Project MOSI: rationale and pilot-study results of an initiative to help protect zoo animals from mosquito-transmitted pathogens and contribute data on mosquito spatio–temporal distribution change
Mosquito-borne pathogens pose major threats to both wildlife and human health and, largely as a result of unintentional human-aided dispersal of their vector species, their cumulative threat is on the rise. Anthropogenic climate change is expected to be an increasingly significant driver of mosquito dispersal and associated disease spread. The potential health implications of changes in the spatio-temporal distribution of mosquitoes highlight the importance of ongoing surveillance and, where necessary, vector control and other health-management measures. The World Association of Zoos and Aquariums initiative, Project MOSI, was established to help protect vulnerable wildlife species in zoological facilities from mosquito-transmitted pathogens by establishing a zoo-based network of fixed mosquito monitoring sites to assist wildlife health management and contribute data on mosquito spatio-temporal distribution changes. A pilot study for Project MOSI is described here, including project rationale and results that confirm the feasibility of conducting basic standardized year-round mosquito trapping and monitoring in a zoo environment
Analysis of Functional Differences between Hepatitis C Virus NS5A of Genotypes 1–7 in Infectious Cell Culture Systems
Hepatitis C virus (HCV) is an important cause of chronic liver disease. Several highly diverse HCV genotypes exist with potential key functional differences. The HCV NS5A protein was associated with response to interferon (IFN)-α based therapy, and is a primary target of currently developed directly-acting antiviral compounds. NS5A is important for replication and virus production, but has not been studied for most HCV genotypes. We studied the function of NS5A using infectious NS5A genotype 1–7 cell culture systems, and through reverse genetics demonstrated a universal importance of the amphipathic alpha-helix, domain I and II and the low-complexity sequence (LCS) I for HCV replication; the replicon-enhancing LCSI mutation S225P attenuated all genotypes. Mutation of conserved prolines in LCSII led to minor reductions in virus production for the JFH1(genotype 2a) NS5A recombinant, but had greater effects on other isolates; replication was highly attenuated for ED43(4a) and QC69(7a) recombinants. Deletion of the conserved residues 414-428 in domain III reduced virus production for most recombinants but not JFH1(2a). Reduced virus production was linked to attenuated replication in all cases, but ED43(4a) and SA13(5a) also displayed impaired particle assembly. Compared to the original H77C(1a) NS5A recombinant, the changes in LCSII and domain III reduced the amounts of NS5A present. For H77C(1a) and TN(1a) NS5A recombinants, we observed a genetic linkage between NS5A and p7, since introduced changes in NS5A led to changes in p7 and vice versa. Finally, NS5A function depended on genotype-specific residues in domain I, as changing genotype 2a-specific residues to genotype 1a sequence and vice versa led to highly attenuated mutants. In conclusion, this study identified NS5A genetic elements essential for all major HCV genotypes in infectious cell culture systems. Genotype- or isolate- specific NS5A functional differences were identified, which will be important for understanding of HCV NS5A function and therapeutic targeting
Recomendações sobre o uso dos testes de exercício na prática clínica
