The whole is greater than the sum of the parts: Recognising missed opportunities for an optimal response to the rapidly maturing TB-HIV co-epidemic in South Africa
<p>Abstract</p> <p>Background</p> <p>Despite widely acknowledged WHO guidelines for the integration of TB and HIV services, heavily burdened countries have been slow to implement these and thus significant missed opportunities have arisen.</p> <p>Discussion</p> <p>The individual-centred, rights-based paradigm of the SA National AIDS Policy, remains dissonant with the compelling public-health approach of TB control. The existence of independent and disconnected TB and HIV services results in a wastage of scarce health resources, an increased burden on patients' time and finances, and ignores evidence of patients' preference for an integrated service. The current situation translates into a web of unacceptable, ongoing missed opportunities such as failure to maximize collaborative disease surveillance, VCT, adherence support, infection control, and positive prevention. TB services present a readily identifiable cohort for HIV provider-initiated testing. Integrating HAART and DOTS will promote efficient usage of health workers' time and a more navigable experience for patients, ultimately ensuring increased TB treatment completion rates and MDR-TB prevention. As direct observation evolves into a more supportive, empowering experience for patients, adherence to both TB drugs and HAART will be bolstered. Little attention has been paid to the transmission of TB within HIV services. Low cost infection control interventions include: triaging patients, scheduling new and follow-up patients separately; well-ventilated, sheltered waiting rooms; and the use of personal respirators by patients and staff. A more patient-centred approach to TB care may be able to recruit the active participation of TB patients in positive prevention efforts, including maximizing personal infection control, limiting exposure of social contacts to TB during the intensive phase of treatment, advocating isoniazid prophylaxis within the home and patient-centred education efforts to reduce overall transmission. Several model programmes demonstrated synergy, in which the impact of the "whole" or integrated response was greater than the sum of the non-integrated parts.</p> <p>Summary</p> <p>The full potential of an integrated TB-HIV service has not been fully harvested. Missed opportunities discount existing efforts in both programmes, will perpetuate the burden of disease, and prevent major gains in future interventions. This paper outlines simple, readily-implementable strategies to narrow the gap and reclaim existing missed opportunities.</p
The Thermal Design, Characterization, and Performance of the SPIDER Long-Duration Balloon Cryostat
We describe the SPIDER flight cryostat, which is designed to cool six
millimeter-wavelength telescopes during an Antarctic long-duration balloon
flight. The cryostat, one of the largest to have flown on a stratospheric
payload, uses liquid helium-4 to deliver cooling power to stages at 4.2 and 1.6
K. Stainless steel capillaries facilitate a high flow impedance connection
between the main liquid helium tank and a smaller superfluid tank, allowing the
latter to operate at 1.6 K as long as there is liquid in the 4.2 K main tank.
Each telescope houses a closed cycle helium-3 adsorption refrigerator that
further cools the focal planes down to 300 mK. Liquid helium vapor from the
main tank is routed through heat exchangers that cool radiation shields,
providing negative thermal feedback. The system performed successfully during a
17 day flight in the 2014-2015 Antarctic summer. The cryostat had a total hold

Tensor Decomposition Reveals Concurrent Evolutionary Convergences and Divergences and Correlations with Structural Motifs in Ribosomal RNA
Evolutionary relationships among organisms are commonly described by using a
hierarchy derived from comparisons of ribosomal RNA (rRNA) sequences. We propose that
even on the level of a single rRNA molecule, an organism's evolution is composed
of multiple pathways due to concurrent forces that act independently upon different
rRNA degrees of freedom. Relationships among organisms are then compositions of
coexisting pathway-dependent similarities and dissimilarities, which cannot be
described by a single hierarchy. We computationally test this hypothesis in
comparative analyses of 16S and 23S rRNA sequence alignments by using a tensor
decomposition, i.e., a framework for modeling composite data. Each alignment is
encoded in a cuboid, i.e., a third-order tensor, where nucleotides, positions and
organisms, each represent a degree of freedom. A tensor mode-1 higher-order singular
value decomposition (HOSVD) is formulated such that it separates each cuboid into
combinations of patterns of nucleotide frequency variation across organisms and
