    Mechanisms for the Oxonolysis of Ethene and Propene: Reliability of Quantum Chemical Predictions

    Reactions of ozone with ethene and propene leading to primary ozonide (concerted and stepwise ozonolysis) or epoxide and singlet molecular oxygen (partial ozonolysis) are studied theoretically. The mechanism of concerted ozonolysis proceeds via a single transition structure which is a partial diradical. The transition structures and intermediates in the stepwise ozonolysis and partial ozonolysis mechanisms are singlet diradicals. Spin-restricted and unrestricted density functional methods are employed to calculate the structures of the closed-shell and diradical species. Although the partial diradicals exhibit moderate to pronounced instability in their RDFT and RHF solutions, RDFT is required to locate the transition structure for concerted ozonolysis. Spin projected fourth-order Møller–Plesset theory (PMP4) was used to correct the DFT energies. The calculated pre-exponential factors and activation energies for the concerted ozonolysis of ethene and propene are in good agreement with experimental values. However, the PMP4//DFT procedure incorrectly predicts the stepwise mechanism as the favored channel. UCCSD(T) predicts the concerted mechanism as the favored channel but significantly overestimates the activation energies. RCCSD(T) is found to be more accurate than UCCSD(T) for the calculation of the concerted mechanism but is not applicable to the diradical intermediates. The major difficulty in accurate prediction of the rate constant data for these reactions is the wide range of spin contamination for the reference UHF wave functions and UDFT solutions across the potential energy surface. The possibility of the partial ozonolysis mechanism being the source of epoxide observed in some experiments is discussed

    Desire for experiential travel, avoidance of rituality and social esteem: An empirical study of consumer response to tourism innovation

    AbstractThis study investigates tourist consumption responses toward tourism innovation. To measure tourist responses, this study posits three key consumption drivers, namely social esteem, desire for experiential travel, and avoidance against rituality of tourism settings (a subscale of need for uniqueness) and models consumers’ affective response within the context of tourism innovation. It involves 295 respondents in an empirical survey. The findings affirm the three drivers toward tourist consumption behavior. Avoidance of rituality reflects tourist preference toward tourism product and service innovation. Desire for experiential travel and the pursuit of social esteem signify tourism management and marketing innovation. Social esteem, need for status and creative choice have significant influence on tourists’ affective responses. Acquiring unique tourist products, desire for experiential travel and seeking social esteem are important motivations for tourist consumption. The implications of the study enrich the existing literature of consumer behavior and tourist consumption in response to tourism innovation

    Two Years versus One Year of Tianjiu Therapy in Sanfu Days for Chronic Asthma: A Clinical Efficacy Observation Trial

    Background. Tianjiu therapy has established efficacy against chronic asthma with related symptoms or the medication need during asthma attack. This study aimed to explore the optimal duration of Tianjiu therapy for asthma. Methods. This study was a self-comparison-to-the-baseline study, which comparing treatment with Tianjiu therapy for 1 year and 2 years in the same 102 chronic asthma patients. Totally 6 sessions of Tianjiu treatment were provided, 3 sessions in a year as a course of treatment and totally two years treatment. The primary endpoint was the number of asthma related symptoms which frequently appeared in asthma patients and the frequency of bronchodilator used during asthma attack. Results. The frequency of bronchodilator used during asthma attack significantly improved (χ2=46.276, P=0.000). But the number of asthma related symptoms which frequently appeared in asthma patients added by 1.38 points (95% CI, 0.25 to 2.51), 2.93±0.41 in 1-year group and 4.31±0.41 in the 2-years group (P<0.05). Conclusions. The effect of 2 years Tianjiu therapy was not as effective as 1 year such treatment for asthma, but the second year Tianjiu therapy was still needed because it has a role to consolidate the curative effect of Tianjiu therapy for asthma

    The virologic test results of 415 patients with severe acute respiratory syndrome (SARS) were examined. The peak detection rate for SARS-associated coronavirus occurred at week 2 after illness onset for respiratory specimens, at weeks 2 to 3 for stool or rectal swab specimens, and at week 4 for urine specimens. The latest stool sample that was positive by reverse transcription–polymerase chain reaction (RT-PCR) was collected on day 75 while the patient was receiving intensive care. Tracheal aspirate and stool samples had a higher diagnostic yield (RT-PCR average positive rate for first 2 weeks: 66.7% and 56.5%, respectively). Pooled throat and nasal swabs, rectal swab, nasal swab, throat swab, and nasopharyngeal aspirate specimens provided a moderate yield (29.7%–40.0%), whereas throat washing and urine specimens showed a lower yield (17.3% and 4.5%). The collection procedures for stool and pooled nasal and throat swab specimens were the least likely to transmit infection, and the combination gave the highest yield for coronavirus detection by RT-PCR. Positive virologic test results in patient groups were associated with mechanical ventilation or death (p < 0.001), suggesting a correlation between viral load and disease severity

    Fios e desafios da atenção à saúde da criança no estado do Espírito Santo: análise da mortalidade de zero a cinco anos com gestores do Programa Estadual de Saúde da Mulher e da Criança

