    The amino-terminal fragment of pro-brain natriuretic peptide in plasma as a biological marker for predicting mortality in community-acquired pneumonia: a cohort study

    Abstract Aim: Community-acquired pneumonia (CAP) is an infectious disease that causes the highest mortality rates in developed countries. The primary endpoint of this study was to evaluate the relationship between the plasma concentration of the amino-terminal fragment of pro-brain natriuretic peptide (NT-ProBNP) at the time of CAP diagnosis in a hospital emergency room (HER) and its severity, determined as mortality at 30 days. Materials and Methods: A prospective, observational cohort study was used to determine NT-ProBNP (ng/L) in patients with CAP, with a follow-up over 30 days and analysis of the mortality rate. Results: A total of 338 patients were assessed. Thirty patients died within the first 30 days (10.5%). The mean NT-ProBNP values in the deceased patients were 14,035 ng/L (SD: 19,271) compared to 1,711 ng/L (SD: 3,835) in survivors (p<0.0001). The cut-off point of 1,769 ng/L showed a negative predictive value (NPV) of 95.3%, whereas 10,808 ng/L showed a positive predictive value (PPV) of 73.3%. The diagnostic performance of NT-ProBNP reached an AUC of 0.783 (95%CI: 0.731–0.829). Entering the potential confounding variables in a logistic regression model revealed that NT-ProBNP behaved like an independent risk factor. Grouping the NT-ProBNP values by every 300, 500, 1,000, and 2,000 ng/L increased the risk of mortality at 30 days by 3%, 5.1%, 10.5%, and 22%, respectively. Conclusion: The NT-ProBNP values at the time of CAP diagnosis are significantly higher among patients that die than those that survive the first 30 days, and it could be a good predictor of early mortality. Private bankers need to have deep knowledge about an innumerable list of products and their characteristics as well as the suitability of each product for the client’s characteristics to be able to offer an optimal portfolio according to client expectations. Client and portfolio diversity calls for new recommendation and advice systems focused on their specific characteristics. This paper presents PB-ADVISOR, a system aimed at recommending investment portfolios based on fuzzy and semantic technologies to private bankers. The proposed system provides private bankers with a powerful tool to support their decision process and help deal with complex investment portfolios.     SABUMO: towards a collaborative and semantic framework for knowledge sharing

    Knowledge is one the most valuable assets in todays companies. Artificial Intelligence (AI) techniques aim to represent the knowledge in a way that can be applied to solving complex problems or supporting decision making processes. During the last years the semantic web techniques have supported the share of knowledge by means of ontologies. Ontologies have been used for both annotating resources and rea soning. Business Process Management is one of the specific fields in which the use of ontologies has been widely applied. This paper presents SABUMO, a framework based on ontologies that allows experts to represent and share their knowledge with other experts by means of shared and controlled vocabularies. The framework also allows the execution of business processes represented by experts. The execution of this knowledge does not require the installation of complex AI programs. Initial results of the evaluation setup show promising results both in usability and recommendation.Publicad

    SINVLIO: using semantics and fuzzy logic to provide individual investment portfolio recommendations

    Portfolio selection addresses the problem of how to diversify investments in the most efficient and profitable way possible. Portfolio selection is a field of study that has been broached from several perspectives, including, among others, recommender systems. This paper presents SINVLIO (Semantic INVestment portfoLIO), a tool based on semantic technologies and fuzzy logic techniques that recommends investments grounded in both psychological aspects of the investor and traditional financial parameters of the investments.     Neural network and classification approach in identifying customer behavior in the banking sector: A case study of an international bank

    The customer relationship focus for banks is in development of main competencies and strategies of building strong profitable customer relationships through considering and managing the customer impression, influence on the culture of the bank, satisfactory treatment, and assessment of valued relationship building. Artificial neural networks (ANNs) are used after data segmentation and classification, where the designed model register records into two class sets, that is, the training and testing sets. ANN predicts new customer behavior from previously observed customer behavior after executing the process of learning from existing data. This article proposes an ANN model, which is developed using a six-step procedure. The back-propagation algorithm is used to train the ANN by adjusting its weights to minimize the difference between the current ANN output and the desired output. An evaluation process is conducted to determine whether the ANN has learned how to perform. The training process is halted periodically, and its performance is tested until an acceptable result is obtained.     Leishmaniasis represents a group of parasitic diseases caused by a protozoan of the genus Leishmania and is widely distributed in tropical and subtropical regions. Leishmaniasis is one of the major tropical neglected diseases, with 1.5 to 2 million new cases occurring annually. Diagnosis remains a challenge despite advances in parasitological, serological, and molecular methods. Dogs are an important host for the parasite and develop both visceral and cutaneous lesions. Our goal was to contribute to the diagnosis of canine cutaneous leishmaniasis (CL) and visceral leishmaniasis (VL) using the recombinant cysteine proteinase B (F-CPB) from Leishmania braziliensis and its N- and C-terminal domains (N-CPB and C-CPB) as antigens in an enzyme-linked immunosorbent assay (ELISA). Sera from dogs from Northwest Argentina diagnosed with CL were tested by ELISA against a supernatant of L. braziliensis lysate, the F-CPB protein, and its domains. We found values of sensitivity (Se) of 90.7%, 94.4%, and 94.3% and specificity (Sp) of 95.5%, 90.9%, and 91.3% for F-CPB and its N- and C-terminal domains, respectively. In sera from dogs diagnosed with VL from Northeast Argentina, we found Se of 93.3%, 73.3%, and 66.7% and Sp of 92.3%, 76.9%, and 88.5% for F-CPB and its N- and C-terminal domains, respectively. These results support CPB as a relevant antigen for canine leishmaniasis diagnosis in its different clinical presentations. More interestingly, the amino acid sequence of CPB showed high percentages of identity in several Leishmania species, suggesting that the CPB from L. braziliensis qualifies as a good antigen for the diagnosis of leishmaniasis caused by different species.Fil: Bivona, Augusto Ernesto.
