    Transcatheter Aortic Valve Implantation (TAVI) Planning with Dual-Layer Spectral CT Using Virtual Monoenergetic Image (VMI) Reconstructions and 20 mL of Contrast Media

    Transcatheter aortic valve implantation (TAVI) is a less invasive alternative to surgical implantation and its implementation is progressively increasing worldwide. We routinely perform pre-procedural aortic angiography CT to assess aortic dimensions and vascular anatomy. This study aims to evaluate the image quality of CTA for TAVI planning using dual-layer spectral CT, with virtual monoenergetic image reconstructions at 40 keV. Thirty-one patients underwent a CTA protocol with the injection of 20 mL of contrast media. Image quality was assessed by measuring the mean density in Hounsfield Units (HU), the signal-to-noise ratio, and the contrast-to-noise ratio in VMI reconstructions. Additionally, a blinded subjective analysis was conducted by two observers. The results showed significant enhancement at all sampled vascular levels with a gradual decrease in HU from proximal to distal regions. Favourable subjective ratings were given for all parameters, with greater variability in the evaluation of iliac axes. A significant negative correlation (p < 0.05) was observed between BMI and CA at all vascular levels, indicating reduced contrast enhancement with increasing BMI. Spectral CT, along with reducing iodine load, allows for obtaining high-quality images without a significant increase in noise. The reduction in iodine load can have positive implications in clinical practice, improving patient safety and resource efficiency

    The present economic and financial crisis is taking place at the climax of a period in which mainstream economic theory, on the one hand, and global financial liberalization on the other, had given credit once again to the self-regulatory ability of markets, which also seemed to find confirmation in the results of the "Great moderation" (high growth rate and low inflation rate) in spite of some worrying aspects (enormous indebtedness of the USA, frequent "speculative bubbles").;With the present-day return to Keynesian remedies (but without "paying for it" at a theoretical level), the paper reflects on the debate that developed in the 1970s and 1980s in the presence of an equally important financial crisis, albeit in a different form and in a context of discontinuity at the theoretical level (the neo-monetarist counter-revolution).;Fausto Vicarelli gave a fundamental theoretical and interpretative contribution to that debate by tackling questions such as the connections between finance and accumulation, the efficiency and stability of financial systems and, more generally, market vs. state. In the belief that the evolution of theory is not unidirectional, because it is re-proposed with some U-turns and that the evolution of an economy has unequivocal foundations, though in different forms, this study aims to consider these contributions critically to recover important ideas for today, especially on the question of overcoming the autonomization of finance and unidirectional decisions in favour of market efficiency.banks, economic growth, financial crises, financial markets, institutions

    I governi europei stanno discutendo due importanti riforme ed entrambe, per come al momento sono strutturate, possono avere per l’Italia conseguenze molto gravi. Quella che è giunta alle ultime battute e dovrebbe essere approvata entro breve riguarda l’European Stability Mechanism (Esm), il cosiddetto Fondo salva Stati. Questo Fondo, istituito nel settembre del 2012, dovrebbe intervenire in soccorso degli Stati che si trovassero in grave difficoltà. L’aiuto agli Stati in linea con i parametri stabiliti dalle regole del Fondo non richiede particolari requisiti, mentre per quelli non in linea è previsto solo a patto di pesanti condizionalità, tra le quali giudizi sulla sostenibilità del debito e sulla capacità di rimborsarlo, in seguito ai quali può essere richiesta allo Stato in questione una ristrutturazione del debito
