    SOFTWARE-AS-A-SERVICE (SAAS) INNOVATION IN THE CONTEXT OF SOFTWARE INDUSTRY: A RESOURCE ORCHESTRATION PERSPECTIVE

    Cloud computing brings a paradigm shift in the software industry and changes the business model of software vendors (SV). Software as a service (SaaS), the most popular form of cloud computing, has been recognized as the fundamental change in the delivery, utilization, and management of software. While the transformation to SaaS requires changes within the organization, SVs must actively take action to attract customers to accept the SaaS business model, the so-called pull strategy. Drawing on the resource orchestration view, we propose that the antecedents (i.e., structuring cloud resources, developing service bundling capability, and leveraging cloud ecosystem) are related to the likelihood of an innovative SaaS, which, in turn, is associated with SaaS attractiveness to users.     DEVELOPING SUPPLY CHAIN DYNAMIC CAPABILITY TO REALIZE THE VALUE OF INTER-ORGANIZATIONAL SYSTEMS

    In face of increasingly complex supply chain, firms are taking steps to develop different kinds of inter-organizational systems (IOS) to facilitate information sharing and sustain competitive competency. These systems are expected to provide great business value, however many of them do not fulfill the expected promise as these systems are relatively more complicated and the usage is across supply chains. Built upon process theory and the view of dynamic capability, this study has defined two supply chain dynamic capabilities (SDC) -- supply chain integration capability and supply chain cooperation capability and proposed that they might significantly moderate IOS performance. A general survey is conducted in Taiwan PC industry to validate the research model. A linear regression is used to testify the hypotheses. The results show that improving SDC can create greater IOS performance. Furthermore, supply chain integration capability has stronger moderating effect of IOS performance than supply chain cooperation capability. These findings contribute to the literature by confirming the influence of SDC on IOS performance and also by showing which SDC is of primary importance to firms

    Spatial heterogeneity of tectonic stress and friction in the crust: new evidence from earthquake focal mechanisms in Taiwan

    We performed inversions of earthquake focal mechanisms in central Taiwan to investigate the heterogeneity of the stress field and fault strength, and temporal variations of stress parameters, friction and pore pressure associated with the 1999 Chi-Chi earthquake. We divided the focal mechanism data into two groups: before and after the Chi-Chi earthquake, and analysed them separately. With the assumption of a uniform stress field, the friction coefficient is mostly within a range of 0.2–0.4 in central Taiwan, which is lower than the commonly quoted laboratory result, 0.6–0.85. The low friction coefficient is also inferred by the rotation of principal stress axes after the Chi-Chi earthquake. By contrast, if we assume that the friction is constant and failures occur on optimally oriented planes, we find that the resulting stress orientations must be spatially variable. However, a large dispersion of stress orientations is not seen in borehole breakouts and fault slip data, implying a constant friction model might be ruled out. Our analysis suggests that either the distribution of the coefficient of friction or pore pressure changed during the 1999 Chi-Chi earthquake. We infer that the pore pressure probably rose in the Chi-Chi rupture area and northern Longitudinal Valley and dropped in the areas south of the coseismic rupture area after the main shock

    Elderly Patients with Laryngeal and Hypopharyngeal Cancer Undergoing Total Pharyngolaryngectomy with a Radial Forearm, Free Flap-reconstructed Phonation Tube

