    Optimal sampling of MRI slices for the assessment of knee cartilage volume for cross-sectional and longitudinal studies

    BACKGROUND: MRI slices of 1.5 mm thickness have been used in both cross sectional and longitudinal studies of osteoarthritis, but is difficult to apply to large studies as most techniques used in measuring knee cartilage volumes require substantial post-image processing. The aim of this study was to determine the optimal sampling of 1.5 mm thick slices of MRI scans to estimate knee cartilage volume in males and females for cross-sectional and longitudinal studies. METHODS: A total of 150 subjects had a sagittal T1-weighted fat-suppressed MRI scan of the right knee at a partition thickness of 1.5 mm to determine their cartilage volume. Fifty subjects had both baseline and 2-year follow up MRI scans. Lateral, medial tibial and patellar cartilage volumes were calculated with different samples from 1.5 mm thick slices by extracting one in two, one in three, and one in four to compare to cartilage volume and its rate of change. Agreement was assessed by means of intraclass correlation coefficient (ICC) and Bland & Altman plots. RESULTS: Compared to the whole sample of 1.5 mm thick slices, measuring every second to fourth slice led to very little under or over estimation in cartilage volume and its annual change. At all sites and subgroups, measuring every second slice had less than 1% mean difference in cartilage volume and its annual rate of change with all ICCs ≥ 0.98. CONCLUSION: Sampling alternate 1.5 mm thick MRI slices is sufficient for knee cartilage volume measurement in cross-sectional and longitudinal epidemiological studies with little increase in measurement error. This approach will lead to a substantial decrease in post-scan processing time

    Penelitian ini bertujuan untuk memberikan gambaran tentang : 1) efektivitas penerapan Sistem Manajemen Mutu (SMM) ISO 9001:2008 pada SMK Negeri di kota Singaraja dilihat dari konteks, masukan, proses, dan hasil penerapan; 2) kendala-kendala yang dihadapai dalam melaksanakan SMM ISO 9001:2008 pada SMK Negeri dikota Singaraja serta alternatif pemecahannya. Penelitian ini dilaksanakan pada SMK Negeri dikota Singaraja pada tahun pelajaran 2013/2014, dengan menggunakan model evaluasi CIPP dari Stufflebeam yang melibatkan 354 responden. Variabel konteks yang terdiri dari sub variabel SMM sekolah dan keterlibatan komite sekolah. Variabel masukan terdiri dari sub variabel manajemen sekolah dan keterlibatan dunia USAha/dunia industri (DU/DI). Variabel proses yang terdiri dari sub variabel pemeliharaan dan pengadaan sarana pendidikan, kegiatan belajar mengajar oleh guru, dan kegiatan belajar mengajar siswa. Variabel produk dengan sub variabel nilai ujian tahun pemelajaran 2013/2014 (UN, US dan UK). Metode kuesioner digunakan untuk menjaring data SMM sekolah, manajemen sekolah dengan responden semua staff manajemen, keterlibatan dunia USAha/dunia industri (DU/DI), dan proses pembelajaran siswa. Metode wawancara untuk menjaring data keterlibatan komite sekolah. Metode observasi untuk menjaring data kegiatan belajar mengajar oleh guru. Metode studi dokumen untuk menjaring data SMM sekolah, pengadaan dan pemeliharaan sarana pendidikan, dan nilai ujian. Hasil penelitian menunjukkan bahwa : 1) efektif dilihat dari variabel konteks dengan frekuensi kategori positif 54.286% untuk SMM dan frekuensi kategori positif 75% untuk keterlibatan komite; 2) efektif dilihat dari variabel masukan dengan frekuensi kategori positif 57.6271% untuk manajmen sekolah dan frekuensi kategori positif 52.041% untuk keterlibatan DU/DI; 3) kurang efektif dilihat dari variabel proses dengan frekuensi kategori negatif 63.1578% untuk pemeliharaan dan pengadaan saran pendidikan, dan frekuensi kategori positif 57.4713% untuk kegiatan belajar mengajar oleh guru, dan frekuensi kategori positif 52.308% untuk kegiatan belajar mengajar siswa; 4) efektif dilihat dari variabel produk dengan frekuensi kategori positif 52.055.00% untuk nilai ujian. Bertolak dari hasil penelitian tersebut dapat direkomendasikan: 1) meningkatkan koordinasi setiap kebijakan baru; 2) penerapan SMM perlu disosialisasikan secara terus menerus pada setiap kesempatan; 3) meningkatkan komitmen warga sekolah untuk menerapakan SMM; 4) meningkatkan koordinasi dengan industri menuju Manajemen Partisipatif; 5) memberikan tugas dan tanggungjawab yang jelas kepada kepala program

