, compared to the results at 0 degrees. The heat transfer in the turn regions and immediately downstream of the turns in the second passage with flow inward and in the third passage with flow outward was also a function of model orientation with differences as large as 40 to 50 percent occurring between the model orientations with forward flow and rearward flow of coolant

An experimental program was conducted to investigate heat transfer and pressure loss characteristics of rotating multipass passages, for configurations and dimensions typical of modern turbine blades. The immediate objective was the generation of a data base of heat transfer and pressure loss data required to develop heat transfer correlations and to assess computational fluid dynamic techniques for rotating coolant passages. GLP-1 Agonists in Type 1 Diabetes Mellitus: A Review of the Literature
Objective: To review the use of GLP-1 agonists in patients with type 1 diabetes mellitus (T1DM). Data Sources: A search using the MEDLINE database, EMBASE, and Cochrane Database was performed through March 2016 using the search terms glucagon-like peptide 1 (GLP-1) agonists, incretin, liraglutide, exenatide, albiglutide, dulaglutide, type 1 diabetes mellitus. Study Selection and Data Extraction: All English-language trials that examined glycemic end points using GLP- 1 agonists in humans with T1DM were included. Data Synthesis: A total of 9 clinical trials examining the use of GLP-1 agonists in T1DM were identified. On average, hemoglobin A1C (A1C) was lower than baseline, with a maximal lowering of 0.6%. This effect was not significant when tested against a control group, with a relative decrease in A1C of 0.1% to 0.2%. In all trials examined, reported hypoglycemia was low, demonstrating no difference when compared with insulin monotherapy. Weight loss was seen in all trials, with a maximum weight loss of 6.4 kg over 24 weeks. Gastrointestinal adverse effects are potentially limiting, with a significant number of patients in trials reporting nausea. Conclusion: The use of GLP-1 agonists should be considered in T1DM patients who are overweight or obese and not at glycemic goals despite aggressive insulin therapy; however, tolerability of these agents is a potential concern. Liraglutide has the strongest evidence for use and would be the agent of choice for use in overweight or obese adult patients with uncontrolled T1DM
Strategies for the Management of Postoperative Anemia in Elective Orthopedic Surgery
Objective: To assess the use of oral iron, intravenous (IV) iron, and erythropoiesis-stimulating agents (ESAs) for the prevention and management of perioperative anemia in elective orthopedic surgery patients, and to provide a clinical algorithm for use. Data Sources: A PubMed and MEDLINE search was conducted from 1964 through March 2016 using the following search terms alone or in combination: orthopedic, surgery, elective, anemia, blood transfusion, iron, erythropoiesisstimulating agents, and erythropoietin. Study Selection and Data Extraction: All English-language prospective and retrospective human studies and meta-analyses evaluating oral iron, IV iron, or ESA alone or in combination in elective orthopedic surgery patients were evaluated, provided they reported blood transfusion outcomes. Data Synthesis: A total of 9 prospective and retrospective studies and 1 meta-analysis were identified and included. In the preoperative setting, administration of oral iron, IV iron, or ESA alone or in combination to correct underlying anemia led to significantly reduced transfusion rates. Transfusion requirements were generally less with combination therapy (ESA + oral or IV iron). In the short-term perioperative or postoperative period, use of oral or IV iron led to conflicting results, with some reporting a statistically significant reduction in blood transfusions, whereas others reported none. Conclusions: In elective orthopedic surgery, IV or oral iron with or without an ESA may provide benefit in prevention of postoperative anemia and results in blood transfusion reduction without significantly increasing the risk of adverse events. These agents should be considered at the lowest effective dose with emphasis on administration prior to planned surgery
The Impact of an International Interprofessional Experience on Perceptions of Pharmacist-Physician Relationships
Objective. To assess the impact of this international interprofessional learning experience on perceptions of pharmacist-physician relationships and interprofessional teams.
Methods. Medical and pharmacy students completed a one-week interprofessional medical mission experience in the Dominican Republic. Anonymous surveys were administered to 17 students before and after completion to measure perceptions of pharmacist-physician relationships and interprofessional teams. Responses were matched and changes in perceptions were analyzed using the Wilcoxon Signed Rank test. The SPICE-R2 instrument was administered after the experience to measure attitudes toward interprofessional teams.
Results. Of the 17 participants, 100% responded to all surveys. Significant improvements were seen in the perception of pharmacists as an integral part of medical mission trips (P=0.035) and confidence in the ability to communicate with other healthcare disciplines (P=0.033). All students stated they would recommend this experience, and agreed that interprofessional experiences enhance their team work skills and should be incorporated into their education. Student comments supported that this was a meaningful and effective interprofessional experience. The results of the SPICE-R2 demonstrated positive attitudes about interprofessional teams, with all questions having a median score of “agree” or “strongly agree.”