    SummaryBackgroundThe radial forearm, free-flap (RFFF)-reconstructed phonation tube was developed for functional restoration of voice after total pharyngolaryngectomy. We aimed to report the efficacy of RFFF phonation tube after pharyngolaryngectomy with radiotherapy (RT) or concurrent chemoradiation therapy (CCRT) with intensity-modulated radiotherapy (IMRT) for elderly.Materials and methodsTen patients with laryngeal and hypopharyngeal cancer underwent total pharyngolaryngectomy and one-stage reconstruction with an RFFF-accompanied phonation tube, followed by RT or CCRT. Voice restoration was achieved with the RFFF-reconstructed phonation tube. Functional outcomes of phonation and speech were evaluated and scored.ResultsPercentages of stage III and stage IV patients among all participants were 10% and 90%, respectively. The median follow-up time was 31 months (range, 4–67 months). Almost 9 out of 10 (90%) patients experienced phonation efficacy greater than 80%. The maximal phonation time per breath was 70% longer than 3 sec. The graded as mild of wet voice was 90%. Percentage of mild decreased loudness was 60% and that of low and high pitch was 80%. Of the 10 patients, 40% could count more than 10 and 70% could pronounce more than 1 to 5 words per breath. After RT or CCRT, of patients had moderately good to excellent speech intelligibility.ConclusionThe RFFF phonation tube that was used after pharyngolaryngectomy with RT or CCRT with IMRT provided acceptable complications and functional restoration of voice for elderly patients

    Risk factors and outcomes of carbapenem-nonsusceptible Escherichia coli bacteremia: A matched case–control study

    BackgroundInfections due to carbapenem-resistant Enterobacteriaceae have been the emerging problem worldwide. This primary object of this study was to understand the risk factors and clinical outcomes of carbapenem-nonsusceptible Escherichia coli (CNSEc) bacteremia.MethodsWe conducted a matched case–control study in a 3,715-bed tertiary care medical center in northern Taiwan. The controls were selected among patients with carbapenem-susceptible E coli and were matched with CNSEc for bacteremia.ResultsFifty-one patients were included in this study (17 cases and 34 controls). Bivariate analysis showed that prior exposure to carbapenems (p<0.001), stay in intensive care units (p=0.016), placement of central venous catheters (p=0.001), chronic liver diseases (p<0.001), uremia with regular dialysis (p=0.004), and mechanical ventilation (p=0.004) were associated with CNSEc bacteremia. Multivariate analysis revealed that prior exposure to carbapenems [odds ratio (OR), 29.17; 95% confidence interval (CI), 1.76–484.70; p=0.019], uremia with regular dialysis (OR, 98.58; 95% CI, 4.02–999; p=0.005) and chronic liver diseases (OR, 27.86; 95% CI, 2.31–335.83; p=0.009) were independent risk factors for CNSEc bacteremia. Compared with carbapenem-susceptible E coli group, CNSEc group had a longer hospital stay (68.4 days vs. 35.8 days; p=0.04) and a higher disease severity, as indicated by a Pittsburgh bacteremia score greater than or equal to 4 (5.6% vs. 2.5%; p=0.015). Patients with CNSEc bacteremia had a higher overall in-hospital mortality rate (94.12% vs. 50.00%; p=0.002), but there was no difference in the 28-day mortality between these two groups.ConclusionsCNSEc bacteremia would lead to a poor outcome among patients with prior exposure to carbapenems, chronic liver disease, and uremia with regular dialysis

    The Volcanic Earthquake Swarm of October 20, 2009 in the Tatun Area of Northern Taiwan

    On October 20, 2009, a series of felt earthquakes with local magnitudes ranging from 2.8 - 3.2 occurred in the Tatun volcanic area off the northern tip of Taiwan. Although there was no damage caused by those earthquakes, many residents in the Taipei metropolitan area, particularly for people who live near the Yangminshan National Park, felt strong ground shaking. In order to know what the possible mechanisms were that generated those earthquakes, we carefully examined seismic data recorded by a dense seismic array in the Tatun volcanic area. During the period between October 18 and 22, 2009 we detected at least 202 micro-earthquakes. Most of the earthquakes were relocated using the double-difference method and were clustered in the shallow crust beneath the Dayoukeng area, which is the strongest fumarole in the Tatun volcanic area. Among these earthquakes, 72 focal mechanisms were determined by polarizing the first P-wave motion. Most earthquakes belonged to normal faulting. An extremely high b-value of 2.17 was obtained from those earthquakes. Based on the seismic variations in both the temporary and spatial distribution as well as an extremely high b-value, we conclude that the earthquake sequence on October 20, 2009 was a typically seismic swarm associated with possible active volcanism in the Tatun volcanic area
