    Evidence for the exacerbation of lymphedema of geochemical origin, podoconiosis, by onchocerciasis

    SummaryThe study was conducted to investigate a variation in the distribution of endemic elephantiasis previously determined to be of geochemical origin in three neighbouring and essentially homogenous villages, Bambili, Bambui and Finge of the Bambui Health District of NW Cameroon. A total of 301 subjects were examined for onchocerciasis and lymphatic filariasis in the area using standard procedures. The onchocercal microfilarial prevalence varied from 6.5% in Bambili through 20.4% in Bambui to 60.4% in Finge. The onchocercal serological prevalence based on IgG4 detection followed a similar trend. By contrast, blood microfilariae were absent in the area as verified by use of sensitive techniques. The community prevalence of elephantiasis varied from 1.1% in Bambili to 4.4% in Bambui and 10.4% in Finge. The correlation between the parasitological prevalence of onchocerciasis and the prevalence of lymphedema in the three villages was strong (r=0.99, p<0.05). We confirm that the elephantiasis in the area is of geochemical origin and the results suggest that it is being exacerbated by onchocercal lymphadenitis

    Isolation and molecular identification of Salmonella with high multidrug resistance to first line typhoid antibiotics in Southwest Cameroon

    Background:The prevalence of typhoid fever in Cameroon is high. However, studies on susceptibility of Salmonella to the recommended antibiotics are few and not extensive. This study investigated the susceptibility of Salmonella isolated from clinical specimens in the South West region of Cameroon and the genes responsible for their antibiotic resistance.&nbsp;Methods:&nbsp;Salmonellawas isolated from clinical specimens collected from some medical laboratories in the study area by culture on Salmonella Shigella agar. Identification and resistance genes analysis were done using biochemical tests, PCR and gene sequencing. Disc diffusion test was done using 11 commercial antibiotics and minimum inhibitory concentrations determined by microdilution test to assess antibiotic susceptibility profile. The Multi-Antibiotic Resistance (MAR) index values were calculated.&nbsp;Results:&nbsp;Salmonella was isolated from 50 out of 115 specimens, of which 46 (92 %) were multidrug resistant (MDR) with MAR indices &gt; 0.2.The MDR isolates showed high resistance to penicillins and low resistance to ciprofloxacin. PCR revealed that antibiotic resistance was largely due to the&nbsp;tem-1,&nbsp;Sul 1,&nbsp;dfrA1,&nbsp;floR&nbsp;genes. Sequencing revealed double mutations at codons 83 (Ser83-Phe) and 87 (Asp87-Gly) and one mutation at codon 83 (Ser83-Trp) in the quinolone resistance determining regions of the&nbsp;gyrA gene in two isolates.&nbsp;Conclusion:&nbsp;This study has revealed presence of MDR&nbsp;Salmonella enterica&nbsp;isolates in Southwest Cameroon with several resistance genes.&nbsp;This finding will guide use of antibiotics in treatment of Salmonella infections in the study area and justifies the search for new efficacious antibacterials against the MDR strains

    Selective activity of extracts of Margaritaria discoidea and Homalium africanum on Onchocerca ochengi

    <p>Abstract</p> <p>Background</p> <p>The current treatment of onchocerciasis relies on the use of ivermectin which is only microfilaricidal and for which resistant parasite strains of veterinary importance are increasingly being detected. In the search for novel filaricides and alternative medicines, we investigated the selective activity of crude extracts of <it>Margaritaria discoidea </it>and <it>Homalium africanum </it>on <it>Onchocerca ochengi</it>, a model parasite for <it>O. volvulus</it>. These plants are used to treat the disease in North West Cameroon.</p> <p>Methods</p> <p>Sixteen crude extracts were prepared from various parts of <it>M. discoidea </it>and <it>H. africanum </it>using different organic solvents. The filaricidal activities were determined <it>in vitro</it>. Cytotoxicity of the active extracts was assessed on monkey kidney epithelial cells <it>in vitro </it>and the selectivity indices (SI) of the extracts determined. Acute toxicity of the promising extracts was investigated in mice.</p> <p>Results</p> <p>Four out of the 16 extracts showed microfilaricidal activity based on motility reduction, whereas, none showed macrofilaricidal activity based on the MTT/formazan assay. The methylene chloride extract of <it>H. africanum </it>leaves (HLC) recorded the lowest IC<sub>50 </sub>of 31.25 μg/mL and an IC<sub>100 </sub>of 62.5 μg/mL. The SI for the active extracts ranged from 0.5 - 2.63. No form of acute toxicity was observed in mice. Phytochemical analysis revealed the presence of anthraquinones, sterols and terpenoids in the promising extracts.</p> <p>Conclusions</p> <p>The non-polar extracts of <it>M. discoidea </it>and <it>H. africanum </it>are potential sources of new microfilaricidal lead compounds, and the results support their use in traditional medicine.</p

