    Impacto de la ecografía volumétrica portátil en el sondaje vesical por retención urinaria en una unidad de medicina interna

    Aim: To estimate the impact of the use of portable bladder volumetric ultrasound on bladder catheterization due to suspicion of urinary retention in an internal medicine unit. Methodology: Study of retrospective cohorts, comparing the cohort exposed to the availability of bladder ultrasound, with the not exposed the previous year. All records in the Electronic Medical Record (EHR) of short-term permanent bladder catheters in adult patients admitted to the Internal Medicine hospitalization unit of the University Hospital Fundación Alcorcón (HUFA) during the years 2015 and 2016 were analyzed. The urinary retention frequency is estimated as the cause of the catheterization after the device has been incorporated into the unit and compared with the frequency of catheterization during the same period of the previous year in the same unit. It is estimated that the impact of having this device on the urinary retention frequency is the reason for the catheterization. Results: 134 catheters are included in 113 patients, 62 in the group without ultrasound and 72 in the group with ultrasound. The frequency of catheterizations due to retention is reduced from 47.5% to 21.4% after introducing the ultrasound unit into the unit. This represents a 50% reduction (adjusted RR=0.48; CI95%:0.27-0.84, p=0.01) in the frequency of urinary catheterization for suspected urinary retention.Objetivo: Estimar el impacto que el uso de ecógrafo vesical tiene en los sondajes vesicales por sospecha de retención urinaria de una unidad de Medicina interna.Metodología: Estudio de cohortes retrospectivas, comparando la cohorte expuesta a la disponibilidad del ecógrafo vesical, con la no expuesta el año previo. Se analizan todos los registros en la Historia Clínica Electrónica (HCE) de sondajes vesicales permanentes de corta duración en pacientes adultos que ingresaron en la unidad de hospitalización de Medicina Interna del Hospital Universitario Fundación Alcorcón (HUFA) durante los años 2015 y 2016. Se estima la frecuencia de retención urinaria como causa del sondaje después de la incorporación del dispositivo en la unidad y se compara con la frecuencia en sondajes durante el mismo periodo del año anterior en la misma unidad. Se estima el impacto que disponer de este dispositivo tiene en la frecuencia de retención urinaria como motivo del sondaje.Resultados: Se incluyen 134 sondajes en 113 pacientes, 62 en el grupo sin ecógrafo y 72 en el grupo con ecógrafo. La frecuencia de sondajes por retención se reduce del 47.5% al 21.4% después de introducir el ecógrafo en la unidad. Esto supone una reducción del 50% (RR ajustado= 0.48; IC95%:0.27-0.84, p=0.01) en la frecuencia de sondaje vesical por sospecha de retención urinaria

    Areview is offered of the coverage given by the British newspaper The Guardian and the Spanish daily El País in the days following the bombings in Madrid on 11th March 2004 using two different databases. The next days to the bombings, both British and Spanish media gave images and information that reflected the massacre from different points of view. Through this work, we were able to locate and analyze both the images and the information about the bombings and its consequences on the Spanish general elections of 14 March, including different main viewpoints of the political parties and official security institutionsSe analiza la visión que los periódicos The Guardian y El País difundieron en los días posteriores a los atentados de Madrid del 11 de marzo de 2004, utilizando para ello dos bases de datos.     Supporting autonomy in physical education: Perception versus reality.

    Esta investigación se centró, por una parte en la percepción de apoyo a la autonomía en las clases de Educación física, que tienen tanto el alumnado como el profesorado, y por otra, en la coherencia entre percepción y la realidad observada en las clases. Se aplicaron dos escalas al profesorado y al alumnado con el objeto de conocer con qué frecuencia se producen comportamientos instructivos favorables al desarrollo de la autonomía, y se observaron y grabaron en video clases impartidas por el profesorado. Los resultados muestran que la frecuencia de los comportamientos instructivos de apoyo a la autonomía durante la intervención de enseñanza es significativamente menor que la percibida por el alumnado y el profesorado. La realidad observada muestra un perfil de profesorado con gran margen de mejora en comportamientos como: calidad de la comunicación, funcionalidad de las tareas, fomento del pensamiento del alumnado, situaciones en las que el alumnado pueda expresar su opinión sobre las tareas y motivación intrínsecaThis investigation, on the one hand it focuses on students´ and teachers´ perception about autonomy support during Physical Education instruction, and on the other hand, in the coherence between perception and reality observed in the classes. Two scales were administered to determine the frequency of instructional behaviors favoring the development of autonomy, and teacher´s classes were observed and videotaped. The results show that the observed frequency of instructional behavior favoring autonomy support during classes is significantly lower than that perceived by students and teachers.     Evaluación morfométrica del complejo estilohioideo mediante tomografía computarizada multicorte

