    Social capital and factors associated with the caries experience in adults : a population-based study in Brazil

    The objective of this study was to investigate the experience of caries related to social capital and associated factors in adults in large-scale population-based study. A Cross-sectional study was performed in 163 municipalities in the State of São Paulo, Brazil (SBSP-2015). 17,560 people were evaluated, of which 6051 were adults aged 35-44 years. Hierarchical logistic regression analysis was proposed. Outcome variables (decayed teeth, missing teeth and DMFT) and independent variables were included in the model, considering the distal (income and schooling); intermediate (social capital) and proximal levels (sex and ethnicity). Results showed that income up to 1,500 reais - US$ 367.6 in 11/11/2019 - (OR = 1.91;1.75-2.08), schooling up to 8 years (OR = 1.32;1.12-1.56) and non-white ethnicity (OR = 1.54;1.35-1.76) were more likely to have decayed teeth. Income up to 1500 reais (OR = 1.29;1.15-1.44), schooling up to 8 years (OR = 2.13;1.90-2.38), low social capital (OR = 1.84;1.65-2.04), medium social capital (OR = 1.15;1.01-1.30) and females were more likely to have lost teeth (OR = 1.13;1.03-1.23). Schooling up to 8 years (OR = 1.51;1.35-1.69), low social capital (OR = 1.25; 1.14-1.37) and female (OR = 1.40,1.19-1.53) were associated with DMFT. It was concluded that sociodemographic factors and low social capital were associated with the experience of caries, which should be taken into account in the formulation of public policies3

    Malocclusion and dental appearance in underprivileged brazilian adolescents

    Satisfaction with dental aesthetics is a subjective indicator used in epidemiological studies and is related to health behaviours. Little is known about the factors that influence this indicator, particularly among adolescents who live in a situation of social vulnerability. The aim of this study was to investigate the relationship between malocclusion and dental appearance in underprivileged Brazilian adolescents. This analytical cross-sectional study was conducted in Piracicaba, Brazil, and evaluated 884 adolescents from 13 to 19 years of age. The dependent variable was satisfaction with dental appearance, and the independent variables were classified as individual (components of the Dental Aesthetic Index - DAI, sex and age) and contextual (social exclusion index). For statistical analysis, multilevel regression models were estimated. The individual variables were considered Level 1, and the contextual variable was considered Level 2, with a level of significance of 5%. The mean age of the adolescents was 15.3 years. Female adolescents more frequently affirmed that they were satisfied with their dental appearance than did male individuals. There was an increase in dissatisfaction with oral health with the increase in anterior maxillary overjet, midline diastema, larger anterior irregularity in the maxilla, larger anterior irregularity in the mandible, anterior open bite and antero-posterior molar relation.     Individual And Contextual Factors Associated With Malocclusion In Brazilian Children

    Objective: To assess the association between the prevalence of malocclusion in Brazilian 12 years-olds with individual and contextual variables. Methods: A cross-sectional, analytical study was conducted with data from the Brazilian Oral Health Survey-SBBrazil 2010. The outcome studied was malocclusion, categorized as absent, set, severe and very severe. The independent variables were classifi ed as individual and contextual. Data were analyzed using a multilevel model with a 5% signifi cance level. Results: It was found that the prevalence of severe and very severe malocclusion in 12-year-olds did not differ between the Brazilian regions, although variation between the cities was signifi cant (p < 0.001). Male children (p = 0.033), those on lower income (p = 0.051), those who had visited a dentist (p = 0.009), with lower levels of satisfaction with mouth and teeth (p < 0.001) and embarrassed to smile (p < 0.001) had more severe malocclusion. The characteristics of the cities also affected the severity of malocclusion; cities with more families on social benefi ts per 1,000 inhabitants, with lower scores on the health care system performance index and lower gross domestic product per capita were signifi cantly associated with malocclusion.     Influence Of Socioeconomic Clinical And Demographic Variables On Caries Experience Of Preschool Children In Piracicaba, Sp

    AIM: To evaluate the influence of socioeconomic, clinical and demographic variables on caries experience in 5-year-old preschool children in Piracicaba. METHODOLOGY: The sample consisted of 728 children attending 22 public (n=428) and 18 private (n=300) preschools. Dental caries was measured using the dmft/dmfs indexes and by detecting initial caries lesion (IL). Gingivitis, crowding, spacing, fluorosis and mouth breathing were also recorded. The socioeconomic variables (monthly family income, number of people living in the household, parents' educational level, home ownership and car ownership) were collected by means of a semi structured questionnaire answered by the children's parents. RESULTS: The mean (standard deviation) values for dmft and dmfs indexes were 1.30 (2.47) and 3.08 (7.55) respectively; and 62.2% of the children were caries-free. The mean (standard deviation) values for dmft+LI and dmfs+LI were 1.72 (3.36) and 3.45 (7.94), respectively and 59.7% were caries-free. By means of multiple logistic regression analyses, children with fluorosis (OR=0.40) or family income > 4 minimum wages (OR=0.49) presented lower probabilility of having caries experience. Those with gingivitis (OR=1.87) were more prone to having the disease. Considering the diagnostic threshold including IL, the multiple logistic regression analyses showed that children with fluorosis (OR=0.39) or family income > 4 minimum wages (OR=0.52) were less prone to having caries. Those with gingivitis (OR=1.80), crowding (OR=2.63 e OR=1.01) or mouth breathing (OR=1.37) presented more probability of having the disease. CONCLUSION: The preschool children who presented gingivitis, crowding, mouth breathing or monthly family income £ 4 minimum wages had more probability of having caries experience.     Objective: to evaluate the actions of education on the oral health of adolescents within the scope of the Family Health Strategy. Methodology: A questionnaire was applied and a clinical exam conducted evaluating biofilm and gingival inflammation in 80 adolescents before and after interventions. A qualitative study of the brushing technique was also performed on 15 adolescents in the sample. Results: there was an improvement in oral hygiene of adolescents who participated in actions at school and an increase in the number of adolescents who had healthy gums or only mild inflammation in the second test in school groups and home visits. With respect to the consumption of candies, both the weekly group and the school group saw a decrease in candy consumption between the first and second tests. There was an increased frequency of brushing in the school group.     Evaluation Of An Educational Program For Children With High Risk Of Caries

