    Shared decision-making during virtual care regarding rheumatologic and chronic conditions: Qualitative study of benefits, pitfalls, and optimization

    OBJECTIVE: Virtual care (VC) is an accepted modality of care delivery, and shared decision-making (SDM) benefits patients with rheumatologic and chronic conditions (RCCs). Unfortunately, research suggests reduced quality of SDM during VC. This study explores the benefits and shortcomings of SDM regarding RCCs during VC with suggestions for optimally using VC during SDM. METHODS: Following Stiggelbout\u27s framework for SDM, we conducted focus groups of patients with RCCs and providers to understand their experiences with SDM during VC, probing for facilitating and challenging factors. We conducted content analysis of the transcripts, defining themes, and inductively reasoned to identify relationships among themes. We summarized the facilitators, barriers, and opportunities for improving SDM during VC that participants proposed. RESULTS: Virtual SDM shares several similarities with in-person practice, as both draw upon trusting patient-provider relationships, following the same general steps, and relying on effective communication. VC presents solutions for known barriers to in-person SDM, expanding time for making decisions and access to care. Technology and virtual health systems introduce new barriers to SDM, and participants list opportunities for overcoming these concerns. CONCLUSION: VC is a tool that can enhance and even support superior SDM compared with in-person visits when implemented successfully, a condition requiring the development of nuanced skills to correctly identify when and how to best use VC for SDM as well as technology and health care structures that integrate SDM into VC. Therefore, patients, providers, insurance carriers, and policy makers all contribute to the success of SDM among RCCs during VC

    Solar Effects on Tensile and Optical Properties of Hubble Space Telescope Silver-Teflon(Registered Trademark) Insulation

    A section of the retrieved Hubble Space Telescope (HST) solar array drive arm (SADA) multilayer insulation (MLI), which experienced 8.25 years of space exposure, was analyzed for environmental durability of the top layer of silver-Teflon (DuPont) fluorinated ethylene propylene (Ag-FEP). Because the SADA MLI had solar and anti-solar facing surfaces and was exposed to the space environment for a long duration, it provided a unique opportunity to study solar effects on the environmental degradation of Ag-FEP, a commonly used spacecraft thermal control material. Data obtained included tensile properties, solar absorptance, surface morphology and chemistry. The solar facing surface was found to be extremely embrittled and contained numerous through-thickness cracks. Tensile testing indicated that the solar facing surface lost 60% of its mechanical strength and 90% of its elasticity while the anti-solar facing surface had ductility similar to pristine FEP. The solar absorptance of both the solar facing surface (0.155 plus or minus 0.032) and the anti-solar facing surface (0.208 plus or minus 0.012) were found to be greater than pristine Ag-FEP (0.074). Solar facing and anti-solar facing surfaces were microscopically textured, and locations of isolated contamination were present on the anti-solar surface resulting in increased localized texturing. Yet, the overall texture was significantly more pronounced on the solar facing surface indicating a synergistic effect of combined solar exposure and increased heating with atomic oxygen erosion. The results indicate a very strong dependence of degradation, particularly embrittlement, upon solar exposure with orbital thermal cycling having a significant effect

    Effectiveness of a simple lymphoedema treatment regimen in podoconiosis management in southern Ethiopia: one year follow-up

    Background: Podoconiosis is a non-filarial elephantiasis caused by long-term barefoot exposure to volcanic soils in endemic areas. Irritant silicate particles penetrate the skin, causing a progressive, debilitating lymphoedema of the lower leg, often starting in the second decade of life. A simple patient-led treatment approach appropriate for resource poor settings has been developed, comprising (1) education on aetiology and prevention of podoconiosis, (2) foot hygiene (daily washing with soap, water and an antiseptic), (3) the regular use of emollient, (4) elevation of the limb at night, and (5) emphasis on the consistent use of shoes and socks. Methodology/Principal Findings: We did a 12-month, non-comparative, longitudinal evaluation of 33 patients newly presenting to one clinic site of a non-government organization (the Mossy Foot Treatment & Prevention Association, MFTPA) in southern Ethiopia. Outcome measures used for the monitoring of disease progress were (1) the clinical staging system for podoconiosis, and (2) the Amharic Dermatology Life Quality Index (DLQI), both of which have been recently validated for use in this setting. Digital photographs were also taken at each visit. Twenty-seven patients completed follow up. Characteristics of patients completing follow-up were not significantly different to those not. Mean clinical stage and lower leg circumference decreased significantly (mean difference -0.67 (95% CI -0.38 to -0.96) and -2.00 (95% CI -1.26 to -2.74), respectively, p<0.001 for both changes). Mean DLQI diminished from 21 (out of a maximum of 30) to 6 (p<0.001). There was a non-significant change in proportion of patients with mossy lesions (p = 0.375). Conclusions/Significance: This simple, resource-appropriate regimen has a considerable impact both on clinical progression and self-reported quality of life of affected individuals. The regimen appears ideal for scaling up to other endemic regions in Ethiopia and internationally. We recommend that further research in the area include analysis of cost-effectiveness of the regimen

