Patient Perception of Natural Orifice Transluminal Endoscopic Surgery in an Endoscopy Screening Program in Korea
Purpose Natural orifice transluminal endoscopic surgery (NOTES) is a new method of accessing intracavitary organs in order to minimize pain by avoiding incisions in the body wall. The aim of this study is to determine patients' acceptance of NOTES in Korea and to compare their views about laparoscopic surgery and NOTES for benign and malignant diseases. Materials and Methods The target number of total subjects was calculated to be 540. The subjects were classified into 18 sub-groups based on age groups, gender, and history of prior surgery. The questionnaire elicited information about demographic characteristics, medical check-ups, diseases, endoscopic and surgical histories, marital status and childbirth, the acceptance of NOTES, and the preferred routes for NOTES. In addition, the subjects chose laparoscopic surgery or NOTES for a hypothetical cholecystectomy and rectal cancer surgery, and responded to questions regarding the acceptable complication rate of NOTES, the appropriate cost of NOTES, and the reason(s) why they did not select NOTES. Results: 486 of 540 patients (90.0%) who agreed to participate in this study completed the questionnaire. NOTES was preferred by the following patients: elderly; a history of treatment due to a disease; having regular check-ups; and a history of an endoscopic procedure (p<0.05). The most preferred route for NOTES was the stomach (67.1%). Eighty-four percent of the patients choosing NOTES responded that the complication rate of the new surgical method should be the same or lower than laparoscopic surgery. Vague anxiety over a new surgical method was the most common reason why NOTES was not selected in benign and malignant diseases (64% and 73%), respectively. Conclusion: Patients appear to be interested in the potential benefits of NOTES and would embrace it if their concerns about safety are met. We believe that qualified surgical endoscopists can meet these safety concerns, and that NOTES development has the potential to flourish
Antinociceptive and Anti-Inflammatory Effects of Ethanolic Extracts of Glycine max (L.) Merr and Rhynchosia nulubilis Seeds
The aim of this study was to assess the in vivo potential of ethanolic extracts of Glycine max (L.) Merr. (SoRiTae) and Rhynchosia nulubilis (Yak-Kong) seeds as natural anti-nociceptive and anti-inflammatory agents. To assess the anti-nociceptive and anti-inflammatory potential, the ethanolic extracts of SoRiTae and Yak-Kong seeds were tested in arachidonic acid-induced ear edema, carrageenan induced paw edema, formalin-induced licking time, acetic acid induced writhing and hot plate-induced thermal stimulation in mice. The administration of ethanolic extracts of SoRiTae and Yak-Kong seeds evoked a significant effect of anti-nociceptive and anti-inflammatory activities as compared to standards aminopyrine and indomethacin. The ear edema, paw edema, paw licking time, pain and writhes in mice were significantly reduced (p < 0.05) as compared to the control. The results obtained in this study indicate that both SoRiTae and Yak-Kong soybeans possesses potential anti-nociceptive and anti-inflammatory activities
Genome-Based Construction of the Metabolic Pathways of Orientia tsutsugamushi and Comparative Analysis within the Rickettsiales Order
Orientia tsutsugamushi, the causative agent of
scrub typhus, is an obligate intracellular
bacterium that belongs to the order of
Rickettsiales. Recently, we have reported that
O. tsutsugamushi has a unique
genomic structure, consisting of highly
repetitive sequences, and suggested that it may
provide valuable insight into the evolution of
intracellular bacteria. Here, we have used
genomic information to construct the major
metabolic pathways of
O. tsutsugamushi and performed a
comparative analysis of the metabolic genes and
pathways of O. tsutsugamushi
with other members of the Rickettsiales order.
While O. tsutsugamushi has the
largest genome among the members of this order,
mainly due to the presence of repeated
sequences, its metabolic pathways have been
highly streamlined. Overall, the metabolic
pathways of O. tsutsugamushi
were similar to Rickettsia but
there were notable differences in several
pathways including carbohydrate metabolism, the
TCA cycle, and the synthesis of cell wall
components as well as in the transport systems.
