    Qualitative results from a phase II pilot randomised controlled trial of a lymphoma nurse-led model of survivorship care

    Purpose: To explore and describe lymphoma survivors’ thoughts and perceptions of the components of a nurse led lymphoma survivorship clinic intervention. Methods: An exploratory, qualitative descriptive study using interviews from 10 participants who had transitioned post-treatment into the survivorship phase via a nurse-led lymphoma survivorship clinic intervention. Results: Thematic analysis revealed three major themes: Reassurance and individualised care; Information and support; and Empowerment. Participants described the reassurance they gained from having contact with a health professional post-treatment who individualised information and support. A survivorship care plan and treatment summary was developed for this study and was believed to be very patient-centred and helpful. This enabled participants to take back control of their health and well-being and to rebuild confidence. Conclusions: In this study, participants expressed a need for patient-centred follow-up care that addressed their concerns and supported them in the survivorship phase to get their life back on track. Nurse-led follow-up may offer a viable model of post-treatment survivorship care to lymphoma cancer survivors

    Girl or boy? Prenatal lead, cadmium and mercury exposure and the secondary sex ratio in the ALSPAC study

    AbstractThe aim of this study was to evaluate the effect of prenatal exposure to lead, cadmium and mercury levels on the secondary sex ratio. Whole blood samples were collected from pregnant women enrolled in the Avon Longitudinal Study of Parents and Children (ALSPAC) study at a median gestational age of 11 weeks and were analyzed for lead, cadmium and mercury. Regression analysis was used to identify associations between maternal lead, cadmium and mercury levels and the secondary sex ratio with adjustment for confounders. There was no evidence for associations between maternal lead, cadmium or mercury levels and the secondary sex ratio in this sample. It appears unlikely that alterations in the secondary sex ratio are influenced by exposure to heavy metals, but further work should be done in large cohorts in other countries to confirm these findings

    Intake of game birds in the UK:assessment of the contribution to the dietary intake of lead by women of childbearing age and children

    AbstractObjectiveConcern has recently been expressed about Pb levels in Pb-shot game meat. Our aim was to determine the consumption of game birds in a representative sample population in the UK, and in children and women of childbearing age in particular.DesignPopulation-based cross-sectional cohort study. Data from 4 d diet diaries from the UK National Diet and Nutrition Survey (NDNS; 2008–2010) were extracted to analyse data on game bird consumption in the sample population, in women of childbearing age (15–45 years old) and in children ≤6 years old.SettingHome-based study in representative areas of the UK.SubjectsParticipants in the NDNS (2008–2010; n 2126, age 1·5 to &gt;65 years).ResultsFifty-eight participants (2·7 %) reported eating game birds. The mean intake was 19·5 (sd 18·1) g/d (median 15·6, range 1·3–92·9 g/d). In women of childbearing age (15–45 years), 11/383 (2·9 %) reported eating game birds, with a mean intake of 22·4 (sd 25·8) g/d (median 15·6, range 2·0–92·9 g/d). In children aged ≤6 years old, 3/342 (0·9 %) were reported as eating game birds, with a mean intake of 6·8 (sd 9·7) g/d (median 2·4, range 1·3–23·2 g/d).ConclusionsThe prevalence of consumption of game birds by women of childbearing age and children ≤6 years old was relatively low and intakes were small. However, any exposure to Pb in these two groups is undesirable. As are uncertainties about the ability of the diet diary method to capture the consumption of food items that are infrequently consumed, alternative methods of capturing these data should be used in future studies.</jats:sec

    Identification of criticality in neuronal avalanches: II. A theoretical and empirical investigation of the Driven case

    The observation of apparent power laws in neuronal systems has led to the suggestion that the brain is at, or close to, a critical state and may be a self-organised critical system. Within the framework of self-organised criticality a separation of timescales is thought to be crucial for the observation of power-law dynamics and computational models are often constructed with this property. However, this is not necessarily a characteristic of physiological neural networks—external input does not only occur when the network is at rest/a steady state. In this paper we study a simple neuronal network model driven by a continuous external input (i.e. the model does not have an explicit separation of timescales from seeding the system only when in the quiescent state) and analytically tuned to operate in the region of a critical state (it reaches the critical regime exactly in the absence of input—the case studied in the companion paper to this article). The system displays avalanche dynamics in the form of cascades of neuronal firing separated by periods of silence. We observe partial scale-free behaviour in the distribution of avalanche size for low levels of external input. We analytically derive the distributions of waiting times and investigate their temporal behaviour in relation to different levels of external input, showing that the system’s dynamics can exhibit partial long-range temporal correlations. We further show that as the system approaches the critical state by two alternative ‘routes’, different markers of criticality (partial scale-free behaviour and long-range temporal correlations) are displayed. This suggests that signatures of criticality exhibited by a particular system in close proximity to a critical state are dependent on the region in parameter space at which the system (currently) resides

    Identification of criticality in neuronal avalanches: I. A theoretical investigation of the non-driven case

    In this paper, we study a simple model of a purely excitatory neural network that, by construction, operates at a critical point. This model allows us to consider various markers of criticality and illustrate how they should perform in a finite-size system. By calculating the exact distribution of avalanche sizes, we are able to show that, over a limited range of avalanche sizes which we precisely identify, the distribution has scale free properties but is not a power law. This suggests that it would be inappropriate to dismiss a system as not being critical purely based on an inability to rigorously fit a power law distribution as has been recently advocated. In assessing whether a system, especially a finite-size one, is critical it is thus important to consider other possible markers. We illustrate one of these by showing the divergence of susceptibility as the critical point of the system is approached. Finally, we provide evidence that power laws may underlie other observables of the system that may be more amenable to robust experimental assessment