Conclusions. An international interprofessional experience improved the perception of pharmacist-physician relationships. The experience provided understanding of the other healthcare discipline, an appreciation for the importance of interprofessional teamwork, increased student confidence in communicating with the other discipline, and cultivated interest in future interprofessional collaboration
Polyethylene Oxide Nanofiber Production by Electrospinning
Electrospinning is an inexpensive technique that is used to produce nanofibers for a variety of applications. In electrospinning, a polymer solution is dispensed from a hypodermic-like syringe where an intense electric field attracts the solution to a collector while drawing the polymer into a very thin fiber. The diameter of the fiber can be controlled by tuning the process parameters such as the applied electric field, solution flow rate, distance between syringe tip and collector, and the collector geometry. In this paper we describe results from electrospinning poly(ethylene oxide) (PEO), a likely candidate for applications involving scaffolding for tissue engineering. The PEO nanofibers were fabricated from different polymer solution concentrations ranging from 14% - 22% (by weight). Each sample was then imaged using a scanning electron microscope. The morphology of the fibers produced from varying solution concentrations is discussed
Hybrid Online Delivery of a Pharmacy Residency and Fellowship Elective Course
Objective: To describe and evaluate the transition of a pharmacy residency and fellowship (PRF) elective course to a hybrid online platform.
Innovation: In 2016, the 1-credit hour PRF elective was transitioned from a live, synchronous course to a hybrid online platform. Over the course of the semester, students completed eight modules along with assignments that pertained to a different component of PRF. Course grades and evaluations, as well as PRF placement rates, were compared between 2015 (live, synchronous course) and 2016 (hybrid online course). There were no differences in overall course grades or student evaluations of individual relevant course objectives between the two course formats. However, more students rated the course as excellent during the 2015 live, synchronous course. Placement rates were similar between students who took the course in 2015 and 2016.
Critical Analysis: Following the transition of a PRF elective to a hybrid online platform, course grades, evaluation of individual relevant course objectives, and PRF placement rates remained similar to previous years. Creative educational venues can help meet the student demand while simultaneously allowing faculty to manage their time. However, instructors should balance this with desire of students to have more face-to-face in class time.

Evaluation of the long-term efficacy and safety of an imidacloprid 10%/flumethrin 4.5% polymer matrix collar (Seresto®) in dogs and cats naturally infested with fleas and/or ticks in multicentre clinical field studies in Europe
<p>Abstract</p> <p>Background</p> <p>The objective of these two GCP multicentre European clinical field studies was to evaluate the long-term efficacy and safety of a new imidacloprid/flumethrin collar (Seresto<sup>®</sup>, Bayer AnimalHealth, Investigational Veterinary Product(IVP)) in dogs and cats naturally infested with fleas and/or ticks in comparison to a dimpylat collar ("Ungezieferband fuer Hunde/fuer Katzen", Beaphar, Control Product (CP)).</p> <p>Methods</p> <p>232 (IVP) and 81 (CP) cats and 271(IVP) and 129 (CP) dogs were treated with either product according to label claims and formed the safety population. Flea and tick counts were conducted in monthly intervals for up to 8 months in the efficacy subpopulation consisting of 118 (IVP) + 47 (CP) cats and 197 (IVP) + 94 (CP) dogs. Efficacy was calculated as reduction of infestation rate within the same treatment group and statistically compared between the two treatment groups.</p> <p>Results</p> <p>Preventive efficacy against fleas in cats/dogs varied in the IVP group between 97.4%/94.1% and 100%/100% (overall mean: 98.3%/96.7%) throughout the 8 month period and in the CP group between 57.1%/28.2% and 96.1%/67.8% (overall mean: 79.3%/57.9%). Preventive efficacy against ticks in cats/dogs varied in the IVP group between 94.0%/91.2% and 100%/100% (overall mean: 98.4%/94.7%) throughout the 8 month period and in the CP group between 90.7%/79.9% and 100%/88.0% (overall mean: 96.9%/85.6%). The IVP group was statistically non-inferior to the CP group, and on various assessment days, statistical superiority was proven for flea and tick count reduction in dogs and cats. Both treatments proved to be safe in dogs and cats with mainly minor local observations at the application site. There was moreover, no incidence of any mechanical problem with the collar in dogs and cats during the entire study period.</p> <p>Conclusions</p> <p>The imidacloprid/flumethrin collar proved to reduce tick counts by at least 90% and flea counts by at least 95% for a period of at least 7-8 months in cats and dogs under field conditions. Therefore, it can be used as sustainable long-term preventative, covering the whole flea and tick season.</p