    Neuropathic pain associated with first metatarsophalangeal joint osteoarthritis: frequency and associated factors.

    OBJECTIVE: To determine whether neuropathic pain is a feature of first metatarsophalangeal (MTP) joint osteoarthritis (OA). METHODS: Ninety-eight participants (mean age 57.4 years, standard deviation 10.3) with symptomatic radiographic first MTP joint OA completed the painDETECT questionnaire (PDQ), which incorporates nine questions regarding the intensity and quality of pain. The likelihood of neuropathic pain was determined using established cut-points of the PDQ. Participants with unlikely neuropathic pain were then compared to those with possible/likely neuropathic pain in relation to age, sex, general health (Short Form [SF] 12), psychological wellbeing (Depression, Anxiety and Stress Scale), pain characteristics (self-efficacy, duration, and severity), foot health (Foot Health Status Questionnaire [FHSQ]), first MTP dorsiflexion range of motion and radiographic severity. Effect sizes (Cohen's d) were also calculated. RESULTS: Thirty (31%) participants had possible/likely neuropathic pain (possible n=19, [19.4%], likely n=11 [11.2%]). The most common neuropathic symptoms were sensitivity to pressure (56%), sudden pain attacks/electric shocks (36%) and burning (25%). Compared to those with unlikely neuropathic pain, those with possible/likely neuropathic pain were significantly older (d=0.59, p=0.010), had worse SF12 physical (d=1.10, p<0.001), pain self-efficacy (d=0.98, p<0.001), FHSQ pain (d=0.98, p<0.001) and FHSQ function (d=0.82, p<0.001) scores, and had higher pain severity at rest (d=1.01, p<0.001). CONCLUSION: A significant proportion of individuals with first MTP joint OA report symptoms suggestive of neuropathic pain, which may partly explain the suboptimal responses to commonly used treatments for this condition. Screening for neuropathic pain may assist in the selection of targeted interventions and improve clinical outcomes

    Associations between systemic bone mineral density and early knee cartilage changes in middle-aged adults without clinical knee disease: a prospective cohort study

    Background: Osteoarthritis has a high prevalence in people with high bone mineral density (BMD). Nevertheless, whether high systemic BMD predates early structural features of knee osteoarthritis is unclear. This study examined the association between systemic BMD and knee cartilage defect progression and cartilage volume loss in middle-aged people without clinical knee disease.Methods: Adults (n = 153) aged 25-60 years had total body, lumbar spine, and total hip BMD assessed by dual-energy X-ray absorptiometry at baseline (2005-2008), and tibial cartilage volume and tibiofemoral cartilage defects assessed by magnetic resonance imaging at baseline and follow up (2008-2010).Results: Higher spine BMD was associated with increased risk for progression of medial (OR = 1.45, 95% CI 1.10, 1.91) and lateral (OR = 1.30, 95% CI 1.00, 1.67) tibiofemoral cartilage defects. Total hip BMD was also positively associated with the progression of medial (OR = 1.63, 95% CI 1.10, 2.41) and lateral (OR = 1.53, 95% CI 1.08, 2.18) tibiofemoral cartilage defects. Greater total body, spine, and total hip BMD were associated with increased rate of lateral tibial cartilage volume loss (for every 1 g/10 cm2 increase in total body BMD: B = 0.44%, 95% CI 0.17%, 0.71%; spine BMD: 0.17%, 95% CI 0.04%, 0.30%; total hip BMD: 0.29%, 95% CI 0.13%, 0.45%), with no significant associations for medial tibial cartilage volume loss.Conclusion: In middle-aged people without clinical knee disease, higher systemic BMD was associated with increased early knee cartilage damage. Further work is needed to clarify the effect of systemic BMD at different stages of the pathway from health through to disease in knee osteoarthritis, as new therapies targeting bone are developed for the management of knee osteoarthritis

