Feasibility of inguinal hernioplasty under local anaesthesia in elderly patients.
BACKGROUND: The aim of this study is to evaluate the feasibility and the safety of hernioplasty under local anaesthesia in elderly patients with significant comorbidity. METHODS: A total of 218 patients underwent inguinal hernia repair with mesh between June 2009 and July 2012. Presence of comorbid conditions and complications were compared between patients younger and older than 70 years. RESULTS: Hernia repair in older patients were more likely associated with comorbid conditions than in their younger counterparts ( hypertension: 25% vs 8.16%; cardiovascular diseases: 50% vs 22%; benign prostatic hypertrophy 60% vs 30%). The most common postoperative complications in both groups were recurrence, wound infection, urinary retention. There was a slightly higher rate of complication in elderly group. CONCLUSIONS: Inguinal hernia repair with local anaesthesia is quite safe and results in a good success rate in elderly patients despite a higher rate of comorbidity typical of this kind of patient
Non-malleable encryption: simpler, shorter, stronger
In a seminal paper, Dolev et al. [15] introduced the notion of non-malleable encryption (NM-CPA). This notion is very intriguing since it suffices for many applications of chosen-ciphertext secure encryption (IND-CCA), and, yet, can be generically built from semantically secure (IND-CPA) encryption, as was shown in the seminal works by Pass et al. [29] and by Choi et al. [9], the latter of which provided a black-box construction. In this paper we investigate three questions related to NM-CPA security: 1. Can the rate of the construction by Choi et al. of NM-CPA from IND-CPA be improved? 2. Is it possible to achieve multi-bit NM-CPA security more efficiently from a single-bit NM-CPA scheme than from IND-CPA? 3. Is there a notion stronger than NM-CPA that has natural applications and can be achieved from IND-CPA security? We answer all three questions in the positive. First, we improve the rate in the scheme of Choi et al. by a factor O(λ), where λ is the security parameter. Still, encrypting a message of size O(λ) would require ciphertext and keys of size O(λ2) times that of the IND-CPA scheme, even in our improved scheme. Therefore, we show a more efficient domain extension technique for building a λ-bit NM-CPA scheme from a single-bit NM-CPA scheme with keys and ciphertext of size O(λ) times that of the NM-CPA one-bit scheme. To achieve our goal, we define and construct a novel type of continuous non-malleable code (NMC), called secret-state NMC, as we show that standard continuous NMCs are not enough for the natural âencode-then-encrypt-bit-by-bitâ approach to work. Finally, we introduce a new security notion for public-key encryption that we dub non-malleability under (chosen-ciphertext) self-destruct attacks (NM-SDA). After showing that NM-SDA is a strict strengthening of NM-CPA and allows for more applications, we nevertheless show that both of our resultsâ(faster) construction from IND-CPA and domain extension from one-bit schemeâalso hold for our stronger NM-SDA security. Peripheral blood mono-nuclear cells implantation in patients with Peripheral Arterial Disease: a pilot study for clinical and biochemical outcome of neoangiogenesis.
BACKGROUND: Substantial progresses in the management of peripheral arterial disease (PAD) have been made in the past two decades. Progress in the understanding of the endothelial-platelet interaction during health and disease state has resulted in better antiplatelet drugs that can prevent platelet aggregation, activation and thrombosis during angioplasty and stenting. A role in physiological and pathological angiogenesis in adults has been recently shown in bone marrowâderived circulating endothelial progenitors (BM-DCEPs) identified in the peripheral blood. These findings have paved the way for the development of therapeutic neovascularization techniques using endothelial progenitors. METHODS: This pilot study includes five patients, aged 60 to 75, with a history of claudication and recruited from September 2010 to February 2011 at the A.O.U. Federico II of Naples. PBMNCs have been implanted three times in the limb with the worst ABI value in all the patients included in the study. The clinical follow up was performed during the subsequent 12 months from the beginning of the treatment. RESULTS: In four patients there was a regression of ulcerative lesions. One patientâs condition improved after the first implantation but later did not respond to the further treatments. All patients achieved a pain relief as judged by the numeric pain scale. Pain relief remained satisfactory in three patients for one year. Pain gradually returned to the pre-treatment level in two patients. All patients referred an ameliorating in their quality of life expressed even by an improvement in claudication free walking distance. These improvements are reflected also by intra-arterial digital subtraction angiography (IADSA) that shows an improvement of arterial vascularization. CONCLUSIONS: The data from this study suggest an efficacy of BM-DCEPs implantation in terms of improvement of the vascularization and quality of life in patients affected by Peripheral Arterial Disease. Nevertheless a double-blind placebo-controlled study is needed to confirm our findings
Quantum correlations in Newtonian space and time: arbitrarily fast communication or nonlocality
We investigate possible explanations of quantum correlations that satisfy the
principle of continuity, which states that everything propagates gradually and
continuously through space and time. In particular, following [J.D. Bancal et
al, Nature Physics 2012], we show that any combination of local common causes
and direct causes satisfying this principle, i.e. propagating at any finite
speed, leads to signalling. This is true even if the common and direct causes
are allowed to propagate at a supraluminal-but-finite speed defined in a
Newtonian-like privileged universal reference frame. Consequently, either there
is supraluminal communication or the conclusion that Nature is nonlocal (i.e.

