    The Effects of Breastfeeding on Childhood BMI: A Propensity Score Matching Approach

    Background Many studies have found a statistical association between breastfeeding and childhood adiposity. This paper investigates whether breastfeeding has an effect on subsequent childhood body mass index (BMI) using propensity scores to account for confounding. Methods We use data from the Millennium Cohort Study, a nationally representative UK cohort survey, which contains detailed information on infant feeding and childhood BMI. Propensity score matching is used to investigate the mean BMI in children breastfed exclusively and partially for different durations of time. Results We find statistically significant influences of breastfeeding on childhood BMI, particularly in older children, when breastfeeding is prolonged and exclusive. At 7 years, children who were exclusively breastfed for 16 weeks had a BMI 0.28 kg/m 2 (95% confidence interval 0.07 to 0.49) lower than those who were never breastfed, a 2% reduction from the mean BMI of 16.6 kg/m 2 . Conclusions For this young cohort, even small effects of breastfeeding on BMI could be important. In order to reduce BMI, breastfeeding should be encouraged as part of wider lifestyle intervention. This evidence could help to inform public health bodies when creating public health guidelines and recommendations

    Family Lifestyle Dynamics and Childhood Obesity: Evidence from the Millennium Cohort Study

    Using data from the Millennium Cohort Study, we investigate the dynamic relationship between underlying family lifestyle and childhood obesity during early childhood. We use a dynamic latent factor model, an approach that allows us to identify family lifestyle, its evolution over time and its influence on childhood obesity and other observable outcomes. We find that family lifestyle is persistent and has a significant influence on childhood weight status as well as other outcomes for all family members. Interventions should therefore be prolonged and persuasive and target the underlying lifestyle of a family as early as possible during childhood in order to have the greatest cumulative influence. Furthermore, the results indicate that to reduce inequalities in childhood obesity, policy makers should target disadvantaged families and design interventions specifically for these families

    This paper investigates the effect of breastfeeding on childhood body mass index (BMI). We use data from the Millennium Cohort Study (MCS), a nationally representative UK cohort survey, containing detailed infant feeding information, which allow us to explore the effects of a range of breastfeeding variables on the mean BMI of children breastfed for different durations and for exclusive and partial breastfeeding. Using propensity score matching, we find statistically significant influences of breastfeeding on childhood BMI, particularly in older children and when breastfeeding is prolonged and exclusive. The effects of breastfeeding on BMI are small in magnitude but large relative to the mean BMIs of children this age. At this young age, there is not a large difference in BMI between children who are identified as obese and those who are identified as normal weight, so even a small difference in BMI could mean the difference between children being overweight or a healthy weight. We suggest that breastfeeding should be encouraged as part of wider lifestyle interventions in order to help reduce BMI as well as improve other childhood outcomes. This could be important evidence for public health bodies when creating public health guidelines and recommendations

    Gender Differences In Academic Ethics With Recommendations For Curricular Change

    An extensive study has been performed on the importance of building ethical principles into secondary school and college curricula.  In published surveys, females are almost universally found to be more ethical, but experience tells us females lag behind males in their ability to maintain and act upon their convictions in the workplace.  We examined these issues by administering a survey on academic ethics to an undergraduate business school population, focusing heavily on gender differences. Careful analysis of survey results using one-way ANOVA, the Tukey-Kramer procedure, and two-way ANOVA procedures provided an understanding of differences in ethical beliefs and ethical behaviors based on gender and other demographic characterizations.  Predictive analysis was completed using logistic regression and discrete choice modeling to determine the likelihood of ethical behavior in the future and evolution of ethical beliefs.  The accumulated results of the analyses were used to guide the authors in the development of a strong and pervasive ethics-based curriculum for secondary schools and universities.  We have combined elements of classroom instruction, technology, active learning, games and extracurricular activities to embed ethical concepts and particularly encourage strength in convictions across the entire curricula.

