Efectes de la eritropeitina i el ferro en la reducció de la mortalitat en cirurgia cardÃaca valvular en pacients anèmics
INTRODUCCIÓ: L'anèmia en cirurgia valvular té pitjor evolució. L'objectiu va ser avaluar si corregint-la podria millorar el pronòstic. MÈTODES: Comparació d'una cohort tractada amb eritropoietina i ferro abans de la cirurgia amb una cohort històrica. RESULTATS: No hi va haver diferències entre els dos grups excepte en l'Euroscore i l'hemoglobina basal. L'administració d' eritropoietina i ferro va disminuir la mortalitat, les complicacions i les transfusions. L'anà lisis de regressió logÃstica mostra que la eritropoietina i ferro és un factor protector per la mortalitat i les complicacions. CONCLUSIONS: Tractar l'anèmia amb eritropoietina i ferro abans de la cirurgia cardÃaca disminueix la morbimortalitat
Pronòstic de l'Anèmia en el sÃndrome coronari agut
L'objectiu de l'estudi va ser avaluar l'anèmia en el pronòstic dels pacients que ingressen per sÃndrome coronari agut (SCA). El grup d'estudi va ser una cohort consecutiva de 174 pacients amb SCA de l'any 2009. Es va observar que els pacients que presentaven anèmia a l'ingrés tenien més comorbilitats, més complicacions hemorrà giques i una major tassa de mortalitat intrahospitalà ria. El risc relatiu d'aquests pacients durant el seguiment realitzat de 3.15 mesos (0,26-6,72), en la variable combinada de complicacions cardiovasculars i mortalitat va ser de 3 (IC 95%:1,1-8,0). Els pacients que no ingressaven anèmics i presentaven valors de PCR 5 mg/dl tenien una major probabilitat de desenvolupar anèmia durant l'ingrés
¿Es el sexo femenino un factor de riesgo quirúrgico en la cirugÃa de valvular?
RESULTADOS: Se intervino un 60,7% de los pacientes de SV aórtica, un 25,7% de SV mitral , un 13,6% de doble SV. Además, en el 22,5% se realizó revascularización coronaria. Las mujeres presentaban más edad, SV mitral, reintervención, fibrilación auricular, anemia y peor clase funcional. Los varones presentaron más tabaquismo, SV aórtica, enfermedad coronaria, cirugÃa de revascularización coronaria, función sistólica deprimida y tiempo de clampaje aórtico. En el análisis bivariado relacionado con mortalidad hospitalaria (8,35%) fueron significativos la edad, el sexo, la clase funcional, la posición de la prótesis, la superficie corporal, la anemia, el número de concentrados de hematÃes y el tiempo de clampaje aórtico. Se constató como factores de riesgo independientes el sexo (OR 2,92; 95% intervalo confianza 1,05-8,14) y la anemia (OR 4,23; 95% intervalo confianza 1,66-10,8) tras ajustarlos con los factores de riesgo, factores confusores y el año de intervención. CONCLUSIONES: El sexo femenino es un factor de riesgo independiente para la mortalidad hospitalaria en la cirugÃa de sustitución valvular.El sexe femenà s'inclou als models d'avaluació de risc per cirurgia de substitució valvular (SV) encara que les publicacions són poques i discordants. L'estudi examina una cohort prospectiva de 491 pacients intervinguts de SV entre 1998 i 2010. Les dones presentaven més edat, reintervenció, anèmia i pitjor classe funcional. Els homes presentaven més tabaquisme, SV aòrtica, cirurgia de revascularització, funció sistòlica deprimida i temps de clampatge aòrtic. Després d'ajustar per any d'intervenció i factors de risc van ser predictors independents per mortalitat el sexe (OR 2.92), l'anèmia (OR 4.23) i l'Euroscore sense sexe
Vasopressina, un nou biomarcador en insuficiència cardÃaca
Introducció. Les concentracions plasmà tiques dels pèptids natriürètics són biomarcadors útils en el diagnòstic i en el maneig de la insuficiència cardÃaca (IC). L'objectiu de l'estudi va consistir en avaluar un nou biomarcador, la vasopressina, per predir la mortalitat i el reingrés a mig termini dels pacients que ingressen per IC aguda. Mètodes. Des del novembre de 2004 fins setembre de 2010 s'estudien de forma prospectiva el pacients que ingressen al Servei de Cardiologia per IC aguda i es distribueixen en dos grups: grup 1, pacients amb nivells baixos de vasopressina ( ≤ 2'9 pg/mL) i grup 2, pacients amb nivells alts de vasopressina ( 2'9 pg/mL). Es realitza una anà lisi multivariada de Cox i es construeixen corbes de Kaplan-Meier. Resultats. S'inclouen 322 pacients, amb una mediana de seguiment de 14,6 mesos. Els pacients del grup 1 presenten una major supervivència als 12 i 24 mesos del 93% i 