Causality - Complexity - Consistency: Can Space-Time Be Based on Logic and Computation?
The difficulty of explaining non-local correlations in a fixed causal
structure sheds new light on the old debate on whether space and time are to be
seen as fundamental. Refraining from assuming space-time as given a priori has
a number of consequences. First, the usual definitions of randomness depend on
a causal structure and turn meaningless. So motivated, we propose an intrinsic,
physically motivated measure for the randomness of a string of bits: its length
minus its normalized work value, a quantity we closely relate to its Kolmogorov
complexity (the length of the shortest program making a universal Turing
machine output this string). We test this alternative concept of randomness for
the example of non-local correlations, and we end up with a reasoning that
leads to similar conclusions as in, but is conceptually more direct than, the
probabilistic view since only the outcomes of measurements that can actually
all be carried out together are put into relation to each other. In the same
context-free spirit, we connect the logical reversibility of an evolution to
the second law of thermodynamics and the arrow of time. Refining this, we end
up with a speculation on the emergence of a space-time structure on bit strings
in terms of data-compressibility relations. Finally, we show that logical
consistency, by which we replace the abandoned causality, it strictly weaker a

The Impact of the COVID-19 pandemic on outdoor physical activities for people with disabilities, including the risks for psychophysical well-being
The restrictions and social distancing measures due to the COVID-19 pandemic have created many obstacles in the practice of outdoor physical activity (OPA) throughout the world, particularly for the most vulnerable people, such as those with disabilities. The aim of this study was to explore the impact of pandemic-related limitations on the OPA of an Italian cohort of people with disabilities practicing sports. A retrospective observational study was conducted using an online survey. The questionnaire was distributed to 121 disabled athletes who practiced different outdoor physical activities. A total of 96 completed the survey, which collected demographic data, information about daily outdoor physical activity and sports habits, and about physical and psychological health before and during the pandemic. The frequency of daily OPA per week, along with the hours of physical activity, significantly decreased during the pandemic compared to those of the year before (p < 0.0001). A statistically significant deterioration was also found in the physical and mental well-being of disabled athletes during the pandemic (p < 0.0001) when compared to those from the year before the advent of COVID-19. This research demonstrated the negative impact of COVID-19 restrictions on OPA levels and on the physical and mental well-being of athletes with disabilities. It also highlighted a new challenge regarding the sustainability and integration of the national health system, demonstrating the necessity of improving the consistent accessibility of people with disabilities to OPA, both under normal conditions and emergency situations, in order to guarantee their psychophysical well-being
The non-euphoric phytocannabinoid cannabidivarin counteracts intestinal inflammation in mice and cytokine expression in biopsies from UC pediatric patients
Patients with ulcerative colitis (UC) using marijuana have been reported to experience symptomatic benefit. Cannabidivarin (CBDV) is a safe non-psychoactive phytocannabinoid able to activate and desensitize TRPA1, a member of the TRP channels superfamily, which plays a pivotal role in intestinal inflammation. Here, we have investigated the potential intestinal anti-inflammatory effect of CBDV in mice and in biopsies from pediatric patients with active UC. Colonic inflammation was induced in mice by dinitrobenzenesulfonic acid (DNBS). The effect of orally administered CBDV on macroscopic and microscopic damage, inflammatory parameters (i.e. myeloperoxidase activity, intestinal permeability and cytokine production) and faecal microbiota composition, was evaluated 3 days after DNBS administration. TRPA1 expression was studied by RT-PCR in inflamed colons of mice as well as in mucosal colonic biopsies of children with active UC, whose response to incubation with CBDV was also investigated. CBDV attenuates, in a TRPA1-antagonist sensitive manner, DNBS-induced signs of inflammation including neutrophil infiltration, intestinal permeability, and cytokine (i.e. IL-1\u3b2, IL-6 and the chemokine MCP-1) production. CBDV also alters the dysregulation of gut microbiota associated to colitis. Finally, CBDV lessens cytokine expression in colonic biopsies from pediatric patients with ulcerative colitis, a condition in which TRPA1 was up-regulated. Our preclinical study shows that CBDV exerts intestinal anti-inflammatory effects in mice via TRPA1, and in children with active UC. Since CBDV has a favorable safety profile in humans, it may be considered for possible clinical trials in patients with UC
The prevalence of diabetes and stress hyperglycemia in the acute myocardial infarction patients