    Objective: tomography (MCT). Materials and methods: This was a descriptive, retrospective cross-sectional study. The sample was made up of 238 MCT studies performed in patients of both sexes between 20 to 87 years of age, with indication of tomography of the craniofacial complex. The length of the SHC was measured in lateral views of 3D volumetric reconstructions. The distribution of these cases was obtained according to age, sex, laterality, type, and the reason for the examination. Results: The mean length of the SHC was 30.66 ± 10.58 mm. Of the total sample, 127 (53.4%) individuals showed an elongated SHC; of these, 63.8% were women and 64.6% of the patients presented a bilateral compromise of the complex. Most of the subjects with an elongated SHC were between 30 and 59 years old. The type of ossification most frequently found was type I (continuous elongation). Regarding the reason for the examination, most of the individuals were referred for study of the temporomandibular joint. Conclusions: MCT is a useful tool for the identification and morphometric study of ossification of the SHC, both in length and type. Examination of an ossified SHC is important for the differential diagnosis of cervico-facial pain and temporomandibular joint dysfunction.Objetivo: Estudiar morfométricamente el complejo estilohioideo (CEH) mediante tomografía computarizada multicorte (TCM). Materiales y métodos: Investigación descriptiva, retrospectiva y transversal. La muestra estuvo conformada por 238 estudios de TCM, pertenecientes a pacientes de ambos sexos con edades entre 20 y 87 años, con indicación de tomografía para el macizo craneofacial. Se realizó la medición de la longitud del CEH en vistas laterales de reconstrucciones volumétricas en 3D. Se obtuvo la distribución de estos casos de acuerdo con edad, sexo, lateralidad, tipo de osificación y motivo de indicación del examen. Resultados: La longitud media del CEH fue de 30,66 ± 10,58 mm. Del total de la muestra, 127 (53,4%) individuos mostraron un CEH elongado; de estos, un 63,8% fueron mujeres y un 64,6% de los pacientes presentó un compromiso bilateral del complejo. La mayoría de los sujetos con un CEH elongado tenían edades comprendidas entre 30 y 59 años. El tipo de osificación encontrada con mayor frecuencia fue del tipo I (elongación continua). En cuanto al motivo de indicación del examen, la mayoría de los pacientes fueron referidos para estudio de la articulación temporomandibular. Conclusiones: La TCM es una herramienta útil en la identificación y el estudio morfométrico de la osificación del CEH, tanto en su longitud como tipo.     Recycled wastewater as a potential source of microplastics in irrigated soils from an arid-insular territory (Fuerteventura, Spain)

    In this work, the occurrence of microplastics (MPs) in irrigation recycled wastewaters (RWWs) and a desalinated brackish water (DBW) from the arid territory of Fuerteventura (Canary Islands, Spain) was studied. Besides, the presence of MPs in two types of soils (sandy-loam and clay-loamy; with no mulch film or fertilization with sewage sludge applied) irrigated with both water qualities was addressed. Results showed the prevalence presence of cellulosic and polyester microfibers (between 84.4 and 100%) of blue and transparent colors (up to 55.6 and 33.3%, respectively), with an average length of 786.9 ± 812.1 μm in the water samples. DBW had the lowest MP concentration (2.0 ± 2.0 items·L−1) while RWW showed concentrations up to 40.0 ± 19.0 items·L−1. Similarities were also observed between the MPs types and sizes found in both soils top layer (0–5 cm), with an average concentration three times greater in soil irrigated with RWW than in soil under DBW irrigation (159 ± 338 vs. 46 ± 92 items·kg−1, respectively). In addition, no MPs were extracted from non-irrigated/non-cultivated soils, suggesting agricultural activities as the unique source of MPs in soils of this arid area. Results show that RWWs constitute a potential source of MPs in irrigated soils that should be considered among other pros and cons linked to the use of this water quality in agricultural arid land

    Molecular characterization of autophagic and apoptotic signaling induced by sorafenib in liver cancer cells

