Łagodna hipotermia terapeutyczna po nagłym zatrzymaniu krążenia w przebiegu ostrego zespołu wieńcowego — doświadczenia z wdrażania metody
Introduction. A mild therapeutic hypothermia (MTH) is a promising adjunct treatment in patients suffering from cardiac arrest.Material and methods. In 2012–2013 a 13 out of 216 consecutive patients admitted due to cardiac arrest were qualified to MTH. Cooling was started at the admission with the intravenous infusion of saline at a degree of 4°C, then continued with intravascular automated cooling system.Results. The study group consisted of five women and eight men (mean age 59 years). The most common mechanism of cardiac arrest was ventricular fibrillation or ventricular tachycardia due to acute myocardial infarction (92.3%). The mean time of return of spontaneous circulation (ROSC) was 15 min 54 s. The mean time from cardiac arrest to the start of the cooling was 1h 10 min. During MTH an increase in white blood cell count and serum level of amylase and CRP was observed. The in-hospital mortality was 7/13 patients (53.8%). Patients who survived were younger (53.8 vs. 63.6 years), had lower incidence of coronary artery disease and ventricular fibrillation was the mechanism of cardiac arrest. In addition, patients who survived had prolonged time to ROSC (median time 19 min vs. 13 min) and reduced time: to the start of cold saline infusion (median time 58 min vs. 85 min), from cardiac arrest to the beginning of the intravascular cooling (median time 3 h 20 min vs. 3 h 50 min) and hospital stay (median time 471 h vs. 1232 h) in comparison with patients with fatal outcome. Five patients were discharged from the hospital in a good neurological condition (4 patients — 0 points and 1 patient — 1 point in Rankin scale).Conclusions. All the patients who survived and were treated with MTH, were discharged in a good neurological condition. The implementation of endovascular cooling device greatly improves precise temperature control in patients undergoing MTH.Mild therapeutic hypothermia after cardiac arrest due to ACS.Wstęp. Łagodna hipotermia terapeutyczna (MTH) jest obiecującą metodą wspomagającą leczenie chorych po nagłym zatrzymaniu krążenia (NZK).Materiał i metody. W latach 2012–2013 spośród kolejnych 216 chorych po NZK do MTH zakwalifikowano 13 pacjentów. Schładzanie rozpoczynano w chwili przyjęcia wlewem dożylnym soli fizjologicznej o temperaturze 4°C, następnie kontynuowano hipotermię za pomocą zautomatyzowanego systemu wewnątrznaczyniowego.Wyniki. Badaną grupę stanowiło 5 kobiet i 8 mężczyzn (średni wiek 59 lat). Najczęstszym mechanizmem NZK były migotanie komór lub częstoskurcz komorowy w przebiegu zawału serca (92,3%). Średni czas do uzyskania spontanicznego krążenia (ROSC) wynosił 15 min 54 s, a od NZK do rozpoczęcia chłodzenia — 1 h 10 min. Podczas MTH obserwowano zwiększone leukocytozę, aktywności amylazy i stężenie CRP. W szpitalu zmarło 7 chorych (53,8%). Grupę pacjentów, którzy przeżyli, charakteryzowały: młodszy wiek (53,8 v. 63,6 roku), rzadszy wywiad choroby niedokrwiennej serca, częstsze występowanie migotania komór jako mechanizmu NZK, dłuższy czas do ROSC (mediana 19 v. 13 min), krótszy czas do rozpoczęcia wlewu zimnej soli fizjologicznej (mediana 58 v. 85 min), krótszy czas od NZK do rozpoczęcia chłodzenia śródnaczyniowego (mediana 3 h 20 min v. 3 h 50 min) oraz krótszy czas pobytu w szpitalu (mediana 471 v. 1232 h). Pięć osób wypisano ze szpitala w bardzo dobrym stanie neurologicznym (4 pacjentów — 0 pkt., 1 pacjent — 1 pkt w skali Rankina).Wnioski. Wszyscy pacjenci, którzy przeżyli, a byli leczeni za pomocą MTH, zostali wypisani w bardzo dobrym stanie neurologicznym. Stosowanie urządzenia do wewnątrznaczyniowego chłodzenia pozwala na precyzyjną kontrolę temperatury u pacjentów poddanych procedurze
Species specificity, surface exposure, protein expression, immunogenicity, and participation in biofilm formation of Porphyromonas gingivalis HmuY