Resumo: A elaboração deste documento pelo grupo de trabalho da European Respiratory Society tem como objectivo apresentar as recomendações sobre o uso clÃnico dos testes de exercÃcio em doentes com patologia cardiorrespiratória, dando particular ênfase à avaliação funcional, à avaliação do prognóstico e à avaliação das intervenções terapêuticas.A intolerância ao esforço fÃsico é um dos sintomas mais frequentes, condicionando a perda de qualidade de vida do doente com patologia cardiorrespiratória crónica. Pode definir-se âintolerância ao exercÃcioâ numa perspectiva clÃnica à incapacidade que o doente apresenta para realizar tarefas que os indivÃduos saudáveis considerariam toleráveis.A intolerância ao exercÃcio, considerada em termos do pico de consumo de oxigénio atingido no esforço máximo (VâO2pico) não pode ser prevista por parâmetros avaliados em repouso, como o volume expiratório máximo no primeiro segundo (FEV1), a transferência alvéolo-capilar do monóxido de carbono (DLCO), a fracção de ejecção do ventrÃculo esquerdo ou o Ãndice de massa corporal (IMC). A avaliação em exercÃcio impõe alguns desafios técnicos: a aplicação de protocolos especÃficos de incremento de carga de forma precisa e reprodutÃvel, com o recurso habitual a ergómetros, tais como a bicicleta ergométrica e o tapete rolante.A prova de exercÃcio cardiorrespiratória (CPET) é considerada o gold standard na avaliação das causas de intolerância ao exercÃcio em doentes com doença cardÃaca e pulmonar e é baseado no princÃpio de que a falência do sistema ocorre tipicamente quando o sistema (seja ele músculo-energético, cardiovascular ou pulmonar) se encontra sob stress. A CPET compreende a imposição de um exercÃcio com cargas crescentes (ou seja, incremental) limitado por sintomas, enquanto se monitorizam as variáveis cardiopulmonares (exemplo: consumo de oxigénio (VâO2), produção de dióxido de carbono (VâCO2), ventilação minuto (VâE), frequência cardÃaca (fC)), a percepção de sintomas (exemplo: a dispneia e o desconforto nos membros inferiores) e, quando necessárias, as avaliações da dessaturação arterial do oxigénio relacionada com o esforço, da hiperinsuflação dinâmica e da força muscular dos membros. Os sistemas são forçados até ao seu limite tolerável, de forma controlada, o que permite detectar respostas que identificam padrões de alteração e que podem ser relacionadas com padrões de referência previamente estudados e publicados pelas sociedades respiratórias europeia e americanas1-3.Neste documento, é descrito o papel da CPET como auxiliar no diagnóstico e na avaliação funcional e prognóstica. A CPET pode: â Fornecer uma medição objectiva da capacidade para o exercÃcio; â Identificar os mecanismos que limitam a tolerância ao exercÃcio; â Estabelecer Ãndices de prognóstico; â Monitorizar a progressão da doença e a resposta à s intervenções terapêuticas. â Auxiliar no diagnóstico, em situações de broncoconstrição induzida pelo exercÃcio e de dessaturação arterial do oxigénio. Na identificação das causas de intolerância ao exercÃcio, a CPET pode detectar: â Alterações na entrega de oxigénio (desde a sua entrada nas vias aéreas, passando pelo sistema de transporte cardiocirculatório, até à entrega à s mitocôndrias das fibras musculares); â Limitação ventilatória no exercÃcio; â Alteração do controlo ventilatório; â Alteração das trocas gasosas pulmonares; â Percepção excessiva de sintomas (exemplos: dispneia, precordialgia, fadiga muscular periférica); â Disfunção metabólica muscular; â Descondicionamento; â Fraco esforço dispendido. Com um bom esforço realizado, se o valor do pico do consumo de oxigénio atingido foi normal e o motivo para parar a prova foi dispneia ou fadiga muscular, então pode considerar-se que o indivÃduo estudado tem uma normal tolerância ao exercÃcio. Este cenário irá excluir doença pulmonar (DPOC, doença intersticial pulmonar, doença vascular pulmonar) ou cardÃaca (insuficiência cardÃaca congestiva) significativas como causa de intolerância.A prova de exercÃcio cardiopulmonar pode auxiliar no diagnóstico diferencial entre limitação no esforço de origem pulmonar ou cardiocirculatória. Pode fornecer um perfil de respostas que caracterizam determinadas doenças; exemplo: na DPOC são frequentes a limitação ventilatória, a hiperinsuflação dinâmica, a dessaturação arterial com o exercÃcio, a dispneia, a disfunção dos músculos periféricos; na doença intersticial pulmonar são frequentes a dispneia, a restrição ventilatória mecânica e as alterações graves das trocas gasosas. Outros padrões de respostas podem ser encontrados na broncoconstrição induzida pelo exercÃcio, na doença vascular