    Human aquaporins: regulators of transcellular water flow

    Background: Emerging evidence supports the view that (AQP) aquaporin water channels are regulators of transcellular water flow. Consistentwith their expression in most tissues, AQPs are associatedwith diverse physiological and pathophysiological processes. Scope of review: AQP knockout studies suggest that the regulatory role of AQPs, rather than their action as passive channels, is their critical function. Transport through all AQPs occurs by a common passive mechanism, but their regulation and cellular distribution varies significantly depending on cell and tissue type; the role of AQPs in cell volumeregulation (CVR) is particularly notable. This reviewexamines the regulatory role of AQPs in transcellular water flow, especially in CVR.We focus on key systems of the human body, encompassing processes as diverse as urine concentration in the kidney to clearance of brain oedema. Major conclusions: AQPs are crucial for the regulation of water homeostasis, providing selective pores for the rapidmovement ofwater across diverse cellmembranes and playing regulatory roles in CVR. Gatingmechanisms have been proposed for human AQPs, but have only been reported for plant andmicrobial AQPs. Consequently, it is likely that the distribution and abundance of AQPs in a particular membrane is the determinant of membrane water permeability and a regulator of transcellular water flow. General significance: Elucidating the mechanisms that regulate transcellular water flow will improve our understanding of the human body in health and disease. The central role of specific AQPs in regulating water homeostasis will provide routes to a range of novel therapies. This article is part of a Special Issue entitled Aquaporins

    Costs of food safety certification on fresh produce farms in Vermont

    This article addresses the economic costs of good agricultural practices (GAPs) audits of small and medium size farms in Vermont. It focuses on the costs of infrastructure, equipment, and labor required to successfully pass a U.S. Department of Agriculture (USDA) GAPs audit. In-depth interviews and surveys of produce farmers in 2011 revealed that the cost of GAPs certification ranges between 37and37 and 54 per acre, and an additional 7 hours were required each week during the growing season. Based on this exploratory research, certifying all the farms in Vermont would cost between 228,216and228,216 and 3,019,114. Our study explored all the criteria of the certification and measured the costs of GAPs from planning stages to daily record keeping more than one year after the certification was achieved. This study provides information to farmers who are considering GAPs certification. It also provides background information to agricultural service providers and policymakers planning for the future of the fresh produce industry

    Anchoring Communities Throughout COVID-19: Results from the 2020 Healthy Food in Health Care Survey in New England.

    Health Care Without Harm has been surveying their network of facilities since 2009. In 2020 the effort was expanded to include the full population health care facilities throughout New England. This new iteration of the Healthy Food in Health Care Survey focused on hospitals\u27 foodservice operations response to COVID-19 and contributions as anchor institutions in their communities. To supplement the survey results, three in-depth case studies demonstrate various experiences throughout the pandemic and highlight the motivations and ways these facilities successfully navigated the incredible strain put on the health care system while still being an essential community resource. The finding suggests that health care facilities played an important role in supporting their communities beyond procurement by establishing new initiatives that promoted food access and local producers. Based on the survey and case-study findings, recommendations and key takeaways for stakeholders hoping to increase their engagement as an anchor institution were developed. These recommendations highlight the resources used to support food access initiatives throughout COVID-19 and further develop relationships with local producers

    Survival of the Fattest: How Body Fat and Migration Influence Survival in Highly Seasonal Environments

    1. Energy stores and migration are important adaptations for animals in seasonal environments, but their roles may vary relative to an animal\u27s endogenous and exogenous environment. In partially migratory populations, migrants and residents experience different seasonal environments; thus, the influence of energy stores on survival may differ relative to migratory tactic, with potential consequences to survival and fitness. 2. Using data from Sierra Nevada bighorn sheep (Ovis canadensis sierrae; hereafter, Sierra bighorn), we tested the hypothesis that body fat (energy stores) buffers animals against their environment, but that buffering capacity differs across environments experienced by high-elevation residents (using a single range year round), traditional migrants (making 1 round-trip movement between high- and low-elevation ranges during winter) and vacillating migrants (making ≥2 round trips between high- and low-elevation ranges during winter). We predicted that: for animals with high levels of body fat, survival would be high regardless of migratory tactic; residents would require larger stores of body fat to survive than migrants; energy stores would be least influential to survival for vacillating migrants. 3. High levels of body fat in autumn (≥14% for females and ≥19% for males) largely buffered animals against harsh environments (survival \u3e0.90) regardless of migratory tactic. At lower levels of body fat, traditional migrants had higher survival than residents. Vacillating migrants exhibited nearly 100% survival with no detectable effect of body fat on survival. 4. Collectively, these results support the hypothesis that body fat buffers animals against harsh environments but that the buffering capacity differed relative to the environment and highly flexible behaviours (i.e. vacillating migration) can allow animals to decouple survival from body fat. 5. Our work reveals that synergies between physiological and behavioural adaptations of animals in highly seasonal environments carry potential fitness consequences for individuals and demographic consequences for populations

