Breast ultrasound scans – surgeons' expectations
Recent years have witnessed a dynamic development of mammary gland imaging techniques,particularly ultrasonography and magnetic resonance imaging. A challengerelated to these studies is the increase in the precision of the anatomical assessment of breast, particularly for early detection of subclinical lesions, performance of ultrasound-guided biopsy procedures, and accurate preoperative location of pathological lesions so as to optimize the surgical treatment. Ultrasound imaging is a primary and baseline diagnostic procedure the patient with suspected pathological lesions withinbreast is referred to by the surgeon. Lesions visualized in ultrasound scans are classified according to the BI-RADS US assessment categories. The successive categories (2 through 6) encompass individual pathological lesions, estimating the risk of malignancy and provide guidelines for further diagnostic and therapeutic management. This article described the important aspects of ultrasonographic imaging of focal lesions within the breasts as signifi cant from the standpoint of surgical treatment of patients falling within BI-RADS US categories 3, 4, 5, and 6. Attention is drawn to the importance of ultrasound scans in the assessment of axillary fossa lymph nodes before the decision regarding the surgical treatment
Metastatic tumors of pancreas — whether and when surgical intervention is gainful for diseased people. Retrospective analysis of data from three surgery centers
Introduction. Metastatic tumors in the pancreas constitute a small (2–5%), but steadily growing percentage of all neoplasms of this organ. The aim of the study was to analyze the surgical treatment of metastases in the pancreas, in particular its effectiveness and the risk of surgical complications. Material and methods. The retrospective analysis was performed on 21 patients treated surgically in three surgical centers. This group included 16 women (76%) and 5 men (24%). The median age at the time of diagnosis of the underlying disease was 55.5 years (41–71). We analyzed the location of secondary lesions, their size, number, type and radicality of surgical intervention, histopathological diagnosis of the disease, as well as the occurrence of postoperative complications. Results. The starting point for metastatic lesions in 16 patients (76%) was renal cell carcinoma (RCC), in other cases — skin melanoma, eyeball melanoma, adenocarcinoma of the rectum, non-small-cell bronchi carcinoma. R0 radical surgery was performed in 15 patients (74%) and perioperative mortality was 5% (1 patient died). The median total survival time was 151 months (10–342), with 168 months for RCC (25–344) and 62 months for the remaining cancers (10–241). Conclusions. The main factor that determines the patient’s future fate is the type of primary neoplasm and surgical treatment of metastatic tumors in the pancreas is the best solution for selected RCC cases
Risk of malignancy in adrenal tumors in patients with a history of cancer
PurposeAdrenal gland is a common site of metastasis and on the other hand, metastases are the most frequent malignant adrenal tumors. The aim of this study was to estimate the risk of malignancy in suspicious adrenal mass in patients with a history of cancer.MethodsThis is a single-center retrospective analysis of patients with adrenal tumors treated previously for different types of cancers. Between 2004 and 2021 a hundred and six such patients were identified. Mean age of patients was 62.6 years (30-78), and mean time from oncologic treatment was 55.8 months (0-274). The most common primary cancer was kidney (RCC): 29 (27.4%), colon/rectum (CRC): 20 (18.9%) and lung (NSCLC): 20 (18.9%).ResultsOf 106 patients, 12 had hormonally active (HA) (11,3%) and 94 (88,7%) non active (HNA) tumors In group of patients with HA tumours 4 had hypercortisolaemia and 8 had elevation of urinary metanephrines. In the first group of HA patients pathology confirmed preoperative diagnosis of adrenocortical cancer and no metastasis was found. In all patients from the second group pheochromocytomas were confirmed. Primary (PM) and secondary (SM) malignancies were found in 50 patients (47.2%). In hormone inactive group only SM - 46/94 (48.9%) were diagnosed. The odds that adrenal lesion was a metastasis were higher if primary cancer was RCC (OR 4.29) and NSCLC (OR 12.3). Metastases were also more likely with high native tumor density, and bigger size in CT. The cut-off values for tumor size and native density calculated from receiver operating characteristic (ROC) curves were 37mm and 24, respectively.ConclusionRisk of malignancy of adrenal mass in a patient with a history of cancer is high (47,2%), regardless of hormonal status. 47,2% risk of malignancy. In preoperative assessment type of primary cancer, adrenal tumour size and native density on CT should be taken into consideration as predictive factors of malignancy. Native density exceeding 24 HU was the strongest risk factor of adrenal malignancy (RR 3.23), followed by history of lung or renal cancer (RR 2.82) and maximum tumor diameter over 37 mm (RR 2.14)
Guzy przerzutowe trzustki — czy i kiedy leczenie operacyjne jest korzystne dla chorego. Analiza retrospektywna materiału trzech ośrodków chirurgicznych
Wstęp. Guzy przerzutowe trzustki stanowią niewielki (2–5%), ale stale rosnący, odsetek wszystkich nowotworów tego narządu. Celem pracy była analiza leczenia operacyjnego przerzutów nowotworowych do trzustki, w szczególności jego skuteczności i ryzyka powikłań chirurgicznych.