BACKGROUND: Porphyromonas gingivalis is a major etiological agent of chronic periodontitis. The aim of this study was to examine the species specificity, surface exposure, protein expression, immunogenicity, and participation in biofilm formation of the P. gingivalis heme-binding protein HmuY. RESULTS: HmuY is a unique protein of P. gingivalis since only low amino-acid sequence homology has been found to proteins encoded in other species. It is exposed on the cell surface and highly abundant in the outer membrane of the cell, in outer-membrane vesicles, and is released into culture medium in a soluble form. The protein is produced constitutively at low levels in bacteria grown under high-iron/heme conditions and at higher levels in bacteria growing under the low-iron/heme conditions typical of dental plaque. HmuY is immunogenic and elicits high IgG antibody titers in rabbits. It is also engaged in homotypic biofilm formation by P. gingivalis. Anti-HmuY antibodies exhibit inhibitory activity against P. gingivalis growth and biofilm formation. CONCLUSIONS: Here it is demonstrated that HmuY may play a significant role not only in heme acquisition, but also in biofilm accumulation on abiotic surfaces. The data also suggest that HmuY, as a surface-exposed protein, would be available for recognition by the immune response during chronic periodontitis and the production of anti-HmuY antibodies may inhibit biofilm formation
Diphosphonucleotide phosphatase/phosphodiesterase (PPD1) from yellow lupin (Lupinus luteus L.) contains an iron–manganese center
AbstractYellow lupin diphosphonucleotide phosphatase/phosphodiesterase (PPD1) represents a novel group of enzymes. Here we report that it possesses one iron atom and one manganese atom (1:1 molar ratio) per subunit. The enzyme exhibits visible absorption maximum at ∼530nm. Prolonged oxidation of PPD1 leads to loss of the charge-transfer band and catalytic activity, whereas after reduction PPD1 remains active. Replacement of conserved amino-acid residues coordinating metals results in the loss of enzymatic activity. Despite low amino-acid sequence homology of PPD1 to well-characterized ∼55-kDa purple acid phosphatases, their overall fold, topology of active center and metal content are highly similar
The structure of a purple acid phosphatase involved in plant growth and pathogen defence exhibits a novel immunoglobulin-like fold
Phosphatases function in the production, transport and recycling of inorganic phosphorus, which is crucial for cellular metabolism and bioenergetics, as well as in bacterial killing, since they are able to generate reactive oxygen species via Fenton chemistry. Diphosphonucleotide phosphatase/phosphodiesterase (PPD1), a glycoprotein plant purple acid phosphatase (PAP) from yellow lupin seeds, contains a bimetallic Fe–Mn catalytic site which is most active at acidic pH. Unlike other plant PAPs, PPD1 cleaves the pyrophosphate bond in diphosphonucleotides and the phosphodiester bond in various phosphodiesters. The homohexameric organization of PPD1, as revealed by a 1.65 Å resolution crystal structure and confirmed by solution X-ray scattering, is unique among plant PAPs, for which only homodimers have previously been reported. A phosphate anion is bound in a bidentate fashion at the active site, bridging the Fe and Mn atoms in a binding mode similar to that previously reported for sweet potato PAP, which suggests that common features occur in their catalytic mechanisms. The N-terminal domain of PPD1 has an unexpected and unique fibronectin type III-like fold that is absent in other plant PAPs. Here, the in vitro DNA-cleavage activity of PPD1 is demonstrated and it is proposed that the fibronectin III-like domain, which `overhangs' the active site, is involved in DNA selectivity, binding and activation. Does permanent cardiac stimulation through atrial natriuretic peptide secretion influence the basic parameters of renal function?
