The use of concepts such as private sector partnership development, quality improvement, expense management, and cost analysis can provide public health practitioners means for growth and improvement, as well as the ability to communicate value to decision makers

    Evidence for the Role of Resource-sharing Networks in Coalition Development

    Background: Accreditation bodies and sponsors of community health projects increasingly require the use of health coalitions in community health planning efforts to ensure buy-in, leverage resources, and distribute health information. Despite a substantive body of research documenting the characteristics of successful health coalitions, little is known about how team dynamics in these coalitions evolve. Purpose: The goal of this study was to employ social network analysis techniques to evaluate whether coalitions’ relative stages in Tuckman’s stages of team development model were associated with specific patterns of advice-, information-, and resource-sharing among the eight coalitions participating in a region-wide forum. Methods: Data were collected during a community health forum in June 2014, during which coalitions from an eight-county development district in Kentucky gathered to share information about their activities. Observational field notes and a questionnaire measuring relational ties among coalitions were collected during the forum and analyzed in spring 2015. Results: The results indicate that less developed coalitions most frequently seek advice, information, and resource-sharing opportunities from coalitions that have successfully moved through the earlier stages of team development. Implications: The study suggests that well-established coalitions may serve as “mentor coalitions” for groups in more formative stages of development, as they are a trusted source of information, advice, resources, best practices, and lessons learned

    Social Security offers two types of benefits for spouses: spousal and survivor benefits. Regardless of his or her own work history, a married individual can claim spousal Social Security benefits, which are equal to half of his or her spouse’s Social Security benefits. Furthermore, a widow or widower can claim survivor benefits and receive or his or her deceased spouse’s full benefit if it is larger than his or her own benefit. Ideally, married individuals think about the impact of their Social Security choices on their spouse. However, if people do not fully understand the rules for the spousal and survivor benefits, they may make suboptimal choices, not only about Social Security claiming, but perhaps also about labor and marriage decisions. In this paper we make use of new data from the Understanding America Study to assess households’ understanding of these benefits. Overall, our results suggest that knowledge of spousal and survivors benefits is low. Furthermore, our results suggest that people’s perceptions of their knowledge is misaligned with their actual knowledge, with many perceiving that they know more about Social Security than they actually do. The results in this paper suggest particular areas where policymakers might be able to increase knowledge of spousal and survivors benefits.     Cross Jurisdictional Boundaries to Build a Health Coalition: A Kentucky Case Study

    Cross-jurisdictional sharing is accomplished through collaboration across jurisdictional boundaries to deliver essential public health services and solve problems that cannot be easily addressed by single organizations or jurisdictions. Partners across 10 counties and three public health jurisdictions of the Barren River Area Development District (BRADD) convened as Barren River Initiative to Get Healthy Together (BRIGHT), a community health improvement coalition. Focus groups and interviews with BRIGHT members indicate that the use of effective strategies to focus collaborative health improvement efforts fosters a cohesive coalition even when the group is populated by individuals from across public health jurisdictional boundaries. Focusing strategies identified included: the importance of organizing workgroups so members can draw upon expertise, adoption of a community engagement model for health assessment and improvement; and use of a facilitator, who offers guidance and administrative support to groups and focuses members on accomplishing goals

    Beam-recoil polarization transfer in the nucleon resonance region in the exclusive e→p→eâ€ČK+Λ→ and e→p→eâ€ČK+Σ→0 reactions at the CLAS spectrometer

    Beam-recoil transferred polarizations for the exclusive e→p→eâ€ČK+Λ→,Σ→0 reactions have been measured using the Continuous Electron Beam Accelerator Facility\u27s large acceptance spectrometer (CLAS) at the Thomas Jefferson National Accelerator Facility. New measurements have been completed at beam energies of 4.261 and 5.754 GeV that span a range of momentum transfer Q2 from 0.7 to 5.4 GeV2, invariant energy W from 1.6 to 2.6 GeV, and the full center-of-mass angular range of the K+ meson. These new data add to the existing CLAS K+Λ measurements at 2.567 GeV, and provide the first-ever data for the K+ÎŁ0 channel in electroproduction. Comparisons of the data with several theoretical models are used to study the sensitivity to s-channel resonance contributions and the underlying reaction mechanism. Interpretations within two semiclassical partonic models are made to probe the underlying reaction mechanism and the ssÂŻ quark-pair creation dynamics

    First Measurement of Transferred Polarization in the Exclusive e⃗ p→eâ€ČK+Λ⃗Reaction

    The first measurements of the transferred polarization for the exclusive e⃗ p→eâ€ČK+Λ⃗ reaction have been performed at Jefferson Laboratory using the CLAS spectrometer. A 2.567 GeV beam was used to measure the hyperon polarization over Q2 from 0.3 to 1.5   (GeV/c)2, W from 1.6 to 2.15 GeV, and over the full K+ center-of-mass angular range. Comparison with predictions of hadrodynamic models indicates strong sensitivity to the underlying resonance contributions. A nonrelativistic quark-model interpretation of our data suggests that the ssÂŻ quark pair is produced with spins predominantly antialigned. Implications for the validity of the most widely used quark-pair creation operator are discussed

