    Clinical significance of obstructive sleep apnea in patients with acute coronary syndrome in relation to diabetes status.

    Objective: The prognostic significance of obstructive sleep apnea (OSA) in patients with acute coronary syndrome (ACS) according to diabetes mellitus (DM) status remains unclear. We aimed to elucidate the association of OSA with subsequent cardiovascular events in patients with ACS with or without DM. Research design and methods: In this prospective cohort study, consecutive eligible patients with ACS underwent cardiorespiratory polygraphy between June 2015 and May 2017. OSA was defined as an Apnea Hypopnea Index ≥15 events/hour. The primary end point was major adverse cardiovascular and cerebrovascular events (MACCEs), including cardiovascular death, myocardial infarction, stroke, ischemia-driven revascularization, or hospitalization for unstable angina or heart failure. Results: Among 804 patients, 248 (30.8%) had DM and 403 (50.1%) had OSA. OSA was associated with 2.5 times the risk of 1 year MACCE in patients with DM (22.3% vs 7.1% in the non-OSA group; adjusted HR (HR)=2.49, 95% CI 1.16 to 5.35, p=0.019), but not in patients without DM (8.5% vs 7.7% in the non-OSA group, adjusted HR=0.94, 95% CI 0.51 to 1.75, p=0.85). Patients with DM without OSA had a similar 1 year MACCE rate as patients without DM. The increased risk of events was predominately isolated to patients with OSA with baseline glucose or hemoglobin A1c levels above the median. Combined OSA and longer hypoxia duration (time with arterial oxygen saturation22 min) further increased the MACCE rate to 31.0% in patients with DM. Conclusions: OSA was associated with increased risk of 1 year MACCE following ACS in patients with DM, but not in non-DM patients. Further trials exploring the efficacy of OSA treatment in high-risk patients with ACS and DM are warranted

    Disrupted Brain Network Efficiency and Decreased Functional Connectivity in Multi-sensory Modality Regions in Male Patients With Alcohol Use Disorder

    Background: Recent studies have reported altered efficiency in selective brain regions and functional networks in patients with alcohol use disorder (AUD). Inefficient processing can reflect or arise from the disorganization of information being conveyed from place to place. However, it remains unknown whether the efficiency and functional connectivity are altered in large-scale topological organization of patients with AUD.Methods: Resting-state functional magnetic resonance imaging (rsfMRI) data were experimentally collected from 21 right-handed males with AUD and 21 right-handed, age-, gender- and education-matched healthy controls (HCs). Graph theory was used to investigate inter-group differences in the topological parameters (global and nodal) of networks and inter-regional functional connectivity. Correlations between group differences in network properties and clinical variables were also investigated in the AUD group.Results: The brain networks of the AUD group showed decreased global efficiency when compared with the HC group. Besides, increased nodal efficiency was found in the left orbitofrontal cortex (OFC), while reduced nodal efficiency was observed in the right OFC, right fusiform gyrus (FFG), right superior temporal gyrus, right inferior occipital gyrus (IOG), and left insula. Moreover, hypo-connectivity was detected between the right dorsolateral prefrontal cortex (DLPFC) and right superior occipital gyrus (SOG) in the AUD group when compared with the HC group. The nodal efficiency of the left OFC was associated with cognitive performance in the AUD group.Conclusions: AUD patients exhibited alterations in brain network efficiency and functional connectivity, particularly in regions linked to multi-sensory modalities. These disrupted topological properties may help to obtain a more comprehensive understanding of large-scale brain network activity. Furthermore, these data provide a potential neural mechanism of impaired cognition in individuals with AUD

    Pseudoaneurysm with Median Nerve Injury Caused by Right Radial Artery Puncture: A Case Report

    Background: Pseudoaneurysm with median nerve injury is a serious complication of radial artery puncture. It is very important to summarize the prevention and treatment experience of this complication through case discussion. Case report: A 66-year-old woman was admitted to the hospital because of “paroxysmal chest tightness and suffocation for 5 days.” Coronary angiography was performed. During insertion of the arterial sheath, the patient experienced severe pain in the right forearm, which radiated to the palm. The puncture sheath did not return blood after the sheath core was withdrawn. The sheath was removed and local compression was used to stop bleeding. There was no obvious bleeding at the puncture point, and the compression was removed 6 hours after the procedure. Local swelling and increased tension were seen in the right forearm. At the 1-week follow-up she exhibited swelling, high local tension, small blisters, and bluish-purple skin of the right forearm, with an acceptable right radial artery pulsation. She had severe pain in the affected limb, which radiated to the thumb, index finger, and middle finger.     A genome-wide association study explores the genetic determinism of host resistance to Salmonella pullorum infection in chickens