    Sorafenib is the unique accepted molecular targeted drug for the treatment of patients in advanced stage of hepatocellular carcinoma. The current study evaluated cell signaling regulation of endoplasmic reticulum (ER) stress, c-Jun-N-terminal kinase (JNK), Akt, and 5′AMP-activated protein kinase (AMPK) leading to autophagy and apoptosis induced by sorafenib. Sorafenib induced early (3–12 hr) ER stress characterized by an increase of Ser51P-eIF2α/eIF2α, C/EBP homologous protein (CHOP), IRE1α, and sXBP1, but a decrease of activating transcription factor 6 expression, overall temporally associated with the increase of Thr183,Tyr185P-JNK1/2/JNK1/2, Thr172P-AMPKα, Ser413P-Foxo3a, Thr308P-AKt/AKt and Thr32P-Foxo3a/Foxo3a ratios, and reduction of Ser2481P-mammalian target of rapamycin (mTOR)/mTOR and protein translation. This pattern was related to a transient increase of tBid, Bim EL, Beclin-1, Bcl-xL, Bcl-2, autophagy markers, and reduction of myeloid cell leukemia-1 (Mcl-1) expression. The progressive increase of CHOP expression, and reduction of Thr308P-AKt/AKt and Ser473P-AKt/AKt ratios were associated with the reduction of autophagic flux and an additional upregulation of Bim EL expression and caspase-3 activity (24 hr). Small interfering-RNA (si-RNA) assays showed that Bim, but not Bak and Bax, was involved in the induction of caspase-3 in sorafenib-treated HepG2 cells. Sorafenib increased autophagic and apoptotic markers in tumor-derived xenograft model. In conclusion, the early sorafenib-induced ER stress and regulation of JNK and AMPK-dependent signaling were related to the induction of survival autophagic process. The sustained drug treatment induced a progressive increase of ER stress and PERK-CHOP-dependent rise of Bim EL, which was associated with the shift from autophagy to apoptosis.     Mediating effect of job crafting dimensions on influence of burnout at self-efficacy and performance: revisiting health-impairment process of JD-R theory in public administration

    In recent years, job crafting has greatly interested Work and Organizational Psychology. Different research studies have shown its positive impact on people and organizational performance. However, it knows little about the differential effect of the two dimensions that make up this variable (prevention-focused and promotion-focused) and its role in the health impairment spiral process of the job demand-resources theory (JD-R). This research aims to analyze the mediating effect of the different dimensions of job crafting on the influence of burnout on performance and self-efficacy in the workplace. The study used a sample of 339 administrative employees of a university. : The results indicate that promotion-focused job crafting is a mediating variable in the relationship between the influence of burnout on performance and self-efficacy. Unexpectedly, prevention-focused job crafting does not have this mediating role in the same relationship. : These findings confirm the adverse impact of burnout on personal and organizational improvement, while showing the absence of prevention/protection responses of employees when they are burned out. The theoretical and practical implications show an advance in knowledge about the process of health deterioration and about the spiral of health deterioration in the JD-R theor

    Microplastics determination in gastrointestinal tracts of european sea bass (Dicentrarchus labrax) and gilt-head sea bream (Sparus aurata) from Tenerife (Canary Islands, Spain)

    Microplastic pollution has an extremely widespread distribution, to the extent that microplastics could be ingested by aquatic organisms, including species of commercial importance for fisheries and aquaculture. In this work, the anthropogenic particles content of the gastrointestinal tracts of 86 individuals of cultivated European sea bass (Dicentrarchus labrax, n = 45) and gilt-head sea bream (Sparus aurata, n = 41) from Tenerife (Canary Islands, Spain) was determined. Samples were bought at local markets and directly transported to the laboratory. After the dissection of the fishes and digestion of the gastrointestinal tracts in 10% KOH (w/v) at 60◦C for 24 h, the digests were filtered (50 µm stainless-steel mesh) and visualized under a stereomicroscope, finding that most of the items were colourless (47.7% for Dicentrarchus labrax and 60.9% for Sparus aurata) and blue (35.3% vs. 24.8%) microfibers, with an average length of 1957 ± 1699 µm and 1988 ± 1853 µm, respectively. Moreover, 15.3% of the microfibres were analysed by Fourier transform infrared spec-troscopy, showing the prevalence of cellulosic fibres together with polyester, polyacrylonitrile, and poly(ether-urethane).     Pharmacogenetics of siponimod: A systematic review

    Multiple sclerosis is a chronic inflammatory neurological disease, and siponimod (Mayzent) is the first oral treatment option for adult patients with secondary progressive multiple sclerosis. We performed a systematic review of the pharmacogenetics of Siponimod, and we found that (430 C>T; rs1799853) and CYP2C9 * 3 (1075 A>C; rs1057910), both translated no-function alleles, have been related to a lower metabolism of siponimod by CYP2C9 enzyme. The FDA-approved drug label and EMA risk management plan for siponimod require testing patients for CYP2C9 genotype before treatment starts. The FDA drug label states that siponimod is contraindicated in patients carrying a CYP2C9 * 3/* 3 genotype, and a daily maintenance dose of 1 mg in patients with CYP2C9 * 1/* 3 and * 2/* 3 genotypes. The EMA reported the potential long-term safety implications in CYP2C9 poor metabolizer patients treated with this drug. Based on this systematic review we concluded that CYP2C9 SNPs influence on siponimod response might be stated by assessing not only CYP2C9 * 2 and CYP2C9 * 3 but other genetic variants resulting in CYP2C9 IM/PM status. CYP2C9 IM phenotype translated from the CYP2C9 * 2 genotype should be revised since it is contradictory compared to other CYP2C9 no-function alleles, and CYP2C9 * 2 might be excluded from PGx testing recommendation before treatment starts with siponimod since it is not translated into a therapeutic recommendation
