    Abdominal Tuberculosis in Adult: 10-Year Experience in a Teaching Hospital in Central Taiwan

    Background/PurposeTuberculosis (TB) is an important communicable disease worldwide. The clinical presentation of abdominal TB often mimics various gastrointestinal disorders and may delay accurate diagnosis. In this study, we conducted a 10-year retrospective study to investigate the clinical manifestations, treatment responses and outcomes of abdominal TB.MethodsThis retrospective study recruited patients presenting between January 1998 and December 2007; all patients ≥ 18 years of age with a diagnosis of abdominal TB were enrolled. Patient charts were thoroughly reviewed and clinical specimens were processed in the laboratory using the BBL MycoPrep System and BACTEC MGIT 960 Mycobacterial Detection System. Mycobacterium tuberculosis complex was confirmed by acid fast stain and the BD ProbeTec ET System.ResultsDuring the study period, 34 patients were diagnosed with abdominal TB. The mean age was 55+18 years. Fourteen patients (41%) had no risk factors; however, 20 patients (59%) had at least one risk factor. Abdominal pain (94.1%), abdominal fullness (91.2%), anorexia (88.2%) and ascites (76.5%) were the most common presenting symptoms. The peritoneum (88%) was the most commonly involved site. Patients with risk factors such as liver cirrhosis, end-stage renal disease and diabetes mellitus had a higher positive rate of acid-fast stain and mycobacterial culture from abdominal specimens (p = 0.02 and 0.05, respectively). The crude mortality rate was 9% and the attributed mortality rate was 3%.ConclusionIn an endemic area like Taiwan, regardless of whether a patient has risk factors for TB, abdominal TB should be seriously considered as a differential diagnosis when a patient presents with gastrointestinal symptoms and unexplained ascites

    Identification of overexpressed cytokines as serum biomarkers of hepatitis C virus-induced liver fibrosis using bead-based flexible multiple analyte profiling

    Hepatic inflammation is the stimulator to activate hepatic stellate cells (HSCs) and triggers fibrogenesis. Cytokines are produced during liver inflammation and maybe considered as liver fibrosis biomarker. The aim of this study was to investigate whether cytokines can be used as reliable biomarkers of liver fibrosis using flexible multi-analyte profiling (xMAP). A total of 61 chronic hepatitis C patients with different severity of liver fibrosis were enrolled. Liver biopsy was used as standard to assess the severity of fibrosis according to METAVIR classification. Afterward, 15 samples from healthy controls were analyzed and totally 50 cytokines were screened using flexible multi-analyte profiling to discover differential biomarkers. Finally, levels of protein expressions of individual stages of liver fibrosis were measured. In histological examination, the necroinflammatory score (histology activity index, HAI) was increased from F1 to F4 stage in hepatitis C virus (HCV) infected patients, indicating that inflammation was accompanied with the progression of liver fibrosis. Using flexible multi-analyte profiling, four serum cytokines, including IFN-α2 (p=0.023), GRO-α (p=0.013), SCF (p=0.047) and SDF-1α p=0.024), were identified under antibody specific recognition and elevated with HAI score. This study reveals the relationship between cytokines and liver fibrosis, and demonstrated that IFN-α2, GRO-α, SCF and SDF-1 α may be used as biomarkers to predict liver fibrosis.     A Multiple Dependent State Repetitive Sampling Plan Based on Performance Index for Lifetime Data with Type II Censoring

