    In terms of existing buildings’ retrofit, historical and non-residential buildings clearly represent two challenging categories. In historical buildings, due to architectural constraints, acting on the technical side is often the only way to reduce consumptions, while non-residential buildings usually require simultaneous production of hot and chilled water. In these cases, Polyvalent heat pumps represent an interesting solution, able to guarantee significant reductions of primary energy consumptions. In detail, a Reference Building for an historical office was simulated in three Mediterranean cities, using EnergyPlus software and some evaluation tools specifically set to emulate the energy performances of the examined HVAC technologies

    Liver resection of hepatocellular carcinoma in HIV-HCV co-infected patients: a retrospective case series

    Abstract Introduction Despite the effectiveness of new therapies and awareness campaigns, the number of seropositive patients is increasing every year. Recently, other causes of death, not directly related to HIV, have emerged, such as chronic liver disease. The risk of hepatocellular carcinoma (HCC) is seven times greater in HIV patients than in noninfected patients, and it is especially attributable to HCV infection. The aim of our study was to evaluate clinical outcomes of HCC in HIV-HCV co-infected patients after liver resection (LR). Materials and methods The current study was conducted on a prospective database and reviewed retrospectively. All consecutive patients with HCC treated by LR from January 2013 to March 2019 at the Luigi Sacco University Hospital in Milan were enrolled. We included patients older than 18 years of age with HCV-related HCC, and in this set of patients, we identified two groups based on the presence of HIV infection. Results We identified 16 patients with HCV infection and precisely five with HIV-HCV co-infection and eleven with HCV infection alone. All HIV patients were male against 72.7% in the non-HIV group (p = 0.509). All patients had optimal HIV virologic control and a normal CD4 T-cell count. The mean diagnosis-to-treatment interval was statistically different between the two groups (HIV versus non-HIV: 1.2 ± 0.55 months versus 2.39 ± 1.09 months, p = 0.039). No other significant differences were found between HIV-HCV co-infected patients and HCV-infected patients. Long-term outcomes in terms of OS and RFS were similar between the two groups. Conclusions With a multidisciplinary approach and intensive support, LR can be a safe and efficacious procedure in HIV-HCV patients. For these reasons, we should not exclude potential patients merely on the basis of their HIV seropositivity

    Toward NZEB by optimizing HVAC system configuration in different climates

    Finding the most appropriate matching between envelope features and HVAC system configurations in function of different climates results fundamental for minimizing buildings’ energy consumptions. The research aims at presenting the most energy-performing HVAC system configurations for high-performing buildings. Different configurations were modeled for new non-residential Reference Buildings in seven European cities, using dynamic simulation software EnergyPlus and some evaluation tools specifically set to emulate the energy performance of some specific HVAC technologies. Finally, the results obtained were compared in order to outline some conclusions, useful as guidelines for optimizing the choice of HVAC systems in function of climate conditions

    The paper deals with the general presentation of the Urban GEO BIG DATA, a collaborative acentric and distributed Free and Open Source (FOS) platform consisting of several components: local data nodes for data and related service Web deploy; a visualization node for data fruition; a catalog node for data discovery; a CityGML modeler; data-rich viewers based on virtual globes; an INSPIRE metadata management system enriched with quality indicators for each dataset.Three use cases in five Italian cities (Turin, Milan, Padua, Rome, and Naples) are examined: 1) urban mobility; 2) land cover and soil consumption at different resolutions; 3) displacement time series.     Inter-Laboratory Concordance of Cerebrospinal Fluid and Serum Kappa Free Light Chain Measurements

    The kappa index (K-Index), calculated by dividing the cerebrospinal fluid (CSF)/serum kappa free light chain (KFLC) ratio by the CSF/serum albumin ratio, is gaining increasing interest as a marker of intrathecal immunoglobulin synthesis. However, data on inter-laboratory agreement of these measures is lacking. The aim was to assess the concordance of CSF and serum KFLC measurements, and of K-index values, across different laboratories. KFLC and albumin of 15 paired CSF and serum samples were analyzed by eight participating laboratories. Four centers used Binding Site instruments and assays (B), three used Siemens instruments and assays (S), and one center used a Siemens instrument with a Binding Site assay (mixed). Absolute individual agreement was calculated using a two-way mixed effects intraclass correlation coefficient (ICC). Cohen’s kappa coefficient (k) was used to measure agreement on positive (5.8) K-index values. There was an excellent agreement in CSF KFLC measurements across all laboratories (ICC (95% confidence interval): 0.93 (0.87–0.97)) and of serum KFLC across B and S laboratories (ICC: 0.91 (0.73–0.97)), while ICC decreased (to 0.81 (0.53–0.93)) when including the mixed laboratory in the analysis. Concordance for a positive K-Index was substantial across all laboratories (k = 0.77) and within S laboratories (k = 0.71), and very good (k = 0.89) within B laboratories, meaning that patients rarely get discordant results on K-index positivity notwithstanding the testing in different laboratories and the use of different platforms/assays
