Comorbidades físicas e psicológicas antes e depois da cirurgia bariátrica : um estudo longitudinal
Introduction: Morbid obesity has multiple implications for
psychological and physical health. Bariatric surgery has been
selected as the treatment of choice for this chronic disease,
despite the controversial impact of the surgery on psychosocial
health. The objective of this study was to describe candidates
for bariatric surgery and analyze changes in weight, psychopathology,
personality, and health problems and complaints at
6- and 12- month follow-up assessments.
Methods: Thirty obese patients (20 women and 10 men) with a
mean age of 39.17±8.81 years were evaluated in different dimensions
before surgery and 6 and 12 months after the procedure.
Results: Six and 12 months after bariatric surgery, patients
reported significant weight loss and a significant reduction in
the number of health problems and complaints. The rates of
self-reported psychopathology were low before surgery, and
there were no statistically significant changes over time. The
conscientiousness, extraversion, and agreeableness dimensions
increased, but neuroticism and openness remained unchanged.
All changes had a medium effect size.
Conclusions: Our results suggest that patients experience
significant health improvements and some positive personality
changes after bariatric surgery. Even though these findings
underscore the role of bariatric surgery as a relevant treatment
for morbid obesity, more in-depth longitudinal studies
are needed to elucidate the evolution of patients after the
procedure.Introdução: A obesidade mórbida tem várias implicações para
a saúde psicológica e física. A cirurgia bariátrica tem sido o
tratamento de escolha para essa doença crônica, apesar da
controvérsia sobre o impacto da cirurgia na saúde psicossocial.
O objetivo deste estudo foi descrever candidatos a cirurgia bariátrica e analisar mudanças no peso, psicopatologia personalidade,
problemas e queixas de saúde desses pacientes em avaliações
realizadas 6 e 12 meses após a cirurgia.
Métodos: Trinta pacientes obesos (20 mulheres e 10 homens)
com idade média de 39,17±8,81 anos foram avaliados em diferentes
dimensões antes da cirurgia e 6 e 12 meses após.
Resultados: Aos 6 e 12 meses após a cirurgia bariátrica, os
pacientes relataram significativa perda de peso e significativa
redução no número de problemas e queixas de saúde. As taxas
de psicopatologia autorrelatada foram baixas antes da cirurgia e
não sofreram mudanças significativas com o tempo. As dimensões
conscienciosidade, extroversão e agradabilidade aumentaram,
mas o neuroticismo e a abertura permaneceram inalteradas.
Todas as mudanças apresentaram um tamanho de efeito médio.
Conclusões: Os nossos resultados sugerem que os pacientes
experimentam melhoras significativas em saúde e algumas mudanças
positivas de personalidade após a cirurgia bariátrica.
Embora esses achados reforcem o papel da cirurgia bariátrica
como um tratamento relevante para a obesidade mórbida, mais
estudos longitudinais e aprofundados são necessários para elucidar
a evolução dos pacientes após a realização do procedimento.(undefined
Association of lifelong occupation and educational level with subclinical atherosclerosis in different European regions. Results from the IMPROVE study
Background and aims
We aimed to examine the association between socioeconomic status (SES) and subclinical atherosclerosis, as assessed by carotid intima-media-thickness (C-IMT) and to investigate whether the effect of social inequality on C-IMT is mediated by cardiovascular (CV) risk factors and whether it is dissimilar in men and women, and in different European countries.
Methods
We assessed the association of lifelong occupation and educational level with C-IMT in the IMPROVE study cohort including 3703 subjects (median age 64.4 years; 48% men) from Southern (Italy), Western (France and the Netherlands) and Northern Europe (Finland and Sweden). Three summary measures of C-IMT (IMTmean, IMTmax, IMTmean-max), obtained from four segments of both carotids, were considered.
Results
After adjusting for conventional CV risk factors, current employment status and diet, C-IMT was higher in manual workers than in white collars (+7.7%, +5.3%, +4.6% for IMTmax, IMTmean-max and IMTmean, respectively; all p<.0001). Similar results were obtained by stratification for educational level. The effect of occupation on C-IMT was comparable in men and women and in different age groups, and was only partially mediated by differences in CV risk factors. Of note, the association of C-IMT with occupation was significant in Western and Northern Europe but not in Italy, with a significant statistical interaction (p = .0005).
Conclusions
Low SES was associated with subclinical atherosclerosis in subjects with at least three CV risk factors. Such association was stronger in Northern and Western Europe than in Italy. This difference was not completely explained by inequalities in CV risk factors and behavioural variables

‘We had the most wonderful times’: seaside nostalgia at a British resort
Morecambe is a traditional British seaside resort that experienced a dynamic ebb and flow of visitors. It still attracts visitors, many of whom are from the North of England and in the second half of their lives. The experiences of such traditional seaside markets have not been examined as carefully by academics in recent years as one might assume. All too often this subject falls between the gap between serious academic study and popular culture, which supports narratives focussing on the apparent decline of an idealised seaside. Instead this paper attempts to gain an understanding of this seaside experience, and is based around ten semi-structured interviews with 55-74 year old repeat North of England visitors to Morecambe. It considers their nostalgic connection and reaction to the resort, which emerged as a significant element of visitor experience. The seaside is considered timeless by these visitors and facilitates a reverie through which one can temporarily revisit a past which is populated by childhood memories of family members. The resort allows visitors to fleetingly transcend time, through immersion in the unchanging resort with its timeless seacape. This reconnection with the past highlights a dissatisfaction with the present which hinges on the loss of childhood. Yet nostalgia also allowed for a positive re-telling of the past which underpinned family narratives and contributed to the cross-generational appeal of the beach