    In this paper, a multiple dependent state repetitive (MDSR) sampling plan based on the lifetime performance index C-L is proposed for lifetime data with type II censoring when the lifetime of a product follows the exponential distribution or Weibull distribution. The optimal parameters of the proposed plan are determined by minimizing the average sample number while satisfying the producer's risk and consumer's risk at corresponding quality levels. Besides, the performance of the proposed plan is compared with that of the existing lifetime sampling plan in terms of the average sample number and operating characteristic curve. Under UV irradiation, the rates of the oxidative polymerization of EDOT monomer along with the reduction of Pt4+ ions were accelerated. In addition, the morphology of PtNPs was also greatly influenced by the UV irradiation; the size of PtNPs was reduced under UV irradiation, which can be attributed to the faster nucleation rate. The immobilized PtNPs showed excellent electrocatalytic activities towards the electroreduction of hydrogen peroxide. The resultant amperometric sensor showed enhanced sensitivity for the detection of H2O2 as compared to that without PtNPs, i.e., only with a layer of PEDOT. Amperometric determination of H2O2 at -0.55 V gave a limit of detection of 1.6 μM (S / N = 3) and a sensitivity of 19.29 mA cm-2 M-1 up to 6 mM, with a response time (steady state, t95) of 30 to 40 s.     Color preferences in engineering are very important, and there exists relationship between color preference and visual comfort. In this study, there are thirty university students who participated in the experiment, supplemented by pre- and posttest questionnaires, which lasted about an hour. The main purpose of this study is to explore the visual effects of different color assignment with subjective color preferences via eye tracking technology. Eye-movement data through a nonlinear analysis detect slight differences in color preferences and visual comfort, suggesting effective physiological indicators as extensive future research discussed. Results found that the average pupil size of eye-movement indicators can effectively reflect the differences of color preferences and visual comfort.     Correlation of virulence genes to clinical manifestations and outcome in patients with Streptococcus dysgalactiae subspecies equisimilis bacteremia

    Background/PurposeStreptococcus dysgalactiae subsp. equisimilis (SDSE) is increasingly recognized as a human pathogen responsible for invasive infection and streptococcal toxic shock syndrome (STSS). The pathogen possesses virulence genes that resemble those found in Streptococcus pyogenes (GAS). We analyzed the association between these specific toxic genes, clinical presentations, and outcome in patients with SDSE infections.MethodsPatients (older than 18 years) with community-acquired invasive bacteremia caused by SDSE bacteremia who were undergoing treatment at China Medical University Hospital from June 2007 to December 2010 were included in this study. Multiplex polymerase chain reaction was performed to identify virulence genes of the SDSE isolates. Demographic data, clinical presentations, and outcome in patients with SDSE infections were reviewed and analyzed.ResultsForty patients with 41 episodes of SDSE bacteremia were reviewed. The median age of the patients with SDSE infection was 69.7 years; 55% were female and 78% had underlying diseases. Malignancy (13, 33%) and diabetes mellitus (13, 33%) were the most common comorbidities. The 30-day mortality rate was 12%. Compared with the survivors, the non-survivors had a higher rate of diabetes mellitus (80% vs. 26%), liver cirrhosis (60% vs.11%), shock (60% vs.17%), STSS (60% vs. 8%), and a high Pittsburgh bacteremia score >4 (40% vs. 6%). Most isolates had scpA, ska, saga, and slo genes, whereas speC, speG, speH, speI, speK, smez, and ssa genes were not detected. speA gene was identified only in one patient with STSS (1/6, 17%). All isolates were susceptible to penicillin, cefotaxime, levofloxacin, moxifloxacin, vancomycin, and linezolid.ConclusionIn invasive SDSE infections, most isolates carry putative virulence genes, such as scpA, ska, saga, and slo. Clinical SDSE isolates in Taiwan remain susceptible to penicillin cefotaxime, and levofloxacin

    Using Sidereal Rotation Period Expressions to Calculate the Sun’s Rotation Period through Observation of Sunspots

    We utilize sidereal rotation period expressions to calculate the sun’s rotation period via sunspot observation. From the well-known astronomical sites, we collected sunspot diagrams for 14 months, from January 2013 to February 2014, to analyze, compare, and implement statistical research. In addition to acquiring the average angular rate of the movement of sunspots, we found that even the same number of sunspots moved at different angular rates, and generally the life of larger sunspots is longer than 10 days. Therefore the larger sunspots moved around the back of the sun, and a handful of relatively smaller sunspots disappeared within a few days. The results show that the solar rotation period varied with the latitude. However, if we take the average of the sunspots at high and low latitudes, we find that the calculated value is very close to the accredited values