Our results will provide a useful guide to the
postgenomic analysis of
O. tsutsugamushi and lead
to a better understanding of the virulence and
physiology of this intracellular pathogen
Reliability Verification of the Performance Evaluation of Multiphase Pump
The crude oil in an oil well exists in various phases such as gas, seawater, and sand, as well as oil. Therefore, a phase separator is needed at the front of a single-phase pump for pressurization and transfer. On the other hand, the application of a multiphase pump can provide such advantages as simplification of the equipment structure and cost savings, because there is no need for a phase separation process. Therefore, the crude oil transfer method using a multiphase pump is being applied to recently developed oil wells. Due to this increase in demand, technical demands for the development of multiphase pumps are sharply increasing, but the progress of research into related technologies is insufficient, due to the nature of multiphase pumps that require high levels of skills. This study was conducted to verify the reliability of pump performance evaluation using numerical analysis, which is the basis of the development of a multiphase pump. For this study, a model was designed by selecting the specifications of the pump under study. The performance of the designed model was evaluated through numerical analysis and experiment, and the results of the performance evaluation were compared to verify the reliability of the result using numerical analysis
Solid–liquid interface temperature measurement of evaporating droplet using thermoresponsive polymer aqueous solution
The present study aims to measure the solid–liquid interface temperature of an evaporating droplet on a heated surface using a thermoresponsive polymer. Poly(N-isopropylacrylamide) (pNIPAM) was used owing to its sensitive optical and mechanical properties to the temperature. We also measured the refractive index variation of the pNIPAM solution by using the surface plasmon resonance imaging (SPRi). In particular, the present study proposed a new method to measure the solid–liquid interface temperature using the correlation among reflectance, refractive index, and temperature. It was found that the reflectance of a pNIPAM solution decreased after the droplet deposition. The solid–liquid interface temperature, estimated from the reflectance, showed a lower value at the center of the droplet, and it gradually increased along the radial direction. The lowest temperature at the contact line region is present because of the maximum evaporative cooling. Moreover, the solid–liquid interface temperature deviation increased with the surface temperature, which means solid–liquid interface temperature should be considered at high temperature to predict the evaporation flux of the droplet accurately
Comparative study of glenoid version and inclination using two-dimensional images from computed tomography and three-dimensional reconstructed bone models
Background This study was performed to compare glenoid version and inclination measured using two-dimensional (2D) images from computed tomography (CT) scans or three-dimensional (3D) reconstructed bone models. Methods Thirty patients who had undergone conventional CT scans were included. Two orthopedic surgeons measured glenoid version and inclination three times on 2D images from CT scans (2D measurement), and two other orthopedic surgeons performed the same measurements using 3D reconstructed bone models (3D measurement). The 3D-reconstructed bone models were acquired and measured with Mimics and 3-Matics (Materialise). Results Mean glenoid version and inclination in 2D measurements were –1.705º and 9.08º, respectively, while those in 3D measurements were 2.635º and 7.23º. The intra-observer reliability in 2D measurements was 0.605 and 0.698, respectively, while that in 3D measurements was 0.883 and 0.892. The inter-observer reliability in 2D measurements was 0.456 and 0.374, respectively, while that in 3D measurements was 0.853 and 0.845. Conclusions The difference between 2D and 3D measurements is not due to differences in image data but to the use of different tools. However, more consistent results were obtained in 3D measurement. Therefore, 3D measurement can be a good alternative for measuring glenoid version and inclination
Mesenteric extraskeletal osteosarcoma with telangiectatic features: a case report
<p>Abstract</p> <p>Background</p> <p>Extraskeletal osteosarcoma is a rare malignant mesenchymal tumor, with a predominant occurrence in the extremities. Only two cases of mesenteric extraskeletal osteosarcoma have been documented. We describe an unusual case of extraskeletal osteosarcoma with telangiectatic features occurring in the mesentery.</p> <p>Case presentation</p> <p>A 67-year-old male presented with blood-tinged stool of 1-month's duration. On colonoscopy, a solid mass was detected protruding from the colon wall. Computed tomography showed a 15 × 9.7 cm heterogeneously enhancing mass, with mottled calcification and a cystic portion, occupying the left upper quadrant of the abdominal cavity. Curative resection of the tumor was performed, and the excised tumor was composed of large multilocular cysts containing old hematomas and necrotic debris. The histology revealed an osteosarcoma showing osteoid formation and blood-filled spaces lined with atypical cells. Despite postoperative chemotherapy, he developed a recurrent peritoneal mass and multiple lung metastases 3 months postoperatively.</p> <p>Conclusion</p> <p>Given the rarity of cases of mesenteric extraskeletal osteosarcoma, its biologic behavior at this location remains to be determined. However, extraskeletal osteosarcoma with telangiectatic features is an uncommon entity to be recognized because of the possible fatal outcome related to the tumors.</p
Explicitly determined sea ice emissivity and emission temperature over the Arctic for surface‐sensitive microwave channels
Data assimilation of satellite microwave measurements is one of the importantkeys to improving weather forecasting over the Arctic region. However, the useofsurface-sensitivemicrowave-soundingchannelmeasurementsfordataassim-ilation or retrieval has been limited, especially during winter, due to the poorlyconstrained sea ice emissivity. In this study, aiming at more use of those channelmeasurements in the data assimilation, we propose an explicit method for speci-fying the surface radiative boundary conditions (namely emissivity and emittinglayer temperature of snow and ice). These were explicitly determined with aradiativetransfermodelforsnowandiceandwithsnow/icephysicalparameters(i.e. snow/ice depths and vertical distributions of temperature, density, salinity,and grain size) simulated from the thermodynamically driven snow/ice growthmodel. We conducted 1D-Var experiments in order to examine whether thisapproach can help to use the surface-sensitive microwave temperature channelmeasurements over the Arctic sea ice region for data assimilation. Results showthat (1) the surface-sensitive microwave channels can be used in the 1D-Varretrieval, and (2) the specification of the radiative boundary condition at thesurface using the snow/sea ice emission model can significantly improve theatmospheric temperature retrieval, especially in the lower troposphere (500hPato surface). The successful retrieval suggests that useful information can beextracted from surface-sensitive microwave-sounding channel radiances oversea ice surfaces through the explicit determination of snow/ice emissivity andemitting layer temperature