    Epidemiology of human immunodeficiency virus-1 and hepatitis B virus co-infection and risk factors for acquiring these infections in the Fako division of Southwest Cameroon

    Background Past studies have demonstrated that a large population of Cameroonians are afflicted with human immunodeficiency virus (HIV) and/or hepatitis B virus (HBV) demonstrating a need for better prevention programs. We aim to describe the prevalence of HIV, HBV and HIV/HBV co-infection; examine the association between HIV and HBV; and determine risk correlates associated with HIV and HBV transmission in Southwest Cameroon. Methods A cross-sectional, community-based surveillance study was conducted among adults in five hospitals , one in each of the five health districts of the Fako division of the Southwest region of Cameroon. Participants underwent pre- and post-test counselling, a 30-question survey and blood draw for HIV and HBV serologic testing. To construct a final model, chi-squared tests and logistic regression were used to investigate associations. Results Among 761 participants, 40.32 % were male, mean age was 35.21 ± 12.42 years, and the prevalence of HIV, HBV and HIV/HBV co-infection was 10.69 % , 9.86 % , and 1.16 % , respectively. There was no association between HIV and HBV infection. However, there was a statistically significant crude associated (p-value < 0.05) between HIV and three high-risk sexual behaviour variables: condom use, number of lifetime sexual partners, and age at first sexual intercourse. After adjustment, HIV status continued to be associated with number of lifetime sexual partners (adjusted odds ratio (AOR) = 2.26; 95 % confidence interval (CI) =1.22–4.17) and age at first sexual intercourse (AOR = 2.63; 95 % CI =1.44–4.81). In contrast, none of the high-risk sexual behaviours was associated with HBV. Conclusions The prevalence of HIV and HBV was relatively high in the Southwest region of Cameroon, emphasizing the importance of intervention and treatment programs in this country. Additionally, the results from this study suggest that unlike HIV, HBV is not associated with sexual risk factors and may provide evidence that HBV is acquired through routes other than sexual transmission, warranting further investigation in this region

    Diversity and behavioral activity of Anopheles mosquitoes on the slopes of Mount Cameroon.

