    A simple tool to predict admission at the time of triage

    Aim To create and validate a simple clinical score to estimate the probability of admission at the time of triage. Methods This was a multicentre, retrospective, cross-sectional study of triage records for all unscheduled adult attendances in North Glasgow over 2 years. Clinical variables that had significant associations with admission on logistic regression were entered into a mixed-effects multiple logistic model. This provided weightings for the score, which was then simplified and tested on a separate validation group by receiving operator characteristic (ROC) analysis and goodness-of-fit tests. Results 215 231 presentations were used for model derivation and 107 615 for validation. Variables in the final model showing clinically and statistically significant associations with admission were: triage category, age, National Early Warning Score (NEWS), arrival by ambulance, referral source and admission within the last year. The resulting 6-variable score showed excellent admission/discharge discrimination (area under ROC curve 0.8774, 95% CI 0.8752 to 0.8796). Higher scores also predicted early returns for those who were discharged: the odds of subsequent admission within 28 days doubled for every 7-point increase (log odds=+0.0933 per point, p&#60;0.0001). Conclusions This simple, 6-variable score accurately estimates the probability of admission purely from triage information. Most patients could accurately be assigned to ‘admission likely’, ‘admission unlikely’, ‘admission very unlikely’ etc., by setting appropriate cut-offs. This could have uses in patient streaming, bed management and decision support. It also has the potential to control for demographics when comparing performance over time or between departments.</p

    International audienceAudiovisual culture often privileges the instantly identifiable: the recognizable face, the well-timed stunt, the perfectly synchronized line of dialogue. Yet order and clarity do not come ‘naturally’ to the moving image. Light, motion, definition, compression: the conditions of recording, storing and screening can subject audiovisual media to countless variations, pulling them towards the indefinite and illegible. Filmmakers and artists often seek out and work with the resulting uncertainty, from the warping of space to the melding of senses, from glare to shadow and blur to glitch. This collection concerns itself with the aesthetics, concepts and politics of indefinite and obscured moving images, examining what is at stake in their foregrounding of materiality and mediation, evanescence and flux. Pursuing a range of approaches (spanning history, theory and close analysis), the authors in this volume investigate techniques, effects and themes that emerge from the wilful excavation of the moving image’s formal and material base

    A conjecture of Kadison and Kastler from 1972 asks whether sufficiently close operator algebras in a natural uniform sense must be small unitary perturbations of one another. For n≥3 and a free, ergodic, probability measure-preserving action of SL&lt;sub&gt;n&lt;/sub&gt;(Z) on a standard nonatomic probability space (X,μ), write M=(L&lt;sup&gt;∞&lt;/sup&gt;(X,μ)⋊SL&lt;sub&gt;n&lt;/sub&gt;(Z))⊗¯¯¯R, where R is the hyperfinite II1-factor. We show that whenever M is represented as a von Neumann algebra on some Hilbert space H and N⊆B(H) is sufficiently close to M, then there is a unitary u on H close to the identity operator with uMu∗=N. This provides the first nonamenable class of von Neumann algebras satisfying Kadison and Kastler’s conjecture. We also obtain stability results for crossed products L&lt;sup&gt;∞&lt;/sup&gt;(X,μ)⋊Γ whenever the comparison map from the bounded to usual group cohomology vanishes in degree 2 for the module L&lt;sup&gt;2&lt;/sup&gt;(X,μ). In this case, any von Neumann algebra sufficiently close to such a crossed product is necessarily isomorphic to it. In particular, this result applies when Γ is a free group

    Stability of trusses by graphic statics.

    This paper presents a graphical method for determining the linearized stiffness and stability of prestressed trusses consisting of rigid bars connected at pinned joints and which possess kinematic freedoms. Key to the construction are the rectangular areas which combine the reciprocal form and force diagrams in the unified Maxwell-Minkowski diagram. The area of each such rectangle is the product of the bar tension and the bar length, and this corresponds to the rotational stiffness of the bar that arises due to the axial force that it carries. The prestress stability of any kinematic freedom may then be assessed using a weighted sum of these areas. The method is generalized to describe the out-of-plane stability of two-dimensional trusses, and to describe three-dimensional trusses in general. The paper also gives a graphical representation of the 'product forces' that were introduced by Pellegrino and Calladine to describe the prestress stability of trusses