    Public Health Interventions Guided by Houston\u27s Wastewater Surveillance Program During the COVID-19 Pandemic

    Since the start of the COVID-19 pandemic, wastewater surveillance has emerged as a powerful tool used by public health authorities to track SARS-CoV-2 infections in communities. In May 2020, the Houston Health Department began working with a coalition of municipal and academic partners to develop a wastewater monitoring and reporting system for the city of Houston, Texas. Data collected from the system are integrated with other COVID-19 surveillance data and communicated through different channels to local authorities and the general public. This information is used to shape policies and inform actions to mitigate and prevent the spread of COVID-19 at municipal, institutional, and individual levels. Based on the success of this monitoring and reporting system to drive public health protection efforts, the wastewater surveillance program is likely to become a standard part of the public health toolkit for responding to infectious diseases and, potentially, other disease-causing outbreaks

    Associations between cardiorespiratory fitness, physical activity and clustered cardiometabolic risk in children and adolescents: the HAPPY study

    Clustering of cardiometabolic risk factors can occur during childhood and predisposes individuals to cardiometabolic disease. This study calculated clustered cardiometabolic risk in 100 children and adolescents aged 10-14 years (59 girls) and explored differences according to cardiorespiratory fitness (CRF) levels and time spent at different physical activity (PA) intensities. CRF was determined using a maximal cycle ergometer test, and PA was assessed using accelerometry. A cardiometabolic risk score was computed as the sum of the standardised scores for waist circumference, blood pressure, total cholesterol/high-density lipoprotein ratio, triglycerides and glucose. Differences in clustered cardiometabolic risk between fit and unfit participants, according to previously proposed health-related threshold values, and between tertiles for PA subcomponents were assessed using ANCOVA. Clustered risk was significantly lower (p < 0.001) in the fit group (mean 1.21 ± 3.42) compared to the unfit group (mean -0.74 ± 2.22), while no differences existed between tertiles for any subcomponent of PA. Conclusion These findings suggest that CRF may have an important cardioprotective role in children and adolescents and highlights the importance of promoting CRF in youth

    The Earth BioGenome Project (EBP) is an audacious endeavor to obtain whole-genome sequences of representatives from all eukaryotic species on Earth. In addition to the project's technical and organizational challenges, it also faces complicated ethical, legal, and social issues. This paper, from members of the EBP's Ethical, Legal, and Social Issues (ELSI) Committee, catalogs these ELSI concerns arising from EBP. These include legal issues, such as sample collection and permitting; the applicability of international treaties, such as the Convention on Biological Diversity and the Nagoya Protocol; intellectual property; sample accessioning; and biosecurity and ethical issues, such as sampling from the territories of Indigenous peoples and local communities, the protection of endangered species, and cross-border collections, among several others. We also comment on the intersection of digital sequence information and data rights. More broadly, this list of ethical, legal, and social issues for large-scale genomic sequencing projects may be useful in the consideration of ethical frameworks for future projects. While we do not-and cannot-provide simple, overarching solutions for all the issues raised here, we conclude our perspective by beginning to chart a path forward for EBP's work

    The Anthropocene is a prospective epoch/series, not a geological event

    The Anthropocene defined as an epoch/series within the Geological Time Scale, and with an isochronous inception in the mid-20th century, would both utilize the rich array of stratigraphic signals associated with the Great Acceleration and align with Earth System science analysis from where the term Anthropocene originated. It would be stratigraphically robust and reflect the reality that our planet has far exceeded the range of natural variability for the Holocene Epoch/Series which it would terminate. An alternative, recently advanced, time-transgressive ‘geological event’ definition would decouple the Anthropocene from its stratigraphic characterisation and association with a major planetary perturbation. We find this proposed anthropogenic ‘event’ to be primarily an interdisciplinary concept in which historical, cultural and social processes and their global environmental impacts are all flexibly interpreted within a multi-scalar framework. It is very different from a stratigraphic-methods-based Anthropocene epoch/series designation, but as an anthropogenic phenomenon, if separately defined and differently named, might be usefully complementary to it
