    Pressure-Equalized Rainscreen Wall System: A Full-Scale Experiment

    The pressure equalized rainscreen wall, considered as the most effective building envelope against wind induced rain penetration, requires continuous investigations to reach better performance. This research seeks the optimum pressure equalization process under external pressure conditions and wall parameters that have not previously been studied in detail. For this purpose, a single compartment full-scale wall model was built in a controlled facility at the University of Western Ontario. The cavity pressure response to external fluctuations was experimentally examined with respect to the rainscreen venting area ratio, under two types of real wind pressure distributions generated mechanically at zero degree incidence: 1) single pressure and, 2) pressure gradient caused by the application of three different signals varying horizontally across the rainscreen. As the rainscreen venting area ratio increases, the pressure equalization performance improves, irrespective of the nature of the applied pressure, implying an increase in the critical damping frequency. However, an applied pressure gradient leads to a lower degree of pressure equalization at a constant venting area. Moreover, the change of the vent openings layout has an impact on the wall performance, mainly at low venting areas. Locating the vent openings at the bottom of the rainscreen gives better pressure equalization rather than distributing them between top and bottom. Using a numerical model, the cavity pressure measurements were underestimated under a uniform pressure and overestimated when subject to a pressure gradient. The agreement in the frequency domain between experimental and predicted signals was satisfactory in the high frequency regions at high venting area ratios. However, transfer functions and phase angles were overpredicted at low venting rates. Based on numerical simulations, the cavity volume change does not significantly affect the performance of the model under an external pressure gradient. When a single pressure is applied, the pressure equalization is reduced at a larger cavity depth, which is only apparent at low venting areas

    New Constraints on the Energetics, Progenitor Mass, and Age of the Supernova Remnant G292.0+1.8 Containing PSR J1124-5916

    We present spatially resolved spectroscopy of the supernova remnant (SNR) G292.0+1.8 with the Chandra X-ray observatory. This SNR contains the 135 ms pulsar, J1124-5916. We apply non-equilibrium ionization (NEI) models to the data. By comparing the derived abundances with those predicted from nucleosynthesis models, we estimate a progenitor mass of 30-40 solar masses. We also derive the intrinsic parameters of the supernova explosion such as its energy, the age of the SNR, the blast wave velocity, and the swept-up mass. In the Sedov interpretation, our estimated SNR age of 2,600 years is close to the pulsar's characteristic age of 2,900 years. This confirms the pulsar/SNR association and relaxes the need for the pulsar to have a non-canonical value for the braking index, a large period at birth or a large transverse velocity. We discuss the properties of the pulsar wind nebula (PWN) in the light of the Kennel and Coroniti model and estimate the pulsar wind magnetization parameter. We also report the first evidence for steepening of the power law spectral index with increasing radius from the pulsar.Comment: 5 pages, 3 figures.     Identifying and addressing the support needs of family caregivers of people with motor neurone disease using the Carer Support Needs Assessment Tool

    Family caregivers of people with motor neurone disease (MND) experience adverse health outcomes as a result of their caregiving experience. This may be alleviated if their support needs are identified and addressed in a systematic and timely manner. The objective of the present study was to assess the feasibility and relevance of the Carer Support Needs Assessment Tool (CSNAT) in home-based care during the period of caregiving from the perspectives of the family caregivers of people with MND and their service providers. The study was conducted during 2014 in Western Australia. Some 30 family caregivers and 4 care advisors participated in trialling the CSNAT intervention, which involved two visits from care advisors (6–8 weeks apart) to identify and address support needs. The feedback from family caregivers was obtained via telephone interviews and that of care advisors via a self-administered questionnaire. A total of 24 caregivers completed the study (80% completion rate) and identified the highest support priorities as “knowing what to expect in the future,” “knowing who to contact if concerned,” and “equipment to help care.” The majority found that this assessment process adequately addressed their needs and gave them a sense of validation, reassurance, and empowerment. Care advisors advocated the CSNAT approach as an improvement over standard practice, allowing them to more clearly assess needs, to offer a more structured follow-up, and to focus on the caregiver and family. The CSNAT approach for identifying and addressing family caregivers' support needs was found to be relevant and feasible by MND family caregivers and care advisors. The tool provided a formal structure to facilitate discussions with family caregivers and thus enable needs to be addressed. Such discussions can also inform an evidence base for the ongoing development of services, ensuring that new and improved services are designed to meet the explicit needs of the family caregivers of people with a motor neurone disease

    An assets-based approach to bereavement care

    A survey of bereaved clients of four funeral providers in Australia confirms a public health model predicting that over half the group would demonstrate low risk of complicated grief, another third moderate risk, while a small minority would meet the criteria for prolonged grief disorder. The survey also shows differing patterns of need and sources of support for each of the three groups. While our findings support targeting the bereavement care provided by health services, our primary interest is in the care received by most bereaved people. Some is provided in the community through the everyday activities of healthcare professionals. Most comes from a range of people already involved in the everyday lives of those recently bereaved. We contend that the most effective way to provide bereavement care is to support these ‘everyday assets’, ensuring that their care is recognised, appreciated, and not disrupted by over-reach from professional services

