    Reducing parental anxiety using a family based intervention for youth mental health : a randomized controlled trial

    This paper presents findings on parent anxiety and attachment relationship style from the Deakin Family Options (DFO) pilot study, a randomized controlled pilot study comparing a family-based treatment (BEST Plus), versus a youth only treatment (CBT) versus a group who received both of these treatments (COMBINED). Eligible participants were families with a young person (aged 12 - 25 years) with a high prevalence mental health problem. Youth from participating families scored in the clinical or subclinical range for depression, anxiety and/or substance misuse symptoms on standardized measures during the initial assessment. The collected sample was drawn from regional and urban centers in Victoria, Australia and allocated to treatment condition using a simple randomization procedure (parallel design). It was hypothesized that families receiving the BEST Plus would experience greater reductions in youth and parent mental health symptoms, and improved parent-child relationships, compared with those in the CBT condition. This paper describes and discusses changes in parent anxiety and parent attachment, according to whether the parent participated in a treatment (BEST Plus) or did not (NONBEST Plus). Participants were blind to the study hypotheses. In total 71 parent participants returned pre data and were allocated to a treatment group. In this paper, data from parent participants who completed pre and post measures (n = 48) and pre, post, and 6-month follow-up measures (n = 28) on anxiety and attachment were analyzed by group (BEST Plus versus NONBEST Plus). The results of this study suggest that parent anxiety decreased significantly more following parent involvement in a group treatment, than for parents that did not receive treatment. Unexpectedly, avoidant attachment increased in the no treatment group, but remained relatively stable following the BEST Plus group. There were no significant findings in relation to compulsive traits and anxious attachment. These findings are discussed in light of the study limitations.<br /

    Periodic Orbits and Spectral Statistics of Pseudointegrable Billiards

    We demonstrate for a generic pseudointegrable billiard that the number of periodic orbit families with length less than ll increases as πb0l2/a(l)\pi b_0l^2/\langle a(l) \rangle, where b0b_0 is a constant and a(l)\langle a(l) \rangle is the average area occupied by these families. We also find that a(l)\langle a(l) \rangle increases with ll before saturating. Finally, we show that periodic orbits provide a good estimate of spectral correlations in the corresponding quantum spectrum and thus conclude that diffraction effects are not as significant in such studies.Comment: 13 pages in RevTex including 5 figure

    Regular Tunnelling Sequences in Mixed Systems

    We show that the pattern of tunnelling rates can display a vivid and regular pattern when the classical dynamics is of mixed chaotic/regular type. We consider the situation in which the dominant tunnelling route connects to a stable periodic orbit and this orbit is surrounded by a regular island which supports a number of quantum states.     We review some properties of periodic orbit families in polygonal billiards and discuss in particular a sum rule that they obey. In addition, we provide algorithms to determine periodic orbit families and present numerical results that shed new light on the proliferation law and its variation with the genus of the invariant surface.     Semiclassical Quantisation Using Diffractive Orbits

    Diffraction, in the context of semiclassical mechanics, describes the manner in which quantum mechanics smooths over discontinuities in the classical mechanics. An important example is a billiard with sharp corners; its semiclassical quantisation requires the inclusion of diffractive periodic orbits in addition to classical periodic orbits. In this paper we construct the corresponding zeta function and apply it to a scattering problem which has only diffractive periodic orbits.     Semiclassical Inequivalence of Polygonalized Billiards

    Polygonalization of any smooth billiard boundary can be carried out in several ways. We show here that the semiclassical description depends on the polygonalization process and the results can be inequivalent. We also establish that generalized tangent-polygons are closest to the corresponding smooth billiard and for de Broglie wavelengths larger than the average length of the edges, the two are semiclassically equivalent.Comment: revtex, 4 ps figure

    Uniform approximations for pitchfork bifurcation sequences

    In non-integrable Hamiltonian systems with mixed phase space and discrete symmetries, sequences of pitchfork bifurcations of periodic orbits pave the way from integrability to chaos. In extending the semiclassical trace formula for the spectral density, we develop a uniform approximation for the combined contribution of pitchfork bifurcation pairs. For a two-dimensional double-well potential and the familiar H\'enon-Heiles potential, we obtain very good agreement with exact quantum-mechanical calculations. We also consider the integrable limit of the scenario which corresponds to the bifurcation of a torus from an isolated periodic orbit. For the separable version of the H\'enon-Heiles system we give an analytical uniform trace formula, which also yields the correct harmonic-oscillator SU(2) limit at low energies, and obtain excellent agreement with the slightly coarse-grained quantum-mechanical density of states.Comment: LaTeX, 31 pp., 18 figs.     Level statistics and eigenfunctions of pseudointegrable systems: dependence on energy and genus number