    METHODS - A randomised controlled trial of METhotrexate to treat Hand Osteoarthritis with Synovitis: study protocol for a randomised controlled trial

    Background: Hand osteoarthritis is a common and disabling problem without effective therapies. Accumulating evidence suggests the role of local inflammation in causing pain and structural progression in hand osteoarthritis, and hand osteoarthritis with synovitis is a commonly encountered clinical phenotype. Methotrexate is a well-established, low-cost, and effective treatment for inflammatory arthritis with a well-described safety profile. The aim of this multicentre, randomised, double-blind, placebo-controlled trial is to determine whether methotrexate reduces pain over 6 months in patients with hand osteoarthritis and synovitis.Methods: Ninety-six participants with hand osteoarthritis and synovitis will be recruited through the Osteoarthritis Clinical Trial Network (Melbourne, Hobart, Adelaide, and Perth), and randomly allocated in a 1:1 ratio to receive either methotrexate 20 mg or identical placebo once weekly for 6 months. The primary outcome is pain reduction (assessed by 100 mm visual analogue scale) at 6 months. The secondary outcomes include changes in physical function and quality of life assessed using Functional Index for Hand Osteoarthritis, Australian Canadian Osteoarthritis Hand Index, Health Assessment Questionnaire, Michigan Hand Outcomes Questionnaire, Short-Form-36, tender and swollen joint count, and grip strength, and structural progression assessed using progression of synovitis and bone marrow lesions from magnetic resonance imaging and radiographic progression at 6 months. Adverse events will be recorded. The primary analysis will be by intention to treat, including all participants in their randomised groups.Discussion: This study will provide high-quality evidence to address whether methotrexate has an effect on reducing pain over 6 months in patients with hand osteoarthritis and synovitis, with major clinical and public health importance. While a positive trial will inform international clinical practice guidelines for the management of hand osteoarthritis, a negative trial would be highly topical and change current trends in clinical practice

    A cross sectional study of requests for knee radiographs from primary care

    <p>Abstract</p> <p>Background</p> <p>Knee pain is the commonest pain complaint amongst older adults in general practice. General Practitioners (GPs) may use x rays when managing knee pain, but little information exists regarding this process. Our objectives, therefore, were to describe the information GPs provide when ordering knee radiographs in older people, to assess the association between a clinical diagnosis of osteoarthritis (OA) and the presence of radiographic knee OA, and to investigate the clinical content of the corresponding radiologists' report.</p> <p>Methods</p> <p>A cross sectional study of GP requests for knee radiographs and their matched radiologists' reports from a local radiology department. Cases, aged over 40, were identified during an 11-week period. The clinical content of the GPs' requests and radiologists' reports was analysed. Associations of radiologists' reporting of i) osteoarthritis, ii) degenerative disease and iii) individual radiographic features of OA, with patient characteristics and clinical details on the GPs' requests, were assessed.</p> <p>Results</p> <p>The study identified 136 cases with x ray requests from 79 GPs and 11 reporting radiologists. OA was identified clinically in 19 (14%) of the requests, and queried in another 31 (23%). The main clinical descriptor was pain in 119 cases (88%). Radiologists' reported OA in 22% of cases, and the features of OA were mentioned in 63%. Variation in reporting existed between radiologists. The commonest description was joint space narrowing in 52 reports (38%). There was an apparent although non significant increase in the reporting of knee OA when the GP had diagnosed or queried it (OR 1.95; 95% CI 0.76, 5.00).</p> <p>Conclusion</p> <p>The features of radiographic OA are commonly reported in those patients over 40 whom GPs send for x ray. If OA is clinically suspected, radiologists appear to be more likely to report its presence. Further research into alternative models of referral and reporting might identify a more appropriate imaging policy in knee disorders for primary care.</p