    Trata-se de um estudo de abordagem qualitativa que objetivou discutir a mortalidade de zero a cinco anos, no estado do Espírito Santo, no lapso de agosto de 2011 a agosto de 2012, a partir de matérias veiculadas por um jornal diário da mídia impressa de grande circulação, a saber A Gazeta. As referidas matérias constituíram uma hemeroteca que subsidiou a criação de um painel reprográfico. Os sujeitos da investigação foram os técnicos que compõem a equipe da Coordenação do Programa Estadual de Saúde da Mulher e da Criança, e a produção do material de estudo se deu a partir da análise de um grupo focal, com roteiro semiestruturado, tendo como partida a análise de uma cópia do painel contendo todas as máterias. Todo o material foi gravado e filmado. A Análise Institutucional foi a baliza norteadora de toda a elaboração e descrição do estudo. Conforme preconiza este quadro teórico proposto por Lourau, a etapa final do projeto constituiu-se em uma restituição concreta parte do procedimento científico, tratando-se da discussão das produções na pesquisa com os interessados, de modo a possibilitar a sua interferência direta neste processo. O estudo demonstrou que os sujeitos, a partir do dispositivo analisador natural, a morte de crianças menores de cinco anos, conseguiram fazer uma reflexão sobre o quanto é necessário buscar uma interlocução com os demais setores e perceber que a análise institucional, com sua potência de provocar a autoanálise e a autogestão, proporcionou-lhes uma possibilidade de repensar seus processos de trabalho na atenção à saúde da criança

    Immunoregulatory Protein Profiles of Necrotizing Enterocolitis versus Spontaneous Intestinal Perforation in Preterm Infants

    Necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) are the most common acute surgical emergencies associated with high morbidity and mortality in preterm infants. We aimed to compare the profiles of immunoregulatory proteins and identify novel mediators in plasma of NEC and SIP infants. We also investigated the expression of target genes in resected intestinal tissues and an enterocyte cell line. Using Cytokine Antibody Array assay, we reported the first comparative profiles of immunoregulatory proteins in plasma of NEC and SIP infants, and showed that dysregulated proteins belonged to functionally diversified categories, including pro- and anti-inflammation, angiogenesis, cell growth, wound healing, anti-apoptosis, cell adhesion and extracellular matrix reorganization. Validation by ELISA confirmed significantly higher concentrations of interleukin (IL)-6, angiopoietin (Ang)-2, soluble type II interleukin-1 receptor (sIL-1RII), and soluble urokinase-type plasminogen activator receptor (suPAR) in NEC infants compared with gestational age-matched control, and a lower level of an epidermal growth factor receptor, secreted form of receptor tyrosine-protein kinase ErbB3 (sErbB3), compared with SIP infants. mRNA expressions of IL1-RII and uPAR were up-regulated in resected bowel tissues from NEC infants, indicating that immunoregulation also occurred at the cellular level. In FHs-74 Int cells, Ang-2, IL1-RII and uPAR mRNA expressions were significantly induced by the combined treatment with lipopolysaccharide (LPS) and platelet activating factor (PAF). Our study provided plasmatic signatures of immunoregulatory proteins in NEC and SIP infants, and demonstrated involvement of multiple functional pathways. The magnitude of changes in these proteins was significantly more extensive in NEC infants, reflecting the different nature of injury and/or severity of inflammation. We speculate that dysregulation of IL-6, Ang-2, IL-1RII and uPAR occurred at both systemic and cellular levels, and probably mediated via LPS and endogeneous PAF signals. Such exaggerated immunologic responses may account for the high morbidity and mortality in NEC compared with SIP patients

    Acute renal impairment in coronavirus-associated severe acute respiratory syndrome

    Acute renal impairment in coronavirus-associated severe acute respiratory syndrome.BackgroundSevere acute respiratory syndrome (SARS) is a newly emerged infection from a novel coronavirus (SARS-CoV). Apart from fever and respiratory complications, acute renal impairment has been observed in some patients with SARS. Herein, we describe the clinical, pathologic, and laboratory features of the acute renal impairment complicating this new viral infection.MethodsWe conducted a retrospective analysis of the plasma creatinine concentration and other clinical parameters of the 536 SARS patients with normal plasma creatinine at first clinical presentation, admitted to two regional hospitals following a major outbreak in Hong Kong in March 2003. Kidney tissues from seven other patients with postmortem examinations were studied by light microscopy and electron microscopy.ResultsAmong these 536 patients with SARS, 36 (6.7%) developed acute renal impairment occurring at a median duration of 20 days (range 5–48 days) after the onset of viral infection despite a normal plasma creatinine level at first clinical presentation. The acute renal impairment reflected the different prerenal and renal factors that exerted renal insult occurring in the context of multiorgan failure. Eventually, 33 SARS patients (91.7%) with acute renal impairment died. The mortality rate was significantly higher among patients with SARS and acute renal impairment compared with those with SARS and no renal impairment (91.7% vs. 8.8%) (P < 0.0001). Renal tissues revealed predominantly acute tubular necrosis with no evidence of glomerular pathology. The adjusted relative risk of mortality associated with the development of acute renal impairment was 4.057 (P < 0.001). By multivariate analysis, acute respiratory distress syndrome and age were the most significant independent risk factors predicting the development of acute renal impairment in SARS.ConclusionAcute renal impairment is uncommon in SARS but carries a high mortality. The acute renal impairment is likely to be related to multi-organ failure rather than the kidney tropism of the virus. The development of acute renal impairment is an important negative prognostic indicator for survival with SARS
