    The Process of Leaving School and Meaning of Schooling: The Case of Turkish School Leavers in Germany

    Germany is still among the countries where school performance is closely linked to the students social backgrounds. The proportion of school leavers among children of immigrants is almost two and a half times higher than their German counterparts. Most research points to the disadvantaged backgrounds and/or cultural frames of the immigrant children as reasons for leaving school. While providing valuable insights into what might cause leaving school, these explanations, however, do not explain how students leave school. Therefore, the present study focuses, with qualitative interviews and ethnography, on a group of Turkish school leavers as a case study in order to understand, from their perspectives, the process by which they disengaged from school over time. The findings point out regular interactions of students with structural limitations, common stigmatizing practices and social-psychological frustrations. These commonalities are used for the theoretical description of the disengagement model. Additionally, the findings also strikingly show that the Turkish school leavers damn self-seeking, competition and egoism as pro-school attitudes and consider Germanness in relation to such values, whereas they praise collectivity, brotherhood, cooperation and solidarity and relate Turkishness to these values. For the emergence of such common perceptions, which has a strong detrimental effect on the way schooling is seen, the data points out the structural limitations embedded in family, neighborhood and school practices

    Comparison of Effects of Smoking and Smokeless Tobacco “Maras Powder” Use on Humoral Immune System Parameters

    Background. The aim of this study is to assess the impacts of “Maras powder” and cigarette smoking on the parameters of the humoral immune system. Material and Methods. One hundred seventy seven subjects were included in the study. The IgA, IgG, IgM, C3 and C4 levels were detected via nephelometric method. Results. In 1.4% of the control group IgM levels were below normal where it was 10.8% and 18.6% in Maras powder group and in cigarette smoking group respectively. The IgM levels of both groups were significantly lower compared to the control group (P < .05). Nonetheless, the IgE levels of Maras powder group and smoking group were found to be remarkably higher compared to the control group (P < .01). Conclusion. Effects of Maras powder on humoral immune response were found to be similar to that of smoking

    Characteristics of obstetric fistulas and the need for a prognostic classification system

    Introduction: To evaluate the need for a prognostic classification system for obstetric fistula (OF) with the data obtained by a voluntarily action for OF repair in a regional hospital and Niger, Africa. Material and methods: Obstetric fistula (OF) characteristics of 51 women with vesicovaginal fistula in a fistula campaign in Maradi Regional Hospital, Niger were evaluated. Initial basic gynecological examination, methylene blue (MBT) test and direct cystoscopy were used to describe the characteristics of the lesions. Demographic and clinical data were compared with the existing literature. Results: In 31 (60 %) cases were the sizes of the fistula greater than 4 cm. The urethra was circumferentially lost in 8 (15.7%) women. In 18 (35.3%) women the trigone was involved. Extensive fibrosis was present in 10 (19.6%) patients. Only in 11(21.6%) patients without any obliterating scarring, neither the trigone nor urethra were damaged. Five (9.8%) patients had severe infection. Five (9.8%) had multiple lesions. Thirty-four (66.6%) had a history of at least one previous attempt for fistula repair. A significant proportion of women were divorced or abandoned from their husband, and socially isolated. Conclusions: A simple, reproducible and universally accepted scientific classification or staging system for OF dealing with outcomes rather than anatomic landmarks should replace the present proposed classification systems for prognostic and ethical purposes

    Znaczenie badania krwi pobranej z żył tarczowych górnych i dolnych w rozpoznawaniu raka tarczycy

    Introduction: We aimed to determine whether levels of thyroglobulin measured in blood from the inferior-superior thyroid veins and the peripheral antecubital vein could predict the presence of thyroid carcinoma in patients undergoing surgery for thyroid diseases. Material and methods: Sixty-one patients were prospectively enrolled in the study. Levels of thyroglobulin were analysed. Sensitivity, specificity, positive predictive values (PPV) and negative predictive values (NPV) of these markers were investigated. Results: Twenty-six out of 61 patients (42.6%) with malignancy were diagnosed. The levels of thyroglobulin in the inferior-superior thyroid veins were higher than those in the peripheral antecubital vein (p = 0.001). The levels of thyroglobulin in the blood taken from the antecubital vein and the inferior-superior thyroid veins did not differ between benign and malignant thyroid disorders. For thyroglobulin, sensitivity was 33.3%, specificity 60.6%, PPV 27.8%, and NPV 66.7% respectively. Conclusion: Thyroglobulin levels in the antecubital vein compared to the inferior-superior thyroid veins were not significant either in benign or malignant disorders.Wstęp: Celem badania było ustalenie, czy oznaczenie stężeń tyreoglobuliny we krwi pobranej z żył tarczowych górnych i dolnych oraz z żyły przedłokciowej ma wartość prognostyczną w wykrywaniu nowotworów tarczycy u chorych poddawanych zabiegom chirurgicznym z powodu chorób tarczycy. Materiał i metody: Do tego prospektywnego badania włączono 61 chorych. Analizowano wartości stężeń tyreoglobuliny. Badano czułość, swoistość oraz wartość prognostyczną dodatnią (PPV) i ujemną (NPV) tych wskaźników. Wyniki: U 26 spośród 61 chorych (42,6%) rozpoznano chorobę nowotworową. Stężenia tyreoglobuliny w żyłach tarczowych dolnych i górnych były wyższe niż stężenia mierzone we krwi pobranej z żyły przedłokciowej (p = 0,001). Stężenia tyreoglobuliny w próbkach krwi pobranych z żyły przedłokciowej i żył tarczowych nie różniły się istotnie między osobami z łagodnymi i złośliwymi guzami tarczycy. Oznaczenie tyreoglobuliny cechowało się czułością wynoszącą 33,3%, swoistością równą 60,6%, PPV &#8212; 27,8% i NPV &#8212; 66,7%. Wnioski: Porównanie stężeń tyreoglobuliny w żyle przedłokciowej i w żyłach tarczowych górnych i dolnych nie miało istotnego znaczenia diagnostycznego w wykrywaniu zarówno zmian łagodnych, jak i złośliwych