OBJETIVOS: Determinar a prevalĂȘncia do diabetes melito (DM) e da hiperglicemia de estresse (HE) em pacientes com infarto agudo do miocĂĄrdio (IAM) admitidos em unidade de emergĂȘncia cardiolĂłgica. MĂTODOS: AnĂĄlise retrospectiva de 2.262 pacientes com IAM, avaliando, alĂ©m da prevalĂȘncia de diabetes referido, o diagnosticado e a hiperglicemia de estresse. RESULTADOS: Apesar de referido em 12,1% dos pacientes (H: 10,7%, M: 15,8%), o DM ocorria efetivamente em 24,8% (H: 22,9%, M: 29,7%) e a HE em 13,6% (H: 14,3%, M: 11,7%) dos indivĂduos dessa população. Portanto, alteraçÔes glicĂȘmicas ocorreram em 37,4% dos indivĂduos com IAM (H: 37,2%, M: 41,4%). Nos pacientes com DM, observou-se maior precocidade etĂĄria do IAM, maior prevalĂȘncia de Ăłbitos (DM: 20,7%, ND:13,8%, HE: 13,4%) e de procedimentos cirĂșrgicos (ND: 33,8%, HE: 18,0%, DM: 21,7%). CONCLUSĂO: A elevada prevalĂȘncia de DM e hiperglicemia de estresse observada em nosso estudo indica que as alteraçÔes glicĂȘmicas constituem um dos mais importantes fatores de risco para o IAM.OBJECTIVES: To evaluate in our population the real prevalence of diabetes (DM) and stress hyperglycemia (HE) in patients with myocardial infarction (IAM) admitted in a cardiologic emergency unit. METHODS: A retrospective analysis of 2262 patients with AMI evaluating the prevalence of DM (referred and diagnosed) and stress hyperglycemia. RESULTS: Besides 12,1% of subjects were previously referred to be diabetic (men: 10.7% and women: 15.8%), diabetes was effectively diagnosed in 24,8% (M: 22,9%, W: 29,7%) and stress hyperglycemia in 13,6% HE of the patients (M: 14,3%, W: 11,7%) indicating that glycemic alterations were effectively observed in 37.2.% of the patients with IAM (M: 37,2%, W: 41,4%). In DM subjects IAM events occurred earlier, total intra-hospital mortality was higher (DM: 20.7%, ND: 13,8%, HE: 13,4%) and less surgical procedures were performed (ND 33.8%, DM: 21.7%, HE: 18.0%). CONCLUSION: The elevated DM and stress hyperglycemia prevalence observed in our study indicates that glycemic alterations is one of the most important risk factors for IAM
Non-Malleable Codes, Extractors and Secret Sharing for Interleaved Tampering and Composition of Tampering
Non-malleable codes were introduced by Dziembowski, Pietrzak, and Wichs (JACM 2018) as a generalization of standard error correcting codes to handle severe forms of tampering on codewords. This notion has attracted a lot of recent research, resulting in various explicit constructions, which have found applications in tamper-resilient cryptography and connections to other pseudorandom objects in theoretical computer science.
We continue the line of investigation on explicit constructions of non-malleable codes in the information theoretic setting, and give explicit constructions for several new classes of tampering functions. These classes strictly generalize several previously studied classes of tampering functions, and in particular extend the well studied split-state model which is a ``compartmentalized model in the sense that the codeword is partitioned a prior into disjoint intervals for tampering. Specifically, we give explicit non-malleable codes for the following classes of tampering functions.
(1) Interleaved split-state tampering: Here the codeword is partitioned in an unknown way by an adversary, and then tampered with by a split-state tampering function.
(2) Affine tampering composed with split-state tampering: In this model, the codeword is first tampered with by a split-state adversary, and then the whole tampered codeword is further tampered with by an affine function. In fact our results are stronger, and we can handle affine tampering composed with interleaved split-state tampering.
Our results are the first explicit constructions of non-malleable codes in any of these tampering models. As applications, they also directly give non-malleable secret sharing schemes with binary shares in the split-state joint tampering model and the stronger model of affine tampering composed with split-state joint tampering. We derive all these results from explicit constructions of seedless non-malleable extractors, which we believe are of independent interest.
Using our techniques, we also give an improved seedless extractor for an unknown interleaving of two independent sources
ESPRESSO: The next European exoplanet hunter
The acronym ESPRESSO stems for Echelle SPectrograph for Rocky Exoplanets and
Stable Spectroscopic Observations; this instrument will be the next VLT high
resolution spectrograph. The spectrograph will be installed at the
Combined-Coud\'e Laboratory of the VLT and linked to the four 8.2 m Unit
Telescopes (UT) through four optical Coud\'e trains. ESPRESSO will combine
efficiency and extreme spectroscopic precision. ESPRESSO is foreseen to achieve
a gain of two magnitudes with respect to its predecessor HARPS, and to improve
the instrumental radial-velocity precision to reach the 10 cm/s level. It can
be operated either with a single UT or with up to four UTs, enabling an
additional gain in the latter mode. The incoherent combination of four
telescopes and the extreme precision requirements called for many innovative
design solutions while ensuring the technical heritage of the successful HARPS
experience. ESPRESSO will allow to explore new frontiers in most domains of
astrophysics that require precision and sensitivity. The main scientific
drivers are the search and characterization of rocky exoplanets in the
habitable zone of quiet, nearby G to M-dwarfs and the analysis of the
variability of fundamental physical constants. The project passed the final
design review in May 2013 and entered the manufacturing phase. ESPRESSO will be
installed at the Paranal Observatory in 2016 and its operation is planned to
