Patrón de crecimiento facial y su relación con la permeabilidad de la vía aérea superior

El propósito de este estudio es determinar la relación que existe entre el patrón de crecimiento facial y la permeabilidad de la vía aérea superior. El patrón de crecimiento facial es el resultado de la interacción de diferentes factores etiológicos durante el periodo de crecimiento. Los problemas aéreos, tales como adenoides, amígdalas hiperplásicas o vías aéreas bloqueadas podrían afectar la posición mandibular y así determinar el patrón de crecimiento facial, en la actualidad no se ha aclarado totalmente el tema debido a que si bien es cierto la mayoría de las investigaciones afirma la relación entre el patrón de crecimiento facial con la permeabilidad de la vía aérea superior no existe una clara explicación de cómo están relacionadas. El estudio es descriptivo, comparativo, retrospectivo y transversal. La muestra está constituida por 754 radiografías laterales de cráneo de pacientes ingresantes al Servicio de Ortodoncia y Ortopedia Maxilar del IESN con las características de selección atribuidas por la investigadora. Para la recolección de datos se prosiguió a realizar el trazado cefalométrico siguiendo los criterios establecidos por Bjork Jarabak para luego determinar el patrón de crecimiento facial. Posteriormente se midió en milímetros la nasofaringe y la orofaringe según el análisis de Mc Namara. Los datos se organizaron en tablas y gráficas usando estadística descriptiva, hallando frecuencias, porcentajes; además de la prueba estadística no paramétrica Chi cuadrado con un nivel de significancia del 0.05 para determinar la asociación entre las variables. Luego se aplicó la prueba de Tau b de Kendall para evaluar la dirección y magnitud de la relación. Según la prueba estadística realizada X2 encontramos que existe una relación altamente significativa (p<0.0001) entre los tipos de patrón de crecimiento facial y la permeabilidad de la vía aérea superior de una población de niños de 10 a 15 años de edad. Al realizar la prueba de Tau b de Kendall se concluyó que los pacientes con patrón de crecimiento facial horizontal tienden a una mayor permeabilidad de la vía aérea así como los pacientes con patrón de crecimiento facial vertical tienden a una menor permeabilidad de la vía aérea superior. Facial growth pattern is the result of the interaction of different etiologic factors during the growth period. The air problems, such as adenoids, tonsils, hyperplastic or blocked airway may affect mandibular position and determine the pattern of facial growth, at present not fully clarified the issue because although most research affirms the relationship between facial growth pattern to the permeability of the upper airway there is no clear explanation of how they are related. Subsequently the distance in millimeters the nasopharynx and oropharynx as analyzed by McNamara. The data were organized in tables and graphs using descriptive statistics, finding frequencies, percentages, in addition to the nonparametric statistical test X2 with a significance level of 0.05 to determine the association between variables. Then he applied the test of Kendall's Tau b to assess the direction and magnitude of the relationship. According to the chi-square statistical test performed found that there is a highly significant (p <0.0001) between the types of facial growth pattern and permeability of the upper airway of a population of children 10 to 15 year old. The buccal fat pad due to its size, vascular richness and versatility is a reliable flap. The technique which uses this anatomic element as flap is better tolerated by the patient, easy development and with fewer complications compared to other techniques. For the advantages of this technique is important to highlight its usefulness in the closure of oroantral comunications. This paper describes the technique used in a case of recurrent oroantral fistula which was recidivant to other treatments, treated in The National Institute of Child´s Health.La bola adiposa de Bichat debido a su volumen, riqueza vascular y versatilidad es un colgajo confiable. La técnica, que utiliza a este elemento anatómico como colgajo, es mejor tolerada por el paciente, de fácil desarrollo y con menos complicaciones comparada con otras técnicas. Por las ventajas que presenta esta técnica es importante resaltar su utilidad en el cierre de las comunicaciones bucosinusales.
