A Study on Factors Affecting the Abstention of Drug Abuse in Private Rehabilitation Institutes in Taiwan — Operation Dawn Taiwan as an Example
The purpose of this study is to understand problems of the abstention of drug abuse in Taiwan. Comparing the model of Operation Dawn Taiwan with that of the government’s official rehabilitation institutes, this paper aims to identify advantages and disadvantages of the model in private institutes. The opinions of all inmates and tutors in Operation Dawn Taiwan, the biggest private rehabilitation institute in Taiwan, were collected via questionnaires. This study’s findings show that factors of relapse for a drug abuser after leaving rehabilitation institutes include: 1) the enticement from drug fellows; 2) incapability to resist the allure of drugs; 3) unemployment or hindrances to job-finding; 4) psychological depression; and 5) his/her desire for drugs. Inmates and tutors hold the same opinion on all models of the abstention of drug abuse. Specific suggestions of the abstention of drug abuse in Taiwan will also be discussed in this paper. Finally, inmates consider that career planning has significant positive correlations with abstention
Pseudozyma spp. and Barnettozyma spp. effectively kill cancer cells in vitro
AbstractCancer is the overall leading cause of death in developed countries and also worldwide, and being able to exploit an effective anticancer drug is the aim of all cancer scientists. However, many of the synthetic drugs produced so far usually cause serious side effects, which reduces their therapeutic efficacy. Discovering new drugs or auxiliary therapies derived from natural products might thus provide a novel opportunity for cancer therapy. A recent study reported that some lethal toxins can maintain their activity after being injected into mice. We therefore used two Pseudozyma spp. and three Barnettozyma spp. to examine whether these killer yeasts can preserve their lethal effect on cancer cells under the physical environment (optimum pH, temperature and osmolality, supporting a living cell accomplishes to proliferate, metabolize, differentiate and survive). Our preliminary results showed that both Barnettozyma spp. and Pseudozyma spp. have stronger cytotoxicity against HepG2 than Chang’s liver cells. According to the results of two-dimensional difference gel electrophoresis (2D-DIGE), a total of 115 and 27 proteins differentially expressed by 1.5-fold or more were observed for HepG2 and Chang’s liver cells, respectively. Feasibility of Pulverized Oyster Shell as a Cementing Material
This research intends to study the cementing potential of pulverized oyster shell, rich in calcium, when mixed with fly ash and soil. Cylindrical compacted soil and cubic lime specimens with different proportions of the shells and fly ash are made to study the strength variance. Soil, which is classified as CL in the USCS system, commercialized pulverized oyster shell, F-type fly ash, and lime are mixed in different weight percentages. Five sample groups are made to study the compressive strength of soil and lime specimens, respectively. The lime cubes are made with 0.45 W/B ratio and the cylindrical soils are compacted under the standard Procter compaction process with 20% moisture content. The results show that increment of shell quantity result to lower strength on both the soil and lime specimens. In a 56-day curing, the compressive strength of the lime cubes containing fly ash increases evidently while those carrying the shell get little progress in strength. The soil specimens containing fly ash gradually gain strength as curing proceeds. It suggests that mixtures of the shell and fly ash do not process any Pozzolanic reaction nor help to raise the unconfined strength of the compacted soil through the curing
Metastatic Gallbladder Cancer Presenting as a Gingival Tumor and Deep Neck Infection
Gallbladder cancer has an extremely poor prognosis because it is often diagnosed at an advanced stage. We describe a 63-year-old woman who was treated 4 years previously for gallbladder cancer, with laparoscopic cholecystectomy and secondary hepatectomy after presenting with acute cholecystitis and gallbladder rupture. At her second presentation, she had a left lower gingival tumor and deep neck infection. Incision and drainage and tumor biopsies were performed, and pathology at both sites revealed adenocarcinoma. Positron emission tomography revealed other tumors in the left breast and left lower lung field, which were both proven to be adenocarcinoma by biopsy. The patient's presentation with a metastatic oral tumor was rare. Although the incidence is very low, physicians should consider the possibility of metastatic cancer in a patient with a history of cancer, who presents with new oral tumor or deep neck infection

Comparative global immune-related gene profiling of somatic cells, human pluripotent stem cells and their derivatives: implication for human lymphocyte proliferation.