Materiał i metody. Analizie retrospektywnej poddano 21 chorych leczonych operacyjnie w trzech ośrodkach chirurgicznych. W tej grupie znajdowało się 16 kobiet (76%) i 5 mężczyzn (24%). Mediana wieku chorych w chwili rozpoznania choroby podstawowej wyniosła 55,5 roku (41–71). Przeanalizowano lokalizację zmian wtórnych, ich wielkość, liczbę, typ oraz radykalność interwencji chirurgicznej, rozpoznanie histopatologiczne choroby, a także wystąpienie powikłań pooperacyjnych.
Wyniki. Punktem wyjścia zmian przerzutowych u 16 chorych (76%) był rak nerkowokomórkowy nerki (renal cel carcinoma, RCC), w pozostałych przypadkach — czerniak skóry, czerniak gałki ocznej, rak gruczołowy odbytnicy, rak niedrobnokomórkowy oskrzela. Operację o radykalności R0 udało się wykonać u 15 chorych (74%), a śmiertelność okołooperacyjna wynosiła 5% (zmarł 1 chory). Mediana całkowitego czasu przeżycia wynosiła 151 miesięcy (10–342), przy czym dla RCC było to 168 miesięcy (25–344), a dla pozostałych nowotworów 62 miesiące (10–241).
Wnioski. Głównym czynnikiem, który determinuje dalszy los chorego, jest rodzaj nowotworu pierwotnego, a leczenie operacyjne guzów przerzutowych trzustki jest najlepszym rozwiązaniem dla wyselekcjonowanych przypadków RCC

Wyzwaniem dla tych badań pozostaje jeszcze bardziej precyzyjna ocena anatomii piersi, zwłaszcza pod kątem wczesnego wykrywania zmian subklinicznych, monitorowanie zabiegów biopsyjnych pod kontrolą obrazu ultrasonograficznego, dokładna lokalizacja przedoperacyjna nieprawidłowych zmian w celu optymalizacji leczenia chirurgicznego. Badanie ultrasonograficzne jest podstawowym i wyjściowym badaniem diagnostycznym, na które chirurg kieruje pacjentkę z podejrzeniem zmian patologicznych w piersiach. Uwidocznione w tym badaniu zmiany są na podstawie cech morfologicznych przydzielane do konkretnej kategorii wg klasyfikacji BIRADS-usg. Kolejne stopnie tej klasyfikacji (od 2 do 6) obejmują poszczególne nieprawidłowe zmiany (patologie piersi), szacują ryzyko ich złośliwości i zawierają wskazówki dotyczące dalszego postępowania diagnostyczno-terapeutycznego. W artykule omówiono ważne elementy obrazowania ultrasonograficznego zmian ogniskowych w piersiach, istotne z punktu widzenia leczenia chirurgicznego w poszczególnych kategoriach BIRADS-usg 3, 4, 5 i 6. Cribriform carcinoma mimicking breast abscess – case report. Diagnostic and therapeutic management
The authors presents a case of cribriform breast carcinoma in a cyst that clinically imitated
an abscess. The case concerns a 71-year-old female patient treated for ankylosing
spondylitis, with a positive family history of breast cancer. The patient presented at
the surgical clinic for incision of an abscess of the mammary gland localized in the lower
inner quadrant that was a consequence of previous trauma to the right breast. The abscess
was incised and the serosanguineous contents were evacuated. The wound was drained
and antibiotics (Dalacin with Metronidazol) were administered for the period of 10 days.
During the treatment, a cutaneous fistula was formed. At the incision site, a hard thickening
was palpable (tumor). Core needle biopsy of the clinically palpable tumor was
performed and the purulent material from the fistula was collected for a culture test.
