    Through the eyes of others - The social experiences of people with dementia: A systematic literature review and synthesis

    Psychosocial models suggest that the lived experience of dementia is affected by interpersonal factors such as the ways in which others view, talk about, and behave toward the person with dementia. This review aimed to illuminate how informal, everyday interpersonal relationships are experienced by people with dementia within their social contexts. A systematic review of qualitative literature published between 1989 and May 2016 was conducted, utilizing the electronic databases PsycINFO, MEDLINE, and CINAHL-Complete. This was followed by a critical interpretative synthesis to understand how people with dementia perceive the attitudes, views, and reactions of other people toward them, and the subjective impact that these have. Four major themes were derived from the findings of the 23 included studies: being treated as an “other” rather than “one of us”; being treated as “lesser” rather than a full, valued member of society; the impact of others’ responses; and strategies to manage the responses of others. Thus, people with dementia can feel outcast and relegated, or indeed feel included and valued by others. These experiences impact upon emotional and psychological well-being, and are actively interpreted and managed by people with dementia. Experiences such as loss and diminishing identity have previously been understood as a direct result of dementia, with little consideration of interpersonal influences. This review notes that people with dementia actively engage with others, whose responses can foster or undermine social well-being. This dynamic relational aspect may contribute to emerging understandings of social health in dementia

    Electronic Health Record Availability and Anxiety Treatment in Office Based Practices

    Objective: This study compared the probability of receiving anxiety treatment during a physician visit to primary care practices with and without an electronic health record (EHR). Methods: The 2007–2010 National Ambulatory Medical Care Survey was used to identify visits for anxiety (N=290). The outcome was receipt of anxiety treatment. The independent variable was the presence of a fully functioning EHR. Logistic regression was used to conduct the analysis. Results: Patients who were seen in practices with a fully functioning EHR had lower odds of being offered antianxiety medication (adjusted odds ratio [AOR]=.37, 95% confidence interval [CI]=.15–.90, p=.028), mental health counseling (AOR=.43, CI=.18–1.04, p=.061), and any anxiety treatment (AOR=.40, CI=.15–1.05, p=.062) compared with patients at practices without a fully functioning EHR. Conclusions: EHRs may have a negative impact on the delivery of care for anxiety during primary care visits. Future studies should monitor the impact of EHRs on delivery and quality of care

    Sequential measurement of δ15N, δ13C and δ34S values in archaeological bone collagen at the Scottish Universities Environmental Research Centre (SUERC): a new analytical frontier

    Rationale: The use of multi‐isotopic analysis (δ15N, δ13C and δ34S values) of archaeological bone collagen to assist in the interpretation of diet, movement and mobility of prehistoric populations is gradually increasing, yet many researchers have traditionally avoided investigating sulphur due to its very low concentrations (<0.3%) in mammalian collagen. For this reason, and as a consequence of analytical detection limits, sulphur is usually measured separately from carbon and nitrogen, which leads to longer analytical times and higher costs. Methods: A Thermo ScientificTM EA IsoLinkTM IRMS system, with the ability to rapidly heat a gas‐chromatography (GC) column and concentrate the sample gas online without cryo‐trapping, was used at the Radiocarbon Laboratory at SUERC. Optimisation of the GC temperature and carrier gas flow rate in the elemental analyser resulted in improved signal‐to‐noise ratio and sensitivity for SO2. This allowed for routine sequential N2, CO2 and SO2 measurements on small samples of bone collagen. Results: Improvements in sample gas transfer to the mass spectrometer allows for sequential δ15N, δ13C and δ34S values to be measured in 1–1.5 mg samples of bone collagen. Moreover, the sensitivity and signal‐to‐noise ratio of the sample gas, especially SO2, is improved, resulting in precisions of ±0.15‰ for δ15N values, ±0.1‰ for δ13C values and ±0.3‰ for δ34S values. Previous instrumentation allowed for the analysis of ~30 unknown samples before undertaking maintenance; however, ~150 unknown samples can now be measured, meaning a 5‐fold increase in sample throughput. Conclusions: The ability to sequentially measure δ15N, δ13C and δ34S values rapidly in archaeological bone collagen is an attractive option to researchers who want to build larger, more succinct datasets for their sites of interest, at a much‐reduced analytical cost and without destroying larger quantities of archaeological material

    Genetic Relationships in the Toxin-Producing Fungal Endophyte, \u3ci\u3eAlternaria oxytropis\u3c/i\u3e Using Polyketide Synthase and Non-Ribosomal Peptide Synthase Genes

    The legume Oxytropis sericea hosts a fungal endophyte, Alternaria oxytropis, which produces secondary metabolites (SM), including the toxin swainsonine. Polyketide synthase (PKS) and non-ribosomal peptide synthase (NRPS) enzymes are associated with biosynthesis of fungal SM. To better understand the origins of the SM, an unannotated genome of A. oxytropis was assessed for protein sequences similar to known PKS and NRPS enzymes of fungi. Contigs exhibiting identity with known genes were analyzed at nucleotide and protein levels using available databases. Software were used to identify PKS and NRPS domains and predict identity and function. Confirmation of sequence for selected gene sequences was accomplished using PCR. Thirteen PKS, 5 NRPS, and 4 PKS-NRPS hybrids were identified and characterized with functions including swainsonine and melanin biosynthesis. Phylogenetic relationships among closest amino acid matches with Alternaria spp. were identified for seven highly conserved PKS and NRPS, including melanin synthesis. Three PKS and NRPS were most closely related to other fungi within the Pleosporaceae family, while five PKS and PKS-NRPS were closely related to fungi in the Pleosporales order. However, seven PKS and PKS-NRPS showed no identity with fungi in the Pleosporales or the class Dothideomycetes, suggesting a different evolutionary origin for those genes

    Using technology to improve human performance is critical to maximizing the benefits of future combat systems. This study explores the effects of interruptions during high and low cognitive load states when completing dynamic tasks. Furthermore, it provides insight into how to integrate artificial intelligence and virtual assistants into future aircraft effectively. This research and the following analysis provided the Holistic Situation Awareness and Decision Making (HSA-DM) program office with meaningful data and recommendations that will enable them to reduce the impact of interruptions while improving the performance of future pilots. Specifically, this study collected and examined heart rate variability, subjective cognitive load, flight metrics, interruption lag, and task resumption lag while participants piloted an aircraft and performed dynamic tasks in a flight simulator. There were three different modalities used to assist participants with completing interrupted tasks while performing their primary task.     Evidence for Distinct Components of the Galactic Stellar Halo from 838 RR Lyrae Stars Discovered in the LONEOS-I Survey

    We present 838 ab-type RR Lyrae stars from the Lowell Observatory Near Earth Objects Survey Phase I (LONEOS-I). These objects cover 1430 deg^2 and span distances ranging from 3-30 kpc from the Galactic Center. Object selection is based on phased, photometric data with 28-50 epochs. We use this large sample to explore the bulk properties of the stellar halo, including the spatial distribution. The period-amplitude distribution of this sample shows that the majority of these RR Lyrae stars resemble Oosterhoff type I, but there is a significant fraction (26 %) which have longer periods and appear to be Oosterhoff type II. We find that the radial distributions of these two populations have significantly different profiles (rho_{OoI} ~ R^(-2.26 +- 0.07) and rho_{OoII} ~ R^(-2.88 +- 0.11). This suggests that the stellar halo was formed by at least two distinct accretion processes and supports dual-halo models.Comment: 18 pages, 28 figures, apjemulated, minor corrections and clarifications.