    BACKGROUND: Malaria remains endemic in Cameroon, with heterogeneous transmission related to eco-climatic variations, vector diversity and spatial distribution. The intensification of malaria prevention and control through the free distribution of insecticide-treated nets in recent years may have altered the composition, geographic distribution and natural infection rate of Anopheles species, with implications for malaria transmission dynamics. The present study seeks to assess the vectorial diversity, dynamics and infectivity across different seasons and altitudes in relationship to parasite prevalence around the slopes of Mount Cameroon, southwestern region. METHOD: Mosquitoes were sampled (indoors and outdoors) in 11 eco-epidemiological settings at low (18-197 m), intermediate (371-584 m) and high (740-1067 m) altitude by nightly human landing catches. The mosquitoes were identified morphologically and Anopheles gambiae sibling species identified by PCR. Parity status was ascertained by examining the ovaries and the entomological inoculation rates (EIR) determined by Plasmodium falciparum circumsporozoite antigen ELISA of the head-thorax. The prevalence of Plasmodium infection across target communities was assessed using rapid diagnostic tests. RESULTS: A total of 7327 (18.0 mosquitoes/trap/night) mosquitoes were trapped, mainly during the rainy season (5678, 77.5%) and at low altitude (3669, 50.1%). Anopheles spp. (5079, 69.3%) was the most abundant genera and An. gambiae complex (2691, 36.7%) the major vector, varying with altitude (χ2 = 183.87, df = 8, P < 0.001) and season (χ2 = 28.14, df = 4, P < 0.001). Only An. gambiae (s.s.) was identified following molecular analysis of An. gambiae complex siblings. The overall biting peak for An. gambiae complex was 2-3 a.m. Anopheles cinctus was the most abundant secondary vector in the area. The average EIR in the area was 2.08 infective bites per person per night (ib/p/n), higher at low (2.45 ib/p/n) than at intermediate altitude (1.39 ib/p/n) and during the rainy (1.76 ib/p/n) compared to the dry season (0.34 ib/p/n). Anopheles funestus was most infectious overall (28.1%, 16/57) while An. gambiae had the highest inoculation rates averaging 1.33 ib/p/n. Most Anopheles species across all altitudes and seasons were parous, highest in communities with the highest proportion of malaria parasite infections. CONCLUSION: Anopheles gambiae (s.s.) remains the major malaria vector in the area and An. cinctus possibly a secondary vector of the disease in the slopes of Mt. Cameroon. The seasonal and altitudinal effects on the distribution of these mosquitoes may have implications for the transmission of malaria and its control strategies in the area. Regular monitoring of the bionomics of local Anopheles vector species and targeted control interventions in the 'hotspots' is necessary to curb the prevalence of the infection and incidence of disease

    Mycobacterium tuberculosisis the causative agent of tuberculosis in the southern ecological zones of Cameroon, as shown by genetic analysis

    BACKGROUND: Tuberculosis (TB) is a major cause of mortality and suffering worldwide, with over 95% of TB deaths occurring in low- and middle-income countries. In recent years, molecular typing methods have been widely used in epidemiological studies to aid the control of TB, but this usage has not been the case with many African countries, including Cameroon. The aims of the present investigation were to identify and evaluate the diversity of the Mycobacterium tuberculosis complex (MTBC) isolates circulating in two ecological zones of Cameroon, seven years after the last studies in the West Region, and after the re-organization of the National TB Control Program (NTBCP). These were expected to shed light also on the transmission of TB in the country. The study was conducted from February to July 2009. During this period, 169 patients with symptomatic disease and with sputum cultures that were positive for MTBC were randomly selected for the study from amongst 964 suspected patients in the savannah mosaic zone (West and North West regions) and the tropical rainforest zone (Central region). After culture and diagnosis, DNA was extracted from each of the MTBC isolates and transported to the BecA-ILRI Hub in Nairobi, Kenya for molecular analysis. METHODS: Genetic characterization was done by mycobacterial interspersed repetitive unit–variable number tandem repeat typing (MIRU-VNTR) and Spoligotyping. RESULTS: Molecular analysis showed that all TB cases reported in this study were caused by infections with Mycobacterium tuberculosis (98.8%) and Mycobacterium africanum (M. africanum) (1.2%) respectively. We did not detect any M. bovis. Comparative analyses using spoligotyping revealed that the majority of isolates belong to major clades of M. tuberculosis: Haarlem (7.6%), Latin American-Mediterranean (34.4%) and T clade (26.7%); the remaining isolates (31.3%) where distributed among the minor clades. The predominant group of isolates (34.4%) corresponded to spoligotype 61, previously described as the “Cameroon family. Further analysis based on MIRU-VNTR profiles had greater resolving power than spoligotyping and defined additional genotypes in the same spoligotype cluster. CONCLUSION: The molecular characterization of MTBC strains from humans in two ecological regions of Cameroon has shown that M. tuberculosis sensu stricto is the predominant agent of TB cases in the zones. Three decades ago, TB was reported to be caused by M. africanum in 56.0% of cases. The present findings are consistent with a major shift in the prevalence of M. tuberculosis in Cameroon
