Valutazione comorbilitĂ e valutazione psicosociale in 20 soggetti sottoposti a chirurgia correttiva
Abbiamo valutato attentamente lâanamnesi fisiologica e patologica remota di venti giovani sottoposti ad intervento correttivo sec. Nesbit e/o Yachia in quanto affetti da recurvatum
penieno congenito negli anni 2006-2009; abbiamo poi invitato
i soggetti a visite di follow-up durante le quali venivano chieste notizie circa il lavoro, la famiglia, la componente affettiva
e le capacitĂ di normali intercourse amorose, la sensazione
di felicitĂ o stati di frustrazione o depressivi. Infine abbiamo
indagato su eventuali fallimenti terapeutici o complicanze a
noi imputabili. Tutti i soggetti (19-35 anni) appartengono a
tipo costituzionale longitipo normosplancnico, tutti con normale BMI, genitali esterni normo o plus sviluppati, tutti con
dimensioni di pene maggiore di 8,5 cm in flacciditĂ e over
16,5 in erezione con elasticitĂ normale (100%); scolaritĂ : tutti
diploma di scuola superiore, nessun laureato. Nessuna chirurgia in anamnesi, sviluppo normale, riferito uso di occhiali fin
dallâinfanzia in 8 soggetti (40%), apparecchio correttivo per
malocclusione dentale in 8 soggetti (40%), nessuno ha dovuto
ricorrere a presidi di terapia per scoliosi, solo 2 soggetti erano
mancini (10%), solamente 8 avevano avuto rapporti sessuali
completi prima dellâintervento chirurgico (40%) ovvero gli
stessi che avevano un rapporto sentimentale stabile. In 10
casi siamo stati informati che i soggetti facevano uso di droga
anche se saltuariamente (50%). Nessun paziente ci ha informato di vivere condizioni di disagio famigliare o condizione
di disoccupazione. Nel follow-up registriamo dati lusinghieri
circa i risultati sia morfologici che funzionali, nessun paziente
ha avuto guarigione incompleta o riferita tale, nessuno ci ha
riferito di brevitĂ da fibrosi chirurgica, nessuno ha avuto successivamente dolore durante lâerezione o uno scarso risultato
estetico da circoncisione. Con lâausilio di un collega psichiatra
che ha curato il counselling post-operatorio, abbiamo avuto
informazioni circa lo stato di benessere e qualitĂ della vita
del campione: il 30% dei ragazzi risulta depresso, il 30% ha
affermato di non avere avuto miglioramento della vita dalla
chirurgia, il tasso di legami affettivi nel follow-up non si Ăš modificato (da 18 a 36 mesi).
In conclusione, evidenziamo che ci pare importante sottolineare la correlazione nel gruppo tra i dati antropometrici e dei
genitali, interessante anche lâalta percentuale di malocclusione dentale, lenti correttive e lâuso saltuario di droghe
VEGF and HIF in renal cell carcinogenesis.
Seventy-five percent of all cases of renal cell carcinoma have clear cell histology (CCRCC). In CCRCC alterations of the promoter of the VHL gene is the defining somatic genetic event. The high vascularity of RCC suggests a fundamental role of angiogenesis to its pathogenesis and is deeply associated with proliferation and metastasis. Many HIF-α target genes appear to be involved in these pathways, targeting VEGF and resulting in angiogenesis, increased vascular permeability, endothelial cell (EC) proliferation, survival, migration, and differentiation and promotion of degradation of the extracellular matrix.
Molecular markers might predict the responsiveness of molecular targeted therapy. While in case of localised disease, radical nephrectomy is highly effective therapy, for locally invasive tumors, surgical approaches are less effective and not curative for metastatic disease. In advanced cases, immune systematic cytokine therapies have resulted in low response rates, with a smaller percentage exhibiting complete remission upon treatment. Systemic treatment with traditional chemotherapy, radiation, and hormonal therapy have not been effective. The recent development of molecularly target agents have radically changed the management of metastatic CCRCC and the availability of reliable biomarkers to monitor therapy response and of anti-angiogenic drugs evasive to resistance could render targeted therapy more effective
Prognostic value of CD44 expression in penile squamous cell carcinoma: a pilot study.
BACKGROUND: Several studies have reported on the prognostic value of molecular markers for metastasis risk and survival in penile squamous cell carcinoma (SCC) patients. The usefulness of CD44 expression as such a marker has been studied in different tumors, but not in penile SCC. Our aim was to determine whether CD44 expression may serve as a prognostic marker for lymph node metastasis and survival in penile SCC patients.
METHODS: CD44 immunoistochemical expression was investigated in tissue specimens from 39 patients with penile SCC. CD44 cell positivity, staining intensity and distribution were analyzed and correlated with tumor stage, grade, lymph node status and disease-specific survival.
RESULTS: CD44 expression was detected in epithelial cells of both intratumoral and normal tissues with different intensities and staining distributions. In normal tissues CD44 protein was mainly detected in cell membranes, whereas in the tumor compartments it was found in both the cell membranes and the cytoplasm. The intensities and percentages of CD44 expressing cells did not correlate with tumor stage and/or grade. Seventy-three percent of the patients with lymph node metastasis showed high intensities of CD44 staining, as compared to 44% of the patients without lymph node metastasis (Pâ=â0.03). Lymph node-positive patients showed both cytoplasmic and membranous CD44 expression. High CD44 expression was found to be significantly correlated with a decreased 5 year overall survival (Pâ=â0.01).
CONCLUSIONS: CD44 levels and patterns of expression can be considered as markers for penile SCC aggressiveness and, in addition, may serve as predictive markers for lymph node metastasis, also in patients with clinically negative lymph nodes. CD44 expression may provide prognostic information for penile SCC patients, next to classical clinical-pathological factors