Human pluripotent stem cells (hPSCs), including embryonic stem cells (ESCs) and induced PSCs (iPSCs), represent potentially unlimited cell sources for clinical applications. Previous studies have suggested that hPSCs may benefit from immune privilege and limited immunogenicity, as reflected by the reduced expression of major histocompatibility complex class-related molecules. Here we investigated the global immune-related gene expression profiles of human ESCs, hiPSCs and somatic cells and identified candidate immune-related genes that may alter their immunogenicity. The expression levels of global immune-related genes were determined by comparing undifferentiated and differentiated stem cells and three types of human somatic cells: dermal papilla cells, ovarian granulosa cells and foreskin fibroblast cells. We identified the differentially expressed genes CD24, GATA3, PROM1, THBS2, LY96, IFIT3, CXCR4, IL1R1, FGFR3, IDO1 and KDR, which overlapped with selected immune-related gene lists. In further analyses, mammalian target of rapamycin complex (mTOR) signaling was investigated in the differentiated stem cells following treatment with rapamycin and lentiviral transduction with specific short-hairpin RNAs. We found that the inhibition of mTOR signal pathways significantly downregulated the immunogenicity of differentiated stem cells. We also tested the immune responses induced in differentiated stem cells by mixed lymphocyte reactions. We found that CD24- and GATA3-deficient differentiated stem cells including neural lineage cells had limited abilities to activate human lymphocytes. By analyzing the transcriptome signature of immune-related genes, we observed a tendency of the hPSCs to differentiate toward an immune cell phenotype. Taken together, these data identify candidate immune-related genes that might constitute valuable targets for clinical applications
Feasibility of Bispectral Index-Guided Propofol Infusion for Flexible Bronchoscopy Sedation: A Randomized Controlled Trial
There are safety issues associated with propofol use for flexible bronchoscopy (FB). The bispectral index (BIS) correlates well with the level of consciousness. The aim of this study was to show that BIS-guided propofol infusion is safe and may provide better sedation, benefiting the patients and bronchoscopists.After administering alfentanil bolus, 500 patients were randomized to either propofol infusion titrated to a BIS level of 65-75 (study group) or incremental midazolam bolus based on clinical judgment to achieve moderate sedation. The primary endpoint was safety, while the secondary endpoints were recovery time, patient tolerance, and cooperation.The proportion of patients with hypoxemia or hypotensive events were not different in the 2 groups (study vs. control groups: 39.9% vs. 35.7%, p = 0.340; 7.4% vs. 4.4%, p = 0.159, respectively). The mean lowest blood pressure was lower in the study group. Logistic regression revealed male gender, higher American Society of Anesthesiologists physical status, and electrocautery were associated with hypoxemia, whereas lower propofol dose for induction was associated with hypotension in the study group. The study group had better global tolerance (p<0.001), less procedural interference by movement or cough (13.6% vs. 36.1%, p<0.001; 30.0% vs. 44.2%, p = 0.001, respectively), and shorter time to orientation and ambulation (11.7±10.2 min vs. 29.7±26.8 min, p<0.001; 30.0±18.2 min vs. 55.7±40.6 min, p<0.001, respectively) compared to the control group.BIS-guided propofol infusion combined with alfentanil for FB sedation provides excellent patient tolerance, with fast recovery and less procedure interference.ClinicalTrials. gov NCT00789815
Case report: Aortoesophageal fistula—an extremely rare but life-threatening cardiovascular cause of hematemesis
Aortoesophageal fistula (AEF) is an extremely rare cardiovascular etiology of hematemesis and upper gastrointestinal bleeding. As such, its recognition and diagnosis are challenging and may be delayed when such patients present to the emergency department (ED). Without timely surgical intervention, AEF is almost always fatal. Awareness of AEF as a possible diagnosis and consequently early identification of these patients presenting to the ED are therefore crucial in optimizing clinical outcomes. We report a 45-year-old male presenting to the ED with the classical triad of an AEF (Chiari's triad)—midthoracic pain or dysphagia, a sentinel episode of minor hematemesis, then massive hematemesis with risk of exsanguination. The case report highlights the importance of considering the differential diagnosis of AEF when evaluating patients presenting to the ED with hematemesis, especially if they have predisposing risk factors such as prior aortic or esophageal surgeries, aortic aneurysms, or thoracic malignancies. Patients suspected of having AEF should be prioritized for early computed tomography angiography to expedite diagnosis and treatment
Metrology Camera System of Prime Focus Spectrograph for Subaru Telescope
The Prime Focus Spectrograph (PFS) is a new optical/near-infrared multi-fiber
spectrograph designed for the prime focus of the 8.2m Subaru telescope. PFS
will cover a 1.3 degree diameter field with 2394 fibers to complement the
imaging capabilities of Hyper SuprimeCam. To retain high throughput, the final
positioning accuracy between the fibers and observing targets of PFS is
required to be less than 10um. The metrology camera system (MCS) serves as the
optical encoder of the fiber motors for the configuring of fibers. MCS provides
the fiber positions within a 5um error over the 45 cm focal plane. The
information from MCS will be fed into the fiber positioner control system for
the closed loop control. MCS will be located at the Cassegrain focus of Subaru
telescope in order to to cover the whole focal plane with one 50M pixel Canon
CMOS camera. It is a 380mm Schmidt type telescope which generates a uniform
spot size with a 10 micron FWHM across the field for reasonable sampling of
PSF. Carbon fiber tubes are used to provide a stable structure over the
operating conditions without focus adjustments. The CMOS sensor can be read in
0.8s to reduce the overhead for the fiber configuration. The positions of all
fibers can be obtained within 0.5s after the readout of the frame. This enables
the overall fiber configuration to be less than 2 minutes. MCS will be
installed inside a standard Subaru Cassgrain Box. All components that generate
heat are located inside a glycol cooled cabinet to reduce the possible image
motion due to heat. The optics and camera for MCS have been delivered and
tested. The mechanical parts and supporting structure are ready as of spring