    International audienceAbstractBackgroundSalmonella infection is a serious concern in poultry farming because of its impact on both economic loss and human health. Chicks aged 20 days or less are extremely vulnerable to Salmonella pullorum (SP), which causes high mortality. Furthermore, an outbreak of SP infection can result in a considerable number of carriers that become potential transmitters, thus, threatening fellow chickens and offspring. In this study, we conducted a genome-wide association study (GWAS) to detect potential genomic loci and candidate genes associated with two disease-related traits: death and carrier state.MethodsIn total, 818 birds were phenotyped for death and carrier state traits through a SP challenge experiment, and genotyped by using a 600 K high-density single nucleotide polymorphism (SNP) array. A GWAS using a single-marker linear mixed model was performed with the GEMMA software. RNA-sequencing on spleen samples was carried out for further identification of candidate genes.ResultsWe detected a region that was located between 33.48 and 34.03 Mb on chicken chromosome 4 and was significantly associated with death, with the most significant SNP (rs314483802) accounting for 11.73% of the phenotypic variation. Two candidate genes, FBXW7 and LRBA, were identified as the most promising genes involved in resistance to SP. The expression levels of FBXW7 and LRBA were significantly downregulated after SP infection, which suggests that they may have a role in controlling SP infections. Two other significant loci and related genes (TRAF3 and gga-mir-489) were associated with carrier state, which indicates a different polygenic determinism compared with that of death. In addition, genomic inbreeding coefficients showed no correlation with resistance to SP within each breed in our study.ConclusionsThe results of this GWAS with a carefully organized Salmonella challenge experiment represent an important milestone in understanding the genetics of infectious disease resistance, offer a theoretical basis for breeding SP-resistant chicken lines using marker-assisted selection, and provide new information for salmonellosis research in humans and other animals

    Concomitant Retrograde Coronary Venous Infusion of Basic Fibroblast Growth Factor Enhances Engraftment and Differentiation of Bone Marrow Mesenchymal Stem Cells for Cardiac Repair after Myocardial Infarction.

    AIM: Basic fibroblast growth factor (bFGF) increases the migration and viability of bone marrow mesenchymal stem cells (MSCs) in vitro. Retrograde coronary venous infusion can provide both increased regional bFGF concentrations and homogeneous cell dissemination. We determined whether retrograde delivery of bFGF enhances the potency of transplanted MSCs for cardiac repair in a canine infarct model. METHODS AND RESULTS: Under hypoxic conditions, cellular migration was significantly increased in MSCs co-cultured with bFGF compared to vascular endothelial growth factor or insulin-like growth factor, and bFGF promoted MSCs differentiation into a cardiomyocyte phenotype. A canine infarct model was employed by coronary ligation. One week later, animals were subjected to retrograde infusion of combination bFGF (200ng/mL) and MSCs (1×10(8) cells) (n=5), MSCs (1×10(8) cells, n=5), bFGF (200ng/mL, n=5), or placebo (phosphate-buffered saline, n=3). Four weeks after infusion, only the bFGF+MSCs therapy exhibited significantly increased left ventricular ejection fraction (LVEF) by echocardiography (p CONCLUSIONS: Retrograde coronary venous bFGF infusion augments engraftment and differentiation capacity of transplanted MSCs, recovering cardiac function and preventing adverse remodeling. This novel combined treatment and delivery method is a promising strategy for cardiac repair after ischemic injury

    Effects of Sublethal Concentrations of Chlorpyrifos on Olfactory Learning and Memory Performances in Two Bee Species, Apis mellifera and Apis cerana

    Chlorpyrifos is a widely used organophosphorus insecticide. The acute oral 24 h median lethal dose (LD50) value of chlorpyrifos in Apis mellifera and in Apis cerana was estimated to assess differential acute chlorpyrifos toxicity in both bee species. The LD50 values of chlorpyrifos in A. mellifera and in A. cerana are 103.4 ng/bee and 81.8 ng/bee, respectively, which suggests that A. cerana bees are slightly more sensitive than A. mellifera bees to the toxicity of chlorpyrifos. Doses half the acute LD50 of chlorpyrifos were selected to study behavioral changes in both bee species using proboscis extension response assay. A. mellifera foragers treated with chlorpyrifos showed significantly lower response to the 10% sucrose solution compared to control bees after 2, 24 and 48 h. Chlorpyrifos significantly impaired the olfactory learning abilities and 2 h memory retention of forager bees regardless of honey bee species, which may affect the foraging success of bees exposed to chlorpyrifos