pulmonar, na insuficiência cardÃaca e em cardiopatias congénitas. A avaliação cardiorrespiratória no exercÃcio fornece ainda indicadores prognósticos em várias doenças. Descrevem-se neste documento vários trabalhos que estudaram os parâmetros indicadores de prognóstico em doenças como a DPOC, a doença intersticial pulmonar, a hipertensão pulmonar primária, a fibrose quÃstica e a insuficiência cardÃaca.Este documento demonstra ainda a utilidade dos testes de exercÃcio na definição das respostas à s intervenções terapêuticas, em avaliações seriadas.O grupo de trabalho envolvido neste documento considerou importante apresentar as indicações baseadas na evidência para a realização dos testes de exercÃcio na prática clÃnica. A evidência actual é clara quanto à utilidade da prova de exercÃcio cardiopulmonar, das provas de marcha e das provas de carga constante na avaliação do grau de intolerância ao exercÃcio, do prognóstico e dos efeitos das intervenções terapêuticas em doentes adultos com doença pulmonar crónica (DPOC, doença intersticial pulmonar, hipertensão pulmonar primária), em crianças e adultos com fibrose quÃstica, em crianças e adultos com broncospasmo induzido pelo exercÃcio, em adultos com insuficiência cardÃaca congestiva e em crianças e adolescentes com cardiopatias congénitas.Na elaboração deste documento, os autores pretenderam fornecer as respostas à s perguntas que se colocam com frequência na prática clÃnica: â Quando se deve pedir uma avaliação da intolerância ao esforço? â Qual o teste mais adequado? â Quais as variáveis a seleccionar na avaliação do prognóstico de determinada doença ou na avaliação do efeito de uma intervenção terapêutica particular? O documento contém ainda um suplemento que pode ser obtido on-line e que descreve as bases fisiológicas subjacentes aos parâmetros avaliados nas provas de exercÃcio cardiopulmonar
IBRD Operational Decision Framework
The IBRD Operational Decision Framework in this document is an expansion of an emerging general risk management framework under development by an interagency working group. It provides the level of detail necessary to develop a general Consequence Management Guidance Document for biological contamination remediation and restoration. It is the intent of this document to support both wide area and individual site remediation and restoration activities. This product was initiated as a portion of the IBRD Task 1 Systems Analysis to aid in identification of wide area remediation and restoration shortcomings and gaps. The draft interagency general risk management framework was used as the basis for the analysis. The initial Task 1 analysis document expanded the draft interagency framework to a higher level of resolution, building on both the logic structure and the accompanying text explanations. It was then employed in a qualitative manner to identify responsible agencies, data requirements, tool requirements, and current capabilities for each decision and task. This resulted in identifying shortcomings and gaps needing resolution. Several meetings of a joint LLNL/SNL working group reviewed and approved the initial content of this analysis. At the conclusion of Task 1, work continued on the expanded framework to generate this Operational Decision Framework which is consistent with the existing interagency general risk management framework. A large LLNL task group met repeatedly over a three-month period to develop the expanded framework, coordinate the framework with the biological remediation checklist, and synchronize the logic with the Consequence Management Plan table of contents. The expanded framework was briefed at a large table top exercise reviewing the interagency risk management framework. This exercise had representation from major US metropolitan areas as well as national agencies. This product received positive comments from the participants. Upon completion of the Operational Decision Framework, another joint LLNL/SNL working group conducted a day-long review. Avaliação de títulos minerários de uso agrícola nas bacias logísticas da agropecuária brasileira.