    Changes in oceanic radiocarbon and CFCs since the 1990s

    Anthropogenic perturbations from fossil fuel burning, nuclear bomb testing, and chlorofluorocarbon (CFC) use have created useful transient tracers of ocean circulation. The atmospheric 14C/C ratio (∆14C) peaked in the early 1960s and has decreased now to pre-industrial levels, while atmospheric CFC-11 and CFC-12 concentrations peaked in the early 1990s and early 2000s, respectively, and have now decreased by 10%–20%. We present the first analysis of a decade of new observations (2007 to 2018–2019) and give a comprehensive overview of the changes in ocean ∆14C and CFC concentration since the WOCE surveys in the 1990s. Surface ocean ∆14C decreased at a nearly constant rate from the 1990–2010s (20‰/decade). In most of the surface ocean ∆14C is higher than in atmospheric CO2 while in the interior ocean, only a few places are found to have increases in ∆14C, indicating that globally, oceanic bomb 14C uptake has stopped and reversed. Decreases in surface ocean CFC-11 started between the 1990 and 2000s, and CFC-12 between the 2000–2010s. Strong coherence in model biases of decadal changes in all tracers in the Southern Ocean suggest ventilation of Antarctic Intermediate Water was enhanced from the 1990 to the 2000s, whereas ventilation of Subantarctic Mode Water was enhanced from the 2000 to the 2010s. The decrease in surface tracers globally between the 2000 and 2010s is consistently stronger in observations than in models, indicating a reduction in vertical transport and mixing due to stratification

    Factors contributing to delayed diagnosis of cancer among Aboriginal people in Australia: a qualitative study.

    BACKGROUND/OBJECTIVES: Delayed presentation of symptomatic cancer is associated with poorer survival. Aboriginal patients with cancer have higher rates of distant metastases at diagnosis compared with non-Aboriginal Australians. This paper examined factors contributing to delayed diagnosis of cancer among Aboriginal Australians from patient and service providers' perspectives. METHODS: In-depth, open-ended interviews were conducted in two stages (2006-2007 and 2011). Inductive thematic analysis was assisted by use of NVivo looking around delays in presentation, diagnosis and referral for cancer. PARTICIPANTS: Aboriginal patients with cancer/family members (n=30) and health service providers (n=62) were recruited from metropolitan Perth and six rural/remote regions of Western Australia. RESULTS: Three broad themes of factors were identified: (1) Contextual factors such as intergenerational impact of colonisation and racism and socioeconomic deprivation have negatively impacted on Aboriginal Australians' trust of the healthcare professionals; (2) health service-related factors included low accessibility to health services, long waiting periods, inadequate numbers of Aboriginal professionals and high staff turnover; (3) patient appraisal of symptoms and decision-making, fear of cancer and denial of symptoms were key reasons patients procrastinated in seeking help. Elements of shame, embarrassment, shyness of seeing the doctor, psychological 'fear of the whole health system', attachment to the land and 'fear of leaving home' for cancer treatment in metropolitan cities were other deterrents for Aboriginal people. Manifestation of masculinity and the belief that 'health is women's domain' emerged as a reason why Aboriginal men were reluctant to receive health checks. CONCLUSIONS: Solutions to improved Aboriginal cancer outcomes include focusing on the primary care sector encouraging general practitioners to be proactive to suspicion of symptoms with appropriate investigations to facilitate earlier diagnosis and the need to improve Aboriginal health literacy regarding cancer. Access to health services remains a critical problem affecting timely diagnosis

    Expansion of the nodal-adjoint method for simple and efficient computation of the 2d tomographic imaging jacobian matrix

    This paper focuses on the construction of the Jacobian matrix required in tomographic reconstruction algorithms. In microwave tomography, computing the forward solutions during the iterative reconstruction process impacts the accuracy and computational efficiency. Towards this end, we have applied the discrete dipole approximation for the forward solutions with significant time savings. However, while we have discovered that the imaging problem configuration can dramatically impact the computation time required for the forward solver, it can be equally beneficial in constructing the Jacobian matrix calculated in iterative image reconstruction algorithms. Key to this implementation, we propose to use the same simulation grid for both the forward and imaging domain discretizations for the discrete dipole approximation solutions and report in detail the theoretical aspects for this localization. In this way, the computational cost of the nodal adjoint method decreases by several orders of magnitude. Our investigations show that this expansion is a significant enhancement compared to previous implementations and results in a rapid calculation of the Jacobian matrix with a high level of accuracy. The discrete dipole approximation and the newly efficient Jacobian matrices are effectively implemented to produce quantitative images of the simplified breast phantom from the microwave imaging system

    Staying just one step ahead: providing care for patients with motor neurone disease

    Introduction: There is limited information about the experiences and educational needs of health professionals who may be required to provide care for people with Motor Neurone Disease(MND) especially in the later stages of the disease. The aim of this study was to determine the experiences of, and need for, education of these health professionals. Methods: Interviews and focus groups were conducted with 31 health professionals with some experience in providing palliative care for people with MND. Thematic content analysis was used to identify common themes. Results: A key theme, Just One Step Ahead, emerged, that describes the central capability health professionals identified as necessary to help individuals plan and prepare for disease and lifestyle changes just before they arise. Two subthemes also emerged: Expertise in MND and Bespoke Communication. Expertise in MND described the required understanding of the disease and the particular individual’s version of the disease to allow the health professional to plan, advise, support and anticipate the needs of the person living with MND. Bespoke Communication was the facility to tailor care messages sensitively and effectively to the range of people involved in care (patients, family, healthcare team members).Conclusions: Care of people with MND requires up-to-date expertise about the disease and skilled communication abilities to manage complexity and change. Timely and focused education and specialist MND support for care providers are essential to tailored and responsive care and a widely available education programme has been developed to address these needs