    Early ACEI/ARB use and in-hospital outcomes of acute myocardial infarction patients with systolic blood pressure <100 mmHg and undergoing percutaneous coronary intervention: Findings from the CCC-ACS project

    BackgroundFew studies have evaluated whether acute myocardial infarction (AMI) patients with relatively low blood pressure benefit from early ACEI/ARB use in the era of percutaneous coronary intervention (PCI).ObjectivesThis study evaluated the associations of ACEI/ARB use within 24 h of admission with in-hospital outcomes among AMI patients with SBP < 100 mmHg and undergoing PCI.MethodsThis study was based on the Improving Care for Cardiovascular Disease in China-ACS project, a collaborative registry and quality improvement project of the American Heart Association and the Chinese Society of Cardiology. Between November 2014 and December 2019, a total of 94,623 patients with AMI were enrolled. Of them, 4,478 AMI patients with SBP < 100 mmHg and undergoing PCI but without clinically diagnosed cardiogenic shock at admission were included. Multivariable logistic regression and propensity score-matching analysis were used to evaluate the association between early ACEI/ARB use and in-hospital major adverse cardiac events (MACEs), a combination of all-cause death, cardiogenic shock, and cardiac arrest.ResultsOf AMI patients, 24.41% (n = 1,093) were prescribed ACEIs/ARBs within 24 h of admission. Patients with early ACEI/ARB use had a significantly lower rate of MACEs than those without ACEI/ARB use (1.67% vs. 3.66%, p = 0.001). In the logistic regression analysis, early ACEI/ARB use was associated with a 45% lower risk of MACEs (odds ratio: 0.55, 95% CI: 0.33–0.93; p = 0.027). Further propensity score-matching analysis still showed that patients with early ACEI/ARB use had a lower rate of MACEs (1.96% vs. 3.93%, p = 0.009).ConclusionThis study found that among AMI patients with an admission SBP < 100 mmHg undergoing PCI, early ACEI/ARB use was associated with better in-hospital outcomes. Additional studies of the early use of ACEIs/ARBs in AMI patients with relatively low blood pressure are warranted

    Insight-HXMT observations of Swift J0243.6+6124 during its 2017-2018 outburst

    The recently discovered neutron star transient Swift J0243.6+6124 has been monitored by {\it the Hard X-ray Modulation Telescope} ({\it Insight-\rm HXMT). Based on the obtained data, we investigate the broadband spectrum of the source throughout the outburst. We estimate the broadband flux of the source and search for possible cyclotron line in the broadband spectrum. No evidence of line-like features is, however, found up to 150 keV\rm 150~keV. In the absence of any cyclotron line in its energy spectrum, we estimate the magnetic field of the source based on the observed spin evolution of the neutron star by applying two accretion torque models. In both cases, we get consistent results with B1013 GB\rm \sim 10^{13}~G, D6 kpcD\rm \sim 6~kpc and peak luminosity of >1039 erg s1\rm >10^{39}~erg~s^{-1} which makes the source the first Galactic ultraluminous X-ray source hosting a neutron star.Comment: publishe

    Image Segmentation Method on Quartz Particle-Size Detection by Deep Learning Networks

    In the beneficiation of quartz sand, hydraulic classification is a primary way to obtain quartz production in various size fractions. It is essential for plants to measure the particle size of quartz sand during the classification, in time to evaluate the classification efficiency. However, the traditional manual-screening method consumes labor and time, while the particle-size analyzer is expensive. Thus, a size-detection method of quartz-sand particle is proposed in this paper, which is based on a deep learning semantic-segmentation network Fully Convolutional Networks (FCN)-ResNet50. The FCN-ResNet50 network sand segments images, and the average particle size of quartz sand is obtained after converting the pixel-particle size to physical-particle size. Using deep learning, the quartz sand with particle sizes of &minus;40 + 70 (0.212&ndash;0.38 mm), &minus;70 + 100 (0.15&ndash;0.212 mm), &minus;100 + 140 (0.109&ndash;0.15 mm), and &minus;140 + 400 (0.038&ndash;0.109 mm) meshes, can be measured directly. The results showed that the validation accuracy of the FCN-ResNet50 was over 97%, and the loss value was approximately 0.2. Compared with the UNet-Mobile and Deeplab-Xception, the average error of particle-size detection was approximately 0.01 mm, which was close to the manual calibration-software results. This method has the advantages of quick sampling and low equipment costs, increasing the hydraulic-classification efficiency of quartz sand and promoting automation in the concentrator