    The ocean plays a major role in controlling the concentration of carbon dioxide (CO2) in the atmosphere. Increasing concentrations of CO2 in the atmosphere are a threat to the stability of the earth’s climate. A better understanding of the controlling role of the ocean will improve predictions of likely future changes in climate and the impact of the uptake of CO2 itself on marine eco-systems caused by the associated acidification of the ocean waters. The SNOMS Project (SWIRE NOCS Ocean Monitoring System) is a ground breaking joint research project supported by the Swire Group Trust, the Swire Educational Trust, the China Navigation Company (CNCo) and the Natural Environment Research Council. It collects high quality data on concentrations of CO2 in the surface layer of the ocean. It contributes to the international effort to better quantify (and understand the driving processes controlling) the exchanges of CO2 between the ocean and the atmosphere. In 2006 and 2007 a system that could be used on a commercial ship to provide data over periods of several months with only limited maintenance by the ships crew was designed and assembled by NOCS. The system was fitted to the CNCo ship the MV Pacific Celebes in May 2007. The onboard system was supported by web pages that monitored the progress of the ship and the functioning of the data collection system. To support the flow of data from the ship to the archiving of the data at the Carbon Dioxide Information Analysis Center (CDIAC in the USA) data processing procedures were developed for the quality control and systematic handling of the data. Data from samples of seawater collected by the ships crew and analysed in NOC (730 samples) have been used to confirm the consistency of the data from the automated measurement system on the ship. To examine the data collected between 2007 and 2012 the movements of the ship are divided into 16 voyages. Initially The Celebes traded on a route circum-navigating the globe via the Panama and Suez Canals. In 2009 the route shifted to one between Australia and New Zealand to USA and Canada. Analysis of the data is an on going process. It has demonstrated that the system produces reliable data. Data are capable of improving existing estimates of seasonal variability. The work has improved knowledge of gas exchange processes. Data from the crew-collected-samples are helping improve our ability to estimate alkalinity in different areas. This helps with the study of ocean acidification. Data from the 9 round trips in the Pacific are currently being examined along with data made available by the NOAA-PMEL laboratory forming time series from 2004 to 2012. The data from the Pacific route are of considerable interest. One reason is that the data monitors variations in the fluxes of CO2 associated with the current that flows westwards along the equator. This is one of the major natural sources of CO2 from the ocean into the atmosphere

    Patients' Experiences and Perspectives of Telehealth Coaching with a Dietitian to Improve Diet Quality in Chronic Kidney Disease: A Qualitative Interview Study

    Background: Dietary behavior change interventions for the self-management of chronic kidney disease (CKD) have the potential to slow disease progression and reduce metabolic complications. Telehealth-delivered dietary interventions may assist in the self-management of CKD, although their acceptability by patients is unknown. Objective: This study aims to describe the acceptability and experiences of a telehealth coaching intervention that utilized telephone calls and tailored text messages to improve diet quality in patients with stage 3 to 4 CKD. Design: Semistructured interview study of adults with CKD. Participants/setting: Adults with stage 3 to 4 CKD (n=21) aged 28 to 78 (mean 62) years, who completed a 12-week telehealth-delivered dietary intervention in Queensland, Australia, were interviewed from March to July 2017. Data analysis: Interviews were transcribed verbatim and analyzed thematically. Results: Five themes were identified: valuing relationships (receiving tangible and perceptible support, building trust and rapport remotely, motivated by accountability, readily responding to a personalized approach, reassured by health professional expertise); appreciating convenience (integrating easily into lifestyle, talking comfortably in a familiar environment, minimizing travel and wait time burden); empowered with actionable knowledge (comprehending diet-disease mechanisms, practical problem solving for sustainable dietary behavior); increasing diet consciousness (learning from recurrent feedback, prompted by reiteration of messages); making sense of complexity (contextualizing and prioritizing comorbidities, gaining confidence to make dietary decisions, setting and achieving realistic goals). Conclusions: Among adults with stage 3 to 4 CKD, individualized telehealth coaching for improving diet quality was convenient for patients, and they felt supported and empowered to navigate recommendations and prioritize dietary behavior changes. Telehealth-delivered dietary interventions appear to be well accepted by patients as a way of providing regular, tailored contact with a health professional to support dietary management in CKD

    ‘Back to Life’—Using knowledge exchange processes to enhance lifestyle interventions for liver transplant recipients: A qualitative study

    Interventions to prevent excessive weight gain after liver transplant are needed. The purpose of the present study was to enhance a specialist post-transplant well-being program through knowledge exchange with end-users.The study used an interactive process of knowledge exchange between researchers, clinicians and health system users. Data were collected as focus groups or telephone interviews and underwent applied thematic analysis.There were 28 participants (age 24-68 years; 64% male). The results identified experiences that may influence decisions around health behaviours during the course of transplant recovery. Three over-arching themes were identified that impact on liver transplant recipients post-transplant health behaviours. These include (i) Finding a coping mechanism which highlighted the need to acknowledge the significant emotional burden of transplant prior to addressing long-term physical wellness; (ii) Back to Life encompassing the desire to return to employment and prioritise family, while co-ordinating the burden of ongoing medical monitoring and self-management and (iii) Tailored, Personalised Care with a preference for health care delivery by transplant specialists via a range of flexible eHealth modalities.This person-centred process of knowledge exchange incorporated experiences of recipients into service design and identified life priorities most likely to influence health behaviours post-transplant. Patient co-creation of services has the potential to improve the integration of knowledge into health systems and future directions will require evaluation of effectiveness and sustainability of patient-centred multidisciplinary service development

The book is discussed by Adrienne van den Bogaard and Frank Veraart and by Gerard Alberts, followed by a reply by the author. Van den Bogaard and Veraart join great appreciation for the three-sector division Campbell-Kelly developed to describe the history of the software industry, to a slight criticism of his ad hoc-argumentation in explaining why in each sector some enterprises survive and others do not.
