    Il costo sociale del morbillo in età pediatrica. L’epidemia a Palermo nel 1996-97

    Objective To determine the direct and indirect costs associated with a measles epidemic occurring between September 1996 and August 1997 in Palermo (Italy) in paediatric-aged patients. Design A total of 2,029 cases of measles in a paediatric patient population were identified from a total of 38 paediatricians databases (24% of total). An extrapolation to the general population was then performed to estimate a total of 9,059 cases. Patient information obtained from the database such as patient age, risk factors, complications, vaccination history, as well as caretaker’s profession were included in a questionnaire compiled for each patient. Setting Inpatient and outpatient clinics in Palermo, Italy. Patients and participants Participants were paediatric-aged patients who had been diagnosed with measles. Included in the study was a group of previously vaccinated patients (6%). Main outcome measures and results The average cost of care was 464.000 Italian lire (Lit.) per case with a total cost of Lit. 4,2 billion for the entire epidemic. The direct costs comprehended 46.6% (Lit. 217.000 per case) of the total costs related to the measles epidemic and were subdivided according to in-patient care (55.4%), paediatric outpatient visits (33.5%) and drugs (9.7%). The average health-care cost associated to previously vaccinated patients (6%) was lower than for non-vaccinated patients, Lit. 110.000 vs Lit. 223.000 per case, respectively. Conclusion The demographic and economic data obtained highlights not only the social and economic impact of the epidemic, but also provides relevant information useful for cost-effectiveness analysis

    Evaluating sea cucumbers as extractive species for benthic bioremediation in mussel farms

    Filter-feeding mussels blend suspended particles into faeces and pseudo-faeces enhancing organic matter flows between the water column and the bottom, and strengthening benthic-pelagic coupling. Inside operating farms, high bivalve densities in relatively confined areas result in an elevated rate of organic sinking to the seabed, which may cause a localized impact in the immediate surrounding. Deposit-feeding sea cucumbers are potentially optimal candidates to bioremediate mussel organic waste, due to their ability to process organic-enriched sediments impacted by aquaculture waste. However, although the feasibility of this polyculture has been investigated for a few Indo-Pacific species, little is known about Atlanto-Mediterranean species. Hence, for the first time, in the present study, we conducted a comparative investigation on the suitability of different Mediterranean sea cucumber species, to be reared in Integrated Multitrophic Aquaculture (IMTA) with mussels. A pilot-scale experiment was accomplished operating within a mussel farm where two sea cucumbers species, Holothuria tubulosa and Holothuria polii, were caged beneath the long-line mussel farm of Mytilus galloprovincialis. After four months, H. tubulosa showed high survivorship (94%) and positive somatic growth (6.07%); conversely H. polii showed negative growth (- 25.37%), although 92% of specimens survived. Furthermore, sea cucumber growth was size-dependent. In fact, smaller individuals, independently from the species, grew significantly faster than larger ones. These results evidenced a clear difference in the suitability of the two sea cucumber species for IMTA with M. galloprovincialis, probably due to their different trophic ecology (feeding specialization on different microhabitats, i.e. different sediment layers). Specifically, H. tubulosa seems to be an optimal candidate as extractive species both for polycultures production and waste bioremediation in M. galloprovincialis operating farms

    Generation of flavors and fragrances through biotransformation and de novo synthesis

    Flavors and fragrances are the result of the presence of volatile and non-volatile compounds, appreciated mostly by the sense of smell once they usually have pleasant odors. They are used in perfumes and perfumed products, as well as for the flavoring of foods and beverages. In fact the ability of the microorganisms to produce flavors and fragrances has been described for a long time, but the relationship between the flavor formation and the microbial growth was only recently established. After that, efforts have been put in the analysis and optimization of food fermentations that led to the investigation of microorganisms and their capacity to produce flavors and fragrances, either by de novo synthesis or biotransformation.     Valorisation of Biowastes for the Production of Green Materials Using Chemical Methods

    With crude oil reserves dwindling, the hunt for a sustainable alternative feedstock for fuels and materials for our society continues to expand. The biorefinery concept has enjoyed both a surge in popularity and also vocal opposition to the idea of diverting food-grade land and crops for this purpose. The idea of using the inevitable wastes arising from biomass processing, particularly farming and food production, is, therefore, gaining more attention as the feedstock for the biorefinery. For the three main components of biomass—carbohydrates, lipids, and proteins—there are long-established processes for using some of these by-products. However, the recent advances in chemical technologies are expanding both the feedstocks available for processing and the products that be obtained. Herein, this review presents some of the more recent developments in processing these molecules for green materials, as well as case studies that bring these technologies and materials together into final products for applied usage

    Universal varicella vaccination in the Sicilian paediatric population: rapid uptake of the vaccination programme and morbidity trends over five years

    Following the licensure of the Oka/Merck varicella vaccine in Italy in January 2003, the Sicilian health authorities launched a universal vaccination programme in all nine Local Health Units. A two-cohort vaccination strategy was adopted to minimise the shift of the mean age of varicella occurrence to older age groups, with the goal of vaccinating with one dose at least 80% of children in their second year of life and 50% of susceptible adolescents in their 12th year of life. Two studies were implemented in parallel to closely monitor vaccination coverage as well as varicella incidence. Overall, the programme achieved its target, with 87.5% vaccine coverage for the birth cohort 2005 and 90.2% for adolescents born in 1995 and 1996. Varicella surveillance data obtained from a total of 28,188 children (0-14 years-old) monitored by family paediatricians showed a decline in incidence rates from 95.7 (95% confidence interval (CI): 72.2-126.8) for 1,000 person-years (PY) in 2004 to 9.0 (95% CI: 6.4-12.6) for 1,000 PY in 2007. In Europe, the only similar experience is the routine childhood varicella vaccination programme in Germany that started in 2004 with a single dose at the age of 11-14 months. The two-cohort universal vaccination programme implemented in Sicily, as well as the network for the surveillance study, can offer a model to other European countries that are considering introducing universal childhood varicella vaccinatio
