    Efeitos de uma abordagem multi-profissional sobre variáveis antropométricas e de perfil alimentar de adolescentes com sobrepeso ou obesidade

    Introduction: To investigate the effects of a multiprofessional approach on anthropometric parameters and body composition and the level of food processing in adolescents. Methods: Twelve overweight/obese male adolescents between 13-17 years old participated in a series of 12-week multiprofessional interventioms led by physical education, nutrition and psychology professionals. The participants were assessed before the intervention as well as at 6 and 12 weeks into it. Anthropometric assessments were performed, including body weight, stature and body mass index; body composition, with fat mass (FM), body fat (%BF), skeletal muscle mass (SMM) and resting metabolic rate (RMR) and by filling in a three-day food record, in order to calculate the ingested foods, by the level of processing of the food: fresh, minimally processed, processed and ultra-processed.  Results:  No differences were observed for the anthropometric parameters performed at different stages (p>0.05). Reductions for FM and %BF and increases in SMM and RMR (p<0.05) were identified, all after 12-week of the intervention. There was an increase in the consumption of natural foods/g, and an increase in the caloric consumption of processed foods/kcal, after 12-week of interventions (p<0.05). Conclusion: The model resulted in decreases in FM and %BF, an increase in SMM, RMR, natural foods/g and processed foods/kcal.Introdução: Investigar a qualidade nutricional de adolescentes com sobrepeso ou obesidade é substancial para orientar estratégias de alimentação saudável. Objetivo: Avaliar os efeitos de uma abordagem multiprofissional sobre os parâmetros antropométricos, de composição corporal e o nível de processamento de alimentos em adolescentes. Método: Doze adolescentes do sexo masculino com sobrepeso/ obesidade, entre 13-17 anos participaram de uma série de intervenções multiprofissionais ao longo de 12 semanas, que foram conduzidas por profissionais de educação física, nutricionistas e psicólogos. Os participantes foram avaliados antes da intervenção, após 6 e após 12 semanas. As avaliações antropométricas foram realizadas, incluindo o peso corporal, a estatura e o índice de massa corporal; para a composição corporal foram incluídas: a massa gorda (MG), percentual de gordura corporal (%G), massa muscular esquelética (MME) e taxa metabólica basal (TMB), bem como o preenchimento do registro alimentar de três dias, para avaliar os alimentos ingeridos, via nível de processamento: in natura, minimamente processado, processado e ultraprocessado. Resultados: Não foram observadas diferenças significativas nos parâmetros antropométricos realizados nas diferentes etapas (p>0,05). Reduções na MG e %G e aumentos na MME e TMB (p<0,05) foram identificados após 12 semanas de intervenção. Houve um aumento do consumo de alimentos in natura/g e aumento do consumo calórico de alimentos industrializados/kcal, após 12 semanas de intervenção (p<0,05). Conclusão: O modelo proposto resultou em uma diminuição da massa gorda e percentual de gordura corporal, aumento da massa muscular esquelética, taxa metabólica basal, consumo de alimentos naturais/g e alimentos processados/kcal

    The impact of pain and nocturnal cramps on sleep quality in Charcot Marie Tooth disease: a case-control study

    Introduction: Charcot-Marie-Tooth disease is an inherited neuropathy that presents two main forms - type 1 and type 2 -, differentiated by the speed of the nervous conduction. Our goal was to assess sleep in Charcot-Marie-Tooth disease and its relationship with pain perception and nocturnal cramps. Material and Methods: This was a case-control study. The case group was composed of 10 volunteers diagnosed with the type 1 and 23 with the type 2. The control group was composed of 22 individuals from the same family matched by age and gender. Volunteers underwent clinical screening to assess the presence of nocturnal cramps and filled the brief pain inventory, the Chalder fatigue scale, the Epworth sleepiness scale, and the Pittsburgh sleep quality index. Sleep was evaluated by actigraphy. Results: Type 2 patients presented a more severe perception of pain and fatigue, more time spend awake after sleep onset, and had lower sleep efficiency. The individuals who reported nocturnal cramps also had worse perception of pain, reduced sleep latency, and increased sleep fragmentation.     The low natural fertility of soils, the high potassium (K) demand in agriculture, and the reduced Brazilian mineral reserves of K are the causes of the high importation of potassium fertilizers in Brazil. This represents a great and negative impact on Brazil´s trade balance. Forty years ago, trials for utilization of rocks as local K sources did not reach success due to the low natural solubility of minerals or the high cost of industrial processes to obtain fertilizers from them. However, in the last years, other kinds of rocks are promising as K sources, even when applied in natura, just ground. Those findings led to the establishment of a research network to study the potential of using those rocks as K sources nationwide.Conseqüência da baixa fertilidade natural dos solos, da elevada demanda de potássio (K) na agricultura e da reduzida reserva brasileira de minérios de K, a importação de fertilizantes potássicos tem representado grande ônus à balança comercial do País. Há cerca de 30 anos, tentativas de utilização de rochas como fontes locais de K não obtiveram sucesso devido à baixa solubilidade natural dos minerais constituintes ou ao elevado custo dos processos industriais de obtenção de fertilizantes a partir dessas fontes. Entretanto, nos últimos anos, outros tipos de rochas têm se mostrado promissoras no fornecimento de K, mesmo quando aplicadas “in natura”, simplesmente moídas.     Porpouse: To systematically review the mechanisms by WBV improves the ability to learn, think, memorize and all other processes involving cognition. Methods: The present study collected data from three databases using the keywords “whole-body-vibration” and “cognition”. Randomized clinical trials focusing on the association of WBV and cognition were considered. The study was registered in the database of systematic reviews protocols PROSPERO. All included studies used healthy patients, exposed to WBV. The included articles were obtained regarding the risk of bias according to the Cochrane Collaboration criteria, level of evidence and strength of recommendation following the GRADE and Oxford classification. Results: Of the 89 articles published to the eligibility criteria, four were submitted to data extraction. Cognitive parameters were improved in relation to attention, memory or learning in almost all articles evaluated in this systematic review. Conclusion: Intervention with WBV would positive effects on individuals' cognitive ability, although further randomized investigations must be conductedObjetivo: Revisar sistematicamente os mecanismos pelos quais a vibração de corpo inteiro (VCI) melhora a capacidade de aprender, pensar, memorizar e todos os outros processos que envolvem a cognição. Método: O presente estudo coletou dados de três bancos de dados usando as palavras-chave “vibração de corpo inteiro” e “cognição”. Ensaios clínicos randomizados com foco na associação de WBV e cognição foram considerados. O estudo foi registrado no banco de dados de protocolos de revisões sistemáticas PROSPERO. Todos os estudos incluídos usaram pacientes saudáveis, expostos à VCI. Os artigos incluídos foram avaliados quanto ao risco de viés de acordo com os critérios da Colaboração Cochrane, nível de evidência e força de recomendação segundo a classificação GRADE e Oxford. Resultados: Dos 89 artigos publicados, de acordo com os critérios de elegibilidade, quatro foram submetidos à extração de dados. Os parâmetros cognitivos, atenção, memória e aprendizagem demonstraram melhora em quase todos os artigos avaliados nesta revisão sistemática. Conclusão: A intervenção com VCI teria efeitos positivos na capacidade cognitiva dos indivíduos, embora mais ensaios clínicos randomizadas devam ser realizados para avaliação de tais parâmetros