Complete blood count did not reveal leucocytosis. In accordance with the obtained sensitivity
report, the patient was started on antibiotics again. Breast ultrasound performed
upon the completion of the antibiotic therapy, in the right breast, revealed two solidcystic
oval lesions with thick echogenic walls and blurred margins. Both masses contained
dense levels of fluid material and solid polycyclic structures. On sonoelastography,
the lesions were heterogeneous with a high Young’s modulus. In the right axillary
fossa, ultrasound examination revealed three abnormal lymph nodes enlarged to 31 mm
length, which were rounded, hypoechoic and without visible sinuses. Histopathology of
the core needle biopsy performed at admittance and after the antibiotic therapy indicated
a breast abscess (presence of fibrinous and partly fibrinopurulent material). The mass was
finally resected to confirm histopathology. The resected material revealed the presence
of an invasive, moderately differentiated cribriform carcinoma, which developed within
a cyst, with a 40% necrotic component. Eighteen months after the commencement of
treatment, the patient remains under oncological supervision and continues hormonal
therapy. There are no signs of relapse or foci of distant metastases. The occurrence of
breast carcinoma within an abscess emphasises the need for comprehensive assessment
and correlation of the clinical picture with imaging and histopathological findings. It also
highlights the necessity to include breast abscess in the differential diagnosis of rare
forms of carcinomas.Przedstawiono przypadek raka sitowatego piersi w torbieli klinicznie imitujący ropień.
Dotyczy on 71‑letniej
pacjentki leczonej z powodu zesztywniającego zapalenia stawów
kręgosłupa, obciążonej rodzinnie rakiem piersi. Kobieta zgłosiła się do poradni chirurgicznej
w celu nacięcia ropnia gruczołu piersiowego, zlokalizowanego w kwadrancie
dolnym wewnętrznym, który powstał w następstwie urazu prawej piersi. Nacięto ropień,
ewakuując treść surowiczo‑krwistą.
Ranę zdrenowano oraz włączono antybiotykoterapię
(Dalacin z Metronidazolem) na okres 10 dni. W trakcie leczenia doszło do powstania
przetoki skórnej. W miejscu po nacięciu ropnia było wyczuwalne twarde zgrubienie
(guz). Wykonano biopsję gruboigłową klinicznie wyczuwalnego guza oraz pobrano treść
ropną z przetoki na posiew. W badaniu morfologii krwi nie występowała leukocytoza.
Włączono ponownie antybiotykoterapię zgodnie z otrzymanym antybiogramem. W badaniu
ultrasonograficznym piersi, wykonanym po zakończonej antybiotykoterapii, w prawej
piersi uwidoczniono dwie lito‑płynowe
owalne zmiany, z obecnością grubych echogennych
ścian, o zatartych brzegach. W obu zmianach występowały poziomy zagęszczonej
treści płynowej oraz lite struktury o policyklicznych kształtach. W sonoelastografii oceniane
zmiany były heterogenne, o wysokich wartościach modułu Younga. W prawym
dole pachowym w badaniu ultrasonograficznym stwierdzono trzy nieprawidłowe węzły
chłonne, powiększone do 31 mm długości, zaokrąglone, hipoechogeniczne, bez widocznych
zatok. Wynik badania histopatologicznego z biopsji gruboigłowej wykonanej przy
przyjęciu oraz po zakończonej antybiotykoterapii wskazywał na ropień piersi (opisano
obecność mas włóknikowych, częściowo włóknikowo‑ropnych).
Dopiero wycięcie całej
zmiany celem weryfikacji histopatologicznej ujawniło obecność inwazyjnego, średnio
dojrzałego raka sitowatego, który rozwinął się w świetle torbieli, z obszarami martwicy
obejmującej około 40% masy guza. Pacjentka po 18 miesiącach od rozpoczęcia leczenia
pozostaje pod opieką onkologiczną, kontynuuje hormonoterapię, nie ma nawrotu choroby
oraz ognisk przerzutów odległych. Opisany przypadek raka piersi w ropniu wskazuje na
potrzebę kompleksowej oceny i korelacji obrazu klinicznego z wynikami badań obrazowych
oraz histopatologicznych, a także uwzględniania w diagnostyce różnicowej ropnia
piersi rzadko występujących postaci raka