    We study the level statistics (second half moment I0I_0 and rigidity Δ3\Delta_3) and the eigenfunctions of pseudointegrable systems with rough boundaries of different genus numbers gg. We find that the levels form energy intervals with a characteristic behavior of the level statistics and the eigenfunctions in each interval. At low enough energies, the boundary roughness is not resolved and accordingly, the eigenfunctions are quite regular functions and the level statistics shows Poisson-like behavior. At higher energies, the level statistics of most systems moves from Poisson-like towards Wigner-like behavior with increasing gg. Investigating the wavefunctions, we find many chaotic functions that can be described as a random superposition of regular wavefunctions. The amplitude distribution P(ψ)P(\psi) of these chaotic functions was found to be Gaussian with the typical value of the localization volume Vloc0.33V_{\rm{loc}}\approx 0.33. For systems with periodic boundaries we find several additional energy regimes, where I0I_0 is relatively close to the Poisson-limit. In these regimes, the eigenfunctions are either regular or localized functions, where P(ψ)P(\psi) is close to the distribution of a sine or cosine function in the first case and strongly peaked in the second case. Also an interesting intermediate case between chaotic and localized eigenfunctions appears

    Level spacing distribution of pseudointegrable billiard

    In this paper, we examine the level spacing distribution P(S)P(S) of the rectangular billiard with a single point-like scatterer, which is known as pseudointegrable. It is shown that the observed P(S)P(S) is a new type, which is quite different from the previous conclusion. Even in the strong coupling limit, the Poisson-like behavior rather than Wigner-like is seen for S>1S>1, although the level repulsion still remains in the small SS region.     Prevalence and Predictors of Urinary Tract Infection and Severe Malaria Among Febrile Children Attending Makongoro Health Centre in Mwanza City, North-Western Tanzania.

    In malaria endemic areas, fever has been used as an entry point for presumptive treatment of malaria. At present, the decrease in malaria transmission in Africa implies an increase in febrile illnesses related to other causes among underfives. Moreover, it is estimated that more than half of the children presenting with fever to public clinics in Africa do not have a malaria infection. Thus, for a better management of all febrile illnesses among under-fives, it becomes relevant to understand the underlying aetiology of the illness. The present study was conducted to determine the relative prevalence and predictors of P. falciparum malaria, urinary tract infections and bacteremia among under-fives presenting with a febrile illness at the Makongoro Primary Health Centre, North-Western Tanzania. From February to June 2011, a cross-sectional analytical survey was conducted among febrile children less than five years of age. Demographic and clinical data were collected using a standardized pre-tested questionnaire. Blood and urine culture was done, followed by the identification of isolates using in-house biochemical methods. Susceptibility patterns to commonly used antibiotics were investigated using the disc diffusion method. Giemsa stained thin and thick blood smears were examined for any malaria parasites stages. A total of 231 febrile under-fives were enrolled in the study. Of all the children, 20.3% (47/231, 95%CI, 15.10-25.48), 9.5% (22/231, 95%CI, 5.72-13.28) and 7.4% (17/231, 95%CI, 4.00-10.8) had urinary tract infections, P. falciparum malaria and bacteremia respectively. In general, 11.5% (10/87, 95%CI, 8.10-14.90) of the children had two infections and only one child had all three infections. Predictors of urinary tract infections (UTI) were dysuria (OR = 12.51, 95% CI, 4.28-36.57, P < 0.001) and body temperature (40-41 C) (OR = 12.54, 95% CI, 4.28-36.73, P < 0.001). Predictors of P. falciparum severe malaria were pallor (OR = 4.66 95%CI, 1.21-17.8, P = 0.025) and convulsion (OR = 102, 95% CI, 10-996, P = 0.001). Escherichia coli were the common gram negative isolates from urine (72.3%, 95% CI, 66.50-78.10) and blood (40%, 95%CI, and 33.70-46.30). Escherichia coli from urine were 100% resistant to ampicillin, 97% resistant to co-trimoxazole, 85% resistant to augmentin and 32.4% resistant to gentamicin; and they were 100%, 91.2% and 73.5% sensitive to meropenem, ciprofloxacin and ceftriaxone respectively. Urinary tract infection caused by multi drug resistant Escherichia coli was the common cause of febrile illness in our setting. Improvement of malaria diagnosis and its differential diagnosis from other causes of febrile illnesses may provide effective management of febrile illnesses among children in Tanzania