72% respecte els pacients del grup 2 que és del 75% i 61% amb una HR 2.90 (IC 95%: 1.47-5.75, p=0.002), respectivament. Tanmateix s'aprecia una major supervivència cardiovascular en els pacients del grup 1 (97% vs. 87% al 12 mesos i de 87% vs. 75% als 24 mesos) amb HR 2.72 (IC 95%: 1.97 - 6.73, p 0.031). Un 56% i 29% dels pacients del grup 1 estaven lliures de reingrés als 12 i 24 mesos, respectivament, versus el 49% i el 21% del grup 2 amb una HR 1.48 (IC 95%: 1.04 - 2.11, p 0.02). Igualment pel primer reingrés per IC , el 71% i 46% del grup 1 respecte del 60% i 39% del grup 2 als 12 i 24 mesos, respectivament (HR 1.57 (IC 95%: 1.03 - 2.41, p 0.038). Conclusions. Els nivells alts de vasopressina en pacients amb insuficiència cardÃaca aguda estabilitzats són un predictor independent de mortalitat i de reingressos als 12 i 24 mesos de seguiment
Aspectos clÃnicos y de organización en la atención a pacientes domiciliarios con insuficiencia cardiaca
Conegut el fenomen epidèmic associat a la Insuficiència CardÃaca, l'objectiu de l'estudi va ser comparar pacients de la Unitat d'Insuficiència CardÃaca i d'Atenció Primà ria, tres mesos després de l'alta hospitalà ria. Els 112 pacients de la UIC van presentar una freqüència de reingressos de 8,1% i mortalitat de 7,1%, davant els 69 pacients d'AP amb 24,2% i 29,9%, respectivament. Una anà lisi multivariada, va demostrar que el seguiment en AP és un factor independent de major mortalitat i reingrés (OR 2,8 (IC 95%:1,2-6,6) i que rebre betablocadors és un efecte protector de la IC (OR 0,2 (IC 95%: 0,1-0,6).Conocido el fenómeno epidémico asociado a la Insuficiencia Cardiaca, el objetivo del estudio fue comparar pacientes de la Unidad de Insuficiencia Cardiaca y de Atención Primaria, tres meses después del alta hospitalaria. Relación entre la resistencia a la insulina, contenido miocárdico de triglicéridos cuantificado mediante espectroscopia por resonancia magnética y disfunción diastólica subclÃnica
La prevalença de la insuficència cardÃaca segueix en aument. La identificació de nous factors de risc, com els relacionats amb el metabolisme cardÃac, és important per a la seva prevenció. La resistència a la insulina és el tret comú de diferents malalties, havent-se observat una alta prevalença entre pacients amb disfunció cardÃaca. L'excedent de lÃpids que es produeix en aquesta circumstà ncia es distribueix cap a teixits no adiposos com el cor (lipotoxicitat). El contingut miocà rdic de lÃpids pot ser un biomarcador predictor de desenvolupament de disfunció diastòlica i és possible quantificar-lo amb l'ús d'espectroscòpia per ressonà ncia magnèticaLa insuficiencia cardÃaca es un problema de salud cuya prevalencia sigue aumentando. La identificación de nuevos factores de riesgo, como los relacionados con el metabolismo cardÃaco, es importante para su prevención. La resistencia a la insulina es el rasgo común de diferentes enfermedades, habiéndose observado una alta prevalencia entre pacientes con disfunción cardÃaca. El excedente de lÃpidos que se produce en esta circunstancia se distribuye hacia tejidos no adiposos como el corazón (lipotoxicidad). El contenido miocárdico de lÃpidos puede ser un biomarcador predictor de desarrollo de disfunción diastólica y es posible cuantificarlo con el empleo de espectroscopia por resonancia magnética
Ischemic aetiology, self-reported frailty, and gender with respect to cognitive impairment in chronic heart failure patients
Background: decisive information on the parameters involved in cognitive impairment in patients with chronic heart failure is as yet lacking. Our aim was to determine the functional and psychosocial variables related with cognitive impairment using the mini-mental-state examination (MMSE) with age-and education-corrected scores. Methods: a cohort study of chronic heart failure patients included in an integrated multidisciplinary hospital/primary care program. The MMSE (corrected for age and education in the Spanish population) was administered at enrolment in the program. Analyses were performed in 525 patients. Demographic and clinical variables were collected. Comprehensive assessment included depression (Yesavage), family function (family APGAR), social network (Duke), dependence (Barthel Index), frailty (Barber), and comorbidities. Univariate and multivariate logistic regression were performed to determine the predictors of cognitive impairment. Results: cognitive impairment affected 145 patients (27.6 %). Explanatory factors were gender (OR: 2.77 (1.75-4.39) p  3.5 (OR: 0.59 (0.35-0.99) p = 0.048), and beta-blocker treatment (OR: 0.36 (0.17 to 0.76, p = 0.007)). No association was found between cognitive impairment and social support or family function. Conclusion: the observed prevalence of cognitive impairment using MMSE corrected scores was 27.6 %. A global approach in the management of these patients is needed, especially focusing on women and patients with frailty, low albumin levels, and ischemic aetiology heart failure

