Continuous intestinal infusion of levodopa-carbidopa in patients with advanced Parkinson's disease in Spain: Subanalysis by autonomous community
Objetivos: Comparar las características de los pacientes con enfermedad de Parkinson avan-
zada en tratamiento con infusión intestinal continua de levodopa-carbidopa (IICLC) y los datos
de efectividad y seguridad de IICLC entre diferentes comunidades autónomas (CC. AA.).
Métodos: Estudio longitudinal observacional y retrospectivo. Se incluyeron 177 pacientes de
11 CC. AA. que iniciaron tratamiento con IICLC entre enero de 2006 y diciembre de 2011. Se
compararon las características clínicas y demográficas, las variables de efectividad (cambios en
el tiempo OFF, ON con y sin discinesias discapacitantes, cambios en la escala de Hoehn y Yahr
y puntuación de la Unified Parkinson’s Disease Rating Scale, síntomas no motores e Impresión
Clínica Global) y seguridad (acontecimientos adversos), y la tasa de suspensión de IICLC.
Resultados: Se hallaron diferencias significativas entre las CC. AA. en diversas variables basa-
les: duración de la enfermedad hasta el inicio de IICLC, tiempo OFF (34,9-59,7%) y ON (con o
sin discinesias; 2,6-48,0%), Hoehn y Yahr en ON, Unified Parkinson’s Disease Rating Scale-III en
ON y OFF, presencia de ≥ 4 síntomas motores y dosis de IICLC. En el seguimiento (> 24 meses en
9 de 11 CC. AA.) hubo diferencias significativas en el porcentaje de tiempo OFF, tiempo ON sin
discinesias discapacitantes, frecuencia de acontecimientos adversos e Impresión Clínica Global.
La tasa de suspensión fue de entre 20-40% en todas las CC. AA., excepto en 2 (78 y 80%).
Conclusiones: Este estudio muestra una amplia variabilidad en la selección de los pacientes y
en la efectividad y seguridad de IICLC entre las diferentes CC. AA. Podrían influir las caracterís-
ticas basales de los pacientes, la disponibilidad de un equipo multidisciplinar y la experiencia
clínica.Objectives: To compare the characteristics of patients undergoing treatment with continuous
intestinal infusion of levodopa-carbidopa (CIILC) for advanced Parkinson’s disease and the data
on the effectiveness and safety of CIILC in the different autonomous communities (AC) of Spain.
Methods: A retrospective, longitudinal, observational study was carried out into 177 patients
from 11 CAs who underwent CIILC between January 2006 and December 2011. We analysed data
on patients’ clinical and demographic characteristics, variables related to effectiveness (chan-
ges in off time/on time with or without disabling dyskinesia; changes in Hoehn and Yahr scale
and Unified Parkinson’s Disease Rating Scale scores; non-motor symptoms; and Clinical Global
Impression scale scores) and safety (adverse events), and the rate of CIILC discontinuation.
Results: Significant differences were observed between CAs for several baseline variables:
duration of disease progression prior to CIILC onset, off time (34.9-59.7%) and on time (2.6-
48.0%; with or without disabling dyskinesia), Hoehn and Yahr score during on time, Unified
Parkinson’s Disease Rating Scale-III score during both on and off time, presence of ≥ 4 motor
symptoms, and CIILC dose. Significant differences were observed during follow-up (> 24 months
in 9 of the 11 CAs studied) for the percentage of off time and on time without disabling dys-
kinesia, adverse events frequency, and Clinical Global Impression scores. The rate of CIILC
discontinuation was between 20-40% in 9 CAs (78 and 80% in remaining 2 CAs).
