    Maritime Claims in the China Seas: Current State Practices

    This Article comments on the maritime practices of coastal states in East Asia with reference to their territorial, contiguous and economic jurisdictions. The author suggests that there are few other regions of the world where coastal State\u27s claims to maritime jurisdiction are so varied, and that the geographical and political circumstances in the region are so complex that only a dozen countries share a cross-section of all issues before the current UN Law of the Sea Conference. The author presents an examination of the current and potential bilateral and multilateral problems faced by China, North and South Korea, Japan, Vietnam, and the Philippines, and how the Convention of the Law of the Sea will affect these problems

    Inter- and Intra-observer Reliability of MRI for Lumbar Lateral Disc Herniation

    Background: The authors analyzed inter- and intra-observer agreement with respect to interpretation of simple magnetic resonance T1- and T2-weighted axial and sagittal images for the diagnosis of lumbar lateral disc herniation, including foraminal and extraforaminal disc herniations.Methods: Forty-two patients in whom lumbar lateral disc herniation was suspected or confirmed by simple magnetic resonance imaging at one institute between May 2003 and December 2004 were included. The magnetic resonance images consisting of T1- and T2-weighted axial and sagittal images, and these were reviewed blindly and independently by three orthopaedic spine surgeons in a random manner. The images were interpreted as positive or negative for lateral disc herniation on 2 different occasions 3 months apart. Results were analyzed using Cohens kappa statistic, and strengths of agreements were determined using the Landis and Koch criteria.Results: The kappa values for inter-observer agreement averaged 0.234 (0.282, 0.111, and 0.308 respectively) on the first occasion, and 0.166 (0.249, 0.111, and 0.137 respectively) on the second occasion, with an overall mean value of 0.200. Thus, the strength of agreement was only slight-to-fair according to the Landis and Koch criteria. Kappa values for intra-observer agreement averaged 0.479 (0.488, 0.491, and 0.459 respectively), indicating moderate agreement.Conclusions: The present study indicates that simple magnetic resonance imaging is not a reliable imaging modality for diagnosing lumbar lateral disc herniation.     Cytokeratin Autoantibodies: Useful Serologic Markers for Toluene Diisocyanate-Induced Asthma

    To evaluate the clinical significance of autoantibodies to three major epithelial cytokeratins (CK) - CK8, CK18, and CK19 - we compared 66 patients with toluene diisocyanate (TDI)-induced asthma (group I) with three control groups: 169 asymptomatic exposed subjects (group II), 64 patients with allergic asthma (group III), and 123 unexposed healthy subjects (group IV). Serum IgG, specific for human recombinant CKs, were measured by ELISA (enzyme linked immunosorbent assay), and ELISA inhibition tests were performed. The existence of these antibodies was confirmed by IgG immunoblot analysis. Anti-TDI-HSA (human serum albumin) IgE and IgG antibodies were measured by ELISA in the same set of the patients. The prevalence of CK8, CK18, and CK19 auotantibodies in group I was significantly higher than in the other three groups. Results of the ELISA inhibition test showed significant inhibition with the addition of three CKs in a dose-dependent manner. No significant association was found between CK autoantibodies and the prevalence of anti-TDI-HSA IgG and IgE antibodies. These results suggest that autoantibodies to CK18 and CK19 can be used as serologic markers for identifying patients with TDI-induced asthma among exposed workers

    The Effect of Stimulation Frequency on the Ionic Currents in Single Atrial Cells of the Rabbit

    In single atrial cells isolated from rabbit hearts the calcium current and [Caj-dependent transient outward current were recorded using the whole-cell clamp technique and the effect of stimulation frequency on these currents was investigated. Voltage dependent transient outward current, which contributes the initial, rapid repolarization phase of the action potential and is frequency-dependent, was also investigated. Increasing the stimulation frequency from O. 025 Hz to 1 Hz had no effect on the calcium current and [Caj-dependent transient outward current and greatly inhibited voltage-dependent transient outward current. The amplitude of voltage dependent transient outward current increased as the membrane potential became depolarized, its steady-state inactivation spans the voltage range -70 mV to -10 mVand steady-state activation curve -30 mV to 30 mV. Within the range of the resting membrane potential (at -70 mV), the voltage-dependent recovery time constant was 1. 3 s. The reversal potential was about -50 mV. Voltage-dependent transient outward current was inhibited by K-channel blockers and not inhibited by modulation of [Cali. From the above findings, it is concluded that due to the amplitude and voltage-dependent recovery time constant which were the basic mechanisms for frequency-dependency, the voltage- dependent transient outward current contributes the initial, rapid repolarization phase and changed the action potential configuration according to stimulation frequency in the rabbit atrium

    Stress-induced nuclear translocation of CDK5 suppresses neuronal death by downregulating ERK activation via VRK3 phosphorylation