    Geographic Variation in the Structure of Kentucky’s Population Health Systems: An Urban, Rural, and Appalachian Comparison

    Introduction: Research examining geographic variation in the structure of population health systems is continuing to emerge, and most of the evidence that currently exists divides systems by urban and rural designation. Very little is understood about how being rural and Appalachian impacts population health system structure and strength. Purpose: This study examines geographic differences in key characteristics of population health systems in urban, rural non-Appalachian, and rural Appalachian regions of Kentucky. Methods: Data from a 2018 statewide survey of community networks was used to examine population health system characteristics. Descriptive statistics were generated to examine variation across geographic regions in the availability of 20 population health activities, the range of organizations that contribute to those activities, and system strength. Data were collected in 2018 and analyzed in 2020. Results: Variation in the provision of population health protections and the structure of public health systems across KY exists. Urban communities are more likely than rural to have a comprehensive set of population health protections delivered in collaboration with a diverse set of multisector partners. Rural Appalachian communities face additional limited capacity in the delivery of population health activities, compared to other rural communities in the state. Implications: Understanding the delivery of population health provides further insight into additional system-level factors that may drive persistent health inequities in rural and Appalachian communities. The capacity to improve health happens beyond the clinic, and the strengthening of population health systems will be a critical step in efforts to improve population health

    Improving Community Health through Hospital-Public Health Collaboration: Insights and Lessons Learned from Successful Partnerships

    From the introduction: Health care expenditures in the United States currently consume over 17 percent of the nation’s gross domestic product, a much larger share than other developed nations. Yet, despite this large investment, studies by Commonwealth Fund, the Institute of Medicine, and other organizations show the USA lags behind other developed nations on multiple metrics of population health such as infant mortality and life expectancy. Moreover, there is extensive evidence of disparities in access, cost, and quality of health care services. Thus, we are confronted by a striking paradox: the USA spends a large and growing proportion of our resources on health care, but the outcomes in terms of access to services, the quality of those services, and the health of our population do not match other countries whose spending per capita is lower. It is evident that many factors contribute to this paradox — demographic, environmental, genetic, lifestyle, and socioeconomic — and all warrant societal attention. Improving access to outpatient and inpatient medical services and the quality of those services, while important, cannot resolve the paradox. Across the country, there is growing awareness that restraining the increase in health expenditures and improving the health status of families, communities, and society at large will require a broader approach that addresses the full array of factors affecting health status. Greater attention and resources must be devoted to promoting a safer environment, healthy lifestyles, prevention of illnesses and injuries, and early detection and treatment of health problems, as well as dealing with the underlying determinants of health.     Designing for Dissemination: Lessons in Message Design from 1-2-3 Pap

    Despite a large number of evidence-based health communication interventions tested in private, public, and community health settings, there is a dearth of research on successful secondary dissemination of these interventions to other audiences. This article presents the case study of 1-2-3 Pap, a health communication intervention to improve human papillomavirus (HPV) vaccination uptake and Pap testing outcomes in Eastern Kentucky, and explores strategies used to disseminate this intervention to other populations in Kentucky, North Carolina, and West Virginia. Through this dissemination project, we identified several health communication intervention design considerations that facilitated our successful dissemination to these other audiences; these intervention design considerations include (a) developing strategies for reaching other potential audiences, (b) identifying intervention message adaptations that might be needed, and (c) determining the most appropriate means or channels by which to reach these potential future audiences. Using 1-2-3 Pap as an illustrative case study, we describe how careful planning and partnership development early in the intervention development process can improve the potential success of enhancing the reach and effectiveness of an intervention to other audiences beyond the audience for whom the intervention messages were originally designed

    Mismatches in scale between highly mobile marine megafauna and marine protected areas

    Marine protected areas (MPAs), particularly large MPAs, are increasing in number and size around the globe in part to facilitate the conservation of marine megafauna under the assumption that large-scale MPAs better align with vagile life histories; however, this alignment is not well established. Using a global tracking dataset from 36 species across five taxa, chosen to reflect the span of home range size in highly mobile marine megafauna, we show most MPAs are too small to encompass complete home ranges of most species. Based on size alone, 40% of existing MPAs could encompass the home ranges of the smallest ranged species, while only < 1% of existing MPAs could encompass those of the largest ranged species. Further, where home ranges and MPAs overlapped in real geographic space, MPAs encompassed < 5% of core areas used by all species. Despite most home ranges of mobile marine megafauna being much larger than existing MPAs, we demonstrate how benefits from MPAs are still likely to accrue by targeting seasonal aggregations and critical life history stages and through other management techniques.Fil: Conners, Melinda G..