positions, i.e., “eigenpositions” and corresponding nucleotide-specific
segments of “eigenorganisms,” respectively, independent of a-priori
knowledge of the taxonomic groups or rRNA structures. We find, in support of our
hypothesis that, first, the significant eigenpositions reveal multiple similarities
and dissimilarities among the taxonomic groups. Second, the corresponding
eigenorganisms identify insertions or deletions of nucleotides exclusively conserved
within the corresponding groups, that map out entire substructures and are enriched
in adenosines, unpaired in the rRNA secondary structure, that participate in tertiary
structure interactions. This demonstrates that structural motifs involved in rRNA
folding and function are evolutionary degrees of freedom. Third, two previously
unknown coexisting subgenic relationships between Microsporidia and Archaea are
revealed in both the 16S and 23S rRNA alignments, a convergence and a divergence,
conferred by insertions and deletions of these motifs, which cannot be described by a
single hierarchy. This shows that mode-1 HOSVD modeling of rRNA alignments might be
used to computationally predict evolutionary mechanisms
Seasonal and Altitudinal Changes in Population Density of 20 Species of Drosophila in Chamundi Hill
A year long study was conducted to analyze the altitudinal and seasonal variation in a population of Drosophila (Diptera: Drosophilidae) on Chamundi hill of Mysore, Karnataka State, India. A total of 16,671 Drosophila flies belonging to 20 species of 4 subgenera were collected at altitudes of 680 m, 780 m, 880 m and 980 m. The subgenus Sophophora was predominant with 14 species and the subgenus Drosilopha was least represented with only a single species. Cluster analysis and constancy methods were used to analyze the species occurrence qualitatively. Altitudinal changes in the population density, and relative abundance of the different species at different seasons were also studied. The diversity of the Drosophila community was assessed by applying the Simpson and Berger-Parker indices. At 680 m the Simpson Index was low at 0.129 and the Berger- Parker index was high at 1.1 at 980 m. Linear regression showed that the Drosophila community was positively correlated with rainfall but not elevation, Furthermore the density of Drosophila changed significantly in different seasons (F = 11.20, df 2, 9; P<0.004). The distributional pattern of a species or related group of species was uneven in space and time. D. malerkotliana and D. nasuta were found at all altitudes and can be considered as dominant species
An ENU-induced mutation of miR-96 associated with progressive hearing loss in mice.
Progressive hearing loss is common in the human population, but little is known about the molecular basis. We report a new N-ethyl-N-nitrosurea (ENU)-induced mouse mutant, diminuendo, with a single base change in the seed region of Mirn96. Heterozygotes show progressive loss of hearing and hair cell anomalies, whereas homozygotes have no cochlear responses. Most microRNAs are believed to downregulate target genes by binding to specific sites on their mRNAs, so mutation of the seed should lead to target gene upregulation. Microarray analysis revealed 96 transcripts with significantly altered expression in homozygotes; notably, Slc26a5, Ocm, Gfi1, Ptprq and Pitpnm1 were downregulated. Hypergeometric P-value analysis showed that hundreds of genes were upregulated in mutants. Different genes, with target sites complementary to the mutant seed, were downregulated. This is the first microRNA found associated with deafness, and diminuendo represents a model for understanding and potentially moderating progressive hair cell degeneration in hearing loss more generally
CHEK2 1100delC in patients with metachronous cancers of the breast and the colorectum
BACKGROUND: Development of multiple primary tumors is a hallmark of hereditary cancer. At least 1/10 of breast cancers and colorectal cancers occur because of heredity and recently the cell cycle kinase 2, CHEK2 1100delC allele has been identified at a particularly high frequency in families with hereditary breast and colorectal cancer. METHODS: We utilized the Southern Sweden population-based cancer registry to identify women with double primary breast and colorectal cancer and sequenced tumor material in order to assess the contribution of the CHEK2 1100delC to the development of such metachronous tumors. RESULTS: Among the 75 patients successfully analyzed, 2 (2.5%) carried the CHEK2 1100delC allele. which was not significantly different (p = 0.26) from the 1% (3/300) carriers identified in the control group. CONCLUSION: In summary, our data suggest that the CHEK2 1100delC is not a major cause of double primary breast and colorectal cancer in Sweden, which suggests that this patient group should not routinely be screened for the CHEK2 1100delC variant