O presente estudo objetivou avaliar o panorama das matérias-primas base para fertilizantes no contexto das bacias logísticas da agropecuária brasileira. Foram utilizadas informações dos títulos minerários disponíveis no Departamento Nacional de Produção Mineral (DNPM), dados de importação de fertilizantes disponibilizadas pela Associação Nacional para Difusão de Adubos (ANDA) e informações sobre a importação, exportação e comercialização de calcário entre os estados brasileiros disponibilizadas pela Associação Brasileira de Produtores de Calcário Agrícola (ABRACAL). Esses dados foram integrados em um ambiente de Sistema de Informação Geográfica (SIG), para representação espacial dessas variáveis no contexto das bacias logísticas e para determinação dos fluxos internos de calcário. Os resultados das análises dos títulos minerários indicaram baixo número de títulos voltados para uso na agricultura e em fase de concessão de lavra e licenciamento. Também foi possível observar grande movimentação interna de calcário entre os estados via transposição de bacias. A avaliação da importação dos fertilizantes indicou que os portos preferenciais de entrada destes são os mesmos portos utilizados para a saída da maior parte da produção de grãos. Estes resultados servirão de subsídios para o desenvolvimento de novos estudos voltados para a questão da retrologística agropecuária
paediatric respiratory disease past present and future
Paediatric respiratory disease has changed in the past 20 yrs; we could fill a whole issue of the journal paying tribute to our famous forebears. We are posing new challenges to our colleagues in the field of adult respiratory disease. They have to learn to deal with conditions that 20 yrs ago were rare in the adult chest clinic, such as cystic fibrosis (CF) and the long-term consequences of premature birth and congenital malformations of the respiratory tract. Furthermore, studies in childhood are challenging pathophysiological concepts throughout life. The many great prospective birth cohort studies have shed light on the different patterns of wheezing, their risk factors and their evolution through childhood. Who would have thought it was good to be born in a barn! It is becoming increasingly clear that even for "adult" diseases, such as chronic obstructive pulmonary disease (COPD), antenatal and early life events are at least as important as smoking in adulthood 1. CF has become a disease also of adults 2. Although many factors have contributed, the main reason has been the development of expert special CF centres, a model increasingly adopted by adult teams. This can serve as a model for other diseases; how a well-structured multidisciplinary approach to treatment can translate into benefits for patients. Perhaps numerically the most important achievement is in the field of public health. The benefit of the decrease in invasive bacterial infections, due to vaccination programmes for infants, is among the most important achievements of the past. Other areas of change include the survival of ever smaller preterm neonates. These children are reaching adult life with impaired lung function and abnormal computed tomography scans. A standard, single dose of inhaled terbutaline attenuates hyperpnoea-induced bronchoconstriction and mast cell activation in athletes
Release of broncho-active mediators from mast cells during exercise hyperpnoea is a key factor in the pathophysiology of exercise-induced bronchoconstriction (EIB). Our aim was to investigate the effect of a standard, single dose of an inhaled β2-adrenoceptor agonist on mast cell activation in response to dry air hyperpnoea in athletes with EIB. Twenty-seven athletes with EIB completed a randomised, double blind, placebo-controlled, crossover study. Terbutaline (0.5 mg) or placebo was inhaled15 min prior to 8 min of eucapnic voluntary hyperpnoea (EVH) with dry air. Pre- and post-bronchial challenge, urine samples were analysed by enzyme immunoassay for 11β-prostaglandin(PG)F2α. The maximum fall in forced expiratory volume in 1 sec(FEV1) of 14 (12-20)% (median and interquartile range) following placebo was attenuated to 7 (5-9)% with the administration of terbutaline (P<0.001). EVH caused a significant increase in 11β-PGF2α from (27-57) ng·mmol creatinine-1 at baseline to (43-72) ng·mmol creatinine-1 at its peak post-EVH following placebo (P=0.002). The rise in 11β-PGF2α was inhibited with administration of terbutaline: 39 (28-44) ng·mmol creatinine-1 at baseline vs. 40 (33-58) ng·mmol creatinine-1 at its peak post-EVH (P=0.118). These data provide novel in vivo evidence of mast cell stabilisation following inhalation of a standard dose of terbutaline prior to bronchial provocation with EVH in athletes with EIB
Wavelets and graph -algebras
Here we give an overview on the connection between wavelet theory and
representation theory for graph -algebras, including the higher-rank
graph -algebras of A. Kumjian and D. Pask. Many authors have studied
different aspects of this connection over the last 20 years, and we begin this
paper with a survey of the known results. We then discuss several new ways to
generalize these results and obtain wavelets associated to representations of
higher-rank graphs. In \cite{FGKP}, we introduced the "cubical wavelets"
associated to a higher-rank graph. Here, we generalize this construction to
build wavelets of arbitrary shapes. We also present a different but related
construction of wavelets associated to a higher-rank graph, which we anticipate
will have applications to traffic analysis on networks. Finally, we generalize
the spectral graph wavelets of \cite{hammond} to higher-rank graphs, giving a
third family of wavelets associated to higher-rank graphs
