Chirurgie intra-abdominale majeure pour cancer, une analgésie péridurale influence-t-elle la survie sans récidive ?

L association d une analgésie locorégionale à une anesthésie générale semble diminuer la récidive et la mortalité après une chirurgie du cancer du sein ou de la prostate. L objectif de ce travail était d évaluer la récidive tumorale et la survie de patients opérés d un cancer intra abdominal sous anesthésie générale et ayant reçu une analgésie péridurale thoracique (EP) ou parentérale par morphine (AG), en péri opératoire.Nouvelle analyse d une étude prospective avec 2 groupes de patient randomisés : AG et EP. Les stades carcinologiques des patients, les critères histo-pronostiques et la survie à 5 ans, ont été recueillis a posteriori. Le critère principal de jugement était la survie sans récidive tumorale. Une analyse multivariée a été réalisée sur les facteurs pronostiques associés. 132 patients ont été analysés. La répartition des cancers était comparable entre les 2 groupes (p=0.52). Le taux de survie sans récidive locale ni métastases à 5 ans est de 43% [32-55] dans le groupe EP et de 24% [15-36] dans le groupe AG. La différence n est pas statistiquement significative (p=0.16). L analyse multivariée montre que les seuls facteurs pronostiques indépendants sont: une exérèse tumorale complète [RR=4.20 (IC95% :2.62-6.76)], et l indice de performance OMS [0 vs 1 /2, RR=2.21 (IC95% :1.36-3.59)].Notre étude retrouve une tendance non significative en faveur d un bénéfice d une analgésie péridurale sur la survie sans récidive tumorale. Application of the Six Sigma Methodology and Engineering methods to reduce the rate of defective products in the denim line in an industrial laundry in Lima - Peru

Este proyecto de investigación consiste en la elaboración de una propuesta de mejora reducir el índice de productos defectuosos en la línea denim en una lavandería industrial de Lima – Perú, aplicando la metodología Six Sigma y la Ingeniería de Métodos. La realización de este trabajo inició con la búsqueda de información de la situación actual del sector en estudio, sector textil. Una vez concluido con los antecedentes del proyecto de investigación, se procedió a realizar el diagnóstico del problema actual de la empresa en estudio. Primero, se identificó la problemática actual mediante los registros manuales, para traspasar información a una base de datos e identificar su indicador principal y su impacto económico. Seguidamente, se analizaron las causas raíz y se trazaron los objetivos del proyecto. El diseño y desarrollo de la propuesta, se elaboró vinculando cada una de las causas raíz con una solución encontrada en la revisión de la literatura. Seguidamente, se desarrolló el diseño del nuevo modelo propuesto y la descripción de este. Una de las últimas fases del proyecto de investigación fue la validación del proyecto, la cual consistió en aplicar mediante un plan piloto el modelo propuesto. Donde, después de su aplicación, se midieron y compararon los resultados obtenidos con los determinados en la situación inicial. Asimismo, se realizó una evaluación de los impactos del proyecto. La evaluación económica consistió en formular todas las consideraciones para la implementación del proyecto en un contexto real considerando todos los beneficios, costos, gastos, inversiones, entre otros conceptos fundamentales que permitiría observar los resultados económicos a la gerencia de la empresa en estudio. Por último, se evaluaron los impactos no económicos que se observaron durante el desarrollo de todo el proyecto y su implementación.This research project consists of the development of an improvement proposal to reduce the rate of defective products in the denim line in an industrial laundry in Lima - Peru, applying the Six Sigma methodology and Methods Engineering. The completion of this work began with the search for information on the current situation of the sector under study, the textile sector. Once the background of the research project was concluded, the diagnosis of the current problem of the company under study was carried out. First, the current problem was identified through manual records, to transfer information to a database and identify its main indicator and its economic impact. Next, the root causes were analyzed, and the project objectives were outlined. The design and development of the proposal was developed by linking each of the root causes with a solution found in the literature review. Next, the design of the new proposed model and its description were developed. One of the last phases of the research project was the validation of the project, which consisted of applying the proposed model through a pilot plan. Where, after its application, the results obtained were measured and compared with those determined in the initial situation. Likewise, an evaluation of the impacts of the project was carried out. The economic evaluation consisted of formulating all the considerations for the implementation of the project in a real context, considering all the benefits, costs, expenses, investments, among other fundamental concepts that would allow the management of the company under study to observe the economic results.
