    The New Quadrivalent Adjuvanted Influenza Vaccine for the Italian Elderly: A Health Technology Assessment

    Background. The elderly, commonly defined as subjects aged ≥65 years, are among the at-risk subjects recommended for annual influenza vaccination in European countries. Currently, two new vaccines are available for this population: the MF59-adjuvanted quadrivalent influenza vaccine (aQIV) and the high-dose quadrivalent influenza vaccine (hdQIV). Their multidimensional assessment might maximize the results in terms of achievable health benefits. Therefore, we carried out a Health Technology Assessment (HTA) of the aQIV by adopting a multidisciplinary policy-oriented approach to evaluate clinical, economic, organizational, and ethical implications for the Italian elderly. Methods. A HTA was conducted in 2020 to analyze influenza burden; characteristics, efficacy, and safety of aQIV and other available vaccines for the elderly; cost-effectiveness of aQIV; and related organizational and ethical implications. Comprehensive literature reviews/analyses were performed, and a transmission model was developed in order to address the above issues. Results. In Italy, the influenza burden on the elderly is high and from 77.7% to 96.1% of influenza-related deaths occur in the elderly. All available vaccines are effective and safe; however, aQIV, such as the adjuvanted trivalent influenza vaccine (aTIV), has proved more immunogenic and effective in the elderly. From the third payer’s perspective, but also from the societal one, the use of aQIV in comparison with egg-based standard QIV (eQIV) in the elderly population is cost-effective. The appropriateness of the use of available vaccines as well as citizens’ knowledge and attitudes remain a challenge for a successful vaccination campaign. Conclusions. The results of this project provide decision-makers with important evidence on the aQIV and support with scientific evidence on the appropriate use of vaccines in the elderly

    Angiogenesis extent and macrophage density increase simultaneously with pathological progression in B-cell non-Hodgkin's lymphomas

    Node biopsies of 30 benign lymphadenopathies and 71 B-cell non-Hodgkin's lymphomas (B-NHLs) were investigated for microvessel and macrophage counts using immunohistochemistry and morphometric analysis. Both counts were significantly higher in B-NHL. Moreover, when these were grouped into low-grade and high-grade lymphomas, according to the Kiel classification and Working Formulation (WF), statistically significant higher counts were found in the high-grade tumours. Immunohistochemistry and electron microscopy revealed a close spatial association between microvessels and macrophages. Overall, the results suggest that, in analogy to what has already been shown in solid tumours, angiogenesis occurring in B-NHLs increases with tumour progression, and that macrophages promote the induction of angiogenesis via the release of their angiogenic factors. © 1999 Cancer Research Campaig

    The economic impact of moderate stage Alzheimer's disease in Italy: Evidence from the UP-TECH randomized trial

    Background: There is consensus that dementia is the most burdensome disease for modern societies. Few cost-of-illness studies examined the complexity of Alzheimer's disease (AD) burden, considering at the same time health and social care, cash allowances, informal care, and out-of-pocket expenditure by families. Methods: This is a comprehensive cost-of-illness study based on the baseline data from a randomized controlled trial (UP-TECH) enrolling 438 patients with moderate AD and their primary caregiver living in the community. Results: The societal burden of AD, composed of public, patient, and informal care costs, was about �20,000/yr. Out of this, the cost borne by the public sector was �4,534/yr. The main driver of public cost was the national cash-for-care allowance (�2,324/yr), followed by drug prescriptions (�1,402/yr). Out-of-pocket expenditure predominantly concerned the cost of private care workers. The value of informal care peaked at �13,590/yr. Socioeconomic factors do not influence AD public cost, but do affect the level of out-of-pocket expenditure. Conclusion: The burden of AD reflects the structure of Italian welfare. The families predominantly manage AD patients. The public expenditure is mostly for drugs and cash-for-care benefits. From a State perspective in the short term, the advantage of these care arrangements is clear, compared to the cost of residential care. However, if caregivers are not adequately supported, savings may be soon offset by higher risk of caregiver morbidity and mortality produced by high burden and stress.     Socioeconomic Predictors of the Employment of Migrant Care Workers by Italian Families Assisting Older Alzheimer's Disease Patients: Evidence from the Up-Tech Study

    Background: The availability of family caregivers of older people is decreasing in Italy as the number of migrant care workers (MCWs) hired by families increases. There is little evidence on the influence of socioeconomic factors in the employment of MCWs. Method: We analyzed baseline data from 438 older people with moderate Alzheimer's disease (AD), and their family caregivers enrolled in the Up-Tech trial. We used bivariate analysis and multilevel regressions to investigate the association between independent variables - education, social class, and the availability of a care allowance - and three outcomes - employment of a MCW, hours of care provided by the primary family caregiver, and by the family network (primary and other family caregivers). Results: The availability of a care allowance and the educational level were independently associated with employing MCWs. A significant interaction between education and care allowance was found, suggesting that more educated families are more likely to spend the care allowance to hire a MCW. Discussion: Socioeconomic inequalities negatively influenced access both to private care and to care allowance, leading disadvantaged families to directly provide more assistance to AD patients. Care allowance entitlement needs to be reformed in Italy and in countries with similar long-term care and migration systems. � 2015 The Author 2015.     A new tool for modelling the decompression behaviour of CO 2 with impurities using the Peng-Robinson equation of state

    Carbon capture and storage is considered as promising technique by the scientific community to reduce CO2 emissions and is composed by three different stages: CO2 capture, transportation and storage. The capturing plant has to be installed where processes involving large CO2 emissions, as power generation from fossil fuel, concrete and iron plants are present. The captured CO2 in real applications will be likely a CO2 dominated mixture with a content of several impurities, depending on the capture process which presence affects the thermo-physical properties of the pure CO2, especially increasing the vapour pressure curve. Such behaviour plays an important role in the control of the ductile fracture, whenever the CO2 is transported using pipelines. In the unfortunate event of a pipeline failure, the study of the pipeline decompression can be useful to estimate whether the fracture will remain confined or will propagate. The residual pressure acting on the crack tip of the pipeline represents the energy source governing the fracture's propagation. The pressure profile is given by the expansion wave, which propagates along the pipeline during the fracture. In the literature, it is common practice to model the expansion wave by assuming one-dimensional isentropic decompression. In this study, a new code developed by the authors for modelling the decompression behaviour in the fluid is presented. The code is based on the Peng-Robinson equation of state with the Peneloux correction correlation matching the density of pure components to experimental data. The sound velocity in the two-phase region has been modelled with the method proposed by Nichita et al., which increase the accuracy of prediction with respect to the most popular Wood's method. The code was validated against experimental and numerical data relating to hydrocarbon gas mixtures and CO2-rich mixtures. In particular, the results obtained for the hydrocarbon gas mixture decompression have been compared with experimental data and the predictions calculated by the software GASDECOM, in order to assess the accuracy of the code, that was then utilized to calculate the expansion wave curves for CO2-rich mixtures. The results obtained have been compared with experimental data and with predictions generated by the GERG2008 equation of state, found in literature. The simulated expansion wave curves show a good agreement with the experimental data for the tested compositions, especially for the plateau characterising the two-phase transition of rich gas compositions. The plateau corresponds to the saturation pressure, which is a key parameter for the fracture propagation control in CO2 pipelines, since the material toughness required to prevent a propagating ductile fracture is determined by applying the condition that the fracture propagation curve does not cross this value