    The goal of this study was to evaluate a 15-month educational program designed to children. The sample consisted of 60 six-year olds, randomly assigned into control and experimental group. The control consisted of tooth brushing training, once a year. The experimental group received intensive individual tooth brushing training every three months and guidance on oral health. Initially, both groups were assessed using plaque, gingival, dmfs and DMF-S indexes every three months. In the control, no statistically significant difference was observed for plaque and gingival indexes. The experimental group showed a statistically significant reduction in mean values for two indexes. The caries indexes showed no statistically significant difference. The proposed educational program developed was efficient in reducing gingival and plaque indexes as well caries incidence.31424625

    Objective: To evaluate the association between family social risk (FSR), individual oral risk and caries and periodontal disease risk and demographic variables of adults from a Family Health Unit (FHU) in the city of Santo André, SP, Brazil. Method: The sample consisted of 253 families and 324 adults, which corresponds to 16.4% of the families registered at the Jardim Santo Alberto FHU in Santo André, SP. The data of the families were collected using the Basic Attention Information System's A Form and transferred to the instrument "Family Risk Scoring System - Santo André", being classified as low, medium, high and extra high FSR. After this classification, all members of the families were asked to attend for an oral examination by the FHU's oral health team, being classified according to the caries and periodontal disease risk criteria established by the São Paulo State Health Secretary. Data were subjected to descriptive analysis, Chi-square or Fisher's exact test, and multiple logistic regressions. Results: The FSR was only associated with the microarea, and most individuals from the microarea 1 belonged to the high FSR classification. No significant association was found between caries risk and individual oral risk and the demographic variables. Women had 2.290 more chances than men of presenting low risk of periodontal disease.     Relationship Between Malocclusion And Behavioral, Demographic And Socioeconomic Variables: A Cross-sectional Study Of 5-year-olds.

    OBJECTIVE: Epidemiological studies have demonstrated that the prevalence and the severity of malocclusion have increased over the years. The aim of this study was to evaluate the relationship between malocclusions, such as open bite and crossbite, and behavioral, demographic and socioeconomic variables in a sample of 5-year-old children attending preschools. METHODS: The random sample consisted of 728 preschool children attending 22 public (n = 428) and 18 private (n = 300) preschools. A calibrated examiner performed the epidemiological examination outdoors, under natural light, using dental mirror and CPI probe. Information on socioeconomic status and the presence of deleterious oral habits was collected by a questionnaire sent to parents. Univariate and multiple logistic regression analyses were performed. RESULTS: The deleterious oral habits were observed in 83.1% of the children and were significantly associated with social class. Pacifier use (OR = 21.71), thumb sucking (OR = 4.72); atypical swallowing (OR = 7.35) and mouth breathing (OR = 4.65) were risk indicators for open bite. Pacifier use (OR = 1.59), lip interposition (OR = 1.78), mouth breathing (OR = 1.96) and gender (OR = 1.88) were risk indicators for crossbite. CONCLUSIONS: Environmental factors, such as the presence of deleterious oral habits as well as social class, play an important role in identifying children with open and/or crossbite.331757

    Variables Associated With The Performance Of Centers For Dental Specialties In Brazil [variáveis Associadas Ao Desempenho De Centros De Especialidades Odontológicas No Brasil]

    The aim of this study was to evaluate the performance of the Centers for Dental Specialties (CDS) in the country and associations with sociodemographic indicators of the municipalities, structural variables of services and primary health care organization in the years 2004–2009. The study used secondary data from procedures performed in the CDS to the specialties of periodontics, endodontics, surgery and primary care. Bivariate analysis by χ2 test was used to test the association between the dependent variable (performance of the CDS) with the independents. Then, Poisson regression analysis was performed. With regard to the overall achievement of targets, it was observed that the majority of CDS (69.25%) performance was considered poor/ regular. The independent factors associated with poor/regular performance of CDS were: municipalities belonging to the Northeast, South and Southeast regions, with lower Human Development Index (HDI), lower population density, and reduced time to deployment. HDI and population density are important for the performance of the CDS in Brazil.     Type II diabetes mellitus is a highly prevalent disease among the adult Brazilian population, and one that can be controlled by interventions such as physical activity, among others. The aim of this randomized controlled study was to evaluate the impact of a traditional motivational strategy, associated with the activation of intention theory, on adherence to physical activity in patients with type II, diabetes mellitus who are part of the Unified Health System (SUS). Participants were divided into a control group (CG) and an intervention group (IG). In both groups, the traditional motivational strategy was applied, but the activation of intention strategy was only applied to the IG Group. After a two-month follow- up, statistically significant differences were verified between the groups, related to the practice of walking (p = 0.0050), number of days per week (p = 0.0076), minutes per day (p = 0.0050) and minutes walking per week (p = 0.0015). At the end of the intervention, statistically significant differences in abdominal circumference (p = 0.0048) between the groups were observed.