An intensive, structured, mobile devices-based healthcare intervention to optimize the lipid-lowering therapy improves lipid control after an acute coronary syndrome
Despite the evidence, lipid-lowering treatment (LLT) in secondary prevention remains insufficient, and a low percentage of patients achieve the recommended LDL cholesterol (LDLc) levels by the guidelines. We aimed to evaluate the efficacy of an intensive, mobile devices-based healthcare lipid-lowering intervention after hospital discharge in patients hospitalized for acute coronary syndrome (ACS). Ambiespective register in which a mobile devices-based healthcare intervention including periodic follow-up, serial lipid level controls, and optimization of lipid-lowering therapy, if appropriate, was assessed in terms of serum lipid-level control at 12 weeks after discharge. A total of 497 patients, of which 462 (93%) correctly adhered to the optimization protocol, were included in the analysis. At the end of the optimization period, 327 (70.7%) patients had LDLc levels ≤ 70 mg/dL. 40% of patients in the LDLc ≤ 70 mg/dL group were upgraded to very-high intensity lipid-lowering ability therapy vs. 60.7% in the LDLc > 70 mg/dL group, p < 0.001. Overall, 38.5% of patients had at least a change in their LLT. Side effects were relatively infrequent (10.7%). At 1-year follow-up, LDLc levels were measured by the primary care physician in 342 (68.8%) of the whole cohort of 497 patients. In this group, 71.1% of patients had LDLc levels ≤ 70 mg/dL. An intensive, structured, mobile devices-based healthcare intervention after an ACS is associated with more than 70% of patients reaching the LDLc levels recommended by the clinical guidelines. In patients with LDLc measured at 1-year follow-up, 71.1% had LDLc levels ≤ 70 mg/dL
Importance of Tricuspid Regurgitation Velocity Threshold in Risk Assessment of Pulmonary Hypertension-Long-Term Outcome of Patients Submitted to Aortic Valve Replacement
Background: The upper physiological threshold for tricuspid regurgitation velocity (TRV) of 2.8 m/s proposed by the Pulmonary Hypertension (PH) guidelines had been questioned. The aim of this study was to evaluate the prognostic significance of preoperative PH in patients with aortic stenosis, long-term after valve replacement, using two different TRV thresholds (2.55 and 2.8 m/s). Methods: Four hundred and forty four patients were included (mean age 73 ± 9 years; 55% male), with a median follow-up of 5.8 years (98% completed). Patients were divided into three PH probability groups according to guidelines (low, intermediate and high) for both thresholds (TRV ≤ 2.8 m/s and TRV ≤ 2.55 m/s), using right atrial area>18 cm 2 and right ventricle/left ventricle ratio>1 as additional echocardiographic variables. Results: In patients with measurable TRV (n = 304), the low group mortality rate was 25% and 30%, respectively for 2.55 and 2.8 m/s TRV thresholds. The intermediate group with TRV > 2.55 m/s was an independent mortality risk factor (HR 2.04; 95% CI: 1.91 to 3.48, p = 0.01), in contrast to the intermediate group with TRV>2.8 m/s (HR 1.44; 95% CI: 0.89 to 2.32, p = 0.14). Both high probability groups were associated with an increased mortality risk, as compared to their respective low groups. When including all patients (with measurable and non-measurable TRV), both intermediate groups remained independently associated with an increased mortality risk: HR 1.62 (95% CI 1.11 to 2.35 p = 0.01) for the new cut-off point; and HR 1.43 (95% CI: 0.96 to 2.13, p = 0.07) for guidelines threshold. Conclusion: A TRV threshold of 2.55 m/s, together with right cavities measures, allowed a better risk assessment of patients with PH secondary to severe aortic stenosis, with or without tricuspid regurgitation
