    Organization and structure of the chain in the Integrated Projects of Food Chain in Basilicata region: the effects on the new rural dynamics

    The introduction of the Integrated Projects of Food Chain requires the development of models capable of interpreting the dynamics of vertical and horizontal coordination between agents and the definition of the issues that most affect the ability of professionals to provide value added to goods and products to acquire in exchange a competitive advantage. With reference to the Basilicata region, the production structure of the region and the recent development of the Integrated Projects of Food Chain, this research has developed a new model of territorial organization of rural development.     The development of agribusiness sector is given gradually increasing importance. This works aims at identifying the elements that create the successful ecosystem for agricultural development, their benefits and application in agri-food systems; additionally, to bring on a light of the role of agricultural policies in these processes. For this purpose, the authors have carried out an analysis of the concern related literature and present a preliminary theoretical framework/model as a tool for supporting local development and incorporate a successful effective ecosystem. The key implications of this paper include an increased attention to agricultural research, to the role of the collaboration between relevant stakeholders, as well as of the agriculture, which uses information technologies for the better utilization of natural resources, and what is the most important is, the role of agricultural policies

    Tratamiento conservador del fibromixoma odontogénico en mandíbula: reporte de dos casos

    Introduction: Odontogenic fibromyxoma is a rare benign tumor of mesenchymal origin. Small tumors may be asymptomatic and are discovered only during the routine radiographic examination, and large tumors are associated with the painless expansion of bone with the displacement of teeth and bone perforation. The treatment option is surgical and can range from conservative treatment to radical excision. Case Report: We report two cases of fibromyxoma in the mandible in a 17-year-old woman and an 18-year-old man. The diagnosis was anatomopathological. Both underwent conservative enucleation treatment with peripheral osteotomy and extraction of the affected teeth with favorable evolution. Discussion: Conservative treatment with an intraoral approach is a humanistic alternative that maintains its value of success, favorable evolution, and great acceptance by patients.Introducción: El fibromixoma odontogénico es un tumor benigno poco frecuente de origen mesenquimal. Las lesiones menores pueden ser asintomáticas y se descubren sólo durante un examen radiográfico de rutina, y las lesiones mayores se asocian a expansión indolora del hueso involucrado, con desplazamiento de dientes y perforación ósea. La opción de tratamiento es quirúrgica existiendo desde el tratamiento conservador hasta la escisión radical. Reporte de caso: Presentamos dos casos de fibromixoma en mandíbula en una mujer de 17 años y varón de 18 años. El diagnóstico fue anatomopatológico. En ambos se realizó el tratamiento conservador de enucleación con osteotomía periférica y extracción de las piezas dentarias involucradas, con evolución favorable. Discusión: El tratamiento conservador con abordaje intrabucal es una alternativa humanística que mantiene su valor de éxito, evolución favorable y gran aceptación por los pacientes

    Mortality After Pediatric Arterial Ischemic Stroke

    OBJECTIVES: Cerebrovascular disease is among the top 10 causes of death in US children, but risk factors for mortality are poorly understood. Within an international registry, we identify predictors of in-hospital mortality after pediatric arterial ischemic stroke (AIS). METHODS: Neonates (0-28 days) and children (29 days- < 19 years) with AIS were enrolled from January 2003 to July 2014 in a multinational stroke registry. Death during hospitalization and cause of death were ascertained from medical records. Logistic regression was used to analyze associations between risk factors and in-hospital mortality. RESULTS: Fourteen of 915 neonates (1.5%) and 70 of 2273 children (3.1%) died during hospitalization. Of 48 cases with reported causes of death, 31 (64.6%) were strokerelated, with remaining deaths attributed to medical disease. In multivariable analysis, congenital heart disease (odds ratio [OR]: 3.88; 95% confidence interval [CI] : 1.23-12.29; P = .021), posterior plus anterior circulation stroke (OR: 5.36; 95% CI: 1.70-16.85; P = .004), and stroke presentation without seizures (OR: 3.95; 95% CI: 1.26-12.37; P = .019) were associated with in-hospital mortality for neonates. Hispanic ethnicity (OR: 3.12; 95% CI: 1.56-6.24; P = .001), congenital heart disease (OR: 3.14; 95% CI: 1.75-5.61; P < .001), and posterior plus anterior circulation stroke (OR: 2.71; 95% CI: 1.40-5.25; P = .003) were associated with in-hospital mortality for children. CONCLUSIONS: In-hospital mortality occurred in 2.6% of pediatric AIS cases. Most deaths were attributable to stroke. Risk factors for in-hospital mortality included congenital heart disease and posterior plus anterior circulation stroke. Presentation without seizures and Hispanic ethnicity were also associated with mortality for neonates and children, respectively. Awareness and study of risk factors for mortality represent opportunities to increase survival

    Leptospira species and serovars identified by MALDI-TOF mass spectrometry after database implementation

    Background: Leptospirosis, a spirochaetal zoonotic disease of worldwide distribution, endemic in Europe, has been recognized as an important emerging infectious disease, though yet it is mostly a neglected disease which imparts its greatest burden on impoverished populations from developing countries. Leptospirosis is caused by the infection with any of the more than 230 serovars of pathogenic Leptospira sp. In this study we aimed to implement the MALDI-TOF mass spectrometry (MS) database currently available in our laboratory with Leptospira reference pathogenic (L. interrogans, L. borgpetersenii, L. kirschneri, L. noguchii), intermediate (L. fainei) and saprophytic (L. biflexa) strains of our collection in order to evaluate its possible application to the diagnosis of leptospirosis and to the typing of strains. This was done with the goal of understanding whether this methodology could be used as a tool for the identification of Leptospira strains, not only at species level for diagnostic purposes, but also at serovar level for epidemiological purposes, overcoming the limits of serological and molecular conventional methods. Twenty Leptospira reference strains were analysed by MALDI-TOF MS. Statistical analysis of the protein spectra was performed by ClinProTools software. Results: The spectra obtained by the analysis of the reference strains tested were grouped into 6 main classes corresponding to the species analysed, highlighting species-specific protein profiles. Moreover, the statistical analysis of the spectra identified discriminatory peaks to recognize Leptospira strains also at serovar level extending previously published data. Conclusions: In conclusion, we confirmed that MALDI-TOF MS could be a powerful tool for research and diagnostic in the field of leptospirosis with broad applications ranging from the detection and identification of pathogenic leptospires for diagnostic purposes to the typing of pathogenic and non-pathogenic leptospires for epidemiological purposes in order to enrich our knowledge about the epidemiology of the infection in different areas and generate control strategies
