    Forced three-wave interactions of capillary-gravity surface waves

    Editors' suggestionInternational audienceThree-wave resonant interactions constitute an essential nonlinear mechanism coupling capillary surface waves. In a previous work [Haudin et al. Phys. Rev. E 93, 043110 (2016)], we characterized experimentally the generation by this mechanism of a daughter wave, whose amplitude saturates due to the viscous dissipation. Here, we show experimentally the generation of a daughter wave verifying the resonant conditions, but not the dispersion relation. By modeling the response of the free surface at the lowest nonlinear order, we explain this observation as a forced interaction. The bandwidth of the linear transfer function of the free surface is indeed increased by the significant viscous dissipation. The observation of free surface excitations not following the linear dispersion relation then becomes possible. This forced three-wave interaction mechanism could have important consequences for wave turbulence in experimental or natural systems with nonnegligible dissipation

    Cholesterol lowering effect of a soy drink enriched with plant sterols in a French population with moderate hypercholesterolemia

    <p>Abstract</p> <p>Background</p> <p>Plant sterols are an established non-pharmacological means to reduce total and LDL blood cholesterol concentrations and are therefore recommended for cholesterol management by worldwide-renown health care institutions. Their efficacy has been proven in many types of foods with the majority of trials conducted in spreads or dairy products. As an alternative to dairy products, soy based foods are common throughout the world. Yet, there is little evidence supporting the efficacy of plant sterols in soy-based foods. The objective of this study was to investigate the effect of a soy drink enriched with plant sterols on blood lipid profiles in moderately hypercholesterolemic subjects.</p> <p>Methods</p> <p>In a randomized, placebo-controlled double-blind mono-centric study, 50 subjects were assigned to 200 ml of soy drink either enriched with 2.6 g plant sterol esters (1.6 g/d free plant sterol equivalents) or without plant sterols (control) for 8 weeks. Subjects were instructed to maintain stable diet pattern and physical activity. Plasma concentrations of lipids were measured at initial visit, after 4 weeks and after 8 weeks. The primary measurement was the change in LDL cholesterol (LDL-C). Secondary measurements were changes in total cholesterol (TC), non-HDL cholesterol (non-HDL-C), HDL cholesterol (HDL-C) and triglycerides.</p> <p>Results</p> <p>Regular consumption of the soy drink enriched with plant sterols for 8 weeks significantly reduced LDL- C by 0.29 mmol/l or 7% compared to baseline (p < 0.05). TC and non-HDL-C concentrations decreased by 0.26 mmol/l and 0.31 mmol/l (each p < 0.05), respectively. Mean reductions in total, LDL and non-HDL cholesterol were significantly greater than in the placebo group (p < 0.05). HDL-C and triglycerides were not affected. Compliance was very high (>96%), and products were well tolerated.</p> <p>Conclusion</p> <p>Daily consumption of a plant sterol-enriched soy drink significantly decreased total, non-HDL and LDL cholesterol and is therefore an interesting and convenient aid in managing mild to moderate hypercholesterolemia.</p

    From modulational instability to focusing dam breaks in water waves

    We report water wave experiments performed in a long tank where we consider the evolution of nonlinear deep-water surface gravity waves with the envelope in the form of a large-scale rectangular barrier. Our experiments reveal that, for a range of initial parameters, the nonlinear wave packet is not disintegrated by the Benjamin-Feir instability but exhibits a specific, strongly nonlinear modulation, which propagates from the edges of the wavepacket towards the center with finite speed. Using numerical tools of nonlinear spectral analysis of experimental data we identify the observed envelope wave structures with focusing dispersive dam break flows, a peculiar type of dispersive shock waves recently described in the framework of the semi-classical limit of the 1D focusing nonlinear Schr"odinger equation (1D-NLSE). Our experimental results are shown to be in a good quantitative agreement with the predictions of the semi-classical 1D-NLSE theory. This is the first observation of the persisting dispersive shock wave dynamics in a modulationally unstable water wave system

    Risk and Response-Adapted Treatment in Multiple Myeloma

    Myeloma therapeutic strategies have been adapted to patients' age and comorbidities for a long time. However, although cytogenetics and clinical presentations (plasmablastic cytology; extramedullary disease) are major prognostic factors, until recently, all patients received the same treatment whatever their initial risk. No strong evidence allows us to use a personalized treatment according to one cytogenetic abnormality in newly diagnosed myeloma. Retrospective studies showed a benefit of a double autologous transplant in high-risk cytogenetics according to the International Myeloma Working Group definition (t(4;14), t(14;16) or del(17p)). Moreover, this definition has to be updated since other independent abnormalities, namely gain 1q, del(1p32), and trisomies 5 or 21, as well as TP53 mutations, are also prognostic. Another very strong predictive tool is the response to treatment assessed by the evaluation of minimal residual disease (MRD). We are convinced that the time has come to use it to adapt the strategy to a dynamic risk. Many trials are ongoing to answer many questions: when and how should we adapt the therapy, its intensity and duration. Nevertheless, we also have to take into account the clinical outcome for one patient, especially adverse events affecting his or her quality of life and his or her preferences for continuous/fixed duration treatment

    Wave turbulence on the surface of a fluid in a high-gravity environment

    International audienceWe report on the observation of gravity-capillary wave turbulence on the surface of a fluid in a high-gravity environment. By using a large-diameter centrifuge, the effective gravity acceleration is tuned up to 20 times the Earth gravity. The transition frequency between the gravity and capillary regimes is thus increased up to one decade as predicted theoretically. A frequency power-law wave spectrum is observed in each regime and is found to be independent of the gravity level and of the wave steepness. While the timescale separation required by weak turbulence is well verified experimentally regardless of the gravity level, the nonlinear and dissipation timescales are found to be independent of the scale, as a result of the finite size effects of the system (large-scale container modes) that are not taken currently into account theoretically

    Empiric antimicrobial therapy for ventilator-associated pneumonia after brain injury

    International audienceIssues regarding recommendations on empiric antimicrobial therapy for ventilator-associated pneumonia (VAP) have emerged in specific populations.To develop and validate a score to guide empiric therapy in brain-injured patients with VAP, we prospectively followed a cohort of 379 brain-injured patients in five intensive care units. The score was externally validated in an independent cohort of 252 brain-injured patients and its extrapolation was tested in 221 burn patients.The multivariate analysis for predicting resistance (incidence 16.4%) showed two independent factors: preceding antimicrobial therapy ≥48 h (p\textless0.001) and VAP onset ≥10 days (p\textless0.001); the area under the receiver operating characteristic curve (AUC) was 0.822 (95% CI 0.770-0.883) in the learning cohort and 0.805 (95% CI 0.732-0.877) in the validation cohort. The score built from the factors selected in multivariate analysis predicted resistance with a sensitivity of 83%, a specificity of 71%, a positive predictive value of 37% and a negative predictive value of 96% in the validation cohort. The AUC of the multivariate analysis was poor in burn patients (0.671, 95% CI 0.596-0.751).Limited-spectrum empirical antimicrobial therapy has low risk of failure in brain-injured patients presenting with VAP before day 10 and when prior antimicrobial therapy lasts \textless48