    Gebelerde proteinüri tanısında kullanılan yöntemlerin karşılaştırılması

    Amaç Kliniğimizde preeklampsi ön tanısı ile tetkik edilen gebelerde spot idrarda dipstik, protein/kreatinin (P/K) oranı ile 24 saatlik idrarda toplam protein miktarı değerlerinin karşılaştırılması amaçlanmıştır. Gereç ve yöntem Çalışmaya, Ocak 2016–Aralık 2018 tarihleri arasında bir ünivesite hastanesinde preeklampsi ön tanısı ile spot idrarda dipstik yöntemi, P/K oranı ve 24 saatlik idrarda toplam protein analizi yapılan 317 hasta çalışmaya dahil edildi. Hastaların bu üç değeri birbiri ile mukayese edildi. Bulgular Dipstik yönteminin sensitivitesi %67,5; spesifitesi %61,5; doğruluğu %64,3 olarak hesaplandı. 24 saatlik idrarda toplam proteinüri eşik değeri 300 mg/gün olduğunda P/K oranı cut-off değeri 0,34 mg protein/mg kreatinin, sensitivite %68,9, spesifite %78,7 olarak hesaplandı. Spot idrar P/K oranı ile 24 saatlik idrardaki toplam proteinüri değerleri arasında orta düzeyde, anlamlı bir korelasyon gözlendi (R: 0,629 p&lt;0,001). Sonuç 24 saatlik toplam idrar analizi için yeterli zamanın olmadığı acil durumlarda spot idrarda P/K oranının 24 saatlik idrarda toplam protein testine alternatif test olarak kullanılabilir

    Expression of CK-19 and CEA mRNA in peripheral blood of gastric cancer patients

    Aim: To investigate the clinical and pathological relevance of detection of circulating tumor cells (CTC) in the peripheral blood of gastric carcinoma patients before operation. Patients and Methods: Fifty patients with gastric adenocarcinoma were analysed prospectively. Patients were divided into two groups according to the extent of the tumor. Group I (unresectable) consisted of 22, and group II (resectable) consisted of 28 patients. Peripheral blood samples were collected pre-operatively from all 50 patients as well as from ten healthy controls and analyzed for carcinoembryonic antigen (CEA) and cytokeratin-19 (CK-19) messenger ribonucleic acids (mRNAs). Tumor localisation, stage, presence of signet cell formation, nodal metastases, serousal and lymphovascular invasion were recorded for all patients. Results: Expression of CK-19 was detected in 24 (48%), and CEA in 10 (20%) cases. Nine patients (40%) in group I and 15 (53.6%) in group II were positive for CK-19 expression. CEA expression was more frequent among group I patients (6 vs. 4 cases). There was no significant difference between the groups in the expression of CK-19 and CEA mRNA, tumor localisation, presence of signet formation, and presence and extent of nodal metastases. Patients with major vascular invasion (MVI) expressed significantly higher levels of CTC mRNA compared to those without MVI (p = 0.023 for CEA, and p = 0.009 for CK-19). The median 1 and 2-year survival was 9.5 and 10.5 months for group I, and 20 and 28.5 months for group II, respectively (p = 0.001). The mean survival was 6.7 months for patients with MVI, and 30.2 months for those without MVI (p = 0.0001). Conclusions: High levels of CTCs were observed in patients with MVI invasion, rather than other causes of unresectability. It can be suggested that expression of both CEA and CK-19 in the peripheral blood of gastric cancer patients are strong predictors of MVI and significantly worse survival rates.     Changing Trends and Experience with Esophageal Cancer Surgery in a Single University Hospital: Are The Results Similar or Not?

    Objective: The main treatment modality for esophageal cancer remains to be surgery. Over the last decades, surgical strategies have evolved remarkably. When neoadjuvant chemoradiotherapy became standard, discussions about the role, type, and timing of surgery began. In this study, we share results we obtained after operating our patients using various surgical techniques.Material and Methods:Reliable data from 51 esophageal cancer patients were evaluated retrospectively. Of the 51 cases, 31 were operable. These operable cases were further classified according to surgical method and neoadjuvant therapy status. Median survival time in months, complications, hospital mortality, length of hospital stay, and pathology results (total lymph nodes harvested and pathologic tumor node metastasis stage [p_TNM]) were documented for the different surgical approaches.Results: Open surgical methods were performed in 21 cases, while in 10 cases the Minimally Invasive Surgery (MIS) method was used. The MIS group received neoadjuvant therapy more frequently than the open surgical methods group (p=0.013). Although more complications were observed in the MIS group, the difference to the open esophagectomy methods group was not significant. Patients in the MIS group also had longer hospital stays, but again the difference was not significant. Although a pathologic complete response was seen in 8 of the 11 (72.7%) patients in our study who received chemoradiotherapy as neoadjuvant treatment, the surgical results of patients who received chemoradiotherapy were worse, although not to a statistically significant extent.Conclusion: Despite changing trends and treatment options in esophageal cancer surgery, we have yet to see the expected improved results
