LA CALIDAD DE LA EDUCACION, UNA PROBLEMATICA COLOMBIANA - THE QUALITY OF EDUCATION, A PROBLEM COLOMBIAN

    Get PDF

    Get PDF
Overview and Aims: The last decade brought relevant insights into the pathophysiology of preeclampsia (PE), namely the role of the circulating levels of placental growth factor (PlGF) and soluble Fms-like tyrosine kinase-1 (sFlt-1). The purpose of this study is to estimate the financial impact of introducing the sFlt-1/PlGF ratio for the evaluation of women with suspicion of PE in the Portuguese National Healthcare System (SNS). Study Design: budget impact study evaluating short-term costs associated with the introduction of the sFlt-1/PlGF ratio from the SNS payer’s perspective. The time horizon for the study is 1 year. Population: The target population consists of women presenting to the healthcare system with signs or symptoms su ggestive of preeclampsia (estimated in 8500 subjects). Methods: A decision-tree model was used to estimate the budget impact of the introduction of the sFlt-1/PlGF ratio in the SNS. The model compares the management costs in the current clinical practice (“no test” scenario) vs. current diagnostic procedures plus the sFlt-1/PlGF ratio (“test” scenario). Clinical inputs have been derived primarily from literature review and, where data was unavailable, expert opinion. Resources and unit costs have been obtained from Portugal-specific sources. Results: In the current standard practice (no test), total costs were estimated to be €9 863 264 (€1160 per patient), with unnecessary admissions representing about €3,5 million. Total costs in the test scenario sum up to €9 781 194 (€1150 per patient), representing a cost saving to the system of €82 070 (€10 per patient), mainly due to a reduction of false positives and related unnecessary hospitalizations of women not developing PE. Conclusions: There is favorable economic evidence about the introduction of the sFlt- 1/PlGF ratio in the SNS. The generated savings appear to offset the costs related to the test.info:eu-repo/semantics/publishedVersio

Acute kidney injury in pregnancy: a clinical challenge

    Get PDF
The incidence of acute kidney injury in pregnancy declined significantly over the second half of the 20th century; however, it is still associated with major maternal and perinatal morbidity and mortality. A set of systemic and renal physiological adaptive mechanisms occur during a normal gestation that will constrain several changes in laboratory parameters of renal function, electrolytes, fluid and acid-base balances. The diagnosis of acute kidney injury in pregnancy is based on the serum creatinine increase. The usual formulas for estimating glomerular filtration rate are not validated in this population. During the first trimester of gestation, acute kidney injury develops most often due to hyperemesis gravidarum or septic abortion. In the third trimester, the differential diagnosis is more challenging for the obstetrician and the nephrologist and comprises some pathologies that are reviewed in this article: preeclampsia/HELLP syndrome, acute fatty liver of pregnancy and thrombotic microangiopathies

What are the motives underlying Brazilians' food choices? An analysis of the Food Choice Questionnaire and its relationship with different sample characteristics

    Get PDF
ABSTRACT: The first aim of this study was to evaluate the psychometric properties of the Food Choice Questionnaire (FCQ)—using the original model—in a sample of 1480 Brazilian adults (69.5% female). The second aim was to rank the reasons underlying the participants' food choices using average FCQ scores and 95% confidence interval. The third aim was to evaluate the relationship between food choice motives and sample characteristics using multiple logistic regression and odds ratios. The validity, the invariance across different groups, and the reliability of the FCQ were confirmed for the sample. Sensory appeal and price emerged as the most important reasons, while ethical concern was the least valued. The factors associated with greater odds of choosing food for specific reasons were being older, female, and a student; practicing physical activity; dieting frequently; self-rating eating quality as good; having a higher body mass index; and having low income. Practical Applications Assessing food choice is a complex task, as it encompasses several factors, such as sensory characteristics, health status, income, culture, lifestyle, and cognitive-affective issues; therefore, the use of appropriate tools should be encouraged. The set analyses followed confirmed that the FCQ was an adequate instrument to evaluate the reasons for food choice of the participants who valued strongly the sensory aspects of the foods and presented specific characteristics (e.g., diet practice) that may influence their decisions. These findings may guide future research and clinical interventions aimed at producing food choices that are more oriented to health and well-being.FAPESPinfo:eu-repo/semantics/publishedVersio

Facile Synthesis of High Quality Graphene Nanoribbons

Graphene nanoribbons have attracted attention for their novel electronic and spin transport properties1-6, and because nanoribbons less than 10 nm wide have a band gap that can be used to make field effect transistors. However, producing nanoribbons of very high quality, or in high volumes, remains a challenge. Here, we show that pristine few-layer nanoribbons can be produced by unzipping mildly gas-phase oxidized multiwalled carbon nanotube using mechanical sonication in an organic solvent. The nanoribbons exhibit very high quality, with smooth edges (as seen by high-resolution transmission electron microscopy), low ratios of disorder to graphitic Raman bands, and the highest electrical conductance and mobility reported to date (up to 5e2/h and 1500 cm2/Vs for ribbons 10-20 nm in width). Further, at low temperature, the nanoribbons exhibit phase coherent transport and Fabry-Perot interference, suggesting minimal defects and edge roughness. The yield of nanoribbons was ~2% of the starting raw nanotube soot material, which was significantly higher than previous methods capable of producing high quality narrow nanoribbons1. The relatively high yield synthesis of pristine graphene nanoribbons will make these materials easily accessible for a wide range of fundamental and practical applications.Comment: Nature Nanotechnology in pres

Coupled effect of microstructure and topology on the mechanical behavior of Inconel718 additively manufactured lattices

    Get PDF
LPBF-manufactured Inconel718 lattices with six cubic and hexagonal structures and with two material microstructures (as-built, without γ” precipitation, and heat-treated, with γ” precipitates, but with similar cell/grain size, shape and texture) were compressed at room temperature and at 600 °C. The behavior of the base material under identical microstructure and test conditions was also investigated using dedicated LPBF-manufactured specimens with single strut gauges. Irrespective of topology, precipitation led to a transition in the lattice mechanical behavior from bending-dominated to stretch-dominated, which was associated to a change in the strut deformation mode from plastic hinging to elastic buckling, as well as to a decrease in the base material strain to fracture. For all topologies investigated, precipitation led to lattice strengthening, consistent with particle-strengthening of the base material. Additionally, high temperature straining resulted both in a decrease in the lattice yield strength, consistent with softening of the base material, and in a reduction in the width of the stress oscillations in the stretch-dominated lattices, which is associated to the decrease in the base material ductility. This work proves that material microstructure influences strongly the behavior of additively manufactured architectured structures and that it must be considered as a design criterion for performance optimization