Conclusions: This study reveals a marked variability between CAs in terms of patient selection
and CIILC safety and effectiveness. These results may have been influenced by patients’ baseline
characteristics, the availability of multidisciplinary teams, and clinical experience
Current trends in research, development and production of prophylactic vaccines : Report of Vaccipharma 2015 Congress
On June 14-19, 2015, the IUPHAR Section of Immunopharmacology and the Cuban Society of Pharmacology, together with the Latin- American Association of Pharmacology (ALF), Finlay Vaccine Institute and other prestigious Cuban scientific institutions, organized the Congress VACCIPHARMA 2015 (3rd International Congress on Pharmacology of Vaccines), held as part of the First International Convention IMMUNOPHARMACOLOGY–VACCIPHARMA 2015 (Meliá Marina Varadero Hotel, in Varadero beach, Cuba) VACCIPHARMA 2015 was organised into two large Workshops, addressing topics related to the research, development, clinical evaluation, production and quality control of Therapeutic and Prophylactic Vaccines, respectively. At the same time the Workshop on Prophylactic Vaccines was integrated by several Symposiums, focused on meningococcal, pneumococcal, enteric, tuberculosis and pertussis vaccines. About 250 delegates, including 100 international researchers from 15 countries, attended this meeting. The Congress had a remarkable Opening Session, with a Key Lecture given by the outstanding scientist Professor Dr. Shiv Pillai (United States of America), who talked about the changing views in the field of the immunology of vaccination and the challenges ahead. The aim of this review is to give an overview of the main topics discussed in the Prophylactic Vaccines Workshop, not as a complete narration of the events, but to provide an update of the latest state of the art and methodologies being applied to prophylactic vaccines with an expert commentary on the invited speakers
Cocoa husk as precursor of carbonaceous materials for supercapacitors electrodes
Activated carbon materials are widely used as high-performance electrodes for supercapacitors because they allow improving the electrochemical performance of these devices. The development of high surface areas and high porosity contributes to the energy storage by double electric layer, whereas the content of different functional groups on their surface to the pseudocapacitive storage. In addition, they can be obtained from lignocellulosic wastes providing a novel and economic perspective for the development of carbon-based electrodes. In this work, activated carbons were synthesized from cocoa husks using KOH as activating agent. The materials were obtained at three impregnation ratios (1:1, 3:1, 5:1) and three carbonization temperatures (500, 650, and 800 °C), optimizing the best conditions to obtain materials with high surface areas and porosity together with surface functional groups to improve energy storage. The results show that the 800 °C synthesized material with 3:1 impregnation ratio presents a capacitance of 120 F/g at 1 A/g, with a retention capacity of 93 % after 12,000 cycles, being the best electrochemical behavior among the carbons prepared. This is attributed to the adequate micro-mesoporosity, content of surface oxygenated functional groups and high surface area (1444 m2/g).Franz E. Microrreservas marinas artificiales en la línea de costa. Hacia un nuevo modelo de gestión de la biodiversidad en áreas litorales
Recientemente se ha propuesto a la comunidad científica la nueva figura de protección MRMA (Microrreserva Marina Artificial; AMMR en inglés) la cual también ha sido formalmente solicitada a la UNESCO para que esta institución considere reconocerla y consecuentemente, validarla. Se expone la contribución de las MRMAs a la preservación de especies protegidas, particularmente a las consideradas en peligro de extinción que propendan, de forma natural, a establecerse en escolleras y diques de abrigo de instalaciones costeras. Se mencionan las especies protegidas localizadas en las MRMAs, así como sus figuras de protección y disposiciones oficiales que les conciernen. Se