Background: The aim of this study was to evaluate atrial humoral function and renal
function after pacemaker implantation due to atrioventricular conduction disturbances.
We analyzed blood atrial natriuretic peptide (ANP) concentration and basic parameters of
renal function within 1 month of implantation of VVI and DDD pacemakers. We evaluated
correlations between blood ANP values and basic renal function parameters.
Methods: We studied two groups of patients with atrioventricular (AV) conduction disturbances:
second-degree AV block and third-degree AV block. Group I comprised 20 patients
aged 71-90 years (median 77.5 ± 5.9 years) in whom permanent VVI pacing was applied, and
group II consisted of 20 subjects aged 49-81 years (median 68.9 ± 11.9 years) in whom DDD/
/VDD pacemakers were implanted. The control group consisted of 15 healthy volunteers aged
58-80 years (median 72.7 ± 2.8 years). Plasma concentration of ANP was determined by
radioimmunoassay. The parameters of renal function we analyzed with Jaffe’s colorimetric
and kinetic test.
Results: Patients in group I showed a significant decrease in blood concentration of ANP
from 168.1 ± 81.9 pg/1000 mL to 118.0 ± 61.1 pg/1000 mL (p < 0.01) 7 days after implantation.
At 30 days, ANP was 121.4 ± 71.9 pg/1000 mL. In group II, plasma concentration of
ANP decreased significantly from 134.9 ± 8.1 pg/1000 mL to 104.9 ± 6.1 pg/1000 mL (p < 0.01)
7 days after implantation and to 110.8 ± 53.3 pg/1000 mL at 30 days. Patients in group I had
elevated, albeit insignificantly, clearance of creatinine to 76.1 ± 17.8 ml/min at 7 days
(p > 0.05) which increased significantly to 85.0 ± 17.9 ml/min at 30 days. In group II, clearance
of creatinine increased insignificantly to 84.6 ± 13.2 ml/min (p < 0.05) at 7 days and was
significantly elevated to 96.9 ± 18.2 ml/min (p < 0.05) at 30 days. In group I, plasma concentration
of creatinine decreased significantly (p < 0.05) to 1.15 ± 0.30 mg/dl at 7 days and to 1.01 ±
± 0.21 mg/dl at 30 days. In group II, there was a significant decrease (p < 0.05) to 1.15 ± 0.24 mg/dl
at 7 days and to 1.08 ± 0.27 mg/dl at 30 days. There was a positive correlation between
creatinine clearance and plasma ANP concentration in groups I and II (r = 0.301; p < 0.05). Conclusions: In patients with a pacemaker implanted due to atrioventricular disturbances,
blood concentration of ANP was decreased. Renal function was improved after pacemaker

Correlation analysis of atrial natriuretic peptide concentration, echocardiographic left atrial and left ventricular dimensions, and renal function parameters in patients after permanent pacemaker implantation
Background: Atrial endocrine function was established in the second half of the 20th century,
confirming the role of artial natriuretic peptide (ANP) and brain natriuretic peptide (BNP)
in the physiology of the cardiovascular system. The present study was undertaken to evaluate
changes in ANP and echocardiographic parameters within the first month after VVI and DDD
pacemaker implantation and to evaluate correlations between the parameters.
Methods: The study population consisted of group I - 20 VVI patients aged 71-90 years
(mean age 77.5 ± 5.9) and group II - 20 DDD/VDD patients aged 49-81 years (mean age
68.9 ± 11). Fifteen healthy volunteers aged 58–80 years (mean age 72.7 ± 2.8) served as
controls. Correlations between ANP levels and cardiac cavity dimensions and between ANP
and parameters of renal function were studied.