    Although extracellular signal-related kinase 1/2 (ERK 1/2) activity is generally associated with cell survival, prolonged ERK activation induced by oxidative stress also mediates neuronal cell death. Here we report that oxidative stress-induced cyclin-dependent kinase 5 (CDK5) activation stimulates neuroprotective signaling via phosphorylation of vaccinia-related kinase 3 (VRK3) at Ser 108. The binding of vaccinia H1-related (VHR) phosphatase to phosphorylated VRK3 increased its affinity for phospho-ERK and subsequently downregulated ERK activation. Overexpression of VRK3 protected human neuroblastoma SH-SY5Y cells against hydrogen peroxide (H2O2)-induced apoptosis. However the CDK5 was unable to phosphorylate mutant VRK3, and thus the mutant forms of VRK3 could not attenuate apoptotic process. Suppression of CDK5 activity results in increase of ERK activation and elevation of proapoptotic protein Bak expression in mouse cortical neurons. Results from VRK3-deficient neurons were further confirmed the role of VRK3 phosphorylation in H2O2-evoked ERK regulation. Importantly, we showed an association between phospho-VRK3 levels and the progression of human Alzheimer’s disease (AD) and Parkinson’s disease (PD). Together our work reveals endogenous protective mechanism against oxidative stress-induced neuronal cell death and suggest VRK3 as a potential therapeutic target in neurodegenerative diseases.1186Ysciescopu

    Multivariate analysis of prognostic factors in patients with pulmonary actinomycosis

    BACKGROUND: There have been few studies of pulmonary actinomycosis, which is an uncommon anaerobic infection. Consequently, the optimal therapeutic regimen, appropriate duration of treatment, long-term prognosis, and factors predicting prognosis are not well established. METHODS: We retrospectively reviewed the medical records of histopathologically confirmed cases of pulmonary actinomycosis seen between November 2003 and December 2012. RESULTS: The study included 68 patients with a mean age of 58.4 ± 11.6 years. Of the 68, initial surgery was performed in 15 patients (22.1%), while the remaining 53 (77.9%) received antibiotic therapy initially. In the initial antibiotic group, 45/53 (84.9%) were cured without relapse (median antibiotic duration 5.3 months). 5/53 (9.4%) patients were refractory medically (median antibiotic duration 9.7 months), and 3/53 (5.7%) experienced a recurrence (median time to relapse 35.3 months). In the initial surgery group, 14/15 (93.3%) were cured and treatment failure occurred in one (6.7%). In the multivariate analysis, the absence of an antibiotic response at 1 month was the only independent factor associated with a poor treatment outcome, with an adjusted odds ratio of 49.2 (95% CI, 3.34–724.30). There was no significant difference in treatment outcome based on the size of the parenchymal lesion, comorbidities, whether intravenous antibiotics were used, antibiotic therapy duration, or whether the initial treatment was surgical. CONCLUSIONS: Antibiotic treatment with or without surgery was effective for treatment of pulmonary actinomycosis. Nevertheless, treatment failure or recurrence occurred in a considerable proportion of patients, especially those resistant to the initial antibiotic treatment

Vi tolkar att rÄdande lÀroplaner beskriver ett uppdrag som ligger i linje med ett formativt arbetssÀtt. Syftet var att ta reda pÄ om lÀrare i samhÀllskunskap som arbetar pÄ gymnasiet arbetar formativt och hur de gör det. Ett annat vanligt namn för formativ bedömning Àr bedömning för lÀrande (BFL). Genom kvalitativa intervjuer har vi frÄgat gymnasielÀrare i samhÀllskunskap hur de arbetar med bedömning, om de arbetar formativt och om det finns nÄgon samsyn pÄ deras respektive gymnasieskolor kring bedömning. Vi ville ocksÄ veta vad formativt arbete innebÀr. Bedömning för lÀrande fokuserar pÄ lÀrandet och hur eleven kan utvecklas och nÄ framgÄng i lÀrandet. Genom att konkretisera mÄl, syfte och kunskapskvaliteter sÄ att eleven förstÄr dessa kan eleven fÄ syn pÄ sitt eget lÀrande. NÀr eleven ges makt över sitt eget lÀrande leder det till ökad motivation. I formativ bedömning arbetar lÀraren bland annat med feedback och kamrat- och sjÀlvvÀrdering. VÄrt resultat visar att lÀrarna vi intervjuat i stor utstrÀckning arbetar summativt med formativa inslag i varierande grad, förutom en av lÀrarna som arbetar pÄ en gymnasieskola i BorÄs som arbetar enligt bedömning för lÀrande. Studien visar att det rÄder brist pÄ samsyn kring bedömning pÄ gymnasie-skolorna, förutom pÄ skolan i BorÄs dÀr det finns tid avsatt för möten och samtal kring formativ bedömning. Litteraturen visar pÄ de positiva effekterna av bedömning för lÀrande vilket ocksÄ styrks av intervjun med lÀraren i BorÄs

    Anterior Decompression and Fusion for the Treatment of Cervical Myelopathy Caused by Ossification of the Posterior Longitudinal Ligament: A Narrative Review

    Occasionally, ossification of the posterior longitudinal ligament (OPLL) causes cord compression, resulting in cervical myelopathy. OPLL differs from other causes of cervical spondylotic myelopathy in several ways, and the surgical strategy should be chosen with OPLL’s characteristics in mind. Although both the anterior and posterior approaches are effective surgical methods for the treatment of OPLL cervical myelopathy, they each have their own set of benefits and drawbacks. Anterior decompression and fusion (ADF) may improve neurological recovery, restore lordosis, and prevent OPLL mass progression. The benefits can be seen in patients with a high canal occupying ratio or kyphotic alignment. We discussed the benefits, limitations, indications, and surgical techniques of ADF for the treatment of OPLL-induced cervical myelopathy in this narrative
