TONGUE DIAGNOSIS OF TRADITIONAL CHINESE MEDICINE FOR RHEUMATOID ARTHRITIS
Rheumatoid arthritis (RA) is a chronic, systemic autoimmune disease with unknown etiology that causes the immune system to attack the joints (synoviums), causing chronic inflammation. According to the traditional Chinese medicine (TCM), RA falls into the category of Impediment disease (「Bi」 syndrome), i.e., poor circulation of qi and blood (stasis). Tongue diagnosis is an important method of TCM to detect blood stasis.
In this study, 74 RA patients, meeting the pre-set criteria, were recruited via rheumatology outpatient clinic and examined by experienced rheumatologist physicians. Two images-one of the tongue and the other, sublingual vessels-of the same patient were taken by a Canon digital camera in a darkroom with uniform lighting conditions. Relevant features of the tongue were extracted by utilizing image processing techniques. Every tongue was classified into corresponding patterns based on the features identified.
The subjects included 62 females and 12 males with an average age of 49.86±13.81 years old, an average morbidity period of 4.56±3.92 years, an average rheumatoid factor(RF) of 225.3±373.8 IU/mL and an average erythrocyte sedimentation rate of (ESR) 40.9±31.9m/hr. According to our study, 86% of the patients with RA have tongues with sublingual vessels with a width of more than 2.7mm, a length of more than 3/5 from tongue tipto sublingual caruncle , or a count of sublingual vessels more than 2. Moreover, since RA index is highly correlated with blood stasis in TCM, a logistic regression is conducted to predict the probability of presence of RA using RF and ESR as explanatory variables. Also, the logistic regression analysis of RA with respect to the conventional tongue diagnosis criteria was performed. Based on the aforementioned studies, we concluded that tongue diagnosis is helpful in detecting blood stasis of RA
Extremity Exercise Program in Breast Cancer Survivors Suffering from Chemotherapy-Induced Peripheral Neuropathy: A Feasibility Pilot Study
Objectives: To evaluate the feasibility of implementation of an extremity exercise program and to examine its preliminary effects in breast cancer survivors suffering from chemotherapy-induced peripheral neuropathy (CIPN). Sample & Setting: Thirteen breast cancer survivors from one hospital in northern Taiwan. Methods and Variables: A single group with repeated measures, and a quasi-experimental design. The intervention program was a four week, home-based extremity exercise program that was comprised of 10 skilled hand exercises and Buerger-Allen exercises. The Total Neuropathy Scale (clinical version), Functional Assessment of Cancer Therapy/Gynecologic Oncology Group, Neurotoxicity (13-Item Version), Identification Pain Questionnaire, and pain Visual Analogue Scale were used to measure CIPN before exercise (T1), during (T2~T4), and after exercise (T5). Qualitative data were also collected at each time point. Data were analyzed by using descriptive statistics, generalized estimating equations, and directed content analysis. Results: None of the participants reported adverse events during the study period. The extremity exercise program significantly improved patient-reported CIPN after intervention at T4 or T5 but was insignificant on clinician-assessed CIPN. The qualitative data of participant experience indicated that this program is feasible and easy to follow. Conclusion: The extremity exercise program is feasible but needs to increase the sample size and prolong the intervention period for confirmation

Tumor cell expression of podoplanin correlates with nodal metastasis in esophageal squamous cell carcinoma
Podoplanin is a mucin-like glycoprotein
expressed in the lymphatic endothelium. It has been
suggested to play a role in lymphangiogenesis, since
podoplanin deficient mice were found to have dilated
malfunctioning lymphatic vessels and lymphedema.
High podoplanin expression in tumor cells was found to
correlate with lymph node metastasis and poor clinical
outcome in patients with oral squamous cell carcinoma
(SCC). However, the prognostic significance of
podoplanin expression in esophageal SCC remains
unexplored. Herein, we studied podoplanin expression in
59 patients who underwent surgical resection of
esophageal SCC, with 43 of them preceded by
preoperative concurrent chemoradiotherapy (CCRT). We
found that high podoplanin expression strongly
correlated with clinical nodal metastasis (cN1;
p=0.0063), which was associated with short survival
(p=0.012). However, there was no direct association
between high podoplanin expression and short survival.
We also found that lymphatic vessel invasion in the
resected esophagus was strongly associated with
pathological nodal metastasis (pN1; p=0.00092). Our
results suggest that podoplanin could also play a role in
tumor aggressiveness in esophageal SCC, as well as in
oral SCC