exponen criterios generales de designación de MRMAs, posibles vías de solución ante problemas previsibles que puedan surgir (entendimiento entre administraciones, calidad de aguas, medidas de contingencia ante vertidos accidentales, etc.) y se deja entrever las potencialidades de las MRMAs como activo ecológico, medioambiental, urbanístico y educativo, así como el futuro papel que desempeñarán las MRMAs en la gestión del medio litoral y en el campo de la biología de la conservación. Finalmente, se establece una revisión de la legislación actual que más pudiera concernirles, destacándose los aspectos más importantes que en ellas pudieran influir. Al respecto, se destaca que ley 41/2010 de Protección del Medio Marino (Artº 26) establece que podrán formar parte de la red de Áreas Marinas Protegidas “ las áreas protegidas por instrumentos internacionales, sin perjuicio de que su declaración y gestión se ajustará a lo dispuesto en su correspondiente normativa internacional ” por lo que, si la nueva figura de protección “Microrreserva Marina Artificial” consiguiera marchamo UNESCO, aquélla podría incorporarse a la legislación española sin ninguna dificultad especial
IFE Plant Technology Overview and contribution to HiPER proposal
HiPER is the European Project for Laser Fusion that has been able to join 26 institutions and signed under formal government agreement by 6 countries inside the ESFRI Program of the European Union (EU). The project is already extended by EU for two years more (until 2013) after its first preparatory phase from 2008. A large work has been developed in different areas to arrive to a design of repetitive operation of Laser Fusion Reactor, and decisions are envisioned in the next phase of Technology Development or Risk Reduction for Engineering or Power Plant facilities (or both). Chamber design has been very much completed for Engineering phase and starting of preliminary options for Reactor Power Plant have been established and review here
Management of Parkinson's disease and other movement disorders in women of childbearing age: Part 2
Introducción
Muchas enfermedades que cursan con trastornos del movimiento hipercinético comienzan o afectan a mujeres en edad fértil. Es importante conocer los riesgos que tienen las mujeres con estas enfermedades durante el embarazo, así como los posibles efectos de los tratamientos sobre el feto.
Objetivos
Definir las características clínicas y los factores que condicionan la vida de la mujer en edad fértil con distonía, corea, síndrome de Tourette, temblor y síndrome de piernas inquietas. Definir una guía de actuación y manejo del embarazo y lactancia en las pacientes con esta enfermedad.
Desarrollo
Este documento de consenso se ha realizado mediante una búsqueda bibliográfica exhaustiva y discusión de los contenidos llevadas a cabo por un Grupo de Expertos en Trastornos del Movimiento de la Sociedad Española de Neurología (SEN).
Conclusiones
En todas las mujeres que padecen o comienzan con trastornos del movimiento hipercinéticos se debe valorar el riesgo-beneficio de los tratamientos, reducir al máximo la dosis eficaz o administrarlo de forma puntual en los casos en que sea posible. En aquellas enfermedades de causa hereditaria es importante un consejo genético para las familias. Es importante reconocer los trastornos del movimiento desencadenados durante el embarazo como determinadas coreas y síndrome de piernas inquietas.Introduction
Many diseases associated with hyperkinetic movement disorders manifest in women of childbearing age. It is important to understand the risks of these diseases during pregnancy, and the potential risks of treatment for the fetus.
Objectives
This study aims to define the clinical characteristics and the factors affecting the lives of women of childbearing age with dystonia, chorea, Tourette syndrome, tremor, and restless legs syndrome, and to establish guidelines for management of pregnancy and breastfeeding in these patients.
Results
This consensus document was developed through an exhaustive literature search and a discussion of the content by a group of movement disorder experts from the Spanish Society of Neurology.
