Relationship between bone turnover and left ventricular function in primary hyperparathyroidism: The EPATH trial
Observational studies suggested a link between bone disease and left ventricular (LV) dysfunction
that may be pronounced in hyperparathyroid conditions. We therefore aimed to test
the hypothesis that circulating markers of bone turnover correlate with LV function in a
cohort of patients with primary hyperparathyroidism (pHPT). Cross-sectional data of 155
subjects with pHPT were analyzed who participated in the \uaaEplerenone in Primary Hyperparathyroidism
\uba (EPATH) Trial. Multivariate linear regression analyses with LV ejection fraction
(LVEF, systolic function) or peak early transmitral filling velocity (e', diastolic function)
as dependent variables and N-terminal propeptide of procollagen type 1 (P1NP), osteocalcin
(OC), bone-specific alkaline phosphatase (BALP), or beta-crosslaps (CTX) as the
respective independent variable were performed. Analyses were additionally adjusted for
plasma parathyroid hormone, plasma calcium, age, sex, HbA1c, body mass index, mean
24-hours systolic blood pressure, smoking status, estimated glomerular filtration rate, antihypertensive
treatment, osteoporosis treatment, 25-hydroxy vitamin D and N-terminal probrain
B-type natriuretic peptide. Independent relationships were observed between P1NP
and LVEF (adjusted \u3b2-coefficient = 0.201, P = 0.035) and e' (\u3b2 = 0.188, P = 0.042), respectively.
OC (\u3b2 = 0.192, P = 0.039) and BALP (\u3b2 = 0.198, P = 0.030) were each independently
related with e'. CTX showed no correlations with LVEF or e'. In conclusion, high bone formation
markers were independently and paradoxically related with better LV diastolic and,
partly, better systolic function, in the setting of pHPT. Potentially cardio-protective properties of stimulated bone formation in the context of hyperparathyroidism should be explored in
future studies
Effect of eplerenone on parathyroid hormone levels in patients with primary hyperparathyroidism: a randomized, double-blind, placebo-controlled trial
<p>Abstract</p> <p>Background</p> <p>Increasing evidence suggests the bidirectional interplay between parathyroid hormone and aldosterone as an important mechanism behind the increased risk of cardiovascular damage and bone disease observed in primary hyperparathyroidism. Our primary object is to assess the efficacy of the mineralocorticoid receptor-blocker eplerenone to reduce parathyroid hormone secretion in patients with parathyroid hormone excess.</p> <p>Methods/design</p> <p>Overall, 110 adult male and female patients with primary hyperparathyroidism will be randomly assigned to eplerenone (25 mg once daily for 4 weeks and 4 weeks with 50 mg once daily after dose titration] or placebo, over eight weeks. Each participant will undergo detailed clinical assessment, including anthropometric evaluation, 24-h ambulatory arterial blood pressure monitoring, echocardiography, kidney function and detailed laboratory determination of biomarkers of bone metabolism and cardiovascular disease.</p> <p>The study comprises the following exploratory endpoints: mean change from baseline to week eight in (1) parathyroid hormone(1–84) as the primary endpoint and (2) 24-h systolic and diastolic ambulatory blood pressure levels, NT-pro-BNP, biomarkers of bone metabolism, 24-h urinary protein/albumin excretion and echocardiographic parameters reflecting systolic and diastolic function as well as cardiac dimensions, as secondary endpoints.</p> <p>Discussion</p> <p>In view of the reciprocal interaction between aldosterone and parathyroid hormone and the potentially ensuing target organ damage, the EPATH trial is designed to determine whether eplerenone, compared to placebo, will effectively impact on parathyroid hormone secretion and improve cardiovascular, renal and bone health in patients with primary hyperparathyroidism.</p> <p>Trial registration</p> <p>ISRCTN33941607</p
DXA-Derived Indices in the Characterisation of Sarcopenia
Sarcopenia is linked with increased risk of falls, osteoporosis and mortality. No consensus exists about a gold standard “dual-energy X-ray absorptiometry (DXA) index for muscle mass determination” in sarcopenia diagnosis. Thus, many indices exist, but data on sarcopenia diagnosis agreement are scarce. Regarding sarcopenia diagnosis reliability, the impact of influencing factors on sarcopenia prevalence, diagnosis agreement and reliability are almost completely missing. For nine DXA-derived muscle mass indices, we aimed to evaluate sarcopenia prevalence, diagnosis agreement and diagnosis reliability, and investigate the effects of underlying parameters, presence or type of adjustment and cut-off values on all three outcomes. The indices were analysed in the BioPersMed cohort (58 ± 9 years), including 1022 asymptomatic subjects at moderate cardiovascular risk. DXA data from 792 baselines and 684 follow-up measurements (for diagnosis agreement and reliability determination) were available. Depending on the index and cut-off values, sarcopenia prevalence varied from 0.6 to 36.3%. Height-adjusted parameters, independent of underlying parameters, showed a relatively high level of diagnosis agreement, whereas unadjusted and adjusted indices showed low diagnosis agreement. The adjustment type defines which individuals are recognised as sarcopenic in terms of BMI and sex. The investigated indices showed comparable diagnosis reliability in follow-up examination
Expression Profiles of miR-22-5p and miR-142-3p Indicate Hashimoto’s Disease and Are related to Thyroid Antibodies
Hashimoto’s thyroiditis (HT) is the most prevalent autoimmune disorder of the thyroid (AITD) and characterized by the presence of circulating autoantibodies evoked by a, to date, not fully understood dysregulation of the immune system. Autoreactive lymphocytes and inflammatory processes in the thyroid gland can impair or enhance thyroid hormone secretion. MicroRNAs (miRNAs) are small noncoding RNAs, which can play a pivotal role in immune functions and the development of autoimmunity. The aim of the present study was to evaluate whether the expression of 9 selected miRNAs related to immunological functions differ in patients with HT compared to healthy controls. MiRNA profiles were analysed using quantitative reverse transcription polymerase chain reaction (qRT-PCR) in 24 patients with HT and 17 healthy controls. Systemic expressions of miR-21-5p, miR-22-3p, miR-22-5p, miR-142-3p, miR-146a-5p, miR-301-3p and miR-451 were significantly upregulated in patients with HT (p ≤ 0.01) and were suitable to discriminate between HT and healthy controls in AUC analysis. Altered expressions of miR-22-5p and miR-142-3p were associated with higher levels of thyroid antibodies, suggesting their contribution to the pathogenesis of HT

Landscapes of return: Italian-Canadian writing published in Italian by Cosmo Iannone Editore
Drawing on the work of Arjun Appadurai (1996), this article discusses recenttranslations into Italian of Italian-Canadian writing as part of a wider ‘‘landscape’’, centred mainly on the idea of return to the homeland. This landscape consists not only of powerful discourses circulating in Italian cultural institutions and guiding the selection and marketing of the translations of this post-migrant literature, but also of discourses employed by de-territorialized ethnoscapes of Italians in Canada. By presenting paratextual and textual examples relating to these translations, published mainly by Cosmo Iannone Editore in the first decade of the twenty-first century, this article shows how the landscapes of return constructed in Italy differ from those constructed by English-Canadian and French-Canadian publishing houses. Landscapes representing a failed attempt to metaphorically return to the ancestral homeland in Italian-Canadian writing are substituted, in Italy, for landscapes of a nostalgic return home of the migrants to a point prior to their migration