    Purification of a lectin from Cratylia mollis crude extract seed by a single step PEG/phosphate aqueous two-phase system

    The partitioning and purification of lectins from the crude extract of Cratylia mollis seeds (Cramoll 1,4) was investigated in aqueous two-phase systems (ATPS). A factorial design model (24) was used to evaluate the influence of polyethylene glycol (PEG) molar mass (15008000g/mol), PEG concentration (12.517.5% w/w), phosphate (1015% w/w) concentration, and pH (68) on the differential partitioning, purification factor, and yield of the lectin. Polymer and salt concentration were the most important variables affecting partition of lectin and used to find optimum purification factor by experimental BoxBehnken design together with the response surface methodology (RSM). ATPS showed best conditions composed by 13.9% PEG1500, 15.3% phosphate buffer at pH 6, which ensured purification factor of 4.70. Sodium dodecyl sulfatepolyacrylamide gel electrophoresis showed a single band of protein with 26.1kDa. Furthermore, results demonstrated a thermostable lectin presenting activity until 60°C and lost hemagglutinating activity at 80°C.     From Re-Emergence to Hyperendemicity: The Natural History of the Dengue Epidemic in Brazil

    The spread of dengue virus is a major public health problem. Though the burden of dengue has historically been concentrated in Southeast Asian countries, Brazil has become the country that reports the largest number of cases in the world. While prior to 2007 the disease affected mostly adults, during the 2007 epidemic the number of dengue hemorrhagic fever cases more than doubled, and over 53% of cases were in children under 15 years of age. In this paper, we propose that the conditions for the shift were being set gradually since the re-introduction of dengue in 1986 and that they represent the transition from re-emergence to hyperendemicity. Using data from an age stratified seroprevalence study conducted in Recife, we estimated the force of infection (a measure of transmission intensity) between 1986–2006 and used these estimates to simulate the accumulation of immunity since the re-emergence. As the length of time that dengue has circulated increases, adults have a lower probability of remaining susceptible to primary or secondary infection and thus, cases become on average younger. If in fact the shift represents the transition from re-emergence to hyperendemicity, similar shifts are likely to be observed in the rest of Brazil, the American continent and other regions where transmission emerges

    Post-intervention Status in Patients With Refractory Myasthenia Gravis Treated With Eculizumab During REGAIN and Its Open-Label Extension

    OBJECTIVE: To evaluate whether eculizumab helps patients with anti-acetylcholine receptor-positive (AChR+) refractory generalized myasthenia gravis (gMG) achieve the Myasthenia Gravis Foundation of America (MGFA) post-intervention status of minimal manifestations (MM), we assessed patients' status throughout REGAIN (Safety and Efficacy of Eculizumab in AChR+ Refractory Generalized Myasthenia Gravis) and its open-label extension. METHODS: Patients who completed the REGAIN randomized controlled trial and continued into the open-label extension were included in this tertiary endpoint analysis. Patients were assessed for the MGFA post-intervention status of improved, unchanged, worse, MM, and pharmacologic remission at defined time points during REGAIN and through week 130 of the open-label study. RESULTS: A total of 117 patients completed REGAIN and continued into the open-label study (eculizumab/eculizumab: 56; placebo/eculizumab: 61). At week 26 of REGAIN, more eculizumab-treated patients than placebo-treated patients achieved a status of improved (60.7% vs 41.7%) or MM (25.0% vs 13.3%; common OR: 2.3; 95% CI: 1.1-4.5). After 130 weeks of eculizumab treatment, 88.0% of patients achieved improved status and 57.3% of patients achieved MM status. The safety profile of eculizumab was consistent with its known profile and no new safety signals were detected. CONCLUSION: Eculizumab led to rapid and sustained achievement of MM in patients with AChR+ refractory gMG. These findings support the use of eculizumab in this previously difficult-to-treat patient population. CLINICALTRIALSGOV IDENTIFIER: REGAIN, NCT01997229; REGAIN open-label extension, NCT02301624.