Conclusions
We must evaluate the risks and benefits of treatment in all women with hyperkinetic movement disorders, whether pre-existing or with onset during pregnancy, and aim to reduce effective doses as much as possible or to administer drugs only when necessary. In hereditary diseases, families should be offered genetic counselling. It is important to recognise movement disorders triggered during pregnancy, such as certain types of chorea and restless legs syndrome
Stellar spectral-type (mass) dependence of the dearth of close-in planets around fast-rotating stars. Architecture of Kepler confirmed single-exoplanet systems compared to star-planet evolution models
In 2013 a dearth of close-in planets around fast-rotating host stars was
found using statistical tests on Kepler data. The addition of more Kepler and
Transiting Exoplanet Survey Satellite (TESS) systems in 2022 filled this region
of the diagram of stellar rotation period (Prot) versus the planet orbital
period (Porb). We revisited the Prot extraction of Kepler planet-host stars, we
classify the stars by their spectral type, and we studied their Prot-Porb
relations. We only used confirmed exoplanet systems to minimize biases. In
order to learn about the physical processes at work, we used the star-planet
evolution code ESPEM (French acronym for Evolution of Planetary Systems and
Magnetism) to compute a realistic population synthesis of exoplanet systems and
compared them with observations. Because ESPEM works with a single planet
orbiting around a single main-sequence star, we limit our study to this
population of Kepler observed systems filtering out binaries, evolved stars,
and multi-planets. We find in both, observations and simulations, the existence
of a dearth in close-in planets orbiting around fast-rotating stars, with a
dependence on the stellar spectral type (F, G, and K), which is a proxy of the
mass in our sample of stars. There is a change in the edge of the dearth as a
function of the spectral type (and mass). It moves towards shorter Prot as
temperature (and mass) increases, making the dearth look smaller. Realistic
formation hypotheses included in the model and the proper treatment of tidal
and magnetic migration are enough to qualitatively explain the dearth of hot
planets around fast-rotating stars and the uncovered trend with spectral type.Comment: Accepted in A&A. 13 pages, 8 figure
Equity, barriers and cancer disparities: study of the Spanish Society of Medical Oncology on the access to oncologic drugs in the Spanish Regions
[Purpose] The Spanish Society of Medical Oncology (SEOM) has conducted a study on the access to oncologic drugs across the 17 Spanish Regions with the aim of identifying potential heterogeneities and making proposals for eliminating the barriers identified at the different levels.[Methods] An Expert Panel made up of medical oncologists designed a survey on certain indications approved for 11 drugs in the approach of breast cancer, melanoma, lung cancer, prostate cancer and support treatment. This survey was sent to 144 National Health System (NHS) hospitals.
[Results] 77 hospitals answered the survey. The information modules analysed were: scope of the Commission that establishes binding decisions related to drug access; conditions, stages and periods of drug application, approval and administration processes; barriers to accessing drugs.
[Conclusions] The study shows variability in drug access. The SEOM makes proposals addressed to reducing the differences identified and homogenizing drug access conditions.This study was funded by SEOM
Papel del agua en la gelatinización delalmidón de maíz: estudio por calorimetríadiferencial de barrido
The thermal behavior of corn starch (Sigma Aldrich) was studied by differential scanning calorimetry (DSC). The endothermic peak in the DSC thermogram is associated to the starch gelatinization transition process. Initial phase of process and range in which it occurs is governed mainly by starch concentration in the solution, and the botanical source. This study demonstrates that the parameters associated to the observation method, in the DSC analysis, are influential in the determination of the maize starch gelatinization. In this way, the transition peak temperature, gelatinization enthalpy, and range of temperature are parameters that have to be considered when the moisture of sample and heating rate are changed with a homogenous grain size. For the DSC analysis, samples were prepared whit moisture values of 60, 65, 70, 75 and 80% (w/w), and heated at r = 2, 5, and 10◦C/min. Similarly, for fixed values of moisture (80%), the values of the heating rate were 2, 5, 7, 10 y 15◦C/min. Results indicate that the amount of water has influence over the enthalpy transition; however the peak temperature Tp remains invariable. The variation also depends of rate which transformation is made. Analysis allowed corroborating, that starch transition depends on extrinsic factors during the process. De esta forma, la temperatura del picode transición, la entalpía de gelatinización y el intervalo de temperatura degelatinización son los parámetros en estudio cuando se varía la humedad dela muestra y velocidad de calentamiento con un tamaño de grano homogéneo.Para los análisis, se tomaron valores de humedad de 60, 65, 70, 75 y 80%(p/p), para una velocidad de calentamiento de 2, 5 y 10◦C/min. De igualmodo, para un valor fijo de humedad (80%), se utilizó una velocidad de calentamientode 2, 5, 7, 10 y 15◦C/min. Los resultados indican que la cantidad deagua influye significativamente sobre la entalpía de gelatinización del proceso,pero la temperatura del pico Tp de la endoterma se mantiene constante. Elvalor de la entalpía disminuye a medida que la cantidad de agua aumenta.Las variaciones también son dependientes de la rapidez con que se efectúa latransformación. Los análisis permitieron corroborar, que esta transición en elalmidón es dependiente de factores extrínsecos durante el proceso.
