Frecuentemente a nuestra consulta acuden pacientes con recesión gingival, estas constituyen una preocupación estética, esto provoca caries radicular, sensibilidad radicular e irritabilidad del margen gingival causado por la incapacidad del paciente de
remover la placa, además, al estar afectada la estética puede provocar trastornos psicológicos al paciente. Existen diversas técnicas para contrarrestar estos defectos como injerto libres de encía, deslizante lateral, doble papila, tejido conectivo subepitelial, semilunar, regeneración tisular guiada y la técnica de colgajo pediculado deslizado coronalmente. El caso que presentamos se trató de un paciente masculino de 25 años de edad, que acudió a nuestra consulta preocupado por su estética ya que a nivel del 23 la encía se encontraba retraída y poca encía insertada, al medir con la sonda periodontal nos dio una exposición radicular de 5 mm, nos dimos a la tarea de resolver el problema y decidimos realizar una amplia búsqueda que nos demostrara con cual técnica obtendríamos mejores resultados por lo que se decide aplicar la combinación de las técnicas de tejido conectivo subepitelial y deslizante coronal, cumpliéndose el objetivo del trabajo donde queda el 96 % cubierta la raíz y se
aumenta la encía insertada y de esta forma también queda satisfecho el paciente al resolver su problema estético.
ABSTRACT
Gingival recession is a very frequent patients ‘reasons for visiting our office, it constitutes an esthetic concern, they also bring about radicular caries, radicular tenderness and gingival margin irritability due to their incapacity to remove the dental bacterial plaque, as esthetics is affected it may therefor cause patients psychological upsets. There are some techniques to treat these defects, such as free gingival grafting, lateral sliding, double papilla, sub-epithelial connective tissue, semilunar, tisular guided regeneration and pedicles flap with crown sliding. A 25 year old – male patient came to our clinic worried about his look. His gums were retracted in 23 level and a few gum was inserted. Five millimeters of root exposition was revealed at proving. After a careful study of the case it was decided to combine two techniques sub-epithelial connective tissue and crown sliding; 96 % of the root was covered and the inserted gingiva increased. Irisin, in women and men: blood pressure, heart rate, obesity and insulin resistance
BackgroundIrisin is a myokine that increases with leisure time physical activity (LTPA) and for which a cardiovascular protective role has been postulated. Our aim was to assess this role in the general population.MethodsA cross-sectional analysis was performed in a large randomly selected population sample (n=2298 women and 1529 men). Apart from age and sex, we record anthropometrics (blood pressure, heart rate, obesity), lifestyle (LTPA, smoking, alcohol), and biochemical measurements (irisin, lipid profile, insulin resistance). Correlations and regression multivariate models were used to analyze the association of irisin levels with the studied factors.ResultsThe variables more strongly and directly associated with irisin, adjusting the studied factors separately in women and men, were HOMA-2 (p=0.043 and p=0.001, respectively) and LTPA (p<0.001 and p=0.001, respectively). Also heart rate inversely (p=0.005 and p=0.002, respectively) and DBP directly (p<0.005 and p=0.045, respectively) were associated to irisin in both sexes. The waist/height ratio (p<0.001) was inversely associated to irisin only in women, and the alcohol drinking was directly associated (p=0.029) only in men.ConclusionWe provide new findings for irisin, such as its association with DBP and with heart rate; furthermore, in women irisin is associated to abdominal obesity, and in men is associated to the alcohol intake. We also corroborate the association of irisin with LTPA and insulin resistance. The associations detected point towards a protective role of irisin in the maintenance of cardiometabolic health

It manages four observatories in Tenerife including the high altitude Izaña Atmospheric Observatory. The Izaña Atmospheric Observatory was inaugurated in 1916 and since that date has carried out uninterrupted meteorological and climatological observations, contributing towards a unique 100-year record in 2016.
This reports are a summary of the many activities at the Izaña Atmospheric Research Center to the broader community. The combination of operational activities, research and development in state-of-the-art measurement techniques, calibration and validation and international cooperation encompass the vision of WMO to provide world leadership in expertise and international cooperation in weather, climate, hydrology and related environmental issues.[ES]El Centro de Investigación Atmosférica de Izaña (CIAI), que forma parte de la Agencia Estatal de Meteorología de España (AEMET), representa un centro de excelencia en ciencias atmosféricas. Role of age and comorbidities in mortality of patients with infective endocarditis
[Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality.
[Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk.
[Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality.
[Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

Delineating fluid-filled region boundaries in optical coherence tomography images of the retina
We evaluate the ability of a deformable model to yield accurate shape descriptions of fluid-filled regions associated with age-related macular degeneration. Calculation of retinal thickness and volume by the current optical coherence tomography (OCT) system includes fluid-filled regions or lesions along with actual retinal tissue. In order to quantify these lesions independently from the retinal tissue, they must be outlined. A deformable model was applied to OCT images of retinas demonstrating cystoids and subretinal fluid spaces. Several implementation issues were addressed in order to choose appropriate parameters. The use of a nonlinear anisotropic diffusion filter to suppress speckle noise while at the same time preserving the edges of the original image was explored. Once the contours of the lesions were outlined, quantitative analysis of the surface area and volume of the lesions was performed. The deformable model could accurately outline fluid-filled regions within the retina. The detection method tested proved effective in capturing the complexity of fluid-filled regions in OCT images. Deformable models combined with nonlinear anisotropic diffusion filtering show promise in the detection of retinal features of interest for diagnosis in clinical OCT images. Thus, fluid-filled region detection may significantly aid in analysis of treatments and diagnosis

Comparison of PDE-based nonlinear diffusion approaches for image enhancement and denoising in optical coherence tomography
A comparison between two nonlinear diffusion methods for denoising OCT images is performed. Specifically, we compare and contrast the performance of the traditional nonlinear Perona-Malik filter with a complex diffusion filter that has been recently introduced by Gilboa et al.. The complex diffusion approach based on the generalization of the nonlinear scale space to the complex domain by combining the diffusion and the free Schridinger equation is evaluated on synthetic images and also on representative OCT images at various noise levels. The performance improvement over the traditional nonlinear Perona-Malik filter is quantified in terms of noise suppression, image structural preservation and visual quality. An average signal-to-noise ratio (SNR) improvement of about 2.5 times and an average contrast to noise ratio (CNR) improvement of 49% was obtained while mean structure similarity (MSSIM) was practically not degraded after denoising. The nonlinear complex diffusion filtering can be applied with success to many OCT imaging applications. In summary, the numerical values of the image quality metrics along with the qualitative analysis results indicated the good feature preservation performance of the complex diffusion process, as desired for better diagnosis in medical imaging processing
