    Pilot Study of Labor Induction with Titrated oral Misoprostol

    SummaryObjectiveTo evaluate the safety and efficacy of titrated oral misoprostol for labor induction at term.Materials and MethodsSeventy-seven pregnant women (37 nullipara and 40 multipara), with medical or obstetric indications for labor induction after 37 weeks of gestation and unfavorable cervices (Bishop's score < 7), were induced according to the principles of titrated oral doses of misoprostol against uterine response. Our primary outcome measurements were the percentage of patients who had a vaginal delivery within 24 hours of induction and the interval from induction to vaginal delivery. Secondary measurements included oxytocin requirement, total misoprostol dosage, number of cesarean deliveries, induction failure, uterine hyper-stimulation rates and neonatal outcomes.ResultsSeventy-five women (97.4%) experienced active labor within 24 hours, with 72 (93.5%) completing vaginal delivery within 24 hours. The mean interval from induction to vaginal delivery for all the women was 9.7 hours, with a 2.3-hour active phase. The mean misoprostol dosage was 206 mg, with eight women (10.4%) requiring oxytocin augmentation. There was no uterine hyperstimulation or induction failure, except for seven cases of uterine tachysystole (9.1%).ConclusionTitrated oral misoprostol is a safe and effective method of labor induction because the dosage can be adjusted according to individual response

    Q-enhanced fold-and-bond MEMS inductors

    This work presents a novel coil fabrication technology to enhance quality factor (Q factor) of microfabricated inductors for implanted medical wireless sensing and data/power transfer applications. Using parylene as a flexible thin-film device substrate, a post-microfabrication substrate folding-and-bonding method is developed to effectively increase the metal thickness of the surface-micromachined inductors, resulting in their lower self-resistance so their higher quality factor. One-fold-and-bond coils are successfully demonstrated as an example to verify the feasibility of the fabrication technology with measurement results in good agreements with device simulation. Depending on target specifications, multiple substrate folding-and-bonding can be extensively implemented to facilitate further improved electrical characteristics of the coils from single fabrication batch. Such Q-enhanced inductors can be broadly utilized with great potentials in flexible integrated wireless devices/systems for intraocular prostheses and other biomedical implants

    Polysaccharides obtained from mycelia of Cordyceps militaris attenuated doxorubicin-induced cytotoxic effects in chemotherapy

    Objectives: Fungus Cordyceps militaris has been used as a herbal tonic in traditional Chinese medicine, which could be surface liquid-cultured for mycelia production. To evaluate the potential of polysaccharides obtained from mycelia of Cordyceps militaris (PS-MCM) for attenuation of side-effects of chemotherapy.Methods: Doxorubicin was used to induce cytotoxicity in THP-1 monocytes and EL-4 T cells, and the effects of PS-MCM on cell viability and cytokine production were detected on doxorubicin-treated THP-1 and EL-4 cells.Results: PS-MCM reduced doxorubicin-induced cell death and promoted cell proliferation in THP-1 and EL-4 cells. Moreover, PS-MCM elevated the level of cytokines associated with immune-modulation of THP-1 and EL-4 cells.Conclusion: These findings indicate that PS-MCM has potential for development as a functional food to counteract side effects of chemotherapy.Keywords: Cordyceps militaris, traditional Chinese medicine, doxorubicin, chemotherapy, immunotherapy

    Novel microinjector for carrying bone substitutes for bone regeneration in periodontal diseases

    Background/PurposeTraditionally, guide bone regeneration (GBR) was a widely used method for repairing bone lost from periodontal disease. There were some disadvantages associated with the GBR method, such as the need for a stable barrier membrane and a new creative cavity during the surgical process. To address these disadvantages, the purpose of this study was to evaluate a novel microinjector developed for dental applications. The microinjector was designed to carry bone graft substitutes to restore bone defects for bone regeneration in periodontal diseases. The device would be used to replace the GBR method.MethodsIn this study, the injected force and ejected volume of substitutes (including air, water, and ethanol) were defined by Hooke's law (n = 3). The optimal particle size of bone graft substitutes was determined by measuring the recycle ratio of bone graft substitutes from the microinjector (n = 3). Furthermore, a novel agarose gel model was used to evaluate the feasibility of the microinjector.ResultsThe current study found that the injected force was less than 0.4 N for obtaining the ejected volume of approximately 2 mL, and when the particle size of tricalcium phosphate (TCP) was smaller than 0.5 mm, 80% TCP could be ejected from the microinjector. Furthermore, by using an agarose model to simulate the periodontal soft tissue, it was also found that bone graft substitutes could be easily injected into the gel.ConclusionThe results confirmed the feasibility of this novel microinjector for dental applications to carry bone graft substitutes for the restoration of bone defects of periodontal disease

    Effectiveness of Traditional Chinese Medicine for Liver Protection and Chemotherapy Completion among Cancer Patients

    While traditional Chinese medicine (TCM) is widely used among Chinese patients with cancer, studies evaluating the effectiveness of TCM using objective indicators are rare. We examined the effectiveness of TCM for liver protection and completion of chemotherapy among patients with cancer receiving chemotherapy. We used a case-control design to examine the medical records of patients with cancer who received chemotherapy in a teaching hospital in Taipei in 2004. A total of 184 courses of chemotherapy among 89 patients were studied. Of the 184 courses, 42 used TCM jointly with chemotherapy served as cases, while the remaining 142 courses served as controls. Outcome variables included counts of cancelled or delayed chemotherapies and liver function (aspartate aminotransferase, AST and alanine aminotransferase, ALT) 1 week before, during and 2 weeks after chemotherapy. Generalized estimating equations were used to analyze the data. Patients who had concomitant TCM with chemotherapy had lower serum ALT and AST during chemotherapy than the controls given that the age, sex, cancer stage, radiotherapy sites, cancer diagnosis and potential hepatotoxicity of the chemotherapeutic drugs were controlled for in the model [β  = −3.48, 95% confidence interval (CI) −10.08 to 3.11 for AST; β  = −5.95, 95% CI: −11.47 to −0.44 for ALT]. There was no significant difference between the case and control groups for odds of completing one course of chemotherapy. Use of TCM with chemotherapy resulted in protection of the liver during chemotherapy, as manifested by lower serum AST and ALT levels

    System Design of Internet-of-Things for Residential Smart Grid

    Internet-of-Things (IoTs) envisions to integrate, coordinate, communicate, and collaborate real-world objects in order to perform daily tasks in a more intelligent and efficient manner. To comprehend this vision, this paper studies the design of a large scale IoT system for smart grid application, which constitutes a large number of home users and has the requirement of fast response time. In particular, we focus on the messaging protocol of a universal IoT home gateway, where our cloud enabled system consists of a backend server, unified home gateway (UHG) at the end users, and user interface for mobile devices. We discuss the features of such IoT system to support a large scale deployment with a UHG and real-time residential smart grid applications. Based on the requirements, we design an IoT system using the XMPP protocol, and implemented in a testbed for energy management applications.     Surface-Assisted Laser Desorption/Ionization Mass Spectrometry on Titania Nanotube Arrays

    Titania nanotube arrays (NTA) generated from anodizing processes are tested as the substrate for surface-assisted laser desorption/ionization mass spectrometry (SALDI MS). The background generated from titania NTA is very low, making the approach suitable for the analysis of small molecules. The upper detectable mass is ∼29 kDa. Homogeneous sample deposition leads to good shot-to-shot reproducibility and suitability for quantitative analysis. Additionally, phosphopeptides can be selectively trapped on the titania NTA substrate, as illustrated by simply depositing a tryptic digest of β-casein followed by titania NTA SALDI MS analysis.
