    A Model Oriented Approach for Managing Traceability of Biological Samples and Tests of Patients in Assisted Reproduction Clinics

    Assisted reproduction has become a service that more and more people access. Current problems such as the delay in the age of motherhood, single-parent couples, etc. they have proliferated the options and the different treatments that are put at the service of society. A fundamental part of these processes lies in the work of laboratories, in the samples that are handled in clinical processes and then be implanted in future mothers. The management of the samples is a critical aspect that requires all the opportune mechanisms that guarantee the traceability of said samples, avoiding fatal errors. The correct identification, monitoring and control of them is a fundamental aspect and of special relevance. However, the systems currently offered to clinics present important problems. On the one hand, they offer little security, are very expensive or very independent of a specific provider, so that the traceability system cannot be connected to the hospital central management system, or they are very intrusive control systems in the daily work of the laboratory technicians. In this paper a software solution based on the definition of automatic models and protocols is proposed.     Behavior patterns in hormonal treatments using fuzzy logic models

    Assisted reproductive technologies are a combination of medical strategies designed to treat infertility patients. Ideal stimulation treatment has to be individualized, but one of the main challenges which clinicians face in the everyday clinic is how to select the best medical protocol for a patient. This work aims to look for behavior patterns in this kind of treatments, using fuzzy logic models with the objective of helping gynecologists and embryologists to make decisions that could improve the process of in vitro fertilization. For this purpose, a real-world dataset composed of one hundred and twenty-three (123) patients and five hundred and fifty-nine (559) treatments applied in relation to such patients provided by an assisted reproduction clinic, has been used to obtain the fuzzy models.     Retrospectivas en el aula. Una experiencia práctica

    En la docencia tradicional, se imparte clase durante un periodo concreto y, después, se evalúa a los alumnos. Sin embargo, una vez que se realiza esta evaluación no suele quedar tiempo para arreglar cualquier problema que esta evaluación pueda detectar. Las retrospectivas son reuniones y dinámicas de trabajo en grupo que han alcanzado una enorme popularidad en el ámbito del desarrollo de productos tecnológicos innovadores. El objetivo de estas reuniones y técnicas es reunir a todos los participantes en un proyecto, reflexionar sobre cómo se está trabajando y proponer mejoras en el trabajo del día a día. Con el fin de detectar con la mayor antelación posible cualquier elemento disonante dentro del aula y en el proceso de formación, además de favorecer que los alumnos expresen sus opiniones, hemos aplicado en los tres últimos años técnicas de retrospectivas El objetivo de este trabajo es exponer nuestras experiencias aplicando retrospectivas en el aula con el alumnado incluyendo las técnicas que hemos usado, cómo las hemos aplicado y ejemplos de los resultados que hemos obtenido.In traditional teaching, classes are taught during a specific period and, afterwards, students are evaluated. However, once this evaluation is done, there is usually no time to fix any problem that this evaluation can detect. The retrospectives are meetings and dynamics of group work that have achieved enormous popularity in the field of the development of innovative technological products. The purpose of these meetings and techniques is to bring all the participants together in a project, reflect on how they are working and propose improvements in day-to-day work. In order to detect as early as possible any dissonant element within the classroom and in the training process, in addition to encouraging students to express their opinions, we have applied retrospective techniques in the last three years. The objective of this work is to provide our experiences by applying retrospectives in the classroom with the students, including the techniques we have used, how we have applied them and examples of the results we have obtained

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, individuals with clonal mosaic events (clonal mosaicism for chromosome alterations and/or loss of chromosome Y) showed an increased risk of COVID-19 lethality

Dynamic programming of appointments improves the efficiency through algorithmic decision support tools. This technology has used successfully in other industries such as airlines, car rental agencies and hotels. The application of this technique to the health environment has attracted the interest of many academics in the last 50 years because it is very useful to improve the access and the quality of health systems and also reduce the cost. However, the use of these techniques in health settings is not trivial because every decision is vital for patients. In addition, we must consider other important and complex factors such as emergencies.     El Proyecto IDE4ICDS se mueve en el contexto médico para dar solu ción a la gestión digitalizada del ciclo de vida de guías clínicas. Actualmente, las guías clínicas están definidas de forma textual, lo que las hace difíciles de seguir en el día a día. Esto provoca, entre otros factores, variabilidad en la práctica clí nica. En este proyecto se presenta una metodología que permita automatizar la gestión de guías clínicas, así como una plataforma software que le dé soporte. Para ello, la plataforma IDE4ICDS permite definir, ejecutar y monitorizar guías clínicas. Dicha plataforma ha sido validada con profesionales sanitarios del Hos pital Virgen del Rocío (Sevilla), obteniendo resultados prometedores. Actual mente se encuentra en fase de pilotaje en Atención Primaria con pacientes que sufren Diabetes Mellitus Tipo 2