    Testing a workplace physical activity intervention: a cluster randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Increased physical activity levels benefit both an individuals' health and productivity at work. The purpose of the current study was to explore the impact and cost-effectiveness of a workplace physical activity intervention designed to increase physical activity levels.</p> <p>Methods</p> <p>A total of 1260 participants from 44 UK worksites (based within 5 organizations) were recruited to a cluster randomized controlled trial with worksites randomly allocated to an intervention or control condition. Measurement of physical activity and other variables occurred at baseline, and at 0 months, 3 months and 9 months post-intervention. Health outcomes were measured during a 30 minute health check conducted in worksites at baseline and 9 months post intervention. The intervention consisted of a 3 month tool-kit of activities targeting components of the Theory of Planned Behavior, delivered in-house by nominated facilitators. Self-reported physical activity (measured using the IPAQ short-form) and health outcomes were assessed.</p> <p>Results and discussion</p> <p>Multilevel modelling found no significant effect of the intervention on MET minutes of activity (from the IPAQ) at any of the follow-up time points controlling for baseline activity. However, the intervention did significantly reduce systolic blood pressure (B = -1.79 mm/Hg) and resting heart rate (B = -2.08 beats) and significantly increased body mass index (B = .18 units) compared to control. The intervention was found not to be cost-effective, however the substantial variability round this estimate suggested that further research is warranted.</p> <p>Conclusions</p> <p>The current study found mixed support for this worksite physical activity intervention.     The role of context and timeframe in moderating relationships within the theory of planned behaviour

    This study examined the moderating effect of context and timeframe on the predictive ability of Theory of Planned Behaviour (TPB) constructs. Three hundred and eighty-three students completed TPB measures either in a campus bar or a library and were randomly allocated to one of three timeframe conditions: tonight, tomorrow or next week. There was a threeway interaction such that the subjective norms of participants in a bar were more predictive of their intentions to binge drink that night, whereas thesubjective norms of participants in a library were less predictive of intentions to binge drink that night. This research provides empirical evidence that ignoring context may result in underestimation of the importance of normative factors in binge drinking. It also suggests that other research utilising the TPB needs to take greater account of the impact of context of data collection, which has been neglected to date

    The association between care co-ordination and emergency department use in older managed care enrollees

    OBJECTIVE: To investigate the association between care co-ordination and use of the Emergency Department (ED) in older managed care enrollees. DESIGN: Nested case-control with 103 cases (used the ED) and 194 controls (did not use the ED). PATIENTS AND METHODS: Older patients with multiple chronic illnesses enrolled in a care management programme of a large group-model health maintenance organisation with more than 50,000 members over the age of 64. Better care co-ordination was defined as timely follow-up after a change in treatment; fewer decision-makers involved with the care plan; and a higher patient-perceived rating of overall care co-ordination. Logistic regression was used to assess the relationship between ED use (the outcome variable) and measures of care co-ordination (the predictor variables). RESULTS: Self-reported care co-ordination was not significantly different between cases and controls for any of the four classifications of inappropriate ED use. Similarly, no differences were found in the number of different physicians or medication prescribers involved in the patients' care. Four-week follow-up after potentially high-risk events for subsequent ED use, including changes in chronic disease medications, missed encounters, and same day encounters, did not differ between subjects with inappropriate ED use and controls. CONCLUSION: Existing measures of care co-ordination were not associated with inappropriate ED use in this study of older adults with complex care needs. The absence of an association may, in part, be attributable to the paucity of validated measures to assess care co-ordination, as well as the methodological complexity inherent in studying this topic. Future research should focus on the development of new measures and on approaches that better isolate the role of care co-ordination from other potential variables that influence utilisation