Results: Blood levels of ANP decreased after pacemaker implantation: in the VVI group from
168.61 ± 81.95 pg/1000 μL to 118.04 ± 61.06 pg/1000 μL at 7 days and to 121.4 ± 71.90 pg/1000 μL at 30 days; and in the DDD/VDD group from 134.89 ± 83.11 pg/1000 μL to 104.96 ±
± 57.09 pg/1000 μL at 7 days and to 110.82 ± 53.32 pg/1000 μL at 30 days. There was
a significant correlation between ANP levels and left atrial size in the DDD/VDD group -
0.598 (p = 0.005) and 0.593 (p = 0.005) and left ventricular dimensions - 0.499 (p = 0.024)
and 0.485 (p = 0.030).
Conclusions: ANP decreases significantly after pacing implementation in patients selected
for implantation of VVI and DDD/VDD pacemakers. ANP correlates significantly with echocardiographic
measurements in patients selected for DDD/VDD pacemakers, but no significant
correlation is observed in VVI patients qualifying for permanent pacemaker due to atrioventricular
block

Przedstawiamy przypadek 42-letniego mężczyzny po przebytym zawale serca, hospitalizowanego z powodu ustawicznego częstoskurczu komorowego, przebiegającego z objawami zespołu małego rzutu, skutecznie opanowanym skojarzonym leczeniem antyarytmicznym amiodaronem i prokainamidem. Z uwagi na samoistne nawracanie rytmu zatokowego nie wykonano kardiowersji elektrycznej. Zaburzenia hemodynamiczne zostały opanowane dzięki intensywnemu leczeniu kompensującemu układ krążenia i zwolnieniu częstości częstoskurczu komorowego z 180/min do 100–130/min. Skuteczne opanowanie częstoskurczu komorowego nastąpiło po dołączeniu do amiodaronu prokainamidu. Zaobserwowano stopniową poprawę stanu chorego. Na proponowane badanie koronarograficzne chory nie wyraził zgody. Wypisano go do domu w stanie dobrym z zaleceniem stałego przyjmowania amiodaronu i prokainamidu.
W 18 miesięcy po hospitalizacji odstawiono prokainamid. W kontrolnym badaniu wysiłkowym i holterowskim nie stwierdzono progresji arytmii

Anomalia tętnic wieńcowych pod postacią przetok wieńcowo-płucnych - częstość występowania, patofizjologia i znaczenie kliniczne
Przetoki wieńcowe są rzadką anomalią mogącą mieć charakter wrodzony lub nabyty. Objawy
kliniczne zależą od stopnia zaburzeń hemodynamicznych. Nagły zgon bywa często jedynym
i ostatecznym objawem anomalii. Różnorodność objawów klinicznych, niejednoznacznie ustalone
postępowanie w przypadku stwierdzenia przetok są powodem prezentacji przypadku
49-letniej kobiety z przetoką wieńcowo-płucną pomiędzy tętnicą przednią zstępującą a pniem
płucnym oraz między prawą tętnicą wieńcową a pniem płucnym. Chorą przyjęto do szpitala
z powodu niestabilnego przebiegu choroby wieńcowej. Diagnozę przetok postawiono na podstawie
koronarografii, a ich znaczenie hemodynamiczne wykazano w badaniu scyntygraficznym.
Pacjentkę wstępnie zakwalifikowano do przezskórnego zabiegu zamknięcia przetok wieńcowych,
na które nie wyraziła zgody. Intensywna farmakoterapia nadciśnienia tętniczego i hiperlipidemii
oraz zmiana stylu życia spowodowały stabilizację kliniczną. (Folia Cardiol. 2004;

Acute aortic dissection in a 34 year old pregnant women : a case report
A case of a 34 year old pregnant women at 38 weeks gestation, who was admitted to the hospital with acute chest pain,
is presented. Electrocardiogram showed ST segment changes suggestive of acute myocardial infarction. Echocardiography,
however, revealed acute aortic dissection. The patient was transferred to another hospital where caesarean section followed
by cardiac surgery were successfully performed. A subsequent one-year follow-up period was uneventful, both for mother and
child. Diagnosis and treatment of acute aortic dissection during pregnancy are discussed
